具有囊性特征的胰腺导管腺癌与囊腺瘤的多层螺旋CT对比研究 被引量:2

目的:探讨多层螺旋CT对具有囊性特征的胰腺导管腺癌与胰腺囊腺瘤的鉴别诊断价值。方法:回顾性分析具有囊性特征的胰腺导管腺癌与胰腺囊腺瘤的48例患者的临床资料,均行胰腺CT扫描且经过病理证实。其中胰腺导管腺癌12例、浆液性囊腺瘤16... 目的:探讨多层螺旋CT对具有囊性特征的胰腺导管腺癌与胰腺囊腺瘤的鉴别诊断价值。方法:回顾性分析具有囊性特征的胰腺导管腺癌与胰腺囊腺瘤的48例患者的临床资料,均行胰腺CT扫描且经过病理证实。其中胰腺导管腺癌12例、浆液性囊腺瘤16例、黏液性囊腺瘤20例。分析肿瘤位置、单(多)囊、最大囊直径、形态、囊内容物、囊壁特征、囊内分隔、胰管扩张、胰腺萎缩等影像学征象。结果:同时满足4项CT征象(包括形态不规则,囊肿多发,有壁结节,囊壁局部增厚)对于鉴别具有囊性特征的胰腺导管腺癌和囊腺瘤具有较高的敏感度(88%)和特异度(74%)。其他CT表现如囊肿的位置、大小、分隔、胰腺萎缩、淋巴结肿大等对病灶的鉴别诊断无统计学意义(P>0.05)。结论:4项CT征象:形态不规则、囊肿多发、有壁结节、囊壁局部增厚对于鉴别具有囊性特征的胰腺导管腺癌和囊腺瘤具有较高的诊断价值。 展开更多

目的分析腺样囊性癌样特征颌骨造釉细胞瘤与原发颌骨腺样囊性癌临床、病理特征。方法对新疆医科大学第一附属医院2013-02—2022-04原发颌骨腺样囊性癌4例及具有腺样囊性癌特征颌骨造釉细胞瘤14例进行回顾性分析。结果原发颌骨腺样囊性... 目的分析腺样囊性癌样特征颌骨造釉细胞瘤与原发颌骨腺样囊性癌临床、病理特征。方法对新疆医科大学第一附属医院2013-02—2022-04原发颌骨腺样囊性癌4例及具有腺样囊性癌特征颌骨造釉细胞瘤14例进行回顾性分析。结果原发颌骨腺样囊性癌女性多见,中位年龄68岁,以上颌骨发生为主;腺样囊性癌样特征颌骨造釉细胞瘤男性多见,中位年龄46岁,以下颌骨发生为主。原发颌骨腺样囊性癌由腺上皮及肌上皮两类细胞组成,可见腺样及囊腔样结构。腺上皮嗜酸性,免疫组化示腺上皮CD117、CK7阳性,肌上皮SMA、calponin阳性。具有腺样囊性癌特征造釉细胞瘤由网状细胞及极性倒置成釉细胞组成,其中亦可见腺腔样结构,免疫组化示P63、P40弥漫阳性,calponin部分阳性,SMA、S-100阴性。结论具有腺样囊性癌特征造釉细胞瘤与原发颌骨腺样囊性癌样形态有交叉,结合病理形态、免疫组化及分子检测可鉴别二者。 展开更多

目的:探讨具有囊性改变的胰腺导管腺癌(PDAC)的CT及MRI表现。方法:回顾性分析经手术病理证实的、CT/MRI上表现为囊性改变的15例PDAC患者的影像资料,观察内容包括病变的位置、形态、囊性/囊实性,肿瘤最大径、囊性成分的最大径、囊内容物... 目的:探讨具有囊性改变的胰腺导管腺癌(PDAC)的CT及MRI表现。方法:回顾性分析经手术病理证实的、CT/MRI上表现为囊性改变的15例PDAC患者的影像资料,观察内容包括病变的位置、形态、囊性/囊实性,肿瘤最大径、囊性成分的最大径、囊内容物特征、囊壁特征、主胰管是否扩张等征象。结果:15例PDAC中,低分化者4例,中分化者5例,高分化者5例,介于中、高分化之间者1例。病灶表现为纯囊性肿块2例;囊实性肿块13例,其中7例表现为实性肿块中心出现不规则囊性区。15例PDAC中,2例囊内容物信号/密度不均匀(2/15),且T1WI上呈高信号;5例出现分隔(5/15),4例出现壁结节(4/15)。15例肿块的最大径为(4.56±1.52)cm;肿块内囊变的最大径为(2.80±1.90)cm。主胰管及胆总管均扩张4例,仅病变处主胰管扩张2例,仅病变后方主胰管扩张1例。结论:具有囊性改变的PDAC并不多见,纯囊性表现者更为少见,临床上极易误诊为单纯囊肿或假性囊肿;囊实性表现者与胰腺囊实性肿瘤鉴别困难,临床需要加强认识,提高诊断准确性。 展开更多

AIM: To study the relationship between nm23H1 gene genetic instability and its clinical pathological characteristics in Chinese digestive system cancer patients. METHODS: Polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) was used to analyze the microsatellite instability (MSI) and loss of heterozygosity (LOH). Immunohistochemistry was employed to detect the expression of nm23H1. RESULTS: The MSI was higher in TNM stageⅠ + Ⅱ than in stage Ⅲ + Ⅳ of gastric, colonic and gallbladder carcinomas. The LOH was higher in TNM stage Ⅲ + Ⅳ than in stageⅠ + Ⅱ of gastric, colonic and hepatocellular carcinomas. Lymphatic metastasis was also observed. The expression of nm23H1 protein was lower in TNM stage Ⅲ + Ⅳ than in stageⅠ + Ⅱ of these tumors and in patients with lymphatic metastasis.The nm23H1 protein expression was higher in the LOH negative group than in the LOH positive group.CONCLUSION: MSI and LOH may independently control the biological behaviors of digestive system cancers. MSI could serve as an early biological marker of digestive system cancers. Enhanced expression of nm23H1 protein could efficiently inhibit cancer metastasis and improve its prognosis. LOH mostly appears in late digestive system cancer. 展开更多

Fabrication and Properties of Microencapsulated n-octadecane and Paraffin

Microencapsulated n-alkanes as energy- storage materials have promising application prospects. The ndcrocapsules containing 100 - 50 wt% of n - octadecane, 0 -20 wt% of paraffin and 0 - 30 wt% of cyclohexane were synthesized by in-situ polymerization using melamine- formaldehyde polymer as shell. Cyclohexane was removed after heat-treated the microcapsules at 100℃. The morphologies, cell parameters, phase change properties, thermal stable temperatures of these microcapsules were examined. The diameters of these microcapsules are lower than 5 μm. The effect of paraffin in the microcapsules on the cell parameters of n-octadecane is negligible. The paraff'm is effectively used as a nucleating agent to decrease the degree of supercooling. The melting enthalpy is decreased from 132 J/g to 111 J/g due to the increase of the cyclohexane contents. The thermal stable temperature is enhanced 6 - 16℃ after heat-treated the microcapsules at 160℃ for 30 min. 展开更多
