目的比较采用高屈曲度假体与普通后稳定假体的人工全膝关节置换术近期疗效。方法2005年4月-2007年10月,对23例27膝采用高屈曲度假体行人工全膝关节置换(高屈曲度组),同期35例41膝采用普通后稳定假体置换(后稳定组)。高屈曲度组:男2例3膝...目的比较采用高屈曲度假体与普通后稳定假体的人工全膝关节置换术近期疗效。方法2005年4月-2007年10月,对23例27膝采用高屈曲度假体行人工全膝关节置换(高屈曲度组),同期35例41膝采用普通后稳定假体置换(后稳定组)。高屈曲度组:男2例3膝,女21例24膝;年龄(64.3±5.6)岁。体重指数(body mass index,BMI)为27.3±3.9。骨性关节炎20例23膝,类风湿性关节炎3例4膝。病程(5.3±5.6)年。膝关节功能按照美国特种外科医院(HSS)评分为(58.4±7.9)分,美国西部Ontario和McMaster大学骨性关节炎指数(WOMAC)评分为(49.4±6.9)分。膝关节最大屈曲度为(107.6±8.3)°,膝关节活动度为(103.5±7.7)°。后稳定组:男3例3膝,女32例38膝;年龄(65.1±5.9)岁。BMI为27.1±4.1。骨性关节炎33例39膝,类风湿性关节炎2例2膝。病程(5.1±4.9)年。HSS评分为(60.1±10.4)分;WOMAC评分为(47.9±7.2)分。膝关节最大屈曲度为(108.4±9.7)°,膝关节活动度为(105.9±11.4)°。两组一般资料比较差异均无统计学意义(P>0.05),具有可比性。结果术后两组切口均Ⅰ期愈合,无膝关节僵硬、血管及神经损伤、假体松动等并发症发生。患者均获随访,随访时间24~54个月,平均32.8个月。两组术后3、12、24个月的HSS评分、WOMAC评分、膝关节最大屈曲度、关节活动度比较,差异均无统计学意义(P>0.05),但与同组术前比较差异有统计学意义(P<0.05)。两组术后24个月内有5例发生膝前痛,其中高屈曲度组1例,后稳定组4例,发生率分别为3.70%及9.76%,差异有统计学意义(P<0.05)。术后X线片检查未见假体周围透亮区,关节力线正常,无髌骨低位。结论采用高屈曲度假体行人工全膝关节置换术后关节活动度及临床功能评分与普通后稳定假体无明显差异,但膝前痛发生率较小。展开更多

MSI determination was performed in accordance with the NCI criteria using PCR and Genescan. Telomerase activity was determined by the TRAP-assay, an ELISA procedure based on the amplification of telomeric repeat sequences.RESULTS: 6.2% showed high MSI (MSI-H), 10.3% showed low MSI (MSI-L) and 83.5% did not show this alteration (MSS). Positive telomerase activity was detected in 92.8% of the patients. 83.3% of MSI-H tumors showed positive telomerase against 93.8% of MSS tumors. In the overall survival analysis the absence of telomerase activity conferred a better prognosis.CONCLUSION: Previous works have shown that tumors which develop via the MSI pathway present a better prognosis. No link between telomerase activity and MSl status is observed, although sample sizes are small. Patients with telomerase negative tumors had better overall survival than patients with telomerase positive tumors.展开更多
Based on the technology of controlling surrounding rock deformation by constructing concrete pillars in large section finishing cut in backfill coal mining, the characteristics of vertical stress on concrete pillars a...Based on the technology of controlling surrounding rock deformation by constructing concrete pillars in large section finishing cut in backfill coal mining, the characteristics of vertical stress on concrete pillars and main factors influencing pillar stability are analyzed. By building a Winkler elastic foundation mechanical model for the support system constituted of coal pillar, backfill body and concrete pillars, mechanical calculation on stability of concrete pillar is carried out to evaluate the pillar stability and safety. Seven numeral models in three schemes with different pillar sizes, inter-row distances and compression ratios at the stopes were analyzed through numerical simulation according to width reduction principle. The practice of finishing cut at IIIB44 workface at Yangzhuang coal mine shows that: when the actual compression ratio is 86.5%, construction size inside the finishing cut is 2000 mm x 2000 mm and the interval between concrete pillars is 2000 mm x 2000 mm, the pillars can be stable with the maximum movement of two sides of each pillar being only 83 mm and 54 mm, which achieves the expected effect.展开更多
