Setting: Hospital research unit. Patient(s): Eight patients with PCOS and eight fertile women of similar age to those with PCOS. In-tervention(s): Endometrial samples were obtained from women with PCOS (PCOSE) and normal (NE)- women during the midsecretory phase of the menstrual cycle. Main Outcome Measure(s): Expression studies (immunohistoch-emistry, reverse transcription-polymerase chain reaction [RT- PCR] and Western blot). Result(s): Endometria from PCOS exhibit higher levels of messenger RNA (mRNA) and protein for estrogen receptor α and coactivators than NE. Epithelial cells had a greater expression of progesterone receptor in PCOSE, whereas, no differences were observed in gene and protein expression of the nuclear corepressor (NcoR)- and the antiadhesion molecule mucin type- 1 (MUC- 1) between PCOSE and NE. Immunodetection for the coactivator ARA70 was higher in PCOSE than in NE; in contrast, expression of β 3- integrin in epithelia was lower in PCOSE than in control endometria. Conclusion(s): The higher response to steroid hormones of endometria from untreated PCOS-women induces diminished expression of β 3 integrin, which partially explain implantation failure in PCOS patients.展开更多

Multicenter clinical trial of the usefulness of endometrial biopsy in the evaluation of infertility, with women from fertile and infertile couples randomly assigned to midluteal vs. late luteal phase endometrial sampling. Twelve clinical sites of the National Institutes of Health/National Institute of Child Health and Human Developmentsponsored Reproductive Medicine Network. All women in the study had regular menstrual cycles. Fertile volunteers who had delivered a live born infant within the past 2 years without medical intervention were recruited through advertisements at participating sites. Infertile women with regular cycles were recruited from the clinical practices of the sitesphysicians. Interview, informed consent, subjectinterpreted home urine LH testing, and endometrial biopsy in either the midluteal or late luteal phase. Proportion of cases with proliferative endometrium on biopsy. In both fertile and infertile women, more than 7%of endometrial biopsies performed 7-13 days after a positive home urine LH test revealed proliferative endometrium. Nucleolar channel system (NCS) appeared in the endometrial epithelial cells during the early and mid secretory phase of menstrual cycle. The NCS was a hollow ball like structure of different sizes and was composed of 2 to 5 rows of tubules embedded in an amorphous matrix. On its surface there were numerous electron dense particles resembling ribosonies. It was usually located within or associated with the nucleolus. Sometimes, it was close to the nuclear envelope or protruding out from the nucleus. On occasion, NCS with simplified structure was found in the perinuclear cytoplasm. Concepts concerning the genesis, involution and function(s) of the NCS were discussed.展开更多
