腹部微创手术安全入路分析研究

腹腔镜手术与传统手术相比,具有微创、损伤小、术后恢复快、腹壁瘢痕小等优点,越来越多地被广大患者接受。但是,微创手术入路整体损伤及并发症不容忽视。预防措施是采取正确的气腹针穿刺方法,熟悉解剖方位和控制穿刺力度。

目的:探讨腰椎间盘突出症相关侵入类治疗体位(俯卧位)下的后侧安全入路。方法:选择成年腰椎间盘突出症患者22例,用3.0T MRI对其腰椎(L_1-S_1)进行扫描,分别测量双侧上位腰椎下关节突外缘和下位腰椎上关节突外缘至后正中线的水平间距、... 目的:探讨腰椎间盘突出症相关侵入类治疗体位(俯卧位)下的后侧安全入路。方法:选择成年腰椎间盘突出症患者22例,用3.0T MRI对其腰椎(L_1-S_1)进行扫描,分别测量双侧上位腰椎下关节突外缘和下位腰椎上关节突外缘至后正中线的水平间距、双侧腰椎关节突关节后缘至腰背侧皮肤的最近距离,统计其差异。结果:侵入类治疗体表入路安全区间为距离后正中线:L_(1-2)为10.83~17.5 mm、L_(2-3)为13.03~19.17 mm、L_(3-4)为15.72~21.20 mm、L_(4-5)为19.47~25.55 mm、L_5-S_1为24.69~29.04 mm;侵入类治疗体内活动安全区间:L_(1-2)为7.03~7.93 mm、L_(2-3)为6.80~8.10 mm、L_(3-4)为6.57~7.66 mm、L_(4-5)为7.63~8.63 mm、L_5-S_1为6.13~7.27 mm,侵入类治疗的体内进针深度安全区间:L_(1-2)为37.19~39.85 mm、L_(2-3)为38.64~41.45 mm、L_(3-4)为40.23~43.60 mm、L_(4-5)为43.07~46.57 mm、L_5-S_1为45.24~49.00mm。结论:侵入类治疗的进针点、体内活动范围、进针深度,以平行矢状面和垂直冠状面进针较为安全,且应结合腰椎MRI,确保治疗的个体性、精确性、安全性。 展开更多

膝关节微创安全入路治疗后交叉韧带胫骨止点撕脱骨折疗效分析 被引量:5

目的 探讨采用膝关节微创安全入路治疗后交叉韧带(posterior cruciate ligament,PCL)胫骨止点撕脱骨折的疗效。方法 回顾分析2019年2月—2022年3月采用膝关节微创安全入路治疗的26例PCL胫骨止点撕脱骨折患者临床资料。男18例,女8例;年龄... 目的 探讨采用膝关节微创安全入路治疗后交叉韧带(posterior cruciate ligament,PCL)胫骨止点撕脱骨折的疗效。方法 回顾分析2019年2月—2022年3月采用膝关节微创安全入路治疗的26例PCL胫骨止点撕脱骨折患者临床资料。男18例,女8例;年龄33~58岁,平均45.5岁。致伤原因:交通事故伤14例,高处坠落伤7例,运动伤5例。左膝15例,右膝11例。术前膝关节Lysholm评分为(34.4±7.3)分,膝关节屈曲活动度为(69±12)°。骨折按Meyers分型:Ⅱ型8例,Ⅲ型18例。受伤至手术时间1~5 d,平均2 d。结果 手术时间40~70 min,平均55 min;术中出血量10~30 mL,平均15 mL。1例术后出现切口延迟愈合,经保守治疗后切口愈合,其余患者切口均Ⅰ期愈合。术后X线片示骨折复位满意。26例患者均获随访,随访时间3~30个月,平均22个月。术后均未发生神经血管损伤、感染及膝关节伸直障碍等并发症。术后3个月X线片示所有患者骨折均愈合,骨折端无移位;后抽屉试验均为阴性。末次随访时膝关节屈曲活动度为(120±9)°,膝关节Lysholm评分为(90.7±3.8)分,与术前比较差异均有统计学意义(t=16.376,P<0.001;t=47.665,P<0.001)。结论 膝关节微创安全入路治疗PCL胫骨止点撕脱骨折操作简便,手术创伤小,手术入路安全可靠,术后膝关节功能恢复满意。 展开更多

The security and performance are critical to DSPS. The authors suggested a key management scheme by combining the server group structure to improve availability and the key distribution structure needed by proactive security. This paper detailed the implementation of proactive security in DSPS. After thorough performance analysis, the authors concluded that the performance difference between the replicated mechanism and proactive mechanism becomes smaller and smaller with increasing number of concurrent connections; and that proactive security is very useful and practical for large, critical applications. 展开更多

Transvenous embolization of dural arteriovenous fistula of cavernous region by multiple venous routes

Objective:To evaluate the safety and efficiency of transvenous embolization of dural arteriovenous fistula of cavernous region by multiple venous routes. Methods: Twenty seven patients with dural arteriovenous fistula of cavernous region were treated by transvenous embolization with micro-coils. The transvenous routes included inferior petrosal sinus, superior ophthalmic vein and facial vein. Results: Clinical cure was achieved in 23 cases and significant improvement of symptoms in 4 cases. Complete anglographic obliteration was documented in 22 patients (82%). Residual shunting were left in 2 patients via pterygoid drainage and 1 case via inter-cavernous sinus, 2 cases via inferior petrosal sinus, disappeared one month later by manual compression carotid artery. Headache and vomiting were the most common symptoms after embolization. Three patients had diplopia and relieved within two months after embolization. There was no permanent procedure-related morbidity. The clinic follow up ranged from 5 months to 6 years, and there was not recurrence case. Conclusion: Transvenous embolization via different venous routes is a safe and efficient method for dural arteriovenous fistula of cavernous region treatment. 展开更多

According to the SRAM-based FPGA's single event effect problem in space application,single event upset induced multi-block error(SEU-MBE) phenomenon and its mitigation strategy are studied in the paper.After analyzing the place and route result,the paper points out that the essence of SEU-MBE is that some important modules exceed the safe internal distance.Two approaches,area constraint method(ACM) and incremental route algorithm(IRA),are proposed,which can reduce the error rate by manipulating programmable switch matrix and interconnection points within FPGA route resource.Fault injection experiments indicate that error detection rate is above 98.6% for both strategies,and FPGA resources increment and performance penalty are around 10%. 展开更多
