头针电刺激合康复训练治疗小脑卒中共济失调临床研究 被引量:10

目的:观察头针电刺激联合康复训练治疗小脑卒中共济失调的疗效。方法:将140例小脑卒中共济失调患者按随机数字表法分为观察组与对照组各70例,对照组给予基本康复训练,观察组在对照组基础上加用头针电刺激,每天1次,10次1个疗程,共治疗3... 目的:观察头针电刺激联合康复训练治疗小脑卒中共济失调的疗效。方法:将140例小脑卒中共济失调患者按随机数字表法分为观察组与对照组各70例,对照组给予基本康复训练,观察组在对照组基础上加用头针电刺激,每天1次,10次1个疗程,共治疗3个疗程。治疗前后采用世界神经病联合会国际合作共济失调量表(ICARS)、Berg平衡量表(BBS)、Barthel指数(BI)及并发症进行评定。结果:2组治疗后ICARS、BBS与Barthel评分均明显优于治疗前,且观察组改善程度优于对照组;观察组有效率及减少并发症发生率方面均优于对照组。结论:头针电刺激联合康复训练治疗小脑卒中共济失调能明显改善共济失调症状,促进患者平衡能力与日常生活能力的提高,减少并发症的发生。 展开更多

目的观察头针电刺激不同时间点对急性脑梗死大鼠血清CD62P和CD63水平、神经功能缺损的调节作用并探讨其机理。方法将160只健康的雄性Wistar大鼠,随机分为头针电刺激组、普通针刺组、假手术组和模型对照组,各组又根据治疗的时间点不同分... 目的观察头针电刺激不同时间点对急性脑梗死大鼠血清CD62P和CD63水平、神经功能缺损的调节作用并探讨其机理。方法将160只健康的雄性Wistar大鼠,随机分为头针电刺激组、普通针刺组、假手术组和模型对照组,各组又根据治疗的时间点不同分为1 d组、3 d组、7 d组和14 d组4个亚组,每组10只大鼠。头针电刺激组、普通针刺组及模型对照组采用改良的线栓法制作右侧大脑中动脉闭塞局灶性脑缺血大鼠模型,4组大鼠在造模成功1 d起,每天同一时间进行干预,头针电刺激组给予大鼠头针电刺激干预,以疏密波进行电刺激治疗,普通针刺组给予大鼠普通头针干预,选取百会和大椎穴,假手术组及模型组大鼠于同一时间固定于针刺台,不做处理。于治疗1、3、7、14 d后,检测血清中CD62P、CD63的表达。各组大鼠造模结束清醒后及治疗前后均采用Bederson评分法评分。结果头针电刺激组较各组能显著降低CD62P、CD63的表达(P <0.01),头针电刺激组中各治疗时间点比较,血清中CD62P、CD63表达明显减少(P <0.01)。头针电刺激组、普通针刺组及模型对照组Bederson评分比较,3、7及14 d组差异具有统计学意义(P <0.05);头针电刺激组中各治疗时间点评分比较,随着时间增加,评分降低(P <0.05),且以14 d为最佳。结论通过比较各组大鼠血清CD62P、CD63表达及Bederson评分,结果均表明头针电刺激能降低CD62P、CD63表达,改善脑梗死大鼠神经功能,且随着时间延长,改善越明显,以14 d为最佳。 展开更多

研究分析头针电刺激结合康复训练治疗小脑卒中共济失调的临床疗效。方法:选取2017年3月-2020年3月间本院收治的小脑卒中患者72例,采用随机分组法将患者分为两组,对照组与实验组各36例,对照组采用康复训练的方式进行治疗,实验组采用头针... 研究分析头针电刺激结合康复训练治疗小脑卒中共济失调的临床疗效。方法:选取2017年3月-2020年3月间本院收治的小脑卒中患者72例,采用随机分组法将患者分为两组,对照组与实验组各36例,对照组采用康复训练的方式进行治疗,实验组采用头针电刺激结合康复训练的方式进行治疗,治疗前后采用共济失调量表(ICARS)、Berg平衡量表、Barthel生活质量量表对患者进行评定。结果:对比治疗后两组的ICARS、Berg、Barthel评分,发现实验组患者ICARS评分、Berg评分与Barthel量表评分均优于对照组,具有统计学意义(P<0.05)。结论:小脑卒中共济失调是共济失调的主要类型,针对该疾病采用头针电刺激结合康复训练治疗能够有效改善患者共济失调的症状,同时患者平衡能力、生活质量明显提高。 展开更多

目的观察头针电刺激对急性脑梗死大鼠神经胶质细胞及凋亡基因相关蛋白的影响。方法将大鼠分为假手术组、模型组和头针电刺激组,采用线栓法制备动物大脑中动脉栓塞模型,在造模成功24h后头针电刺激组进行电针治疗,刺激时长每日1次,每次30m... 目的观察头针电刺激对急性脑梗死大鼠神经胶质细胞及凋亡基因相关蛋白的影响。方法将大鼠分为假手术组、模型组和头针电刺激组,采用线栓法制备动物大脑中动脉栓塞模型,在造模成功24h后头针电刺激组进行电针治疗,刺激时长每日1次,每次30min,假手术组和模型组每天固定于针刺台上30min,不进行头针电刺激,持续14d。在实验结束时进行大鼠神经功能Bederson′s评分,处死大鼠后对脑组织进行TTC染色,计算梗死体积;检测脑组织中Bax、B细胞淋巴瘤2(Bcl-2)和半胱氨酸天冬氨酸蛋白酶(Caspase-3)蛋白的表达量;用TUNEL法检测神经胶质细胞凋亡情况。结果假手术组大鼠表现正常,未见梗死灶;与模型组相比,头针电刺激组梗死体积明显减小,神经功能Bederson′s评分降低,差异有统计学意义(P<0.05);与假手术组相比,模型组和头针电刺激组大鼠脑组织中细胞凋亡率、Bax和Caspase-3蛋白表达量均增加,Bcl-2蛋白表达量均降低,差异有统计学意义(P<0.05);与模型组比较,头针电刺激组大鼠脑组织中细胞凋亡率、Bax和Caspase-3蛋白表达量均降低,Bcl-2蛋白表达量增加,差异有统计学意义(P<0.05)。结论头针电刺激能减小脑梗死体积,改善急性脑梗死大鼠的神经功能,其机制可能与下调Bax和Caspase-3蛋白表达,上调Bcl-2蛋白表达,抑制神经胶质细胞凋亡有关。 展开更多

目的:观察头针电刺激对不同时间点脑梗死大鼠Bax与Bcl-2 mRNA的变化,探讨其对脑梗死可能作用机制。方法:采用线栓法制作大鼠局灶性脑梗死模型。将180只健康Wistar雌性大鼠,随机分为3组,头针电刺激组、模型对照组和假手术组。各组又依据... 目的:观察头针电刺激对不同时间点脑梗死大鼠Bax与Bcl-2 mRNA的变化,探讨其对脑梗死可能作用机制。方法:采用线栓法制作大鼠局灶性脑梗死模型。将180只健康Wistar雌性大鼠,随机分为3组,头针电刺激组、模型对照组和假手术组。各组又依据脑梗死后治疗时间点不同分为4个亚组,即1天组、3天组、7天组与14天组。3组大鼠造模结束24 h起,给予头针电刺激组大鼠针刺百会穴与大椎穴,并进行电刺激治疗,1次/日,30 min/次,余2组在每天相应时间固定大鼠于针刺台,不予任何处置。疗程治疗结束后,断头取脑。运用RT-PCR检测梗死脑组织Bax与Bcl-2 mRNA。3组大鼠在造模结束清醒后、每日治疗前后均要进行神经功能缺损程度评分,采用Zea-longa 5级评分标准。结果:头针电刺激组中7天与14天的亚组大鼠Zea-longa 5级评分明显低于同组1天与3天的亚组(P<0.05),14天的Zea-longa 5级评分与模型组比较,具有显著性差异(P<0.05);头针电刺激组经RTPCR检测后,Bax mRNA的表达逐渐降低,Bcl-2逐渐增加,且14天的亚组与模型组14天比较,有显著性差异(P<0.05),与假手术组14天比较则无显著性差异(P>0.05)。结论:头针电刺激能明显降低脑梗死大鼠的神经功能缺损程度评分,改善大鼠运动功能;头针电刺激能降低Bax mRNA的表达,提高Bcl-2 mRNA的表达,从而抑制缺血区细胞凋亡,保护脑组织,促进肢体康复的功效,其中以头针电刺激14天疗效最佳。 展开更多

Methods:A total of 300 CP cases a... Objective:To observe the clinical efficacy of Governor Vessel-unblocking and brain-refreshing scalp acupuncture for cerebral palsy (CP) complicated with intellectual disabilities. Methods:A total of 300 CP cases aged between 1 and 5 years were randomly allocated into a treatment group (n=150) and a control group (n=150). Patients in the treatment group were treated with Governor Vessel-unblocking and brain- refreshing scalp acupuncture combined with rehabilitation training, whereas patients in the control group received rehabilitation training alone. The Beijing Gesell developmental (Gesell) scale and gross motor function measure (GMFM) scale were used to assess the motor and intellectual development before and 3 months after the treatment. In addition, the head CT/MRI examination was applied to assess the brain nerve repair before and after the treatment. Results:After the treatment, the total effective rate in the treatment group was 78.0%, versus 42.7% in the control group, showing a statistical significance (P〈0.05). As for scores of five subscales in the Gesell scale, there were significant intra-group differences in the treatment group (allP〈0.05); and the scores in the treatment group were significantly better than those in the control group (allP〈0.05). As for scores of five dimensions in GMFM scale, there were significant intra-group differences in the treatment group (allP〈0.05); and the scores in the treatment group were significantly better than those in the control group (allP〈0.05). According to the head CT/MRI findings, the total effective rate was 73.3% in the treatment group, versus 62.0% in the control group, showing a statistical significance (P〈0.05). Conclusion:When used in combination with rehabilitative training, Governor Vessel-unblocking and brain-refreshing scalp acupuncture can significantly improve the clinical efficacy for cerebral palsy complicated with intellectual disabilities. Objective: To investigate the therapeutic effect of scalp multiple needle insertion by electric stimulation in the treatment of ischemic apoplexy, Methods: Scalp multiple needle insertion by electric stimulation was used to treat ischemic apoplexy and changes in rheoencephalogram, blood rheology, blood lipid and serum endothelin were observed. Results: The therapeutic effect was remarkable (P〈 0,05) in the observation group and there were obvious changes in rheoencephalogram, blood rheology, blood lipids and serum endothelin (P〈 0.05). Conclusion.- Scalp multiple needle insertion by electric stimulation has an exact effect in the treatment of ischemic apoplexy. Objective:To observe the clinical effect of regulating Conception Vessel and unblocking Governor Vessel by acupuncture combined with tuina for cerebral infarction. Methods:A total of 61 eligible cases with cerebral infarction were treated with regulating Conception Vessel and unblocking Governor Vessel by acupuncture combined with tuina, once a day, 12 d for a course. There was a 3-day interval between two courses. These cases were treated for 3 courses. Before treatment, the infarction severity was graded using neurological deficit score. The score changes and correlation between infarction severity and therapeutic efficacy were evaluated after 1, 2 and 3 courses of treatment. Results:Before treatment, the neurological deficit score was (18.65±3.28), versus (13.63±3.91), (9.53±3.22) and (5.57±3.97) respectively after 1, 2 and 3 courses of treatment. When the third course was completed, 4 cases obtained almost full recovery, 33 cases obtained marked effect, 18 cases obtained improvement and 6 cases had no effect. The total effective rate was 90.2%. Conclusion:Regulating Conception Vessel and unblocking Governor Vessel by acupuncture combined with tuina can substantially improve limb function in patients with hemiplegia due to cerebral infarction. 展开更多

Objective:To observe the clinical efficacy of acupuncture for vascular cognitive disorder following a cerebral infarction.Methods:A total of 90 cases were randomly allocated into an observation group and a control group.In addition to the cognitive training and the routine rehabilitation exercise for the paralyzed limb,special needling techniques were employed in the observation group,versus electroacupuncture in the control group.After a session of 4-week treatment,the outcomes were assessed and analyzed usingmini-mental state examination (MMSE),clock drawing test (CDT),delayed story recall (DSR) and functional ability questionnaire (FAQ).Results:There were statistical significances (P0.05) in the scores between before and after treatment in the observation group.Also,there were statistical significances (P0.05) between the observation group and the control group after the treatment.Conclusion:The special needling method in this study could obtain better therapeutic efficacy than electroacupuncture for vascular cognitive disorder. 展开更多

Objective:To observe and compare the therapeutic effect of two different needling methods,liver-soothing and mind-regulating needling method versus conventional needling method,in treating anxiety and depression in tension-type headache.Methods:A total of 120 patients with tension-type headache coupled with anxiety and/or depression were randomized into two groups according to the random number table,a liver-soothing andmind-regulating needling group (group A) and a conventional needling group (group B),60 patients in each group.The self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were used for evaluation before and after treatment.The therapeutic effects of the two groups were compared after 2-treatment courses.Results:The values of SAS and SDS both significantly dropped in the two groups (P0.01),and the differences between the two groups after treatment were statistically significant (P0.01,P0.05).The therapeutic effect in group A was superior to that in group B (P0.01).The 3-month follow-up study showed that the liver-soothing andmind-regulating needling method had better long-term effect in improving anxiety and depression than the conventional needling method (P0.05).Conclusion:The liver-soothing andmind-regulating needling method can effectively relieve the anxiety and depression symptoms in patients with tension-type headache,and the long-term effect is also satisfactory. 展开更多
