METHODS: Three pediatric patients with hepatoblastoma were enrolled in this study. Two underwent right hemi-hepatectomies and one underwent a left hemihepatectomy using the two-surgeon technique by means of saline-linked electric cautery (SLC) and the Cavitron Ultrasonic Surgical Aspirator (CUSA; Valleylab, Boulder, CO) and the LHM. RESULTS: The mean operative time during the parenchymal transections was 50 rain and the mean blood loss was 235 g. There was no bile leakage from the cut surface after surgery. No macroscopic or microscopic-positive margins were observed in the hepatic transections. CONCLUSION: The two-surgeon technique using SLC and CUSA with the LHM is applicable to even pediatric patients with hepatoblastoma.展开更多

The system consists of the following main components: an integrated P...A remote control system has been developed to deliver stimuli into the rat brain through a wireless micro-stimulator for animal behavior training. The system consists of the following main components: an integrated PC control program, a transmitter and a receiver based on Bluetooth (BT) modules, a stimulator controlled by C8051 microprocessor, as well as an operant chamber and an eight-arm radial maze. The micro-stimulator is featured with its changeable amplitude of pulse output for both constant-voltage and constant-current mode, which provides an easy way to set the proper suitable stimulation intensity for different training. The system has been used in behavior experiments for monitoring and recording bar-pressing in the operant chamber, controlling rat roaming in the eight-arm maze, as well as navigating rats through a 3D obstacle route. The results indicated that the system worked stably and that the stimulation was effective for different types of rat behavior controls. In addition, the results showed that stimulation in the whisker barrel region of rat primary somatosensory cortex (SI) acted like a cue. The animals can be trained to take different desired turns upon the association between the SI cue stimulation and the reward stimulation in the medial forehrain bundle (MFB).展开更多

This research presents two five-plane lancet needle designs, the five-plane lancet needle with two back bevels (FLN-B) and five-plane lancet needle with two front bevels (FLN-F), to study the effects of two additional bevel planes on the reduction of soft tissue cutting force. Mathematical models to calculate the inclination angle and rake angle along the cutting edges of FLN-B and FLN-F are developed. By using the grinding process, the prototype FLN-B and FLN-F needles are fabricated. And their inclination and rake angles along the cutting edges are investigated and compared to that of regular lancet needle. Needles insertion tests were conducted on soft PVC phantom which mimics the soft tissue. The initial peak insertion force and steady-state cutting force during needle insertion were identified, and the effect of cutting edge on needle soft tissue cutting force was studied. Compared to lancet needle, FLN-B and FLN-F both have higher inclination and rake angles at the tip cutting edge, could reduce the initial peak needle insertion force and tissue cutting forces, and thus can efficiently cut the soft tissue for medical applications.展开更多

These kinds of productions were widly used and had a close relationship with TCM. The development of this technology had through stages of "yeast production", "solid culture", "solid fermentation", and nowadays toward a series engineering of solid fermentation with promising prospects. The author described the development of its history briefly, and the representative products and the characteristics of production process of every stage, respectively. This research focused on taking "Trametes robinioplila fermented substance", "T. robinioplila & A. membranaceus fermented substance" and "T. wilfordii Hook.f. & G. lucidum fermented substance" as examples to introduce R & D ideas and important findings of"common fermentation" and "bi-directional fermentation" which were recommended as studying hotspots. There were many unique and important functions in the fields of TCM about all kinds of"fermentation substances". It is infered that there may exist some other innovate technologies such as "Multi-fungal fermentation", "Special elements fermentation", etc. which are remained to be researched and developed continuely. The results proved that, chitosan is certain to restrain the reaction of activated sludge, but it do improve the structure of sludge floes and increase the treatment efficiency of activated sludge. AIM: To investigate the use of the Gaumard's Noelle S550.100 Maternal and Neonatal Simulators for teaching forceps delivery.METHODS: Twenty two(n = 22) resident physicians were enrolled in a simulation course on o...AIM: To investigate the use of the Gaumard's Noelle S550.100 Maternal and Neonatal Simulators for teaching forceps delivery.METHODS: Twenty two(n = 22) resident physicians were enrolled in a simulation course on operative forceps deliveries. The physicians enrolled in the course were all part of an accredited Obstetrics and Gynecology residency program and ranged in their training from post graduate year(PGY) 1-4. Each participant received simulation based teaching on the indications, contraindications, proper application, delivery and removal of forceps by a single teacher. The Gaumard's simulator and Simpson forceps were used for this course. Statistical analysis using SPSS statistical software was performed after the completion of the simulation training program. A paired student t-test was performed to compare the cohort's mean pretest and post simulation training scores. Follow up skills assessment scores at one month, 3 mo and 6 mo were compared to thebaseline pretest score using a paired student t-test. RESULTS: There was statistically significant improvement in the post simulation training performance evaluations compared to the pretest, 13.7(SD = 3.14) vs 7.9(SD = 4.92), P < 0.05. Scores at 1 mo, 3 mo, and 6 mo were compared to the pretest score and showed retention of skills: 4.6(SD = 5.5, 95%CI: 2.21-7.07), 4.4(SD = 5.2, 95%CI: 2.13-6.70), and 5.6(SD = 4.8, 95%CI: 3.53-7.75) points, respectively. There were statistically significant differences between residents by post graduate training year on pretest scores, however these differences were not present after simulation training. Pretest scores for PGY 1, 2, 3, 4 were 3.5(SD = 2.27, 95%CI: 2.13-5.00), 7.25(SD = 6.70, 95%CI: 1.50-13.00), 10.75(SD = 1.5, 95%CI: 9.50-12.00), 12.17(SD = 2.57, 95%CI: 10.33-14.00). After simulation training PGY 1 residents did as well as well as the upper level residents. Posttest mean test scores for PGY 1, 2, 3, 4 were 13.75(SD = 1.49, 95%CI: 12.75-14.63), 10.25(SD = 0.24, 95%CI: 4.25-14.00), 15.00(SD = 1.16, 95%CI: 14.00-16.00), 15.17(SD = 0.75, 95%CI: 14.67-15.67). CONCLUSION: Our simulation based training program not only produced short term gains, but participants were able to retain the skills learned and demonstrate their knowledge months later.展开更多

To minimize this impact, many in the healthcare industry are highly interested in supporting prescription adherence among patients. They believe that information technology in general, and mobile technology in particular, can help in developing medical practices that can be highly conducive to high rates of prescription adherence by enhancing communication between patients and healthcare providers. To this end, a number of pharmacy management benefit companies plan to adopt SMS (Short Message Service) communication to reach their customers given the wider acceptance of SMS messaging among mobile phone users. However, most of these pharmacies are reluctant to purchase service agreements from SMS aggregators without a complete understanding of user, service and business requirements related to SMS messaging. Hence, many are in dire needs for prototypes of SMS servers that can help them define and refine these requirements before committing to costly agreements with SMS aggregators. This paper describes such a prototype for a pharmacy benefit management company located in the southeast of the United States.展开更多

The definition of the genetic basis of many human diseases has led to a better understanding of their pathogenesis a...Molecular analyses have become an integral part of biomedical research as well as clinical medicine. The definition of the genetic basis of many human diseases has led to a better understanding of their pathogenesis and has in addition offered new perspectives for their diagnosis, therapy and prevention. Genetically, human diseases can be classified as hereditary monogenic, acquired monogenic and polygenic diseases. Based on this classification, gene therapy is based on six concepts: (1) gene repair, (2) gene substitution, (3) cell therapy, (4) block of gene expression or function, (5) DNA vaccination and (6) gene augmentation. While major advances have been made in all areas of gene therapy during the last years, various delivery, targeting and safety issues need to be addressed before these strategies will enter clinical practice. Nevertheless, gene therapy will eventually become part of the management of patients with various liver diseases, complementing or replacing existing therapeutic and preventive strategies.展开更多
