METHODSOne hundred thirty-nine newly diagnosed cirrhotic patients without variceal bleeding were included in this analysis. Model for end-stage liver disease (MELD), aspartate aminotransferase (AST) to alanine aminotransferase (ALT) ratio (AST/ALT), AST to platelet ratio index (APRI), platelet count to spleen diameter (PC/SD), fibrosis-4-index (FIB-4), fibrosis index (FI) and King’s Score were measured for all participants. All patients underwent endoscopic assessment within two days of hospitalization. The major end point was the first esophageal variceal bleeding (EVB) event. The diagnostic performance of “predictors” for the presence of EV and EVB were assessed by sensitivity and specificity values obtained from the receiver operating characteristics procedure. RESULTSFIB-4 was the only strong and significant “predictor” of esophageal varices (multivariable-adjusted OR = 1.57 for one unit increment; 95%CI: 1.15-2.14). Furthermore, a cut-off value of 3.23 for FIB-4 was a significant predictor of esophageal varices, with a sensitivity of 72%, a specificity of 58% and a proportion of area under the curve (AUC) of 66% (P = 0.01). During the follow-up (median: 31.5 mo; interquartile range: 11-59 mo), 34 patients (24%) experienced a first EVB. FIB-4 was a poor predictor of EVB (the AUC was only 51%) for a cut-off value of 5.02. Furthermore, the AUC of AST/ALT, APRI, PC/SD, FI, MELD and King’s Score ranged from 45% to 55%. None of the non-invasive markers turned out to be a useful predictor of EVB. CONCLUSIONDespite the low diagnostic accuracy, FIB-4 appears the most efficient non-invasive liver fibrosis marker which can be used as an initial screening tool for cirrhotic patients.展开更多

METHODS: Blood samples from 410 unselected schoolboys, 666 students, 500 military personnel, 1286 casual blood donors, 378 voluntary blood donors and 640 pregnant women (total 3880 non-vaccinated residents of rural and metropolitan areas from all over Albania; 2354 (60.7%) male and 1526 (39.3%) female; mean age of 26.3 years) were tested during 2004-2006 for hepatitis B surface antigen (HBsAg) and antibodies to hepatitis B virus (anti-HBs) by ELISA. RESULTS: The HBsAg and anti-HBs prevalence were 9.5% and 28.7%, respectively. The highest HI3sAg prevalence was evident in the younger age group, such as in schoolchildren (11.8%) and the military (10.6%). Consequently, the anti-HBs prevalence increased with age, from 21.2% in schoolchildren (mean age: 15.7 years), to 36.3% in pregnant women (mean age: 26.3 years) and 29.7% in voluntary blood donors (mean age: 40.1 years). There were no significant differences between males and females. CONCLUSION: Despite the estimated two-fold reduction of HBsAg prevalence in the general population from about 18%-19% to 9.5%, Albania remains a highly endemic country (i.e. over 8% of HBsAg prevalence rate).展开更多

In the last decade we notice that banks pay a growing attention to their marketing services and in particular to the use of the latest technologies for their promotions. The use of the lnternet has influenced in the fact that customers now are closer to their banks, their number has increased, and of course through the Internet we have a more efficient exchange of products and services. A questionnaire was developed to help us understand the internet banking acceptance in Albania and other issues related to it. The questionnaire survey was conducted on 9 banks operating in Albania and from the evaluation of the collected data results that all banks that underwent questionnaire, have a separate marketing department. So marketing has an important role in the banking business. The importance of marketing rose by increased interbank competition in recent years. Crisis and economic and financial difficulties have shown that banks, which face these difficult periods, have been those which had not only a healthy bank policy, but also a strong commercial policy. These things depend on many factors, among which we can mention: the tradition, which has taught people to trust "their financial secrets" only to someone who exercises the profession of banker; the cultural and infrastructural level of development of the population, which gives approval and insures the continuousness of any discovery or development in various fields; the ability of this kind of banking to be successful, that is and remains the basis on which to establish a trustful cooperation relationship of any kind. Strong policies to e-marketing have targeted to increase the percentage of elements such as banking and credit granting. E-banking also consists of counseling online with a bank employee or financial advisor. This kind of service is not only effective for the client, who saves his time, but also more economical for the bank, which reduces its costs in human resources (an adviser to the window only deals with a client, while he can handle online more than two credit cases simultaneously, so we decrease the number of employees in general). With its low-cost structure, direct consumer marketing and innovative services, the internet is going to present a real challenge to traditional forms of banking. E-banking is seen from the banks as an opportunity to create competitive advantage over its competitors. It offers a great deal of advantages both to the banks and to the customers. Clients are becoming more and more demanding and they are expecting even higher levels of service from their banks.展开更多

Rrogozhina aquifer represents an important groundwater source of the country whose relevance was highly increased during recent years because of the demographic expansion in this region. This is a typically multi-layered artesian aquifer which consists of sandstone and conglomerate with high heterogeneous permeability. The groundwater mostly belongs to HCO3-Mg-Ca hydrochemical type. The content of soluble iron and manganese in groundwater ranges from 0.004 mg/L to 0.7 mg/L, and from 0.0 mg/L to 1.147 mg/L, respectively. Deeper the groundwater occurs and longer the distance from the recharge zone, higher is the content of Fe2+ in groundwater due to gradual depletion of Oe in groundwater. The content of iron is higher than that WHO and EU recommended, which were 21% and 25% respectively, while the content of manganese were 17% and 23%, respectively. Abstraction of groundwater with high quantities by deeper wells would be accompanied by increase of dissolved iron and manganese content in groundwater which should be treated (with aeration) in order to decrease the content of these two elements in the water before using it.展开更多

Human activities can release substances into the air, some of which can cause problems for humans, plants and animals. There are several main types of pollution and well-known effects of pollution which are commonly discussed. These include smog, acid rain, the greenhouse effect, and "holes" in the ozone layer. Each of these problems has serious implications for our health and well-being as well as for the whole environment. The age and health condition are more important also at the response opposite the pollution of air. During the last decade in our country such as result of freely population movement, development and growth of their industrial activities, growth of vehicle number, collapse of roadway infrastructure etc., an increase of air quality pollution is evident. In this study, we exhibited the causers, such as increasing of number of the old cars, which is an important factor of the air pollution and, at the same time, we exhibited the pollution indicators of the air quality, such as PM I 0, total dust, dust levels outdoors and indoors, which are in the high levels. These factors indicated on human health. We have observed the health indicators, especially lung diseases such as bronchial asthma with higher percentage, chronic bronchitis and lung cancer. There was consistency in the finding that relates the acute increases in urban air pollution and the short-term health effects or patients suffering from chronic obstructive pulmonary diseases.展开更多
The paper sheds light on the factors enabling sustainable development of Medicinal and Aromatic Plants (MAPs) of Albania; socio-economic, legal and regulatory aspects and institutional capacity related to the MAP se...The paper sheds light on the factors enabling sustainable development of Medicinal and Aromatic Plants (MAPs) of Albania; socio-economic, legal and regulatory aspects and institutional capacity related to the MAP sector in the country; sector dynamics, challenges and opportunities; and Albania's positioning in the world markets. Stewardship of MAPs natural base in the country remains a challenge as approximately 95% of the MAPs are collected from the wild. Albania continues to be the leader in Sage exports to the US market accounting for 49% of its market share. Statistical data and direct communication with MAP relevant stakeholders reveal that there is a great demand for the Albanian MAPs from the EU and US markets. However, as clearly indicated in this paper, increased competitiveness of this sector in the world markets should be preceded by sustainable management of the MAPs which can be achieved through (1) continuous training and education of the MAP value chain members prior to harvesting season, (2) organic production and (3) commercial cultivation of MAPs. This paper highlights that sustainable management and sector development require combined efforts of all sector stakeholders in an attempt of all respecting Mother Nature's generosity, and if harvesting as living forever.展开更多

This system is used for compiling the land value map and apartments value map. Due to the lack of experience in the compilation of these maps, the methodology of valuation is based on the experiences of other countries with similar development of real estate market. The mass assessment, which is applied in determining the values for specific purposes, such as expropriation, compensation and taxes for property transfer through the methodology adopted for this aim, is analyzed in the light of the finding of issues that accompany this application. The aim of this research is to highlight the problems to provide the ways of overcoming them in order to approximate the values of map values with market values. The mass appraisal process requires effective valuation models and proper management of resources. Its accuracy depends upon the extent and quality of property data available. Because of this reason, it is recommended the usage of the GIS (geographic information system), an automated system for collection and processing of the data necessary for valuation, as the first point of integration between two process of valuation, individual and mass valuation.展开更多

Overweight and obesity...The aim of this study was to investigate the body weight models among adolescent school students and to establish the national prevalence of being overweight and obesity in Albanian adolescents. Overweight and obesity is a global public health concern. There is limited information on overweight and obesity in Albanian adolescents. A cross-sectional study on a stratified sample of 1,786 adolescents who attended school, 705 (39%) males and 1,081 (61%) females was conducted in Albania in January-December 2011. They were interviewed and examined for weight and height using standardized techniques. Referring to the 2007 WHO classification for the BMI scale it resulted that 4.4% 95% CI 3.5 - 5.4 adolescents resulted overweight where 32.9% were females and 67.1% were males with a statistically significant difference among them, P 〈 0.01. The overall prevalence of overweight in adolescent females was 2.4%, while in males was 7.5%. The prevalence of obesity class I resulted 0.4%, the obesity class II was 0.3% and obesity class III was 0.1% 95% CI 0.02 - 0.3 from the total of participants. A remarkable finding of our study was that 70.8% of adolescent school students were in normal weight, 20% of them underweight, where 71.7% were females, P 〈 0.01.展开更多

A lot of projects and experiences regarding to what mentioned above, lead to the credence that forest biomass assume a fundamental role, inasmuch they constitute a renewable resource, already or easily present, in the environment. The main purpose of undertaking this study is to analyse the obtainable biomass yields from black locust (Robinia pseudoacacia L.) SRF. Four different plantations have been situated on lands of different fertilities in Italy and Albania. Neither irrigation nor fertilization has been applied. Two different methodologies of biennial harvesting have been compared on black locust plantation situated in Italy. The first one is the contemporary cutting and chipping, while the second with cutting, field seasoning of trees and then chipping. In Albania the analyses of planted trees were mainly focused on height and diameter growth. They revealed a good continuation which means that the bed conditions of abandoned terrain, and climate considering the extended drought period of last summer, don't affect the growth.展开更多

Strong policies to e-marketing have targeted to increase the percentage of elements such as banking and credit granting. E-banking also consists of counseling online with a bank employee or financial advisor. This kind of service is not only effective for the client, who saves his time, but also more economical for the bank, which reduces its costs in human resources (an adviser to the window only deals with a client, while he can handle online more than two credit cases simultaneously, so we decrease the number of employees in general). With its low-cost structure, direct consumer marketing and innovative services, the internet is going to present a real challenge to traditional forms of banking. E-banking is seen from the banks as an opportunity to create competitive advantage over its competitors. It offers a great deal of advantages both to the banks and to the customers. Clients are becoming more and more demanding and they are expecting even higher levels of service from their banks.
文摘This paper aims to advance previous studies on the iron and manganese content in groundwater of Rrogozhina aquifer (western Albania) and to show the factors which control the content of both iron and manganese in groundwater of this aquifer. Rrogozhina aquifer represents an important groundwater source of the country whose relevance was highly increased during recent years because of the demographic expansion in this region. This is a typically multi-layered artesian aquifer which consists of sandstone and conglomerate with high heterogeneous permeability. The groundwater mostly belongs to HCO3-Mg-Ca hydrochemical type. The content of soluble iron and manganese in groundwater ranges from 0.004 mg/L to 0.7 mg/L, and from 0.0 mg/L to 1.147 mg/L, respectively. Deeper the groundwater occurs and longer the distance from the recharge zone, higher is the content of Fe2+ in groundwater due to gradual depletion of Oe in groundwater. The content of iron is higher than that WHO and EU recommended, which were 21% and 25% respectively, while the content of manganese were 17% and 23%, respectively. Abstraction of groundwater with high quantities by deeper wells would be accompanied by increase of dissolved iron and manganese content in groundwater which should be treated (with aeration) in order to decrease the content of these two elements in the water before using it.
文摘Hemorrhagic fever with renal syndrome (HFRS) is a disease caused by viruses of the family Bunyaviridae,genus Hantavirus.HFRS from Dobrava virus (DOBV) is a seldom reported disease in Albania.Clinically HFRS is manifested as mild,moderate,or severe.Therefore,the number of cases of Hantavirus' infection may be underestimated,and should be included in the differential diagnosis of many acute infections,hematologic diseases,acute abdominal diseases and renal diseases complicated by acute renal failure.We report here an atypical presentation of HFRS from Dobrava virus complicated by orchitis with a positive outcome.
文摘Air is the ocean we breathe. Air supplies us with oxygen which is essential for our bodies to live. 99.9% air is nitrogen, oxygen, water vapor and inert gases. Human activities can release substances into the air, some of which can cause problems for humans, plants and animals. There are several main types of pollution and well-known effects of pollution which are commonly discussed. These include smog, acid rain, the greenhouse effect, and "holes" in the ozone layer. Each of these problems has serious implications for our health and well-being as well as for the whole environment. The age and health condition are more important also at the response opposite the pollution of air. During the last decade in our country such as result of freely population movement, development and growth of their industrial activities, growth of vehicle number, collapse of roadway infrastructure etc., an increase of air quality pollution is evident. In this study, we exhibited the causers, such as increasing of number of the old cars, which is an important factor of the air pollution and, at the same time, we exhibited the pollution indicators of the air quality, such as PM I 0, total dust, dust levels outdoors and indoors, which are in the high levels. These factors indicated on human health. We have observed the health indicators, especially lung diseases such as bronchial asthma with higher percentage, chronic bronchitis and lung cancer. There was consistency in the finding that relates the acute increases in urban air pollution and the short-term health effects or patients suffering from chronic obstructive pulmonary diseases.
文摘The paper sheds light on the factors enabling sustainable development of Medicinal and Aromatic Plants (MAPs) of Albania; socio-economic, legal and regulatory aspects and institutional capacity related to the MAP sector in the country; sector dynamics, challenges and opportunities; and Albania's positioning in the world markets. Stewardship of MAPs natural base in the country remains a challenge as approximately 95% of the MAPs are collected from the wild. Albania continues to be the leader in Sage exports to the US market accounting for 49% of its market share. Statistical data and direct communication with MAP relevant stakeholders reveal that there is a great demand for the Albanian MAPs from the EU and US markets. However, as clearly indicated in this paper, increased competitiveness of this sector in the world markets should be preceded by sustainable management of the MAPs which can be achieved through (1) continuous training and education of the MAP value chain members prior to harvesting season, (2) organic production and (3) commercial cultivation of MAPs. This paper highlights that sustainable management and sector development require combined efforts of all sector stakeholders in an attempt of all respecting Mother Nature's generosity, and if harvesting as living forever.
文摘Environmental problems in the Republic of Macedonia and Albania are similar in other countries in the region. Numerous problems and deficiencies identified through different projects. The studies and planning documents are evident for environmental media and areas. Such as lack of regional integrated systems for municipal solid waste management. The system for special waste types management, including hazardous waste, insufficient coverage of populated places and municipalities by wastewater collection and treatment systems (especially in agglomerations above 10,000 populations). The need to establish integrated river basin management is in accordance with the new law on waters, air pollution with different intensity in different parts of the country. This paper presents the imperative reforms to protect the natural resources and biodiversity in the countries.
文摘In the last decade, mass valuation system is applied in Albania in addition to individual valuation system for real estate valuation. This system is used for compiling the land value map and apartments value map. Due to the lack of experience in the compilation of these maps, the methodology of valuation is based on the experiences of other countries with similar development of real estate market. The mass assessment, which is applied in determining the values for specific purposes, such as expropriation, compensation and taxes for property transfer through the methodology adopted for this aim, is analyzed in the light of the finding of issues that accompany this application. The aim of this research is to highlight the problems to provide the ways of overcoming them in order to approximate the values of map values with market values. The mass appraisal process requires effective valuation models and proper management of resources. Its accuracy depends upon the extent and quality of property data available. Because of this reason, it is recommended the usage of the GIS (geographic information system), an automated system for collection and processing of the data necessary for valuation, as the first point of integration between two process of valuation, individual and mass valuation.
文摘The paper analyses a special moment-a so-called "revolutionar" campaign that was undertaken in all fields: social, cultural and religious ones in the contemporary history of Albania. And exactly, it analyses the dramatic turn undertaken by the dictatorial communist regime during the years of 1966-1969. As a result, in period of few years, Albanian was transformed into the only atheist country in the world, because since November 13, 1967, the religious activities were banned and this situation lasted until 1976 from the constitutional point of view.

Berlin Congress of 1878 totally neglected interests of the Albanians. Subsequently, Treaty of Berlin became the starting point for creation of the Albanian League of Prizren (1878-1881) as well as future development of the Albanian national movement. Russia as well as other Great Powers played a controversial role in that context.
文摘The issue of linguistic planning, choice of variant for increasing in standard norm, the determination of official status of appropriate language in certain countries, has been a concern, as it was also the orientation of different linguistic studies. In this work, we will try to treat the process of linguistic planning and of the official status of languages in Kosova during 1999-2009. In this context we do not intend to exhaust of handling with this problem, but we will try to present some issues and problems in which languages in Kosova have passed through. This issue is interesting for studies in sociolinguistic field in general and particularly in Albanian language.

The adrenaline as happiness, meaning and pleasure…The topics that this study is about, are two key terms that will guarantee the success of this “adventure project”: The first is the satisfaction of consumers, which gives us a feedback about the product/service and the second part deals with the term adrenaline (pleasure supreme customer); hedonic consumer features, part of their character, the main approaches and a study conducted with customers who have attended at least once rafting, customers attending characteristic of emotions, adrenaline, and adventure to take the pleasure of life. A company should always try and please its consumers. Usually, pleased consumers will always come back for more; they will also spread the word and bring new potential consumers into your business that will pay a fair amount of money for a new upcoming product that set company offers. The measurement of consumer satisfaction is a pure indicator of the consumers repurchases intentions, an indicator of the actual performance and differentiation from the current competition. The consumers mind will always remember the name of the company that they had a bad experience rather than the onethey have been satisfied with. Often when we receive a fixed service, our mind tends to create a very close emotional connection to set service. Often we find that the smallest things bring the greatest joy, regardless of the price. For the majority part, following the adrenaline passion is more than just a need. Lately, the traditional notion of purchasing is left behind and everyone is focusing on providing themselves with pleasure and emotions. The definition of the word “hedonic” comes from Ancient Greece, meaning “taking the maximal pleasure from life” being our primary goal.
文摘Known data on the distribution of Alliocera graeca Saunders, 1845 and Ber& chalybata (Forster, 1771) in Europe are summarized. A. graeca has been reliable recorded from Albania, Croatia and Greece. B. chalybata has been reliable recorded from Austria, Belgium, Bulgaria, Czech Republic, Denmark, England, Finland, France, Germany, Hungary, Ireland, Italy, Netherlands, Norway, Poland, Roumania, Russia, Slovakia, Sweden, Switzeland. A. graeca and B. chalybata, known as European species, are new records for the Turkish Fauna. In this study, the female ofA. graeca and the female ofB. chalybata are presented and photographs of two species are accompanied. Distribution of these species treated is briefly discussed.
文摘The aim of this study was to investigate the body weight models among adolescent school students and to establish the national prevalence of being overweight and obesity in Albanian adolescents. Overweight and obesity is a global public health concern. There is limited information on overweight and obesity in Albanian adolescents. A cross-sectional study on a stratified sample of 1,786 adolescents who attended school, 705 (39%) males and 1,081 (61%) females was conducted in Albania in January-December 2011. They were interviewed and examined for weight and height using standardized techniques. Referring to the 2007 WHO classification for the BMI scale it resulted that 4.4% 95% CI 3.5 - 5.4 adolescents resulted overweight where 32.9% were females and 67.1% were males with a statistically significant difference among them, P 〈 0.01. The overall prevalence of overweight in adolescent females was 2.4%, while in males was 7.5%. The prevalence of obesity class I resulted 0.4%, the obesity class II was 0.3% and obesity class III was 0.1% 95% CI 0.02 - 0.3 from the total of participants. A remarkable finding of our study was that 70.8% of adolescent school students were in normal weight, 20% of them underweight, where 71.7% were females, P 〈 0.01.

文摘Bio-energy and biomass system is a new scientific field, very interesting, where there can find new available information; however, there is also place for other novelties. A lot of projects and experiences regarding to what mentioned above, lead to the credence that forest biomass assume a fundamental role, inasmuch they constitute a renewable resource, already or easily present, in the environment. The main purpose of undertaking this study is to analyse the obtainable biomass yields from black locust (Robinia pseudoacacia L.) SRF. Four different plantations have been situated on lands of different fertilities in Italy and Albania. Neither irrigation nor fertilization has been applied. Two different methodologies of biennial harvesting have been compared on black locust plantation situated in Italy. The first one is the contemporary cutting and chipping, while the second with cutting, field seasoning of trees and then chipping. In Albania the analyses of planted trees were mainly focused on height and diameter growth. They revealed a good continuation which means that the bed conditions of abandoned terrain, and climate considering the extended drought period of last summer, don't affect the growth.

The evaluation and estimations were accomplished by MAHA sottware. This research work has been focused in the evaluation of the opacity coefficient and the sources that can influence in opacity value of the vehicles used in Albania.
