While advancing therapies may add significant longevity, they also bring about new end-of-life decision-making challenges for patients and their families who also must weigh the advan- tages of reduced mortality to the possibility of longer lives consisting of high morbidity, frailty, pain, and poor quality of living. Advance care entails options of withholding or withdrawing therapies, and has become a familiar part of cardiovascular care for older patients in Western countries. However, as advanced cardiovascular practices extend to developing countries, the interrelated concept of advance care is rarely straight forward as it is affected by local cultural traditions and mores, and can lead to very different inferences and use. This paper discusses the concepts of advance care planning, surrogate decision-making, orders for resuscitation and futility in patients with cardiac dis- ease with comparisons of West to East, focusing particularly on the United States versus India.展开更多

While the conductive gel achieves the requirement in making a good contact between the electrodes and skin, several problems appear. A gel-free, non-contact electrode based on capacitive coupling theory was provided in this paper, which was integrated on the print circuit board (PCB). The experimental results showed that clear ECG signals could be acquired in the laboratory conditions by coupling the electrodes to the chest of patients through cotton belts.展开更多

Bollywood and Tamil film has been taking a very popular cultural performance in Sri Lanka. The original Indian spectatorship of these film traditions has been diverted by the cultural and political psychology of Sri Lanka. Hence, this local orientation of the Indian cinema remains a quite metamorphosis and deconstruction in its appreciation and screening in terms of the social, cultural and political features that deeply-rooted in Sri Lankan history, culture and geographical limitations. For this study, several film texts, which include typical Bollywood and Tamil film characteristic features were used and observed on how they have been absorbed in the Sri Lankan cultural society. And on the other hand, some audience surveys specific to different film appreciation contexts were conducted. They included urban slums, semi-rural Muslim societies, north and east areas of Tamil villages, Colombo urban society, Tamil state areas and Kandyan Sinhala rural villages. In the textual analysis of the film content, implicit psychological narrations and their social and aesthetic interpretations were revealed in the analytical explanations. Using the audience surveys, many different opinions that can be traditionally filtered in the screening and appreciation of these film traditions specific to their living cultural settings were also identified. In conclusion, perception in Sinhala film has been considerably designed by the experience of Bollywood and Tamil film spectatorship is deducted by this study. Hence, not only this social trend affected to the appreciation of film in local general audience but also in all the facets of film production (script writing, shooting, acting, dialoguing, music and choreography and screening) has also significantly being influenced by the Bollywood and Tamil film. As a result of this evolution of local film culture, most recent trend in Sinhala film noir is also being followed by the early subjective consequences of Bollywood and Tamil film culture in Sri Lanka.展开更多

On the one hand, Indonesian demographic is showing that the Indonesian people are getting old. The fertility rate has significantly decreased since 1970 when the total fertility rate decreases from 5.0 to 2.1 in 2025. This alone gives rise to various problems of the elderly, among whom are the dependency ratio, aging index, potential support ratio, and the feminization of aging. These problems show the level of vulnerability of the Indonesian elderly. From the perspective of the dependency ratio, for example, the number of people under the age of 15 and above 65 who depend on the labor force (15-64 years) has increased since 1970, from 7.0 to 29.0 in 2050. Again, this shows the high level of vulnerability of Indonesian elderly given that the dependency ratio itself has influenced on health care, living arrangements and family support, and so on. Aside from that, the economic burden will be very visible in the workforce. On the other hand, the policies on the elderly are still far from maximum realization. Law on the Welfare of the Elderly (13/1998) has recommended that every hospital must have geriatrics and gerontology section, yet only 8 hospitals fulfill the requirement. The situation is worsened due to the lack of adequate medical staff at the health center, which can serve elderly patients in a professional and humane way. Again, this brings forth the other side of the state of the elderly in Indonesia, which indicates the high level of their vulnerability. In this paper I will analyze the state of vulnerability of the elderly people in Indonesia from the perspective of ethics of vulnerability and personalism. I consider these two approaches as representing all ethical discussion about the ethical treatment of the elderly due to their characters of valuing the elderly as persons with dignity. Thus, this paper will show that the elderly in Indonesia is in danger of not being treated humanely.展开更多

The indoor environmental factors in workspaces should follow functional as well as behavioral requirements as the spaces are designed as per the user's requirements, needs and aspirations. These play a major role in achieving behavioral impact on the user, as sense of well-being in the environment and physical comfort in the space enhances the productivity and efficiency of the user of the space. The slow and gradual importance of this issue in workspace design with Indian context, post occupancy evaluation can be adjudged as a permanent tool for improving indoor environment in buildings. Occupants should be used as a benchmark of evaluation, showing correlation of physical measurements and subjective parameters. It also suggests recommendations to improve quality of indoor environment in office buildings with analytical study, an important step in post occupancy evaluation. The objective of the paper is to inquire how people experience environmental psychology of the workspace as the determinants are efficiency, productivity and comfort. The success of any design depends upon the degree to which it creates an interface between the user and the environment.展开更多

Using mutant TGF-β1 transgenic (Tx) mice, we first demonstrated that EPL inhibited atrial fibrosis specifically and decreased mac- rophage accumulation in the atria of these mice. Results from immunohistochemistry and western blotting showed that EPL attenuated protein expression of fibrosis-related molecules such as connective tissue growth factor (CTGF) and fibronectin in the atria of Tx mice. In culture, EPL inhibited gene expression of fibrosis-related molecules such as fibronectin, ct-SMA, and CTGF in TGF-β1-stimulated atrial fibroblasts, Finally, using a co-culture system, we showed that TGF-β1 stimulated atrial fi- broblasts induced migration of macrophages and this was blocked by EPL. EPL also blocked TGF-β1 induced gene expression of intedeukin-6 (IL-6) in atrial fibroblasts. Therefore, we conclude that EPL attenuated atrial fibrosis and macrophage infiltra- tion in Tx mice. TGF-I31 and IL-6 were involved in the impacts of EPL on activation of atrial fibroblasts and interactions be- tween fibroblasts and macrophages.展开更多

Following Al exposure, the hippocampal LTP were recorded by field potentiation techniquein vivo and the expression of AMPAR subunit proteins (GluR1 and GluR2) in both total and membrane-enriched extracts from the CA1 area of rat hippocampus were detected by Western blot assay. 〈br〉 ResultsAcute Al treatment produced dose-dependent suppression of LTP in the rat hippocampus and dose-dependent decreases of GluR1and GluR2in membrane extracts; however, no similar changes were found in the total cell extracts, which suggests decreased trafficking of AMPA receptor subunits from intracellular pools to synaptic sites in the hippocampus. Thedose-dependent suppressive effects on LTP and the expression of AMPA receptor subunits both in the membrane and in total extracts were found after subchronic Al treatment, indicating a decrease in AMPA receptor subunit trafficking from intracellular poolsto synaptic sites and an additional reduction in the expression of the subunits. 〈br〉 ConclusionAl(mal)3obviously and dose-dependently suppressed LTP in the rat hippocampal CA1 region in vivo, and this suppression may be related to both trafficking and decreases in the expression of AMPA receptor subunit proteins. However, the mechanisms underlying these observations need further investigation.展开更多

Herein, hollow- channel technique was integrated with lab-on-paper for the simultaneous determination of two different concentrations of Zn2+ based on the origami principle, in which microfiuidic channels were first patterned on a cellulose paper using commercial solid-state wax printer. Hollow-channels were created by laser cutting method as the role of both injecting ending and reaction tank. After screen printing three elec- trodes system, the resulting planar paper sheets were then folded into steric structures and functional- ized by in-situ synthesized reduced graphene oxide. As a proof-of-concept, such lab-on-paper device was employed in the ratiometric electrochemical monitoring of zinc ion from the environment and HepG2 cells extract, by combining with co-catalysis of porous metal-organic frameworks and hemin/ G-quadruplex toward H202 in the linear range of 0.1-7,000 nmol/L. The results indicated that integrating hollow-channel with steric lab-on-paper offered a new methodological approach for the development of metal ions monitoring research. It is believed that it could be useful for various point-of-care related research fields, such as, on-site environmental monitoring, food safety, and disease diagnosis.展开更多

The physical mechanism of educational inequality is manifested in the poor health of migrant workers' children and their devalued behavior. Targeting these physical and psychological mechanisms, we have designed two simple but effective intervention strategies to raise the children's academic achievement: passing on the incremental theory of intelligence and establishing a multiple assessment system. These intervention strategies effectively improved the children's academic performance, increased their identification with learning, and lowered the stereotype threat. It is hoped that these strategies can be applied to the new generation of migrant workers entering the labor market.展开更多
