Objective:The aim of this study was to assess the long-term effect of breast conservation with breast intensitymodulated radiotherapy (IMRT) for early-stage breast cancers.Methods:Pub Med,EMBASE,Cochrane Library,Chinese Biomedical Literature Database were searched to identify relevant original published trails,and the references of eligible studies were manually screened.Randomized controlled trails reported in any language,comparing breast IMRT with conventional radiotherapy (RT) in patients with early-stage breast cancer were eligible for inclusion.Two investigators independently assessed the quality of included trials and extracted data.The RevMan 5.0 software was used for statistical analysis.Results:Four trials involving 1440 patients were included.The results showed that in the IMRT group,the occurrence of acute moist dermatitis,the edema of the mammary gland and the occurrence of hyperpigmentation were lower than those of RT group,the pooled risk ratio value and 95% confidence interval were 0.28 (0.17-0.48),0.03 (0.00-0.21) and 0.06 (0.02-0.15) respectively.The use of IMRT did not correlate with pain and quality of life,but the presence of moist desquamation did significantly correlate with pain and reduced quality of life.Conclusion:The currently available evidence showed that IMRT significantly reduces the occurrence of moist acute dermatitis anywhere in the breast and alleviates the suffering of patients and improves the quality of life of patients.Future large-scale,high-quality,double-blind trials are needed. 展开更多

A medium-temperature waste-heat recovery system based on the organic Rankine cycle (ORC) is designed to recover the exhaust energy from a heavy-duty diesel engine. Analysis of the 1st law of thermodynamics for an ORC system is performed. This analysis contains two parts. The first part is an analysis with undefined heat exchangers to gain an understanding of the ORC and find out suitable organic fluid parameters for a better ORC efficiency. The second part of the analysis uses combined engine test results and two designs of heat exchangers. By comparing the two designs, an improved system of heat exchangers is described. This analysis also quantifies the effect of engine parameters on ORC system. The study concludes that the supercritical Rankine cycle is a better approach towards waste heat recovery. The ORC system is found to perform better under part-load conditions if the medium-high power condition rather than rated working point of the engine is used as the design parameter. The ORC system achieves the highest waste-heat recovery efficiency of up to 10-15% for the optimised heat ex-changer design. 展开更多

We consider the statistical inference for right-censored data when censoring indicators are missing but nonignorable, and propose an adjusted imputation product-limit estimator. The proposed estimator is shown to be consistent and converges to a Gaussian process. Furthermore, we develop an empirical processbased testing method to check the MAR (missing at random) mechanism, and establish asymptotic properties for the proposed test statistic. To determine the critical value of the test, a consistent model-based bootstrap method is suggested. We conduct simulation studies to evaluate the numerical performance of the proposed method and compare it with existing methods. We also analyze a real data set from a breast cancer study for an illustration. 展开更多

Objective: Paraplegia due to traumatic spinal cord injuries is one of the devastating effects of dorsolumbar vertebral fractures. Treatment modalities for such fractures, such as stabilization, have no effect on the neurological recovery. Thus, various pharmacological and biological treatment modalities have been used. The more recent trend of using autologous stem cells from the iliac crest has been used in some clinical trials with varying success. Thus, more clinical studies are required to study the effect of this novel approach Methods: This is a prospective hospital-based cohort study (level IV). The study was conducted in the Dept. of Orthopaedics, University College of Medical Sciences and GTB Hospital, Delhi from November 2010 to March 2012. Ten patients who had sustained traumatic dorsolumbar vertebral fractures with complete paraplegia were recruited for this study. Under suitable anaesthesia, at the beginning of surgery, 100 ml of bone marrow was aspirated. This was centrifuged and buffy coat isolated and then transferred into a sterile tube and sent to the operating room on ice packs. After surgical decompression and stabilization, the buffy coat isolate was injected into the dural sleeve at the site of the injury using a 21G needle. All the patients were evaluated for neurological improvement using the American Spinal Injury Association (ASIA) score and Frankel grade at 6 weeks and 3 months postoperatively. Results: The evaluation at 6 weeks showed some improvement in terms of the ASIA scores in 2 patients but no improvements in their Frankel Grade. The other 8 patients showed no improvements in their ASIA scores or their Frankel Grades. The current pilot study has shown that there has been no improvement in most of the recipients of the transplant (n=8). Some patients (n=2) who did show some improvement in their sensory scores proved to be of no significant functional value as depicted by no change in their Frankel Grades. Conclusion: The outcome of current study shows that although this modality of treatment is safe for the patients, it provides no additional benefits on improvement of quality of life among these patients. 展开更多

OBJECTIVE:To explore the most effective parameters of Yi Zhi Chan Tuina manipulation for improving peripheral blood circulation.METHODS:A total of 45 volunteers were recruited from Pudong district in Shanghai,China,from October to December 2010,and randomly divided into nine groups using computer-generated random numbers.Participants received Yi Zhi Chan Tuina manipulation on Chengjin(BL 56) acupoint;each group received a particular combination of manipulation force and treatment time.We used a two-factor,three-level factorial design to examine the effects of force and treatment time on changes in popliteal artery average volume flow,pulsatility index,and vessel diameter to determine the optimal parameter group.Outcomes were assessed at baseline and after Tuina manipulation by interviewers blind to treatment group status.RESULTS:After manipulation,two of the nine groups showed an increase in popliteal artery volume flow.An inter-participants effect test showed that for main effect of time,F = 0.331,P = 0.720;for main effect of force,F = 2.934,P = 0.066;and for the force-time interaction effect,F = 1.072,P = 0.385,indicating no interaction between force and time.However,a pairwise comparison of the three levels of time showed that a treatment time of 10 min was significantly more effective than that of 2 min(P = 0.024).A pairwise comparison of light force,medium force,and heavy force showed a statistically significant effect for medium force(P = 0.035).CONCLUSION:Yi Zhi Chan Tuina manipulation with vertical force of 9.31 N for 10 min is most effective in improving peripheral circulation. 展开更多

Effect of losing a fight on later agonistic behavior toward unfamiliar conspecifics in male Syrian hamsters

In many species, agonistic interactions result in social relationships that are stable over time. In Syrian hamsters, two unfamiliar males that are placed together will fight vigorously and a clear winner/loser relationship is usually established. In subsequent interactions, the loser will flee soon after detecting the familiar winner. Here we tested the hypothesis that losing a fight with a conspecific will affect future agonistic interactions not only toward that individual (i.e., the familiar winner) but also toward unfamiliar conspecifics. To test this hypothesis we paired two Syrian hamster males in three trials on one day in which the loser had tile opportunity to escape the winner. The next day the loser was paired with an unfarniliar male, also for three trials. If he lost again, he was tested on a third day with a third unfamiliar male. Subjects were those males that were losers on all three days. The latency to escape on the first trial on Days 2 and 3 was significantly shorter than on the first trial on Day l, indicating that losing against the first male affected the response toward unfamiliar males. However, the latency to escape on the first trial on Days 2 and 3 was significantly longer than that on the third trial on the preceding day, indicating that a loser treats unfamiliar males differently than a familiar winner. These results suggest that a defeat during an interaction with one male affects later agonistic behavior towards other, unfamiliar males [Current Zoology 57 (4): 449-452, 2011]. 展开更多

A novel method based on machine learning is developed to estimate event-related potentials from single trial electroencephalography. This paper builds a basic framework using classification and an optimization model based on this framework for estimating event-related potentials. Then the SingleTrialEM algorithm is derived by introducing a logistic regression model, which could be obtained by training before SingleTrialEM is used, to instantiate the optimization model. The simulation tests demonstrate that the proposed method is correct and solid. The advantage of this method is verified by the comparison between this method and the Woody filter in simulation tests. Also, the cognitive test results are consistent with the conclusions of cognitive science. 展开更多
