A total of 379 subjects were recruitedfor the study through community outreach and peerrecruiting methods.Results: Of 379 IDUs, the syphilis prevalence amongIDUs was 15.3% (58/379). Gender, years of education,history of unprotected sex with non-steady sex part-ner in the last month, number of sex partners in thelast 6 months, frequency of sex with non-steady sexpartner in the last 6 months, and number of any newsex partner(s) in the last 6 months were associatedwith syphilis infection after univariate analysis usingthe chi-squared test. Multivariate logistic regressionanalysis showed that ethnicity (OR, 2.85; 95% CI,1.55-5.25), marriage (OR, 1.94; 95% CI, 1.04-3.62)and receiving money for sex in the last 6 months (OR, ,5.60; 95% CI, 2.65-11.85) were significantly associ-ated with syphilis infection among IDUs.Conclusions: This study found that receiving moneyfor sex in the last 6 months was significantly associ-ated with syphilis prevalence among IDUs. It isrecommended that further health education andintervention campaigns be focused on IDUs to promotesafe sexual practices.展开更多

METHODS: Children at the age of 1-9 years were screened for HCV antibodies and alanine aminotransferase (ALT) levels. Every child with elevated ALT and/or detectable HCV antibodies was tested for HCV RNA by RT-PCR and compared with two negative controls for risk factors and signs and symptoms of liver disease.RESULTS: We screened 1042 children, six of them had elevated ALT, negative HCV antibody and positive RNA, likely representing acute hepatitis C cases. Fifteen children were HCV seropositive, 5 of them were HCV RNA positive. Asymptomatic HCV infection was present in 2.02% (positive results for either HCV antibodies or HCV-RNA or both). Symptoms such as diarrhea, abdominal pain, history of fatigue and school absence because of illness and risk factors such as dental care were significantly more common among HCV positive cases than among controls. None of the HCV positive children was diagnosed as having signs of advanced liver disease upon clinical or ultrasonographic examination. CONCLUSION: Asymptomatic HCV infection is detectable in 2.02% Egyptian children.展开更多

This review discusses the pathophysiology,epidemiology,risk factors,classification,clinical evaluation,and current non-operative and operative treatment of hemorrhoids.Hemorrhoids are defined as the symptomatic enlarg...This review discusses the pathophysiology,epidemiology,risk factors,classification,clinical evaluation,and current non-operative and operative treatment of hemorrhoids.Hemorrhoids are defined as the symptomatic enlargement and distal displacement of the normal anal cushions.The most common symptom of hemorrhoids is rectal bleeding associated with bowel movement.The abnormal dilatation and distortion of the vascular channel,together with destructive changes in the supporting connective tissue within the anal cushion,is a paramount finding of hemorrhoids.It appears that the dysregulation of the vascular tone and vascular hyperplasia might play an important role in hemorrhoidal development,and could be a potential target for medical treatment.In most instances,hemorrhoids are treated conservatively,using many methods such as lifestyle modification,fiber supplement,suppositorydelivered anti-inflammatory drugs,and administration of venotonic drugs.Non-operative approaches include sclerotherapy and,preferably,rubber band ligation.An operation is indicated when non-operative approaches have failed or complications have occurred.Several surgical approaches for treating hemorrhoids have been introduced including hemorrhoidectomy and stapled hemorrhoidopexy,but postoperative pain is invariable.Some of the surgical treatments potentially cause appreciable morbidity such as anal stricture and incontinence.The applications and outcomes of each treatment are thoroughly discussed.展开更多

The paper argues that dualistic ways of thinking or viewpoints that radically separate the human as distinct from the earth are hostile to both human and earth flourishing. It further argues that the ecological crisis which has witnessed the destruction and pollution of the ecosystems and other devastations cannot be solved when humans continue to see the earth as an object for conquest and exploitation. Using a critical hermeneutics method, the paper affirms that the African principle of relationality can help in mitigating the environmental crisis. A pedestrian safety analysis of the common right-turn mode at four-phase signalized intersections is presented. Relative risk is used as a measure of the effect of behaviors. The analysis mainly includes five pedestrian factors that affect the conflict process between pedestrians and right-turning vehicles. Pedestrians tend to have a higher risk of being involved in conflicts in the following six situations: crossing with others, running over the crossing, entering the intersection, being near the exit lane, crossing in the middle or at the end of a green light when the right-turn lane is shared, crossing at the beginning of a green light or red period when the right-turn lane is exclusive. It is easier for pedestrians to get priority when crossing the street in the following situations: running over a crossing, entering the intersection, being near the entrance lane, and not using the crosswalk. However, pedestrians are more inclined to yield to right-turning vehicles when pedestrians are crossing in the middle of the green light time. Some measures to alleviate the conflict are put forward according to the conclusion. Video observations also indicate that a clear pedestrian waiting area must be marked for both pedestrian safety and right-turning vehicle efficiency at major flat intersections, particularly when the arms cover the lateral dividing strips.展开更多

If we were to compare the economy of a country to the human body then we would realize that constant and m...The current economical crisis pointed out the gaps and the weak development of the Romanian economical system. If we were to compare the economy of a country to the human body then we would realize that constant and massive loss of "blood" may eventually lead to collapse of the economical system, which require Romania to speed up financial "transfusions". In these conditions, the most accessible and inexpensive source of funding is the non-repayable funding that Romanian economy can acquire by the year of 2013. In this paper we analyze the evolution of non-repayable funds allocated to Romania, in terms of those funds for the approved projects (their implementation has not yet started) but also in terms of those contracted projects or in the process of running (in fact representing "the necessary" amount of money that the Romanian economy needs). The first part of the paper contains an overview of the situation about the requiring and the absorption of non-repayable funding since 2007 (officially since the crisis emerged in the U.S.) to present, also making an analysis of the influence they had on the Romanian economy. We take into account in this analysis that the effects of the crisis in Romania began to be felt in the second half of 2008, economical politics against crisis were not taken into accounted by this date. In this analysis we started from the assumption that one anti-crisis policy, taken into account by public decision members in Romania, is to increase efficiency and absorption of funds by raising the capacity of the Romanian economical environment for the implementation (and not only the winning ) of these funds. The second part of the paper contains an econometrieal development model which highlights the influence of changing the interest rates and exchange rate on the rate of contracting projects during January 2007-March 2010. The Analysis of the results emphasizes the stronger influence of the changing rate on the approved contracting projects compared with the changing influence of the reference interest rate. Following the results of the analysis we highlight the main reasons for which this economical policy of attracting and using outstanding funding represents until now, a failure more than a success. Also we present these several solutions to increase the volume of financial "transfusion" with a direct impact on the Romanian economy, arguing that such measures cannot be implemented without real political will from decision-makers in Romania.展开更多
This paper computed the newest impact solutions of the potentially dangerous asteroid (99942) Apophis based on 4,138 optical observations from March 15.10789 UTC (Universal Time Coordinated), 2004 to February 28.0...This paper computed the newest impact solutions of the potentially dangerous asteroid (99942) Apophis based on 4,138 optical observations from March 15.10789 UTC (Universal Time Coordinated), 2004 to February 28.089569 UTC, 2014 and 20 radar observations from January 27, 2005 through March 15, 2013, as of June 20, 2014. Using the freely available the OrbFit software Package, this paper followed its orbit forward in the searching for close approaches with the Earth and possible impacts up to year 2116. With the different A2 non-gravitational parameter in the motion of the asteroid (99942) Apophis, this paper computed possible impact solutions using the JPL DE405 (Jet Propulsion Laboratory Development Ephemeris) and 25 additional massive perturbed asteroids. Additionally, this paper used weighing and selection methods adopted in the OrbFit software as prepared by the NEODyS (Near Earth Objects--Dynamical Side) Team. Moreover, this paper used method of computing the orbit of Apophis taking into account star catalog debiasing and an error model with assumed astrometric errors RMS (root mean square), deduced from the observational material of the given observatories. JPL's Sentry and NEODyS's CLOMMON2, two automatic monitoring systems routinely scanning for possible impacts in the next hundred years. Only for several dangerous asteroids presented results are computed with the non-gravitational parameters. This paper detected possible impacts of the asteroid (99942) Apophis only with the non-gravitational parameter, A2 〉 0. It was appeared that impacts in 2068, 2087, 2105 and in 2111 were possible only when Apophis rotated in prograde direction.展开更多

In this study, we use a narrative format to review the evidence for a prospecti...The long-term effects of physical activity (PA) or sedentary behavior on cardiovascular health in young people are not well understood. In this study, we use a narrative format to review the evidence for a prospective association with adiposity and other well-established biological cardiovascular risk factors in healthy young people, considering only studies with at least 2 years of follow-up. PA appears to elicit a long-term beneficial effect on adiposity and particularly markers of cardiovascular health. With adiposity, however, a few studies also reported that higher levels of PA were associated with higher levels of adiposity. Time spent sedentary does not appear to be related to adiposity or markers of cardiovascular health independent of PA. We then discuss the uncertainties in the underlying causal chain and consider a number of alternative modeling strategies, which could improve our understanding of the relationship in future studies. Finally, we consider the current methodology for assessing PA and sedentary time.展开更多

METHODS:Stools from 282 subjects suspected...AIM:To study the clinical and epidemiological features of patients with clonorchiasis so as to provide scientific evidences for the diagnosis and prevention of clonorchiasis. METHODS:Stools from 282 subjects suspected of having clonorchiasis were examined for helminth eggs with modified Kato's thick smear and sedimentation methods,and their sera were tested for HAV-DNA,HBV-DNA,HCV-RNA,HDV- RNA and HEV-RNA with polymerase chain reaction (PCR). Clinical symptoms of patients with clonorchiasis only were analyzed,and their blood samples were tested for circulating antigen (CAg) with Dot-ELISA,esoinophilic granulocyte count, and alanine aminotransferase (ALT).Meanwhile,they were asked to provide data of occupation,eating habit,hygienic habit and knowledge of donorchiasis.In addition,the ecosystem of the environment in epidemic areas was surveyed. RESULTS:Among the 282 patients,61 (21.43%) were infected with clonorchis sinensis only,97 (34.64%) were co-infected with clonorchis sinensis and other pathogens, 92 (32.86%) were infected with hepatitis virus only and 31 (11.07%) neither with clonorchis sinensis nor hepatitis virus. Among the 61 patients with clonorchiasis only,there were 14 (22.95%) subjects with discomfort over hepatic region or epigasfrium,12 (19.67%) with general malaise or discomfort and inertia in total body,6 (9.84%) with anorexia,indigestion and nausea,4 (6.56%) with fever,dizziness and headache (6.56%),and 25 (40.98%) without any symptoms;sixty one (100%) with CAg (+),98.33% (59160) with eosinophilic granulocytes increased and 65.00% (39/60) with ALT increased.B-mode ultrasonography revealed 61 cases with dilated and thickened walls of intrahepatic bile duct,and blurred patchy echo acoustic image in liver.Twenty-six cases had stones in the bile duct,39 cases had slightly enlarged liver with diffuse coarse spots in liver parenchyma.Twenty cases had enlarged gallbladder with thickened coarse wall and image of floating plagues,9 cases had slightly enlarged spleen.By analysis of epidemiological data,we found that the ecologic environment was favorable for the epidemiology of clonorchiasis.Most patients with clonorchiasis were lack of knowledge about the disease.Their living environment, hygienic habits,eating habits and their occupations were the related factors that caused the prevalence of the disease. CONCLUSION:The clinical symptoms of clonorchiasis are non-specific,and the main evidences for diagnosis of clonorchiasis should be provided by etiologic examination, B-mode ultrasonography and clinical history.The infection of clonorchis sinensis is related to occupations,bad eating habits and lack of knowledge about prevention of the disease.展开更多

In criminal law the causal relationship between the harmful behavior and harmful results is an intrinsic, essential contact. Endangering behavior (i.e. reason) is generated with contingency; in criminal law causality itself is a dialectical unity of necessity and contingency. One should correctly distinguish causes and conditions in the cause and effect relationship; one cannot equate a causal relationship in criminal law with constituting crime and undertaking criminal responsibility. Criminal law of causality is the main content of a crime in an objective harm, which reflects the intrinsic link of the harm behavior and harm. Attention has been drawn on viral hepatitis in their means of transmission by sexually behaviors, and also, on the homosexuals, assumed to be the high risk group to catch STDs. Coordinated national efforts to control STDs in China have been taken.

文摘A potential energy framework for assessment of grid vulnerability was presented.In the framework,the branch potential energy function model was constructed.Two indexes,current vulnerability and forecasting vulnerability,were calculated.The current vulnerability was used to identify the current vulnerable area through calculating the distance between the current transmitted power and initial transmitted power;and the forecast vulnerability under variation of power injection was used to predict the vulnerable area of next step and verify the current vulnerable area.Numerical simulation was performed under variant operating conditions with IEEE-30 bus system,which shows that almost area of 90% overlaps between current vulnerable area and forecasting vulnerable area,the overlapped area is termed as inherent vulnerable area of grid.When considering N-1 contingency,the assessment results of this method proposed agree with those of optimal power flow.When considering N-2 contingency,optimal power flow fails to obtain correct results,while the method based on energy framework gives reliable results.
文摘Degenerative disc disease is a multifaceted progressive irreversible condition and an inevitable part of aging,which has been found to be a contributing factor for low back pain and might cause radiculopathy,myelopathy,spinal stenosis,degenerative spondylolisthesis,and herniations.Its etiology is complex and multifactorial.Although genetics influence more dominant,the occupational and mechanical influences still persist as a major risk factor.This review emphasizes up-to-date knowledge regarding etiology of disc degeneration with special consideration on occupational,lifestyle factors,and genetic polymorphisms.
文摘The way that humans think about the earth certainly affects the way that they relate and behave towards the earth and all its life forms. The various ideologies or worldviews that humanity has experienced such as dualism, capitalism, holism, animism, ubuntu, ukama, vital force, colonialism, and so forth are all different ways of thinking and relating to the earth. The paper argues that dualistic ways of thinking or viewpoints that radically separate the human as distinct from the earth are hostile to both human and earth flourishing. It further argues that the ecological crisis which has witnessed the destruction and pollution of the ecosystems and other devastations cannot be solved when humans continue to see the earth as an object for conquest and exploitation. Using a critical hermeneutics method, the paper affirms that the African principle of relationality can help in mitigating the environmental crisis. It finds and concludes that if this principle is radically practiced, a better environment will be created.
文摘Epidemiologic studies have suggested a relation between hepatitis C virus (HCV) infection and diabetes mellitus. HCV infection is emerging as a metabolic disease, and diabetes mellitus as a risk factor for HCV infection. However, some data on the prevalence of antibodies to HCV in patients with diabetes are conflicting. These seroprevalence data should be interpreted with caution. Some potential bias may occur in those clinic-based studies that target a specif ic disease group. In this letter we explain some reasons for these conflicting studies.

文摘Due to the fact that there is no protected signal phase for right turns at most signalized intersections, the conflict between pedestrians and right-turning vehicles is one of the most common conflict types for pedestrians. A pedestrian safety analysis of the common right-turn mode at four-phase signalized intersections is presented. Relative risk is used as a measure of the effect of behaviors. The analysis mainly includes five pedestrian factors that affect the conflict process between pedestrians and right-turning vehicles. Pedestrians tend to have a higher risk of being involved in conflicts in the following six situations: crossing with others, running over the crossing, entering the intersection, being near the exit lane, crossing in the middle or at the end of a green light when the right-turn lane is shared, crossing at the beginning of a green light or red period when the right-turn lane is exclusive. It is easier for pedestrians to get priority when crossing the street in the following situations: running over a crossing, entering the intersection, being near the entrance lane, and not using the crosswalk. However, pedestrians are more inclined to yield to right-turning vehicles when pedestrians are crossing in the middle of the green light time. Some measures to alleviate the conflict are put forward according to the conclusion. Video observations also indicate that a clear pedestrian waiting area must be marked for both pedestrian safety and right-turning vehicle efficiency at major flat intersections, particularly when the arms cover the lateral dividing strips.
文摘The current economical crisis pointed out the gaps and the weak development of the Romanian economical system. If we were to compare the economy of a country to the human body then we would realize that constant and massive loss of "blood" may eventually lead to collapse of the economical system, which require Romania to speed up financial "transfusions". In these conditions, the most accessible and inexpensive source of funding is the non-repayable funding that Romanian economy can acquire by the year of 2013. In this paper we analyze the evolution of non-repayable funds allocated to Romania, in terms of those funds for the approved projects (their implementation has not yet started) but also in terms of those contracted projects or in the process of running (in fact representing "the necessary" amount of money that the Romanian economy needs). The first part of the paper contains an overview of the situation about the requiring and the absorption of non-repayable funding since 2007 (officially since the crisis emerged in the U.S.) to present, also making an analysis of the influence they had on the Romanian economy. We take into account in this analysis that the effects of the crisis in Romania began to be felt in the second half of 2008, economical politics against crisis were not taken into accounted by this date. In this analysis we started from the assumption that one anti-crisis policy, taken into account by public decision members in Romania, is to increase efficiency and absorption of funds by raising the capacity of the Romanian economical environment for the implementation (and not only the winning ) of these funds. The second part of the paper contains an econometrieal development model which highlights the influence of changing the interest rates and exchange rate on the rate of contracting projects during January 2007-March 2010. The Analysis of the results emphasizes the stronger influence of the changing rate on the approved contracting projects compared with the changing influence of the reference interest rate. Following the results of the analysis we highlight the main reasons for which this economical policy of attracting and using outstanding funding represents until now, a failure more than a success. Also we present these several solutions to increase the volume of financial "transfusion" with a direct impact on the Romanian economy, arguing that such measures cannot be implemented without real political will from decision-makers in Romania.
文摘This paper computed the newest impact solutions of the potentially dangerous asteroid (99942) Apophis based on 4,138 optical observations from March 15.10789 UTC (Universal Time Coordinated), 2004 to February 28.089569 UTC, 2014 and 20 radar observations from January 27, 2005 through March 15, 2013, as of June 20, 2014. Using the freely available the OrbFit software Package, this paper followed its orbit forward in the searching for close approaches with the Earth and possible impacts up to year 2116. With the different A2 non-gravitational parameter in the motion of the asteroid (99942) Apophis, this paper computed possible impact solutions using the JPL DE405 (Jet Propulsion Laboratory Development Ephemeris) and 25 additional massive perturbed asteroids. Additionally, this paper used weighing and selection methods adopted in the OrbFit software as prepared by the NEODyS (Near Earth Objects--Dynamical Side) Team. Moreover, this paper used method of computing the orbit of Apophis taking into account star catalog debiasing and an error model with assumed astrometric errors RMS (root mean square), deduced from the observational material of the given observatories. JPL's Sentry and NEODyS's CLOMMON2, two automatic monitoring systems routinely scanning for possible impacts in the next hundred years. Only for several dangerous asteroids presented results are computed with the non-gravitational parameters. This paper detected possible impacts of the asteroid (99942) Apophis only with the non-gravitational parameter, A2 〉 0. It was appeared that impacts in 2068, 2087, 2105 and in 2111 were possible only when Apophis rotated in prograde direction.
文摘The long-term effects of physical activity (PA) or sedentary behavior on cardiovascular health in young people are not well understood. In this study, we use a narrative format to review the evidence for a prospective association with adiposity and other well-established biological cardiovascular risk factors in healthy young people, considering only studies with at least 2 years of follow-up. PA appears to elicit a long-term beneficial effect on adiposity and particularly markers of cardiovascular health. With adiposity, however, a few studies also reported that higher levels of PA were associated with higher levels of adiposity. Time spent sedentary does not appear to be related to adiposity or markers of cardiovascular health independent of PA. We then discuss the uncertainties in the underlying causal chain and consider a number of alternative modeling strategies, which could improve our understanding of the relationship in future studies. Finally, we consider the current methodology for assessing PA and sedentary time.
文摘AIM:To study the clinical and epidemiological features of patients with clonorchiasis so as to provide scientific evidences for the diagnosis and prevention of clonorchiasis. METHODS:Stools from 282 subjects suspected of having clonorchiasis were examined for helminth eggs with modified Kato's thick smear and sedimentation methods,and their sera were tested for HAV-DNA,HBV-DNA,HCV-RNA,HDV- RNA and HEV-RNA with polymerase chain reaction (PCR). Clinical symptoms of patients with clonorchiasis only were analyzed,and their blood samples were tested for circulating antigen (CAg) with Dot-ELISA,esoinophilic granulocyte count, and alanine aminotransferase (ALT).Meanwhile,they were asked to provide data of occupation,eating habit,hygienic habit and knowledge of donorchiasis.In addition,the ecosystem of the environment in epidemic areas was surveyed. RESULTS:Among the 282 patients,61 (21.43%) were infected with clonorchis sinensis only,97 (34.64%) were co-infected with clonorchis sinensis and other pathogens, 92 (32.86%) were infected with hepatitis virus only and 31 (11.07%) neither with clonorchis sinensis nor hepatitis virus. Among the 61 patients with clonorchiasis only,there were 14 (22.95%) subjects with discomfort over hepatic region or epigasfrium,12 (19.67%) with general malaise or discomfort and inertia in total body,6 (9.84%) with anorexia,indigestion and nausea,4 (6.56%) with fever,dizziness and headache (6.56%),and 25 (40.98%) without any symptoms;sixty one (100%) with CAg (+),98.33% (59160) with eosinophilic granulocytes increased and 65.00% (39/60) with ALT increased.B-mode ultrasonography revealed 61 cases with dilated and thickened walls of intrahepatic bile duct,and blurred patchy echo acoustic image in liver.Twenty-six cases had stones in the bile duct,39 cases had slightly enlarged liver with diffuse coarse spots in liver parenchyma.Twenty cases had enlarged gallbladder with thickened coarse wall and image of floating plagues,9 cases had slightly enlarged spleen.By analysis of epidemiological data,we found that the ecologic environment was favorable for the epidemiology of clonorchiasis.Most patients with clonorchiasis were lack of knowledge about the disease.Their living environment, hygienic habits,eating habits and their occupations were the related factors that caused the prevalence of the disease. CONCLUSION:The clinical symptoms of clonorchiasis are non-specific,and the main evidences for diagnosis of clonorchiasis should be provided by etiologic examination, B-mode ultrasonography and clinical history.The infection of clonorchis sinensis is related to occupations,bad eating habits and lack of knowledge about prevention of the disease.

文摘Understanding the causal relationship between the criminal law should start with the objective nature of behavior and then set out to determine whether there is the causal relationship between behavior and results, and then combine with the perpetrator' s subjective guilt to determine whether his conduct is harmful behavior, so as to determine the severity of his criminal responsibility. In criminal law the causal relationship between the harmful behavior and harmful results is an intrinsic, essential contact. Endangering behavior (i.e. reason) is generated with contingency; in criminal law causality itself is a dialectical unity of necessity and contingency. One should correctly distinguish causes and conditions in the cause and effect relationship; one cannot equate a causal relationship in criminal law with constituting crime and undertaking criminal responsibility. Criminal law of causality is the main content of a crime in an objective harm, which reflects the intrinsic link of the harm behavior and harm. Correctly solving the criminal law of causality is important for the solution how much criminal responsibility the armful behavior should bear.
