直立位金属钛夹夹闭方式对结直肠息肉EMR后迟发性出血的影响及相关因素分析 被引量:1

目的分析金属钛夹夹闭方式对结直肠息肉内镜黏膜切除术(EMR)后迟发性出血的影响以及相关影响因素。方法回顾性选取2020年3月至2022年6月安徽医科大学第一附属医院收治的60例行结直肠息肉切除术患者作为研究对象,根据金属钛夹夹闭方式不... 目的分析金属钛夹夹闭方式对结直肠息肉内镜黏膜切除术(EMR)后迟发性出血的影响以及相关影响因素。方法回顾性选取2020年3月至2022年6月安徽医科大学第一附属医院收治的60例行结直肠息肉切除术患者作为研究对象,根据金属钛夹夹闭方式不同将患者分为直立位组(n=31)和平行位组(n=29);根据患者术后有无迟发性出血分为出血组(n=10)和未出血组(n=50)。比较直立位组与平行位组患者的围手术期一般资料(手术时间、肠鸣音恢复时间、术中出血量、疼痛评分)、治疗成功率与并发症发生情况。比较出血组和未出血组患者一般临床资料(年龄、性别、合并高血压、合并高血脂、合并糖尿病、息肉数量、息肉形态、息肉分布和切除数量及大小、金属钛夹夹闭方式)。采用Logistic多因素回归分析结直肠息肉患者发生迟发性出血的影响因素。结果两组患者手术时间、肠鸣音恢复时间、视觉模拟评分法(VAS)评分比较,差异均无统计学意义(P>0.05);直立位组患者术中出血量为(49.69±6.57)mL,低于平行位组[(58.41±6.85)mL],差异有统计学意义(P<0.05)。两组患者治疗成功率、肠穿孔、腹痛发生率比较,差异均无统计学意义(P>0.05);直立位组患者迟发性出血率为6.45%,明显低于平行位组(27.59%),差异有统计学意义(P<0.05)。出血组患者合并高血压、合并高血脂、有蒂息肉、多肠段分布、平行位的患者比例分别为70.00%、60.00%、50.00%、50.00%、80.00%,均高于未出血组(34.00%、26.00%、12.00%、12.00%、42.00%),出血组患者切除数量、最大基底直径、平均基底直径分别为(1.92±0.85)枚、(0.97±0.36)cm、(0.82±0.32)cm,均高于未出血组[(1.32±0.76)枚、(0.69±0.30)cm、(0.68±0.28)cm],差异均有统计学意义(P<0.05)。Logistic多因素回归分析结果显示,术中出血量、合并高血压、合并高血脂、切除数量、最大基底直径是结直肠息肉患者发生迟发性出血的危险因素(P<0.05),直立位金属钛夹夹闭方式是结直肠息肉患者发生迟发性出血的保护因素(P<0.05)。结论与平行位金属钛夹夹闭方式相比,直立位金属钛夹夹闭方式用于结直肠息肉EMR术中出血量更少,迟发性出血发生率更低。术中出血量、合并高血压、合并高血脂、切除数量、最大基底直径是结直肠息肉患者发生迟发性出血的危险因素,直立位金属钛夹夹闭方式是结直肠息肉患者发生迟发性出血的保护因素。 展开更多

输卵管银夹异位31例临床分析

In the hydrogen network with the minimum hydrogen utility flow rate,the pinch appears at the point with zero hydrogen surplus,while the hydrogen surpluses of all the other points are positive.In the hydrogen purity profiles,the pinch can only lie at the sink-tie-line intersecting the source purity profile.According to the alternative distribution of the negative and positive regions,the effect of the purification to the hydrogen surplus is analyzed.The results show that when the purification is applied,the pinch point will appear neither above the purification feed nor between the initial pinch point and the purification feed,no matter the purification feed lies above or below the initial pinch point.This is validated by two case studies. 展开更多

Modeling for Orientation Deviation of Workpiece and Analysis of Machining Accuracy

The machining accuracy of workpiece is influenced by its orientation deviation, which is caused by the fixture-workpiece error. Based on the spatial coordinate theory, the orientation deviation of workpiece is measured by using an on-machine verification system, which can take into account the errors resulting from fixture manufacturing, installation and adjustment, location and clamping of workpiece. According to the least square method, the model of orientation deviation is built to determine the relationship between the theoretical and actual coordinate systems. The influence of orientation deviation on machining accuracy is quantified, and it is shown that the orientation deviation only affects the dimensional precision and position precision, rather than shape precision. In the experiment, the compensation processing of realtime errors was conducted, and the perpendicularity and inclination errors of the tetragonal part were reduced by 38.46% and 47.06%, respectively. 展开更多

Objective To observe the clinical efficacy of warm needling moxibustion plus lumbar traction for lumbar disc herniation(LDH)and its effect on Oswestry disability index(ODI)score,present pain intensity(PPI)score,and the degree of straight leg raising.Methods A total of 146 patients with LDH were randomized into a combination group and a control group by the random number table method,with 73 cases in each group.The control group was treated with lumbar traction,and the combination group was treated with lumbar traction plus warm needling moxibustion.The curative efficacy of the two groups was compared.Also,the lumbar function,pain,straight leg raising degree,and inflammatory factor levels before and after treatment were compared.Results The total effective rate of the combination group was 93.2%,which was higher than 78.1%of the control group(P<0.05).After treatment,the lumbar ODI score,the PPI scores of lumbar pain and lower-limb pain,tumor necrosis factor(TNF)-αand interleukin(IL)-6 levels,and straight leg raising test score in both groups were significantly decreased(P<0.05).The Japanese Orthopaedic Association(JOA)score significantly increased(P<0.05).The improvements of the above items in the combination group were all superior to those in the control group(P<0.05).Conclusion Warm needling moxibustion plus lumbar traction in the treatment of LDH can improve lumbar function,relieve pain,and reduce inflammatory response of the body. 展开更多

Investigation on the Pinch Point Position in Heat Exchangers 被引量:2

The pinch point is important for analyzing heat transfer in thermodynamic cycles. With the aim to reveal the importance of determining the accurate pinch point, the research on the pinch point position is carried out by theoretical method. The results show that the pinch point position depends on the parameters of the heat transfer fluids and the major fluid properties. In most cases, the pinch point locates at the bubble point for the evaporator and the dew point for the condenser. However, the pinch point shills to the supercooled liquid state in the near critical conditions for the evaporator. Similarly, it shifts to the superheated vapor state with the condensing temperature approaching the critical temperature for the condenser. It even can shift to the working fluid entrance of the evaporator or the supereritical heater when the heat source fluid temperature is very high compared with the absorb- ing heat temperature. A wrong position for the pinch point may generate serious mistake. In brief, the pinch point should be founded by the itcrativc method in all conditions rather than taking for granted. 展开更多
