Frequent offce visits, every four to six weeks, confer a healthy life style year after year associated with a feeling of good well-being and a positive outlook. Topical eye drop is the most convenient and patient compliant route of drug administration,especially for the treatment of anterior segment diseases. Delivery of drugs to the targeted ocular tissues is restricted by various precorneal,dynamic and static ocular barriers. Also,therapeutic drug levels are not maintained for longer duration in target tissues. In the past two decades,ocular drug delivery research acceleratedly advanced towards developing a novel,safe and patient compliant formulation and drug delivery devices/techniques,which may surpass these barriers and maintain drug levels in tissues. Anterior segment drug delivery advances are witnessed by modulation of conventional topical solutions with permeation and viscosity enhancers. Also,it includes development of conventional topical formulations such as suspensions,emulsions and ointments. Various nanoformulations have also been introduced for anterior segment ocular drug delivery. On the other hand,for posterior ocular delivery,research has been immensely focused towards development of drug releasing devices and nanoformulations for treating chronic vitreo-retinal diseases. These novel devices and/or formulations may help to surpass ocular barriers and associated side effects with conventional topicaldrops. Also,these novel devices and/or formulations are easy to formulate,no/negligibly irritating,possess high precorneal residence time,sustain the drug release,and enhance ocular bioavailability of therapeutics. An update of current research advancement in ocular drug delivery necessitates and helps drug delivery scientists to modulate their think process and develop novel and safe drug delivery strategies. Current review intends to summarize the existing conventional formulations for ocular delivery and their advancements followed by current nanotechnology based formulation developments. Potential risks that are inevitably encountered with traditional methods include complications, secondary injuries, and limited sour...Donor shortages for organ transplantations are a major clinical challenge worldwide. Potential risks that are inevitably encountered with traditional methods include complications, secondary injuries, and limited source donors. Three-dimensional (3D) printing technology holds the potential to solve these limitations; it can he used to rapidly manufacture personalized tissue engineering scaffolds, repair tissue defects in situ with cells, and even directly print tissue and organs. Such printed implants and organs not only perfectly match the patient's damaged tissue, hut can also have engineered material microstructures and cell arrangements to promote cell growth and differentiation. Thus, such implants allow the desired tissue repair to he achieved, and could eventually solve the donor-shortage problem. This review summarizes relevant studies and recent progress on four levels, introduces different types of biomedical materials, and discusses existing problems and development issues with 3D printing that are related to materials and to the construction of extracellular matrix in vitro for medical applications.展开更多

Different tools have been developed, incl...Evaluation and improvement of quality of care provided to the patients are of crucial importance in the daily clinical practice and in the health policy planning and fnancing. Different tools have been developed, includ-ing incident analysis, health technology assessment and clinical audit. The clinical audit consist of mea-suring a clinical outcome or a process, against well-defined standards set on the principles of evidence-based medicine in order to identify the changes needed to improve the quality of care. In particular, patients suffering from chronic renal diseases, present many problems that have been set as topics for clinical audit projects, such as hypertension, anaemia and mineral metabolism management. Although the results of these studies have been encouraging, demonstrating the ef-fectiveness of audit, overall the present evidence is not clearly in favour of clinical audit. These findings call attention to the need to further studies to validate this methodology in different operating scenarios. This review examines the principle of clinical audit, focusing on experiences performed in nephrology settings.展开更多

The availability of new, simple, very sensitive and specific serological tests (anti-gliad...In the past, celiac disease was believed to be a chronic enteropathy, almost exclusively affecting people of European origin. The availability of new, simple, very sensitive and specific serological tests (anti-gliadin, anti- endomysium and anti-transglutaminase antibody assays) have shown that celiac disease is common not only in Europe and in people of European ancestry but also in the developing countries where the major staple diet is wheat (Southern Asia, the Middle East, North West and East Africa, South America), both in the general population and in the groups at risk. Gluten intolerance thus appears to be a widespread public health problem and an increased level of awareness and clinical suspicion are needed in the New World where physicians must learn to recognize the variable clinical presentations (classical, atypical and silent forms) of celiac disease. In the developing countries, both serological screening in the general population and serological testing in groups at risk are necessary for an early identification of celiac patients. The gluten-free diet poses a challenging public health problem in the developing countries, especially since commercial gluten-free products are not available.展开更多

AIM: To investigate the use of the Gaumard's Noelle S550.100 Maternal and Neonatal Simulators for teaching forceps delivery.METHODS: Twenty two(n = 22) resident physicians were enrolled in a simulation course on o...AIM: To investigate the use of the Gaumard's Noelle S550.100 Maternal and Neonatal Simulators for teaching forceps delivery.METHODS: Twenty two(n = 22) resident physicians were enrolled in a simulation course on operative forceps deliveries. The physicians enrolled in the course were all part of an accredited Obstetrics and Gynecology residency program and ranged in their training from post graduate year(PGY) 1-4. Each participant received simulation based teaching on the indications, contraindications, proper application, delivery and removal of forceps by a single teacher. The Gaumard's simulator and Simpson forceps were used for this course. Statistical analysis using SPSS statistical software was performed after the completion of the simulation training program. A paired student t-test was performed to compare the cohort's mean pretest and post simulation training scores. Follow up skills assessment scores at one month, 3 mo and 6 mo were compared to thebaseline pretest score using a paired student t-test. RESULTS: There was statistically significant improvement in the post simulation training performance evaluations compared to the pretest, 13.7(SD = 3.14) vs 7.9(SD = 4.92), P < 0.05. Scores at 1 mo, 3 mo, and 6 mo were compared to the pretest score and showed retention of skills: 4.6(SD = 5.5, 95%CI: 2.21-7.07), 4.4(SD = 5.2, 95%CI: 2.13-6.70), and 5.6(SD = 4.8, 95%CI: 3.53-7.75) points, respectively. There were statistically significant differences between residents by post graduate training year on pretest scores, however these differences were not present after simulation training. Pretest scores for PGY 1, 2, 3, 4 were 3.5(SD = 2.27, 95%CI: 2.13-5.00), 7.25(SD = 6.70, 95%CI: 1.50-13.00), 10.75(SD = 1.5, 95%CI: 9.50-12.00), 12.17(SD = 2.57, 95%CI: 10.33-14.00). After simulation training PGY 1 residents did as well as well as the upper level residents. Posttest mean test scores for PGY 1, 2, 3, 4 were 13.75(SD = 1.49, 95%CI: 12.75-14.63), 10.25(SD = 0.24, 95%CI: 4.25-14.00), 15.00(SD = 1.16, 95%CI: 14.00-16.00), 15.17(SD = 0.75, 95%CI: 14.67-15.67). CONCLUSION: Our simulation based training program not only produced short term gains, but participants were able to retain the skills learned and demonstrate their knowledge months later.展开更多
The paper investigates the contributions of TV Social Programs in UAE channels in promoting the social changes among youths in UAE, and to what extent it effects their attitudes towards the society and its values and ...The paper investigates the contributions of TV Social Programs in UAE channels in promoting the social changes among youths in UAE, and to what extent it effects their attitudes towards the society and its values and traditions. The study used the expecting value model and the third person effect concept in analyzing the perceptions of the UAE's youths towards the practices of local channels. The sample of the study was 200 male and female from several emirates. The study concluded that 88% of the audience in UAE watch the UAE satellite channels. Dubai channel comes in the top most watched channels, followed by Sharjah Channel, then Abu Dhabi channel. Most of the audience watch these channels in their homes. 78% of the audience watch the TV social programs. The most watched program is Khatwa program in Abd Dhabi channels, followed by shabnaa program. The reasons for watching the social programs are: finding relieve from their problems (66%), the diversity of these programs (64%), and the popularity of social programs' anchors (5%). The most discussed issues are Youths problems and unemployment (91%), local problems (73%), divorce issues (31%). They said that these programs encourages for persevering the local heritage (62%), respect the family values (51%), and social traditions (48%). While the most positive features of the social programs are: promote the local anchors (90%), the most negative features are promoting the western values (94%), and discussing some issues which are not fit with the local traditions (88%). In general the study found that the local TV channels played an important role in social changes in UAE.展开更多
In Mexico, garbage represents a high pollution index according to national and international organizations; eighty percent of waste products stay in sanitary landfills, out of doors, where no tailings management exist...In Mexico, garbage represents a high pollution index according to national and international organizations; eighty percent of waste products stay in sanitary landfills, out of doors, where no tailings management exists. Plastic products represent 16% of this pollution (one and one-half kilos daily per person in a country with a population of 107 million people). These sanitary landfills are methane deposits, and consequently emit gases and toxins that cause serious health problems. The object of this is to analyze governmental programs, strategies, policies and procedures which regulate both industrial sectors and society. Society and government share a responsibility since they must implement norms which include the separation, reduction, recycling and reuse of garbage. Results demonstrate that government strategies used to treat railings are complex and are directed more towards sanctions than to motivation. Furthermore these strategies discourage a cultural transformation toward industrial sustainability. Instead of reducing the garbage accumulation problem, they increase the difficulty.展开更多

We plead here already from the perspective of the slogan "One and All" of the first philosopher: Tales, when by the number (which one forebodes) one could go to such an extent into areas of reality (Pythagoras), or when (especially in the human sphere) is being over again actual final cause of Aristotle the philosophy and the mathematics to accomplish far more fruitful encounter with the Being. Alain Badiou (1988) has already pointed that: "Mathematics is ontology," and the category theory in mathematics, having covered by itself other fields of this science, continues to find applications in a series of"non-traditional" domains of reality. In that correlation the philosophy can express its (primary) needs for truth, justice, beauty, ... as well as for the overall development in the sense of purpose--also because of an undreamed power of the technological development (of hardwares and softwares) today. 文摘In the process of TCM intercultural communication, as a source culture, TCM embodies the essences of excellent traditional Chinese culture by absorbing the essence of Confucianism, Daoism and Buddhism and gradually evolving into a profound philosophical and cultural system during its long development, meanwhile, it has accumulated tremendous medical achievements and has been abundant in famous doctors in almost all dynasties. In short, TCM is profound both in medical practice and philosophy and culture, which endows TCM the characteristics of cultural essence and medical uniqueness, in addition to its scientific nature which has been proved historically, all of the above-mentioned factors form the basis and guarantee of the TCM intercultural communication, in which TCM is a source culture. In contrast, the western culture shows significant difference as a target culture, from which the western medicine is developed. The major differences manifest in: western culture distinguishes mankind from heaven while traditional Chinese culture believes in the harmony between man and nature; the difference between micro rational thinking and overall perceptual thinking; the difference between individual standard and organic whole. Despite the great difference, western society welcomes TCM due to its unique effect, therefore, it is of vital significance to compare and analysis the two ends of the TCM intercultural communication, namely the source culture and the target culture, to find effective means to promote the TCM intercultural communication. The current tendency of the integration of TCM and western medicine has proved the possibility and feasibility of TCM intercultural communication.

文摘Although E Maddy (1997) says on naturalism: "This is not, in itself, a philosophy of mathematics [...]" (161), already by its name, or by those whose interest has called on it (Quine, Putnam et al.) ... it anyhow reveals desire to be it. Insofar as otherwise, the semantic potential of the word leaves far behind it (after all scarce) results it achieved from the relation of an exact (mathematical) expression and (overly rich) intuitive reality of Being. We plead here already from the perspective of the slogan "One and All" of the first philosopher: Tales, when by the number (which one forebodes) one could go to such an extent into areas of reality (Pythagoras), or when (especially in the human sphere) is being over again actual final cause of Aristotle the philosophy and the mathematics to accomplish far more fruitful encounter with the Being. Alain Badiou (1988) has already pointed that: "Mathematics is ontology," and the category theory in mathematics, having covered by itself other fields of this science, continues to find applications in a series of"non-traditional" domains of reality. In that correlation the philosophy can express its (primary) needs for truth, justice, beauty, ... as well as for the overall development in the sense of purpose--also because of an undreamed power of the technological development (of hardwares and softwares) today. Namely, the naturalism in mathematics, which developed an abundant reflection on the place (importance of) the mathematical idiom in sciences--in the balance of criticism--has come rather to meager provisions, such as: "preestablished harmony of thinking," "ontic commitment," (Quine 1960) "the hygiene of mind," (Maddy 1996) "success argument," (Putnam 1975) "pragmatic argument," (Resnik 1981) etc., which only are few places from the encounter of an exact expression such as is mathematical one and the reality of natuural sciences. Instead of philosophy of mathematics to radicalize its claims from the perspective of that (powerful) mathematical idiom and the excessive reality of Being and man's place in it--this time, in the spirit of biocosmology (neo-Aristotelism).

Therefore, to promote the TCM intercultural communication, we need to enhance the publicity of TCM cultural tastes, to strengthen the construction of TCM education system in addition to making full use of Chinese students in UK.
