Inflammatory bowel disease(IBD) is often associated with extraintestinal manifestations(EIMs) such as optic neuritis(ON),although this has been described in only a few adult patients so far,all of whom were affected w...Inflammatory bowel disease(IBD) is often associated with extraintestinal manifestations(EIMs) such as optic neuritis(ON),although this has been described in only a few adult patients so far,all of whom were affected with Crohn's disease(CD).Furthermore,ON and demyelinating diseases have been demonstrated to be more frequent in IBD patients than in control populations.In our current case report,we describe a child with active CD who developed sudden blindness due to bilateral ON that was not related to any known cause,and that promptly responded to a high dose of steroids.Investigations and a clinical follow-up have so far ruled out the development of demyelinating diseases in this patient.To our knowledge,this is the first report of ON in a pediatric patient with CD.Possible explanations for this case include an episodic EIM of an active bowel disease,an associated autoimmune disorder such as a recurrent isolated ON,the first manifestation of multiple sclerosis,or another demyelinating disease that could appear in a later follow-up.展开更多

Methods From Feb. 2006 to Mar. 2007, 60 patients presented to ou...Objective To evaluate the effectiveness of shock wave lithortripsy （SWL） for treating patients with calculus after failure of endoscopic lithortripsy. Methods From Feb. 2006 to Mar. 2007, 60 patients presented to our department with the upper urinary tract stones. Thirty-seven patients were renal stones and twentythree were ureteral stones. All patients with average stone burden of 1.5 cm were treated using a Dornier Compact S lithotripter. The total number of shock waves varied from 1 600 to 3 000 and the energy levels ranged from 1 to 6 units. Results For 37 renal stones, the fragmentation was 75. 67% （28cases） after one SWL session, 10.81% （4 cases） after2 SWL sessions, 5. 40% （2 cases） after 3 SWL sessions, and 2. 70% （1 case） after 4 SWL sessions. Two patients （5.40%） failed and changed to open surgery. For23 ureter calculus, the fragmentation was 82.60% （19cases） after one SWL session and l7. 40% （4cases） after 2 SWL sessions. Conclusion SWL is an effective and reliable treatment for patients with calculus after failure of endoscopic lithortripsy. However, it is very important for doctor to choose appropriate therapy. Clinically, this method is mainly used for some acute soft tissue injuries, for instance, needling point Yangchi (TE 4) with reducing method for sprain of external malleolus, needling Qiuxu (GB 40) with reducing method for sprain of dorsal carpus, and needling Tiaokou (ST 38) for lateral shoulder pain. The therapeutic effect given by this point selection method is often superior to that given by the conventional needling method. In clinical practice, the author has found some typical cases, which respond well to this method, and reports them below.展开更多
