In this randomly controlled trial, 86 patients were allocated to two groups, the experimental group given clinical and electro-acupuncture treatments for a period of 4 weeks, and the control group given clinical treatment plus active and/or passive functional exercise. The result showed that the level of impairment and disability in both groups were improvement according to the Chinese Stroke Scale, Brunnstrom-Fugl-Meyer score, and Barthel Index throughout the study and 3 months after. Qing Xin An Shen Fang ([symbol: see text] a formula for clearing away the heart-heat to induce tranquilization) was used in the treatment group and compared with mexiletine and Fu Fang Dan Shen Tablets ([symbol: see text] Compound Salvia Tablets) used in the control group. By several-year clinical observations, 36 cases were treated with satisfactory therapeutic effects as reported in the following.Clinical DataOf 36 in-patients, there were 24 males and 12 females, aged from 44 to 81 years, averaging 64.92 years. Of 36 cases, 24 were at the acute stage and 12 at the recovery stage. All the cases were diagnosed as cerebral vascular diseases by cranial CT scan and MRI, of which, 4 were cerebral infarction, 26 lacunar cerebral infarction, 5 cerebral hemorrhage and 1 mixed type. Of 36 cases, 15 were the first attack of wind-stroke, 15 the second attack, 5 the third attack and 1 the forth attack. There were 26 patients with hypertension among 36 cases, of which, 8 patients suffered from hypertension within 10 years, 6 for more than 10 years, 9 for more than 20 years and 3 for more than 30 years. All the 36 cases were associated with dysphagia and agreeable to Standard on Diagnosis and Evaluation of Therapeutic Effects of Wind-stroke issued by the State Scientific Committee 85-919-01-01, 1995.展开更多
To assess first-year gastroenterology fellows’ ability to address difficult interpersonal situations in the workplace using objective structured clinical examinations (OSCE).METHODSTwo OSCEs (“distracted care team”...To assess first-year gastroenterology fellows’ ability to address difficult interpersonal situations in the workplace using objective structured clinical examinations (OSCE).METHODSTwo OSCEs (“distracted care team” and “frazzled intern”) were created to assess response to disruptive behavior. In case 1, a fellow used a colonoscopy simulator while interacting with a standardized patient (SP), nurse, and attending physician all played by actors. The nurse and attending were instructed to display specific disruptive behavior and disregard the fellow unless requested to stop the disruptive behavior and focus on the patient and procedure. In case 2, the fellow was to calm an intern managing a patient with massive gastrointestinal bleeding. The objective in both scenarios was to assess the fellows’ ability to perform their duties while managing the disruptive behavior displayed by the actor. The SPs used checklists to rate fellows’ performances. The fellows completed a self-assessment survey.RESULTSTwelve fellows from four gastrointestinal fellowship training programs participated in the OSCE. In the “distracted care team” case, one-third of the fellows interrupted the conflict and refocused attention to the patient. Half of the fellows were able to display professionalism despite the heated discussion nearby. Fellows scored lowest in the interprofessionalism portion of post-OSCE surveys, measuring their ability to handle the conflict. In the “frazzled intern” case, 68% of fellows were able to establish a calm and professional relationship with the SP. Despite this success, only half of the fellows were successfully communicate a plan to the SP and only a third scored “well done” in a domain that focused on allowing the intern to think through the case with the fellow’s guidance.CONCLUSIONFellows must receive training on how to approach disruptive behavior. OSCEs are a tool that can assess fellow skills and set a culture for open discussion.展开更多

Three modern optimization algorithms (simulated annealing algorithm, genetic algorithm and ant colony algorithm) are tested to match storms in successive time intervals. Preliminary results indicate that the simulated annealing algorithm and ant colony algorithm are effective and have intuitionally adjustable parameters, whereas the genetic algorithm is unsatisfaetorily constrained by the mode of genetic operations Experiments provide not only the feasibility and characteristics of storm tracking with modern optimization algorithms, but also references for studies and applications in relevant fields.展开更多

The effects of both temp...A metagenomic library recombinant clone CAPL3, an Escherichia coli strain generated by transformed with metagenomic library from deep-sea sediments, can efficiently produce cold active lipase. The effects of both temperature and dissolved oxygen(DO) on cold active lipase production by batch culture of metagenomic library recombinant clone(CAPL3) from deep-sea sediment were investigated. First, a two-stage temperature control strategy was developed, in which the temperature was kept at 34 ℃ for the first 15 h, and then switched to30 ℃. The cold active lipase activity and productivity reached 315.2 U·ml^-1and 8.08 U·ml^-1·h^-1, respectively,increased by both 14.5% compared to the results obtained with temperature controlled at 30℃. In addition, different DO control modes were conducted, based on the data obtained from the different DO control strategies and analysis of kinetics parameters at different DO levels. A step-wise temperature and DO control strategy were developed to improve lipase production, i.e., temperature and DO level were controlled at 34℃, 30% during 0–15 h;30 ℃, 30% during 15–18 h, and 30 ℃, 20% during 18–39 h. With this strategy, the maximum lipase activity reached 354.6 U·ml^-1at 39 h, which was 28.8% higher than that achieved without temperature and DO control(275.3 U·ml^-1).展开更多

The total effective rates of the combination group, the acupuncture group and the pressure on otopoints group were 95.4%, 92.1% and 62.5% respectively; and the cure rates were 38.4%, 15.8% and 5% respectively. The differences in results of the three groups show statistically marked significance, indicating that the therapeutic effectiveness of the combined method of acupuncture with pressure on otopoints is better than the other two therapeutic methods.展开更多

The total effective rate was 91.1% in the former and 86.7% in the latter, with no statistically significant difference between the two groups, indicating that acupuncture only at the points of Yin-Meridians is also an effective therapy for apoplexy.
文摘Osteoarthritis, a nonspecific inflammatory lesion, is a commonly seen joint disease. Clinically, it is mainly characterized by arthralgia, swelling, and motor impairment. Since the knee joint is the load-bearing joint of the human body, and is susceptible to trauma, gonitis tends to have the highest morbidity in the four limbs, which manifests itself arthritis of patella and femur at the early stage; narrowness or disappearance of the medial joint space at the mid stage; and damage of the cartilage accompanied with flexion deformity at the late stage. In recent years, based on the experience of Prof. Cao Yiming, the author has treated 47 cases of gonitis by combined use of Chinese drugs and acupuncture, and obtained satisfactory therapeutic results as reported in the following.
文摘In recent years, the computer drawing technology that deals with three-dimensional (3D) design of footwear has become hot topics. Rhino is a kind of common and practical 3D design software with strong drawing and rendering graphics function, which is widely used to design industrial products. In this paper, through decomposition and modeling, modeling and drawing methods were analyzed in various parts of footwear by Rhino, as well as the smooth technology and adjustments to its profile curve by an example of lady's high boots. Finally, through a series introductions of rendering effects for footwear in color, light perception, grain characteristic, and 3D graphics, the main technical essential is achieved and difficulties in design of overall footwear styles are solved.

文摘To achieve the production information management of the coning,drawing and twisting's workshop,and promote a good and fast development of the textile enterprise,firstly,we analyze the information technology development status of the textile enterprise at home and abroad,study the weaknesses of the existing computer monitoring system,introduce the system design project,and propose a system network model based on client/server(C/S)mode.Secondly,a multi-agent production monitoring and control system is developed,and the communication of the single-port-multi-protocol and a multi-thread data collection technology are studied via the multi-thread and standard template library(STL)technologies.Thirdly,the system management functions,the structure design of the database,and the technical difficulties in the process of software design are also introduced.As proved in practice,the system satisfies the production management requirements of the workshop,and implements the network management of the production information.

文摘A metagenomic library recombinant clone CAPL3, an Escherichia coli strain generated by transformed with metagenomic library from deep-sea sediments, can efficiently produce cold active lipase. The effects of both temperature and dissolved oxygen(DO) on cold active lipase production by batch culture of metagenomic library recombinant clone(CAPL3) from deep-sea sediment were investigated. First, a two-stage temperature control strategy was developed, in which the temperature was kept at 34 ℃ for the first 15 h, and then switched to30 ℃. The cold active lipase activity and productivity reached 315.2 U·ml^-1and 8.08 U·ml^-1·h^-1, respectively,increased by both 14.5% compared to the results obtained with temperature controlled at 30℃. In addition, different DO control modes were conducted, based on the data obtained from the different DO control strategies and analysis of kinetics parameters at different DO levels. A step-wise temperature and DO control strategy were developed to improve lipase production, i.e., temperature and DO level were controlled at 34℃, 30% during 0–15 h;30 ℃, 30% during 15–18 h, and 30 ℃, 20% during 18–39 h. With this strategy, the maximum lipase activity reached 354.6 U·ml^-1at 39 h, which was 28.8% higher than that achieved without temperature and DO control(275.3 U·ml^-1).
文摘The combined method of acupuncture with pressure on otopoints (the combination group) was used to treat 86 patients with facial spasm; and simple acupuncture and simple pressure on otopoints were respectively applied in the other two groups of patients as controls. The total effective rates of the combination group, the acupuncture group and the pressure on otopoints group were 95.4%, 92.1% and 62.5% respectively; and the cure rates were 38.4%, 15.8% and 5% respectively. The differences in results of the three groups show statistically marked significance, indicating that the therapeutic effectiveness of the combined method of acupuncture with pressure on otopoints is better than the other two therapeutic methods.
