快速康复护理干预在半月板损伤手术患者中的应用效果 被引量:5

目的探析快速康复护理干预对半月板损伤手术患者的临床应用效果。方法选取2016年2月~2017年2月本院收治的84例因半月板损伤拟行手术患者,根据入院时间顺序随机分为观察组与对照组,每组42例。观察组实施快速康复护理干预措施,对照组采用... 目的探析快速康复护理干预对半月板损伤手术患者的临床应用效果。方法选取2016年2月~2017年2月本院收治的84例因半月板损伤拟行手术患者,根据入院时间顺序随机分为观察组与对照组,每组42例。观察组实施快速康复护理干预措施,对照组采用常规护理干预措施。比较两组患者术后恢复、费用及视觉模拟法(VAS)评分情况。结果观察组患者住院天数(5.46±0.98)d,住院费用(3 237.45±46.28)元,术后0例并发症,均低于对照组(P<0.05)。观察组出院后1个月膝关节功能评分(86.94±3.17)分,出院时VAS评分(1.12±0.23)分,均明显优于对照组(P<0.05)。结论快速康复护理干预对半月板损伤行关节镜手术的患者护理效果明确,缩短住院时间,减轻医疗费用负担,减少术后并发症,改善患者预后,临床应用推广前景较广。 展开更多

目的探讨强化股四头肌锻炼对半月板损伤关节镜手术患者膝关节功能的影响。方法选取郑州大学第一附属医院180例接受关节镜手术治疗的半月板损伤患者按随机数字表分组,对照组采用常规康复锻炼,观察组联合强化股四头肌锻炼,对比两组患者膝... 目的探讨强化股四头肌锻炼对半月板损伤关节镜手术患者膝关节功能的影响。方法选取郑州大学第一附属医院180例接受关节镜手术治疗的半月板损伤患者按随机数字表分组,对照组采用常规康复锻炼,观察组联合强化股四头肌锻炼,对比两组患者膝关节功能、股四头肌表面肌电信号、膝关节活动度。结果观察组干预后的Lysholm评分、国际膝关节评分委员会(IKDC)评分、膝关节评分量表(HSS)评分分别为(87.52±8.76)分、(88.31±7.03)分、(86.58±8.64)分,均高于对照组的(80.65±8.69)分、(80.62±6.44)分、(78.35±8.42)分(P<0.001);股外侧肌、股内斜肌、骨直肌的肌电信号分别为(53.25±5.84)μV、(58.45±6.13)μV、(65.26±7.24)μV,高于对照组的(49.64±5.47)μV、(47.52±5.39)μV、(54.62±6.35)μV(P<0.001);观察组干预1个月、2个月、3个月后的膝关节活动度分别为(55.48±5.44)°、(65.42±5.83)°、(77.06±6.75)°,均大于对照组的(50.62±5.36)°、(58.69±5.57)°、(68.39±6.43)°(P<0.001)。结论强化股四头肌锻炼可显著增强半月板损伤关节镜手术患者的股四头肌表面肌电信号,提高膝关节活动度,改善膝关节功能。 展开更多

Efficacy of mild moxibustion combined with surgery for meniscal injury and its effect on TGF-β1 and PDGF levels in the fluid of knee joint

Objective To observe the efficacy of mild moxibustion combined with arthroscopic meniscal repair in the treatment of meniscal injury and to explore its action mechanism.Methods Ninety-eight patients with meniscal injury were divided into a surgery group and a moxibustion plus surgery group by the random number table method,with 49 cases in each group.Both groups received arthroscopic meniscal repair,and the moxibustion plus surgery group was treated with add-on mild moxibustion.The Lysholm score,visual analog scale(VAS)score,and mobility of the affected knee were measured before and after treatment,and transforming growth factor(TGF)-β1 and platelet-derived growth factor(PDGF)levels were measured in the fluid of the affected knee joint.The healing of the meniscus was recorded at a follow-up visit 3 months after the surgery.Results After treatment,the Lysholm score increased,the VAS score decreased in both groups,and the intra-group differences in both groups were statistically significant(P<0.05).The Lysholm score was higher in the moxibustion plus surgery group than in the surgery group,and the VAS score was lower in the moxibustion plus surgery group than in the surgery group.The differences between groups in both scores were statistically significant(P<0.05).The mobility of the affected knee joint increased in both groups(P<0.05),and it was greater in the moxibustion plus surgery group than in the surgery group(P<0.05).When compared with pretreatment,the levels of TGF-β1 and PDGF in the fluid of the knee joint increased in both groups(P<0.05),and the levels of TGF-β1 and PDGF in the fluid of the affected knee joint were higher in the moxibustion plus surgery group than in the surgery group(P<0.05).The healing of the meniscus in the moxibustion plus surgery group was significantly better than that in the surgery group at the follow-up visit 3 months after the surgery(P<0.05).Conclusion The effect of mild moxibustion combined with arthroscopic meniscal repair is better than the surgery alone in improving knee function,relieving pain,increasing mobility of the affected knee,and promoting meniscal healing,which may be related to the up-regulation of TGF-β1 and PDGF levels in the fluid of knee joint. 展开更多
