METHODS:Pubmed,Embase,Elsevier ScienceDirect,the China Academic Journals Full-text Database,and Cochrane Controlled Trials Register were searched for the trials comparing the yield of CE with that of DBE. Outcome measure was odds ratio (OR) of the yield. Fixed or random model method was used for data analysis. RESULTS:Eight studies (n = 277) which prospectively compared the yield of CE and DBE were collected. The results of meta-analysis indicated that there was no difference between the yield of CE and DBE 170/277 vs 156/277,OR 1.21 (95% CI:0.64-2.29). Based on sub analysis,the yield of CE was significantly higher than that of double-balloon enteroscopy without combination of oral and anal insertion approaches 137/219 vs 110/219,OR 1.67 (95% CI:1.14-2.44),P < 0.01),but not superior to the yield of DBE with combination of the two insertion approaches 26/48 vs 37/48,OR 0.33 (95% CI:0.05-2.21),P > 0.05). A focused meta-analysis of the fully published articles concerning obscure GI bleeding was also performed and showed similar results wherein the yield of CE was significantly higher than that of DBE without combination of oral and anal insertion approaches 118/191 vs 96/191,fixed model:OR 1.61 (95% CI:1.07-2.43),P < 0.05) and the yield of CE was significantly lower than that of DBE by oral and anal combinatory approaches 11/24 vs 21/24,fixed model:OR 0.12 (95% CI:0.03-0.52),P < 0.01). CONCLUSION:With combination of oral and anal approaches,the yield of DBE might be at least as high asthat of CE. Decisions made regarding the initial approach should depend on patient's physical status,technology availability,patient's preferences,and potential for therapeutic endoscopy.展开更多

METHODS:The d...AIM:Leakage from oesophageal anastomosis is associated with substantial morbidity and mortality.This study presented a novel,safe and effective double stapled technique for oesophago-enteric anastomosis. METHODS:The data were obtained prospectively from hospital held clinical database.Thirty nine patients(26 males,13 females)underwent upper-gastrointestinal resection between 1996 and 2000 for carcinoma(n=36), gastric lymphoma(n=1),and benign pathology(n=2). Double stapled oesophago-enteric anastomosis was performed in all cases. RESULTS:No anastomotic leak was reported.In cases of malignancy,the resected margins were free of neoplasm. Three deaths occurred,which were not related to anastomotic complications. CONCLUSION:Even though the reported study is an uncontrolled one,the technique described is reliable,and effective for oesophago-enteric anastomosis.展开更多
Microstructural features of a duplex-phase Zr-2.5Nb alloy were investigated in detail using electron channeling contrast (ECC) imaging and electron backscatter diffraction (EBSD) technique in an emission gun scann...Microstructural features of a duplex-phase Zr-2.5Nb alloy were investigated in detail using electron channeling contrast (ECC) imaging and electron backscatter diffraction (EBSD) technique in an emission gun scanning electron microscope (FEGSEM). The excellent resolution provided by the FEGSEM promises the combined utilization of both techniques to be quite adequate for characterizing the duplex-phase microstructures. Results show that the microstructure of the Zr-2.5Nb alloy is composed of bulk a grains (majority) in equiaxed or plate shape and thin 13 films (minority) surrounding the bulk grains, with their average grain size and thickness measured to be 1.4 prn and 72 nm, respectively. Analyses on a-grain boundaries reveal a number of low angle boundaries, most of which belong to deformation-induced dislocation boundaries. Measurements on relative propor- tions of various Burgers boundaries suggest very weak (if any) variant selection during 13 ~ a cooling, which should be re- lated to deformation-induced higher nucleation rate of a phases. Compared to earlier attempts, more satisfactory indexing of fine β phases (down to nanoscale) is attained by the FEGSEM-based EBSD. Examples are presented to clearly reveal well-obeyed Burgers orientation relationships between adjacent α and β phases. Finally, it is deduced that continuing applica- tion of the FEGSEM-based EBSD to duplex-phase Zr alloys could help clarify controversies like the deformation priority of the two phases.展开更多
