Implications of small-bowel transit time in the detection rate of capsule endoscopy: A multivariable multicenter study of patients with obscure gastrointestinal bleeding 被引量:10

AIMTo define the role of small-bowel transit time in the detection rate of significant small-bowel lesions.METHODSSmall-bowel capsule endoscopy records, prospectively collected from 30 participating centers in the Lombardy Registry from October 2011 to December 2013, were included in the study if the clinical indication was obscure gastrointestinal bleeding and the capsule reached the cecum. Based on capsule findings, we created two groups: P2 (significant findings) and P0-1 (normal/negligible findings). Groups were compared for age, gender, small-bowel transit time, type of instrument, modality of capsule performance (outpatients vs inpatients), bowel cleanliness, and center volume.RESULTSWe retrieved and scrutinized 1,433 out of 2,295 capsule endoscopy records (62.4%) fulfilling the inclusion criteria. Patients were 67 &#x000b1; 15 years old, and 815 (57%) were males. In comparison with patients in the P0-1 group, those in the P2 group (n = 776, 54%) were older (P &#x0003c; 0.0001), had a longer small-bowel transit time (P = 0.0015), and were more frequently examined in low-volume centers (P &#x0003c; 0.001). Age and small-bowel transit time were correlated (P &#x0003c; 0.001), with age as the sole independent predictor on multivariable analysis. Findings of the P2 group were artero-venous malformations (54.5%), inflammatory (23.6%) and protruding (10.4%) lesions, and luminal blood (11.5%).CONCLUSIONIn this selected, prospectively collected cohort of small-bowel capsule endoscopy performed for obscure gastrointestinal bleeding, a longer small-bowel transit time was associated with a higher detection rate of significant lesions, along with age and a low center volume, with age serving as an independent predictor. 展开更多

AIM To explore the relationship between colonic secretory function and colonic motility.METHODS Using a rat model chronically implanted with intracerebroventricular(ICV) and cecal catheters, we validated the correlation between colonic secretion and colonic motor functions, as well as the role of ICV injection volume.RESULTS Compared to saline controls(5 μL/rat), ICV acyl ghrelin at 1 nmol/5 μL enhanced the total fecal weight, accelerated the colonic transit time, and increased the fecal pellet output during the first hour post-injection, while ICV des-acyl ghrelin at 1 nmol/5 μL only accelerated the colonic transit time. These stimulatory effects on colonic motility and/or secretion from acyl ghrelin and des-acyl ghrelin disappeared when the ICV injection volume increased to 10 μL compared with saline controls(10 μL/rat). Additionally, the ICV injection of 10 μL of saline significantly shortened the colonic transit time compared with the ICV injection of 5 μL of saline. The total fecal weight during the first hour post-injection correlated with the colonic transit time and fecal pellet output after the ICV injection of acyl ghrelin(1 nmol/5 μL), whereas the total fecal weight during the first hour post-injection correlated with the fecal pellet output but not the colonic transit time after the ICV injection of des-acyl ghrelin(1 nmol/5 μL).CONCLUSION Colonic secretion does not always correlate with colonic motility in response to different colonic stimulations. The objective of this work was to determine the effect of transport time (up to 11 hours) on animal welfare. 540 animals (cows, bulls and calves), three transport times of 2, 4 to 6, and 10 to 11 hours, and two space allowances (2 m2 and 1.5 m2 per animal for cows and bulls; and 1.2 m2 and 0.8 m2 per animal for calves) were used for the experiment during transport from farms to the abattoir in Uppsala, Sweden. Measurements were made on five animals on each trip. Stress response parameters considered were cortisol, glucose, lactate, creatine kinase and heart rate. Blood samples were taken before and after transport. Heart rate sensors were mounted on the animals at least 30 minutes before loading and measurements were made continuously from farms to the abattoirs until stunning. The results of heart rate measurement indicated that loading, un-loading and forced movement in the stunning box were the most stressful events. However, the results of statistical analysis confirmed that transport time doesn't have significant effect (P 〈 0.05) on heart rate. Concentration level of cortisol increased by 10 folds during short transport. However, cortisol concentration decreased with an increase of transport times (P 〈 0.01). Glucose concentrations increased with transport time in all animal categories (P ≤ 0.01). Transport time has significant effect on concentration levels of glucose (P ≤ 0.01), creatine kinase (P 〈 0.001) and lactate (P 〈 0.01) particularly after 6 hours journey time. 展开更多

In some of these mystery or detective stories especially featuring the well-known sleuth Sherlock Holmes, Doyle tended to integrate an early subject's experience of shrunken space and reduced time into an unknown fear by delineating his characters who perceive horror and nervousness while facing or riding on a railway transportation, including mainly the steam railway in mysterious tales like "The Lost Special" and "The Man with the Watches" as well as in detective stories like "The Adventure of the Engineer's Thumb", "The Adventure of Bruce-Partington Plan", "Valley of Fear" and several others. How can this spatiotemporal mobility be connected to mysterious affairs which lead Doyle's quasi-detective characters and police power to spring into investigative action? MicroRNA (miRNA) alterations in response to changes in an extracellular microenvironment have been observed and consid- ered as one of the major mechanisms for epigenetic modifications of the cell. While enormous efforts have been made in the understanding of the role of miRNAs in regulating cellular responses to the microenvironment, the mechanistic insight into how extracellular signals can be transduced into miRNA alterations in cells is still lacking. Interestingly, recent studies have shown that ion channels/transporters, which are known to conduct or transport ions across the cell membrane, also exhibit changes in levels of expression and activities in response to changes of extraceUular microenvironment. More importantly, al- terations in expression and function of ion channels/transporters have been shown to result in changes in miRNAs that are known to change in response to alteration of the microenvironment. In this review, we aim to summarize the recent data demonstrating the ability of ion channels/transporters to transduce extracellular signals into miRNA changes and propose a potential link between cells and their microenvironment through ion channels/transporters. At the same time, we hope to pro- vide new insights into epigenetic regulatory mechanisms underlying a number of physiological and pathological processes, in- eluding embryo development and cancer metastasis. 展开更多

Transportation problem on network needs to determine the freight quantity and the transportation route between supply point and demand point. Therefore, taken the uncertainty of freight supply and demand into account, a collaborative optimization model is formulated with transportation capacity constraint. In addition, a two-stage genetic algorithm (GA) is put forward. Herein, the first stage of this GA is adopted a priority-based encoding method for determining the supply and demand relationship between different points. Then supply and demand relationship which the supply and the demand are both greater than zero is a minimum cost flow (MCF) problem on network in the second stage. Aim at the purpose to solve MCF problem, a GA is employed. Moreover, this algorithm is suitable for balance and unbalance transportation on directed network or undirected network. At last, the model and algorithm are verified to be efficient by a numerical example. 展开更多
