Several studies have shown how depression tends to exacerbate coexisting chronic heart failure and its clinical outco...Chronic heart failure and depressive disorders have a high prevalence and incidence in the elderly. Several studies have shown how depression tends to exacerbate coexisting chronic heart failure and its clinical outcomes and vice versa, especially in the elderly. The negative synergism between chronic heart failure and depression in the elderly may be approached only taking into account the multifaceted pathophysiological characteristics underlying both these conditions, such as behavioural factors, neurohormonal activation, inflammatory mediators, hypercoagulability and vascular damage. Nevertheless, the pathophysiological link between these two conditions is not well established yet. Despite the high prevalence of depression in chronic heart failure elderly patients and its negative prognostic value, it is often unrecognized especially because of shared symptoms. So the screening of mood disorders, using reliable questionnaires, is recommended in elderly patients with chronic heart failure, even if cannot substitute a diagnostic interview by mental health professionals. In this setting, treatment of depression requires a multidisciplinary approach including: psychotherapy, antidepressants, exercise training and electroconvulsive therapy. Pharmacological therapy with selective serotonin reuptake inhibitors, despite conflicting results, improves quality of life but does not guarantee better outcomes. Exercise training is effective in improving quality of life and prognosis but at the same time cardiac rehabilitation services are vastly underutilized.展开更多

This paper reports a case study of a distinctive variant of adenoma of the NPCE, which clinically appears as epiretinal membrane in the macular region. Histopathologic studies have revealed this is an adenoma of the NPCE. Identification of this clinic feature is important because it will miss the diagnosis of the adenoma of the NPCE. In this case study, B-scan ultrasonography as well as computerized tomography (CT) has been used to provide help in diagnosing the ciliary body tumor. Because of their anterior location in the ciliary body, partial lamellar sclerou-vectomy is an effective method of treatment.展开更多

Peat soil minerals from three peats and swamps across G...We explored the potential use of combining wavelength-dispersive X-ray spectroscopy(WDX) and micromorphology of thin sections to identify minerals in peat soils. Peat soil minerals from three peats and swamps across Golestan Province in northern Iran were first characterized by micromorphological studies. Soils were composed mainly of quartz, muscovite, biotite, pyroxene,sericitized Fe-nodules, and iron-rich garnet. In addition,micromorphological results indicated that Galougah Coastal Swamp sections contained some inorganic residue with biological origin including oyster and limpet, which may be related to the swamp's location near Gorgan Gulf.In order to determine mineralogical properties of samples,twelve unknown grains were chosen for elemental concentration map studies. Quartz, garnet, ilmenite, calcite,and pyroxene in Suteh samples; epidote and Fe-nodule in Ghaleh-Ghafeh Peat Swamp; and barite, phyllosilicates,and calcite in Galougah were identified by WDX mapping of Si, Al, Fe, Ca, Mg, C, Ba, S, and Ti. Composition of the oysters' body was also analyzed by WDX for Si, Ca, Fe,and C. The results indicated that most of the minerals in all sections likely formed through weathering, inheriting their composition from the parent rock. This research suggests that merging micromorphology and SEM/WDX image techniques can be useful in confirming the presence of mineral particles in soil science.展开更多

AIM:To investigate the patterns and decisive prognostic factors for local recurrence of rectal cancer treated with a multidisciplinary team(MDT) modality.METHODS:Ninety patients with local recurrence were studied,out ...AIM:To investigate the patterns and decisive prognostic factors for local recurrence of rectal cancer treated with a multidisciplinary team(MDT) modality.METHODS:Ninety patients with local recurrence were studied,out of 1079 consecutive rectal cancer patients who underwent curative surgery from 1999 to 2007.For each patient,the recurrence pattern was assessed by specialist radiologists from the MDT using imaging,and the treatment strategy was decided after discussion by the MDT.The associations between clinicopathological factors and long-term outcomes were evaluated using both univariate and multivariate analysis.RESULTS:The recurrence pattern was classified as follows:Twenty-seven(30%) recurrent tumors were evaluated as axial type,21(23.3%) were anterior type,8(8.9%) were posterior type,and 13(25.6%) were lateral type.Forty-one patients had tumors that were evaluated as resectable by the MDT and ultimately received surgery,and R0 resection was achieved in 36(87.8%) of these patients.The recurrence pattern was closely associated with resectability and R0 resection rate(P < 0.001).The recurrence pattern,interval to recurrence,and R0 resection were significantly associated with 5-year survival rate in univariate analysis.Multivariate analysis showed that the R0 resection was the unique independent factor affecting long-term survival.CONCLUSION:The MDT modality improves patient selection for surgery by enabling accurate classification of the recurrence pattern;R0 resection is the most significant factor affecting long-term survival.展开更多

METHODS: From January 2001 to December 2006, 487 patients with histologically confi...AIM: To determine the prognostic value of lymphatic and/or blood vessel invasion (LBVI) in patients with stage 11 gastric cancer. METHODS: From January 2001 to December 2006, 487 patients with histologically confirmed primary gas- tric adenocarcinoma were diagnosed with stage 11 gas- tric cancer according to the new 7th edition American Joint Committee on Cancer stage classification at the Department of Gastric Cancer and Soft Tissue Surgery, Fudan University Shanghai Cancer Center. All patients underwent curative gastrectomy with standard lymph node (LN) dissection. Fifty-one patients who died in the postoperative period, due to various complications or other conditions, were excluded. Clinicopathologicalfindings and clinical outcomes were analyzed. Patients were subdivided into four groups according to the status of LBVI and LN metastases. These four patient groups were characterized with regard to age, sex, tumor site, pT category, tumor grading and surgical procedure (subtotal resection vs total resection), and compared for 5-year overall survival by univariate and multivariate analysis. RESULTS: The study was composed of 320 men and 116 women aged 58.9 ± 11.5 years (range: 23-88 years). The 5-year overall survival rates were 50.7% and the median survival time was 62 too. Stage Ⅱ a cancer was observed in 334 patients, including 268 T3N0, 63 T2N1, and three TIN2, and stage Ⅱb was observed in 102 patients, including 49 patients T3N1, 51 T2N2, one TIN3, and one T4aN0. The incidence of LBVI was 28.0% in stage II gastric cancer with 19.0% (51/269) and 42.5% (71/167) in LN-negative and LN- positive patients, respectively. In 218 patients (50.0%), there was neither a histopathologically detectable LBVI nor LN metastases (LBVI-/LN-, group I); in 51 patients (11.7%), LBVI with no evidence of LN me- tastases was detected (LBVILN-, group 11). In 167 patients (38.3%), LN metastases were found. Among those patients, LBVI was not determined in 96 patients (22.0%) (LBVI-γLN, group Ⅲ), and was determined in 71 patients (16.3%) (LBVI+LN+, group Ⅳ). Correla- tion analysis showed that N category and the number of positive LNs were significantly associated with the presence of LBVI (P 〈 0.001). The overall 5-year sur- vival was significantly longer in LN-negative patients compared with LN-positive patients (56.1% vs 42.3%, P = 0.015). There was a significant difference in the overall 5-year survival between LBVI-positive and LBVI- negative tumors (39.6% vs 54.8%, P = 0.006). Overall 5-year survival rates in each group were 58.8% ( Ⅰ), 45.8% (Ⅱ), 45.7% (Ⅲ) and 36.9% (Ⅳ), and there was a significant difference in overall survival between the four groups (P=-0.009). Multivariate analysis in stage 11 gastric cancer patients revealed that LBVI in- dependently affected patient prognosis in LN-negativepatients (P = 0.018) but not in LN-positive patients (P = 0.508). CONCLUSION: In LN-negative stage 11 gastric cancer patients, LBVI is an additional independent prognostic markeF, and may provide useful information to identify patients with poorer prognosis.展开更多
In order to investigate the transformation among the precipitation,groundwater,and surface water in the Sanjiang Plain,Northeast China,precipitation and groundwater samples which were collected at the meteorological s...In order to investigate the transformation among the precipitation,groundwater,and surface water in the Sanjiang Plain,Northeast China,precipitation and groundwater samples which were collected at the meteorological station of the Sanjiang Mire Wetland Experimental Station,Chinese Academy of Sciences and the surface water which collected from the Wolulan River were used to identify the transformation of three types of water.The isotope composition of different kinds of water sources were analyzed via stable isotope(deuterium and oxygen-18) investigation of natural water.The results show a clear seasonal difference in the stable isotopes in precipitation.During the cold half-year,the mean stable isotope in precipitation in the Sanjiang Plain reaches its minimum with the minimum temperature.The δ18O and δD values are high in the rainy season.In the Wolulan River,the evaporation is the highest in August and September.The volume of evaporation and the replenishment to the river is mostly same.The groundwater is recharged more by the direct infiltration of precipitation than by the river flow.The results of this study indicate that the water bodies in the Sanjiang Plain have close hydrologic relationships,and that the transformation among each water system frequently occurs.展开更多

Analysts point to increasingly automated manufacturing processes, changing domestic demand factors, outsourcing, and over confidence in service export surplnses. However, Asian and European competitors, undergoing the same pressures, have been able to maintain a healthy percentage of their GDP in manufacturing by growing high techjobs while the United States continues to fall behind, Perhaps the most flmdamental and overlooked contribution to this decline is the lackluster performance of the United States education system. After comparing the performance of US students to that of the other G-8 nations in the critical disciplines of math and science literacy, US students rank last. Furthermore, the most recent PISA test results show US students rank in the bottom half of 30 participating nations in the ability to apply math and science concepts to real world problems. This is particularly troubling when one considers that among the compared nations, the US spends the most on math and science teacher compensation. If the US intends to successfully compete in the global manufacturing arena, significant changes to the education system must be enacted. A new strategy aligning the system with real world demands should begin with a national initiative to increase Pre-K enrollments, a paradigm shift from liberal arts to math, science, problem solving and critical thinking, standardized policies aligning high school graduation requirements with college and work place expectations, and standardized college and career readiness assessment programs. These steps combined with more rigorous secondary education teacher certifications, continued education and involvement with universities and manufacturing f'mns within the community, should more adeq^tely prepare high school students for further study or to enter the skilled workforce. At the post secondary level, federal and state funding should be targeted to R&D programs specificly to industrial design, engineering, and alternative energy. Industry investment in such programs should be federally incentivized to foster cooperative relationships between business and academia. Such relationships will ensure faculty spend time focusing on how their discipline relates to manufacturing and instilling the necessary skill sets, knowledge and abilities which graduates will need to compete in the global market. Such a revised system will position the US to create and keep high paying manufacturing positions on its way to sustainable economic growth.展开更多

Objective: The aim of the study was to investigate the level of self-image of patients after breast cancer surgery, and explore factors influencing self-image among patients who have experienced different types of sur...Objective: The aim of the study was to investigate the level of self-image of patients after breast cancer surgery, and explore factors influencing self-image among patients who have experienced different types of surgeries, and set out a foundation for the improvement of care strategies. Methods: The 538 patients with primary breast cancer who underwent surgery in the Department of Breast Surgery, First Hospital of China Medical University, Shenyang, China, from January 2004 to January 2009 were included in the study. The psychological status of the patients was evaluated by body image after breast cancer questionnaire (BIBCQ), social support rating scale (SSRS), self-rating anxiety scale (SRAS), depression rating scale (DRS), and general information questionnaire. The factors influencing the self-image were selected by a stepwise regression analysis. Results: The patients who underwent breast-conserving surgery were the most satisfied with their body image, followed by those underwent surgery of modified radical mastectomy with reconstruction. However, cases treated by modified radical mastectomy without reconstruction had negative outcomes. Regardless of operation type, the self-image was influenced by anxiety, level of abuse by husband, and sexual satisfaction after operation. Conclusion: The self-image of patients who underwent different breast cancer surgeries was influenced by different factors, and individualized nursing should be offered in accordance with the specific situation.展开更多

In this paper I argue that physical literacy includes not only knowledge for performance but also the ability to apply knowledge and use knowledge for innovation. Scholars since the 1930s have addressed the role of knowledge in physical literacy designing curricula centered on transmitting knowledge through a range of interdisciplinary approaches to physical education. This emphasis on physical literacy curricula continues today in the Science, PE, & Me.t and The Science of Healthful Living interdisciplinary curricula.展开更多
AIM To report adalimumab(Ada) efficacy on articulargastrointestinal disease and health-related quality of life(HRQo L) in patients with enteropathic spondyloarthritis(ES).METHODS A cohort of 52 patients with ES was ev...AIM To report adalimumab(Ada) efficacy on articulargastrointestinal disease and health-related quality of life(HRQo L) in patients with enteropathic spondyloarthritis(ES).METHODS A cohort of 52 patients with ES was evaluated in the departments of gastroenterology and internal medicine. At baseline, all patients underwent assessment by an integrated gastro-rheumatologic evaluation of articular and gastrointestinal activity, as well patient reported outcomes(PROs) of the HRQo L questionnaires. After this integrated evaluation and following a specific working flowchart, the Ada anti-tumor necrosis factor(TNF)-inhibitor was assigned to a cohort of 30 patients and its clinical efficacy was evaluated at baseline and after 6-mo and 12-mo treatment by the following tests:(1) Ankylosing Spondylitis Disease Activity ScoreC-Reactive Protein(ASDAS-CRP); Bath Ankylosing Spondylitis Disease Activity Index(BASDAI), Bath Ankylosing Spondylitis Functional Index(BASFI) and Bath Ankylosing Spondylitis Metrology Index(BASMI) for articular activity;(2) Inflammatory Bowel Disease Questionnaire(IBDQ), Crohn's Disease Activity Index(CDAI) and partial Mayo(p Mayo) score for gastrointestinal symptoms and activity; and(3) Health Assessment Questionnaire(HAQ), Patient Global Assessment(PGA) and Short Form-36 health survey(SF-36) questionnaires for PROs of the HRQo L.RESULTS Integrated evaluation and management of the patients affected by ES, carried out simultaneously by a gastroenterologist and a rheumatologist, allowed clinicians to choose the optimal therapeutic strategy. In a cohort of 30 ES patients affected by active articular and gastrointestinal disease, or axial active articular inflammation, Ada led to fast and sustained improvement of both articular and gastrointestinal disease activities. In fact, all the clinimetric evaluation tests exploring articular or gastrointestinal activity, as well as all the HRQo L scores, showed a significant improvement having been achieved at the earliest(6-mo) assessment. This important clinical improvement was maintained at the 12-mo follow-up. Importantly, global and gastrointestinal quality of life significantly correlated with articular disease activity, providing evidence to support that the integrated evaluation is the best option to manage patients with ES.CONCLUSION Ada treatment, upon multidisciplinary(gastrorheumatologic) evaluation, significantly improves both articular and gastrointestinal inflammation, thereby improving the HRQo L in patients affected by ES.展开更多

文摘AIM:To investigate the patterns and decisive prognostic factors for local recurrence of rectal cancer treated with a multidisciplinary team(MDT) modality.METHODS:Ninety patients with local recurrence were studied,out of 1079 consecutive rectal cancer patients who underwent curative surgery from 1999 to 2007.For each patient,the recurrence pattern was assessed by specialist radiologists from the MDT using imaging,and the treatment strategy was decided after discussion by the MDT.The associations between clinicopathological factors and long-term outcomes were evaluated using both univariate and multivariate analysis.RESULTS:The recurrence pattern was classified as follows:Twenty-seven(30%) recurrent tumors were evaluated as axial type,21(23.3%) were anterior type,8(8.9%) were posterior type,and 13(25.6%) were lateral type.Forty-one patients had tumors that were evaluated as resectable by the MDT and ultimately received surgery,and R0 resection was achieved in 36(87.8%) of these patients.The recurrence pattern was closely associated with resectability and R0 resection rate(P < 0.001).The recurrence pattern,interval to recurrence,and R0 resection were significantly associated with 5-year survival rate in univariate analysis.Multivariate analysis showed that the R0 resection was the unique independent factor affecting long-term survival.CONCLUSION:The MDT modality improves patient selection for surgery by enabling accurate classification of the recurrence pattern;R0 resection is the most significant factor affecting long-term survival.
文摘This article discusses the physical principles to be considered in a thermodynamic engineering solar system running under operational control algorithms designed to minimize entropy rates. The proposed plant is based on scientific foundations established by thermodynamics and classical field theory, and empirical disciplines that deal with properties of chemical elements and with technologies of calorific capture from remote sources. The gauge principle was applied in theoretical formalization as an essential tool in the control background of the entropy rates involved. The industrial system presented was planned to recycle condensed matter within the scope of a solid waste management policy.
文摘This paper outlines briefly the role of nuclear medicine in life sciences and health care. Molecular imaging by using isotopic tracers can noninvasively visualize the chemistry or hidden process in the cells and tissues inside the body, obtaining "functional" images to provide early information of any disease and revealing the secrets of life. The vitality of nuclear medicine is its ability to translate bench into new clinical application that can benefits the patients. Although nuclear medicine community in China has made significant achievement with a great effort since 1950s, there are many obstacles to future development. Recommended measures are proposed here in an attempt to solve our existing problems.
文摘AIM: To determine the prognostic value of lymphatic and/or blood vessel invasion (LBVI) in patients with stage 11 gastric cancer. METHODS: From January 2001 to December 2006, 487 patients with histologically confirmed primary gas- tric adenocarcinoma were diagnosed with stage 11 gas- tric cancer according to the new 7th edition American Joint Committee on Cancer stage classification at the Department of Gastric Cancer and Soft Tissue Surgery, Fudan University Shanghai Cancer Center. All patients underwent curative gastrectomy with standard lymph node (LN) dissection. Fifty-one patients who died in the postoperative period, due to various complications or other conditions, were excluded. Clinicopathologicalfindings and clinical outcomes were analyzed. Patients were subdivided into four groups according to the status of LBVI and LN metastases. These four patient groups were characterized with regard to age, sex, tumor site, pT category, tumor grading and surgical procedure (subtotal resection vs total resection), and compared for 5-year overall survival by univariate and multivariate analysis. RESULTS: The study was composed of 320 men and 116 women aged 58.9 ± 11.5 years (range: 23-88 years). The 5-year overall survival rates were 50.7% and the median survival time was 62 too. Stage Ⅱ a cancer was observed in 334 patients, including 268 T3N0, 63 T2N1, and three TIN2, and stage Ⅱb was observed in 102 patients, including 49 patients T3N1, 51 T2N2, one TIN3, and one T4aN0. The incidence of LBVI was 28.0% in stage II gastric cancer with 19.0% (51/269) and 42.5% (71/167) in LN-negative and LN- positive patients, respectively. In 218 patients (50.0%), there was neither a histopathologically detectable LBVI nor LN metastases (LBVI-/LN-, group I); in 51 patients (11.7%), LBVI with no evidence of LN me- tastases was detected (LBVILN-, group 11). In 167 patients (38.3%), LN metastases were found. Among those patients, LBVI was not determined in 96 patients (22.0%) (LBVI-γLN, group Ⅲ), and was determined in 71 patients (16.3%) (LBVI+LN+, group Ⅳ). Correla- tion analysis showed that N category and the number of positive LNs were significantly associated with the presence of LBVI (P 〈 0.001). The overall 5-year sur- vival was significantly longer in LN-negative patients compared with LN-positive patients (56.1% vs 42.3%, P = 0.015). There was a significant difference in the overall 5-year survival between LBVI-positive and LBVI- negative tumors (39.6% vs 54.8%, P = 0.006). Overall 5-year survival rates in each group were 58.8% ( Ⅰ), 45.8% (Ⅱ), 45.7% (Ⅲ) and 36.9% (Ⅳ), and there was a significant difference in overall survival between the four groups (P=-0.009). Multivariate analysis in stage 11 gastric cancer patients revealed that LBVI in- dependently affected patient prognosis in LN-negativepatients (P = 0.018) but not in LN-positive patients (P = 0.508). CONCLUSION: In LN-negative stage 11 gastric cancer patients, LBVI is an additional independent prognostic markeF, and may provide useful information to identify patients with poorer prognosis.

文摘In order to investigate the transformation among the precipitation,groundwater,and surface water in the Sanjiang Plain,Northeast China,precipitation and groundwater samples which were collected at the meteorological station of the Sanjiang Mire Wetland Experimental Station,Chinese Academy of Sciences and the surface water which collected from the Wolulan River were used to identify the transformation of three types of water.The isotope composition of different kinds of water sources were analyzed via stable isotope(deuterium and oxygen-18) investigation of natural water.The results show a clear seasonal difference in the stable isotopes in precipitation.During the cold half-year,the mean stable isotope in precipitation in the Sanjiang Plain reaches its minimum with the minimum temperature.The δ18O and δD values are high in the rainy season.In the Wolulan River,the evaporation is the highest in August and September.The volume of evaporation and the replenishment to the river is mostly same.The groundwater is recharged more by the direct infiltration of precipitation than by the river flow.The results of this study indicate that the water bodies in the Sanjiang Plain have close hydrologic relationships,and that the transformation among each water system frequently occurs.
文摘For decades, the United States' competitive position in global manufacturing has steadily declined. Analysts point to increasingly automated manufacturing processes, changing domestic demand factors, outsourcing, and over confidence in service export surplnses. However, Asian and European competitors, undergoing the same pressures, have been able to maintain a healthy percentage of their GDP in manufacturing by growing high techjobs while the United States continues to fall behind, Perhaps the most flmdamental and overlooked contribution to this decline is the lackluster performance of the United States education system. After comparing the performance of US students to that of the other G-8 nations in the critical disciplines of math and science literacy, US students rank last. Furthermore, the most recent PISA test results show US students rank in the bottom half of 30 participating nations in the ability to apply math and science concepts to real world problems. This is particularly troubling when one considers that among the compared nations, the US spends the most on math and science teacher compensation. If the US intends to successfully compete in the global manufacturing arena, significant changes to the education system must be enacted. A new strategy aligning the system with real world demands should begin with a national initiative to increase Pre-K enrollments, a paradigm shift from liberal arts to math, science, problem solving and critical thinking, standardized policies aligning high school graduation requirements with college and work place expectations, and standardized college and career readiness assessment programs. These steps combined with more rigorous secondary education teacher certifications, continued education and involvement with universities and manufacturing f'mns within the community, should more adeq^tely prepare high school students for further study or to enter the skilled workforce. At the post secondary level, federal and state funding should be targeted to R&D programs specificly to industrial design, engineering, and alternative energy. Industry investment in such programs should be federally incentivized to foster cooperative relationships between business and academia. Such relationships will ensure faculty spend time focusing on how their discipline relates to manufacturing and instilling the necessary skill sets, knowledge and abilities which graduates will need to compete in the global market. Such a revised system will position the US to create and keep high paying manufacturing positions on its way to sustainable economic growth.

文摘Objective: The aim of the study was to investigate the level of self-image of patients after breast cancer surgery, and explore factors influencing self-image among patients who have experienced different types of surgeries, and set out a foundation for the improvement of care strategies. Methods: The 538 patients with primary breast cancer who underwent surgery in the Department of Breast Surgery, First Hospital of China Medical University, Shenyang, China, from January 2004 to January 2009 were included in the study. The psychological status of the patients was evaluated by body image after breast cancer questionnaire (BIBCQ), social support rating scale (SSRS), self-rating anxiety scale (SRAS), depression rating scale (DRS), and general information questionnaire. The factors influencing the self-image were selected by a stepwise regression analysis. Results: The patients who underwent breast-conserving surgery were the most satisfied with their body image, followed by those underwent surgery of modified radical mastectomy with reconstruction. However, cases treated by modified radical mastectomy without reconstruction had negative outcomes. Regardless of operation type, the self-image was influenced by anxiety, level of abuse by husband, and sexual satisfaction after operation. Conclusion: The self-image of patients who underwent different breast cancer surgeries was influenced by different factors, and individualized nursing should be offered in accordance with the specific situation.

文摘A promising bacterial strain for the biodegradation of Microcystins(MCs)was isolated from Dianchi lake in China and identified as Sphingopyxis sp.USTB-05 by the analysis of 16s rDNA.Initial MC-RR of 42.3 mg·L -1 was completely degraded by USTB-05 within 36 h,which was a relatively high biodegradation rate of MC-RR.With the cell-free extract(CE)of Sphingopyxis sp.USTB-05,MC-RR was biodegraded at a more rapid biodegradation rate compared with its strain,so that initial MC-RR of 42.3 mg·L -1 was completely biodegraded within 10 h.During the bio-reaction of MC-RR catalyzed by CE,two intermediate metabolites and a dead-end product of MC-RR were observed on HPLC profiles and all of them had similar scanning profiles in the wavelength from 200 to 300 nm,indicating that the group of Adda in all products of MC-RR remained intact.
文摘Literate individuals possess knowledge and skill and can apply these to perform tasks in novel settings. Knowledge is at the heart of physical literacy and provides the foundation for knowing what to do and how and when to perform. In this paper I argue that physical literacy includes not only knowledge for performance but also the ability to apply knowledge and use knowledge for innovation. Scholars since the 1930s have addressed the role of knowledge in physical literacy designing curricula centered on transmitting knowledge through a range of interdisciplinary approaches to physical education. This emphasis on physical literacy curricula continues today in the Science, PE, & Me.t and The Science of Healthful Living interdisciplinary curricula.
文摘AIM To report adalimumab(Ada) efficacy on articulargastrointestinal disease and health-related quality of life(HRQo L) in patients with enteropathic spondyloarthritis(ES).METHODS A cohort of 52 patients with ES was evaluated in the departments of gastroenterology and internal medicine. At baseline, all patients underwent assessment by an integrated gastro-rheumatologic evaluation of articular and gastrointestinal activity, as well patient reported outcomes(PROs) of the HRQo L questionnaires. After this integrated evaluation and following a specific working flowchart, the Ada anti-tumor necrosis factor(TNF)-inhibitor was assigned to a cohort of 30 patients and its clinical efficacy was evaluated at baseline and after 6-mo and 12-mo treatment by the following tests:(1) Ankylosing Spondylitis Disease Activity ScoreC-Reactive Protein(ASDAS-CRP); Bath Ankylosing Spondylitis Disease Activity Index(BASDAI), Bath Ankylosing Spondylitis Functional Index(BASFI) and Bath Ankylosing Spondylitis Metrology Index(BASMI) for articular activity;(2) Inflammatory Bowel Disease Questionnaire(IBDQ), Crohn's Disease Activity Index(CDAI) and partial Mayo(p Mayo) score for gastrointestinal symptoms and activity; and(3) Health Assessment Questionnaire(HAQ), Patient Global Assessment(PGA) and Short Form-36 health survey(SF-36) questionnaires for PROs of the HRQo L.RESULTS Integrated evaluation and management of the patients affected by ES, carried out simultaneously by a gastroenterologist and a rheumatologist, allowed clinicians to choose the optimal therapeutic strategy. In a cohort of 30 ES patients affected by active articular and gastrointestinal disease, or axial active articular inflammation, Ada led to fast and sustained improvement of both articular and gastrointestinal disease activities. In fact, all the clinimetric evaluation tests exploring articular or gastrointestinal activity, as well as all the HRQo L scores, showed a significant improvement having been achieved at the earliest(6-mo) assessment. This important clinical improvement was maintained at the 12-mo follow-up. Importantly, global and gastrointestinal quality of life significantly correlated with articular disease activity, providing evidence to support that the integrated evaluation is the best option to manage patients with ES.CONCLUSION Ada treatment, upon multidisciplinary(gastrorheumatologic) evaluation, significantly improves both articular and gastrointestinal inflammation, thereby improving the HRQo L in patients affected by ES.
