AIM:To compare diverse endoscopic interventions in the management of occluded uncovered self-expanding metal stents(SEMSs) that had been placed for palliative treatment of unresectable malignant biliary obstruction.ME...AIM:To compare diverse endoscopic interventions in the management of occluded uncovered self-expanding metal stents(SEMSs) that had been placed for palliative treatment of unresectable malignant biliary obstruction.METHODS:A retrospective review was undertaken in 4 tertiary endoscopic centers to determine optimal management of different types of occluded SEMSs.The technical success of performed treatment in occluded SEMSs,the patency of the stent,the need for re-intervention and the financial costs of each treatment were analyzed.RESULTS:Fifty four patients were included in the analysis;21 received Hanaro,19 Wallstent and 14 Flexus.For the relief of obstruction,a plastic stent was inserted in 24 patients,a second SEMS in 25 and mechanical cleaning was performed in 5 patients.The overall median second patency rates between second SEMSs and plastic stents did not differ(133 d for SEMSs vs 106 d for plastic stents;P = 0.856).Similarly,no difference was found between the overall survival of SEMS and plastic stent groups,and no procedure-related complications occurred.Incremental cost analysis showed that successive plastic stenting was a cost-saving strategy at least in Greece.CONCLUSION:Insertion of uncovered SEMSs or plastic stents is a safe and effective treatment for occluded uncovered SEMSs;insertion of plastic stents appears to be the most cost-effective strategy.展开更多

The conventional limit equilibrium method u...A footing may get an eccentric load caused by earthquake or wind, thus the bearing capacity of footing subjected to eccentric load become a fundamental geotechnical problem. The conventional limit equilibrium method used for this problem usually evaluates the material properties only by its final strength. But the classical finite element method(FEM) does not necessarily provide a clear collapse mechanism associated with the yield condition of elements. To overcome these defects, a numerical procedure is proposed to create an explicit collapse mode combining a modified smeared shear band approach with a modified initial stress method. To understand the practical performance of sand foundation and verify the performance of the proposed procedure applied to the practical problems, the computing results were compared with the laboratory model tests results and some conventional solutions. Furthermore, because the proposed numerical procedure employs a simple elasto-plastic model which requires a small number of soil parameters, it may be applied directly to practical design works.展开更多

AMI model was induced...Objectives. To compare the effects of losartan, enalapril and their combination in the prevention ofleft ventricular remodeling (LVRM) after acute myocardial infarction (AMI) in the rat. Methods. AMI model was induced in female SD rats by ligating left coronary artery. Forty-eight hours after the procedure, 83 surviving rats were randomized into one of the following 4 groups: 1 ) AMI control group (n =19), 2) losartan group (n= 22, 3 mg @ kg - 1 @ d - 1 ), 3 ) enalapril group (n = 20, 1 mg @ kg - 1 @ d - 1 ), 4) losartan - enalapril combinative group (n = 22, 3 and 1 mg @ kg- 1 @ d - 1 respectively). 5 ) sham-operated group ( n =10) and 6) normal rats group (n = 10) were selected randomly to serve as non-infarction controls. Losartan and enalapril were delivered by direct gastric gavage. After 4 weeks of medical therapy, hemodynamic studies were performed in each group, then the rat hearts were fixed with 10% formalin and pathologic analysis on them was performed. Complete experimental data was obtained in 56 rats, comprising 1 ) AMI controls (n = 11 ), 2) losartan group (n = 10), 3 ) enalapril group (n = 10), 4) the combination of losartan and enalapril group (n = 11 ),5) sham - operated group (n = 6) and 6) normal controls (n=8). Results. There were no significant differences among the 4 AMI groups in MI size (41.7% ~ 43.4%, all P> 0.05). Compared with sham group, the left ventricular (LV) end diastolic pressure (LVEDP), volume (LVV), long and short axis length (L and D), as well as LV absolute and relative weight (LVAW and LVRW)in AMI group were all significantly increased ( P <0.05 ~ 0. 001 ); whereas the maximum left ventricular pressure rising and droping rates ( + dp/dt) and their corrected values by LV systolic pressure ( + dp/dt/LVSP)were significantly reduced (all P <0.001 ), indicating LVRM occurred and LV systolic and diastolic function impaired after AMI. Compared with AMI group , LVEDP, LVV, LVAW and LVRW were all significantly decreased (P <0.05~0.001 ); while + dp/dt/LVSP were significantly enhanced in all 3 treatment groups (P <0.05~0.001 ) except -dp/dt/LVSP in losartan group (P> 0. 05 ). There were no significant differences in the above indices among the 3 treatment groups (all P> 0.05). Conclusion. Both losartan and enalapril can prevent from LVRM after AMI in the rat and improve LV function with equivalent effects. There seems no additive effect when the 2 drugs are used in combination.展开更多

Based on the shear stress formula of circular shaft under pure torsion in elastic stage, the formula of torque in elastic stage and the definition of yield, it is obtained that the yielding stage of plastic metal shaft under pure torsion is only a surface phenomenon of torque-torsion angle relationship, and the distribution of shear stress is essentially different from that of tensile stress when yielding under uniaxial tension. The pure torsion platform-torsion angle and the shape of torque-torsion angle curve cannot change the distribution of shear stress on the shaft cross-section. The distribution of shear stress is still linear with the maximum shear stress ts. The complete plasticity model assumption is not in accordance with the actual situation of shaft under torsion. The experimental strength data of nine plastic metals are consistent with the calculated results of the new limiting strain energy strength theory (LSEST). The traditional yield stress formula for plastic shaft under torsion is reasonable. The shear stress formula based on the plane assumption in material mechanics is applicable for all loaded stages of torsion shaft.展开更多

A finite difference method over the boundary-fitted orthogonal coordinate system is util- ized to investigate numerically the fully developed steady flow of non-Newtonian yield viscoplastic fluid through concentric and eccentric annuli. The fluid rheology is described with the Herschel-Bulkley model. The numerical simulation based on a continuous viscoplastic approach to the Herschel-Bulkley model is found in poor accordance with the experimental data on volumetric flow rate of a bentonite suspension. A strict mathematical model for Herschel-Bulkley fluid flow is established and the corresponding numerical procedures are proposed. However, only the case of flow of a Herschel-Bulkley fluid in a concentric annulus is resolved based on the presumed flow stnicture by using the common optimization technique. Possible flow structures in an eccentric afinulus are presumed, and further challenges in numerical simulation of the Herschel-Bulkley fluid flow are suggested.展开更多

Among these, voltage-gated potassium(Kv) channels are particularly significant in cardiac electrophysiology, especially during the repolarization phase of the cardiac action potential. In cardiac myocytes, Kv channels are integral to a multitude of sophisticated functions, including electrical conduction. Despite their importance, research on Kv channels in the context of cardiovascular diseases is limited. This review offers a comprehensive summary of the structural complexities of Kv channels, delineating the regulatory mechanisms involved in channel gating, expression, and membrane localization. Additionally, we examine the role of different Kv α-subunits in modulating Kv channels and their impact on cardiac remodeling, and assess the potential of targeting Kv channels for the development of anti-arrhythmic therapies.展开更多
Huanglian Jiedu Decoction(HLJDD)is a quintessential prescription renowned for its heat-clearing and detoxifying properties.It is primarily prescribed to counteract the syndrome characterized by the excessive heat of t...Huanglian Jiedu Decoction(HLJDD)is a quintessential prescription renowned for its heat-clearing and detoxifying properties.It is primarily prescribed to counteract the syndrome characterized by the excessive heat of the Sanjiao fire.Notably,the hyperactivity of liver fire is frequently linked with hypertension,where wind fire and wind toxicity emerge as pivotal pathogenic factors.This study aimed to investigate the impact of HLJDD on the endoplasmic reticulum in spontaneously hypertensive rats(SHR),further delving into the interplay between endoplasmic reticulum stress(ERS)and myocardial remodeling and damage.Fifty SHR rats were stratified randomly into five cohorts:model,low-dose HLJDD,medium-dose HLJDD,high-dose HLJDD,and captopril groups.For comparison,a set of Wistar-Kyoto(WKY)rats served as the baseline control group,with each group comprising 10 rats.While the model and control groups received equivalent volumes of normal saline via gavage,the other groups were administered the respective drug dosages through the same route daily for a span of 6 weeks.Upon the experiment’s conclusion,metrics such as the heart mass index(HWI)and left ventricular mass index(LVWI)were assessed.Cardiac tissue anomalies were identified using H&E staining,while ERS-related protein and mRNA expression levels were ascertained via Western blotting analysis and qPCR.Moreover,TUNEL staining was employed to detect cardiomyocyte apoptosis.The findings indicated that increasing HLJDD concentrations corresponded with escalated HWI and LVWI in rat hearts(P<0.05).There was a marked enhancement in myocardial structural integrity,accompanied by a notable reduction in collagen fibers.The mRNA and protein expressions of myocardial inositol-dependent enzyme 1α(IRE1α),X-box binding protein 1(XBP1),glycoregulatory protein 78(GRP78),and CCAAT enhancer binding protein homologous protein(CHOP)in the medium and high-dose groups saw significant declines(P<0.05).These effects mirrored those observed in the captopril group.The study underscored HLJDD’s efficacy in mitigating myocardial tissue damage in SHR.This therapeutic effect was potentially attributed to the downregulation of IRE1α,XBP1,GRP78,and CHOP,curbing excessive ERS,diminishing cardiomyocyte apoptosis,and thereby conferring cardioprotection.展开更多

文摘odeling · mechanismObjective To investigate mechanisms of anti hypertension and anti cardiovascular remodeling by phenylalanine (phe) in spontaneously hypertensive rats (SHRs) Methods The comparison of blood pressure (BP) increment with the ages and cardiovascular changes of SHRs was made between the 3% phe intervented group (SHR phe) and the control SHRs group Detection of the structural changes with the VIDAS digital vedio frequency processing technique and light and electron microscopy were made The cell growth and proliferation of cultured smooth muscle cells (CSMCs) of the thoracic aortas or myocardial fibroblasts were evaluated by measuring the 3 H thymidine counts per minute (cpm) incorporated into the new synthesized desoxyribonucleic acid (DNA) and determining the cell number with the crystal violet stain technique The Ca 2+ influx was measured in counts/min of 45 CaCl 2 after incubating it with 5 different concentrations of phenylalanine and the intracellular [Ca 2+ ] i by Fura Ⅱ/Am indicator The total messenger ribonucleic acid (mRNA) of the myocardium was extracted and Northern blot analysis was performed with the probe collagen α 2(Ⅰ)cDNA The tyrosine hydroxylase (TH) activity was measured by high performance liquid chromatography (HPLC) with electrochemical detector after having reacted with its substrate tyrosine and other reagents The catecholamine contents in brain homogenat were detected by HPLC method The comparison of pharmacokinetics of phenylalanine among SHR phe, SHRs and control Wistar Kyoto (WKY) rats was made after intravenous injection of 3 H L phe (1?ml/kg) by PK GRAPH Program for kinetic calculation The 3H L phe uptake by CSMCs after incubating for difinite intervals was also detected and compared Results Phenylalanine could prevent the increase of BP with ages and the heart weight (heart/body weight index) The aortic media thickness and the collagen content in the myocardium were decreased significantly in SHR phe Whereas the dearranged cardiovascular structure was much improved The mechanisms might be direct and specific inhibition of the DNA synthesis and proliferation of cardiovascular cells which may be related to the inhibition of collagen α 2(Ⅰ)cDNA, c fos and c myc expression Other mechanisms may include decrease of intracellular [Ca 2+ ] i and an inhibition of central sympathetic activity due to the results of higher TH activity in the caudate nucleus and higher adrenaline content in the posterior hypothalamus Besides, partial recovery of phenylalanine metabolic aberrants existed in SHRs seems to be another possibility for its effectiveness Conclusions Phenylalanine intervention could exert a definite anti hypertension and anti cardiovascular remodeling effects on SHRs like seen in human essential hypertension Its mechanisms might be related to direct inhibition of growth in the cardiovascular cells, decrease of central sympathetic activity, the reverse of the exhibited phenylalanine metabolic aberrants in SHRs, and a decrement of intracellular [Ca 2+ ] i
