目的:观察急性心肌梗死患者外周血粘附分子CD11b和CD54及T淋巴细胞亚群CD4、CD8及CD4/CD8的变化。方法:流式细胞仪技术。结果:急性心肌梗死患者外周血CD11b+、CD54+细胞明显高于对照组。对照组CD11b+细胞(45.6±9.2)%;CD54+细胞(4.... 目的:观察急性心肌梗死患者外周血粘附分子CD11b和CD54及T淋巴细胞亚群CD4、CD8及CD4/CD8的变化。方法:流式细胞仪技术。结果:急性心肌梗死患者外周血CD11b+、CD54+细胞明显高于对照组。对照组CD11b+细胞(45.6±9.2)%;CD54+细胞(4.5±2.3)%。实验组CD11b+细胞(81.51±11.65)%;CD54+细胞(17.88±5.32)%,P<0.01。CD4+、CD8+细胞及CD4+/CD8+比值明显低于对照组。对照组CD4+细胞(49.3±1.1)%;CD8+细胞(24.2±1.95)%;CD4+/CD8+为(2.4±0.2)。实验组:CD4+细胞(31.5±6.2)%;CD8+细胞(22.62±5.7)%;CD4+/CD8+为(1.37±0.55),P<0.01。结论:急性心肌梗死患者存在某种程度的细胞免疫功能紊乱,表现为粘附分子表达增多,而T淋巴细胞亚群中CD4+、CD8+表达降低,CD4+、CD8+比值下降。 展开更多

Acute cardiomyocyte necrosis in the infarcted heart generates damage-associated molecular patterns, activating complement and toll-like receptor/interleukin-1 signaling, and triggering an intense inflammatory response. Iuflammasomes also recognize danger signals and mediate sterile inflammatory response following acute myocardial infarction (AMI), Inflammatory response serves to repair the heart, but excessive inflammation leads to adverse left ventricular remodeling and heart failure. In addition to local inflammation, profound systemic inflammation response has been documented in patients with AMI, which includes elevation of circulating inflammatory cytokines, chemokines and cell adhesion molecules, and activation of peripheral leukocytes and platelets. The excessive inflammatory response could be caused by a deregulated immune system. AMI is also associated with bone marrow activation and spleen monocytopoiesis, which sustains a continuous supply of monocytes at the site of inflammation. Accumulating evidence has shown that systemic inflammation aggravates atherosclerosis and markers for systemic inflammation are predictors of adverse clinical outcomes (such as death, recurrent myocardial in- farction, and heart failure) in patients with AMI. 展开更多
