Only indirect me...The role of the oesophago-gastric junction (OGJ) in gastro-oesophageal reflux disease is still not completely understood, and there is no clinically used method to assess the OGJ function in patients. Only indirect methods such as pH studies are carried out. The OGJ acts a valve controlling the flow of solids, liquids and gases between the oesophagus and the stomach. Manometry can determine if a sphincter is toned or relaxed; but, it cannot confirm that the sphincter region is actually open. Distension is a new technique for measuring function in the OGJ. By measuring the cross-sectional area through the narrow region in the junction during distension of a catheter mounted bag, much more information on the opening and closing patterns of the junction can be determined. This technique has already been demonstrated to show changes in the OGJ after surgical treatments for reflux disease. New measurement ideas around the concept of distending the OGJ offer new hope that a clinically useable test for compliance at the junction can be developed and could potentially help in determining appropriate therapy.展开更多

Radical measures of increasing gas extra...For a low permeability single coal seam prone to gas outbursts, pre-drainage of gas is difficult and inefficient, seriously restricting the safety and efficiency of production. Radical measures of increasing gas extraction efficiency are pressure relief and infrared antireflection. We have analyzed the effect of mining conditions and the regularity of mine pressure distribution in front of the working face of a major coal mine of the Jiaozuo Industrial (Group) Co. as our test area, studied the width of the depressurization zone in slice mining and analyzed gas efficiency and fast drainage in the advanced stress relaxation zone. On that basis, we further investigated and practiced the exploitation technology of shallow drilling, fan dril- ling and grid shape drilling at the working face. Practice and our results show that the stress relaxation zone is the ideal region for quick and efficient extraction of gas. By means of an integrated extraction technology, the amount of gas emitted into the zone was greatly reduced, while the risk of dangerous outbursts of coal and gas was lowered markedly. This exploration provides a new way to control for gas in working faces of coal mines with low permeability and risk of gas outbursts of single coal seams in the Jiaozuo mining area.展开更多

METHODS: A retrospective case-control audit was ...AIM: To characterize the effects of age on the mechanisms underlying the common condition of esophageal dysphagia in older patients, using detailed manometric analysis. METHODS: A retrospective case-control audit was performed on 19 patients aged ≥ 80 years (mean age 85 ± 0.7 year) who underwent a manometric study for dysphagia (2004-2009). Data were compared with 19 younger dysphagic patients (32 ± 1.7 years). Detailed manometric analysis performed prospectively included basal lower esophageal sphincter pressure (BLESP), pre-swallow and nadir LESP, esophageal body pressures and peristaltic duration, during water swallows (5 mL) in right lateral (RL) and upright (UR) postures and with solids. Data are mean ± SE; a P-value < 0.05 was considered significant. RESULTS: Elderly dysphagic patients had higher BLESP than younger patients (23.4 ± 3.8 vs 14.9 ± 1.2 mmHg; P < 0.05). Pre-swallow LESP was elevated in the elderly in both postures (RL: 1 and 4 s P = 0.019 and P = 0.05; UR: P < 0.05 and P = 0.05) and solids (P < 0.01). In older patients, LES nadir pressure was higher with liquids (RL: 2.3 ± 0.6 mmHg vs 0.7 ± 0.6 mmHg, P < 0.05; UR: 3.5 ± 0.9 mmHg vs 1.6 ± 0.5 mmHg, P = 0.01) with shorter relaxation after solids (7.9 ± 1.5 s vs 9.7 ± 0.4 s, P = 0.05). No age-related differences were seen in esophageal body pressures or peristalsis duration. CONCLUSION: Basal LES pressure is elevated and swallow-induced relaxation impaired in elderly dysphagic patients. Its contribution to dysphagia and the effects of healthy ageing require further investigation.展开更多

The classical boundar...A challenge in the study of turbulent boundary layers(TBLs) is to understand the non-equilibrium relaxation process after separation and reattachment due to shock-wave/boundary-layer interaction. The classical boundary layer theory cannot deal with the strong adverse pressure gradient, and hence, the computational modeling of this process remains inaccurate. Here, we report the direct numerical simulation results of the relaxation TBL behind a compression ramp, which reveal the presence of intense large-scale eddies, with significantly enhanced Reynolds stress and turbulent heat flux. A crucial finding is that the wall-normal profiles of the excess Reynolds stress and turbulent heat flux obey a β-distribution, which is a product of two power laws with respect to the wall-normal distances from the wall and from the boundary layer edge. In addition, the streamwise decays of the excess Reynolds stress and turbulent heat flux also exhibit power laws with respect to the streamwise distance from the corner of the compression ramp. These results suggest that the relaxation TBL obeys the dilation symmetry, which is a specific form of self-organization in this complex non-equilibrium flow. The β-distribution yields important hints for the development of a turbulence model.展开更多
