Historically, its indications have been less well-defined than those of endoscopic biliary sphincterotomy, yet it plays a definite and useful role in diseases such as chronic pancreatitis and pancreatic-type sphincter of Oddi dysfunction. In the appropriate setting, it may be used as a single therapeutic maneuver, or in conjunction with other endoscopic techniques such as pancreatic stone extraction or stent placement. The current standard of practice utilizes two different methods of performing pancreatic sphincterotomy: a pull-type sphincterotome technique without prior stent placement, and a needle-knife sphincterotome technique over an existing stent. The complications associated with pancreatic sphincterotomy are many, although acute pancreatitis appears to be the most common and the most serious of the early complications. As such, it continues to be reserved for those endoscopists who perform a relatively high-volume of therapeutic pancreaticobiliary endoscopic retrograde cholangio-pancreatography.展开更多
Big data analytics is emerging as one kind of the most important workloads in modern data centers. Hence,it is of great interest to identify the method of achieving the best performance for big data analytics workloads running on state-of-the-art SMT( simultaneous multithreading) processors,which needs comprehensive understanding to workload characteristics. This paper chooses the Spark workloads as the representative big data analytics workloads and performs comprehensive measurements on the POWER8 platform,which supports a wide range of multithreading. The research finds that the thread assignment policy and cache contention have significant impacts on application performance. In order to identify the potential optimization method from the experiment results,this study performs micro-architecture level characterizations by means of hardware performance counters and gives implications accordingly.展开更多

It may take a long time before the diagnosis can be confirmed if the patients have no typical skin involvements. Here we report an unusual case. A 40-year-old male with lepromatous leprosy showed a gradual onset of bilateral symmetrical neuropathies without characteristic skin manifestations seven years after onset and with pulmonary tuberculosis simultaneously. He was misdiagnosed as having Guillani-Barr6 syndrome and systemic necrotizing vasculitis for 10 years until the skin biopsy was performed. This case indicates that the risk of leprosy exists, though new cases being detected have significantly declined over the last 50 years; neurologists need to pay more attention to leprosy with various manifestations .展开更多

Given the delayed onset of PTA, few studies have as...Purpose: Posttraumatic arthritis (PTA) may develop years after acetabular fracture, hindering joint function and causing significant chronic musculoskeletal pain. Given the delayed onset of PTA, few studies have assessed outcomes of delayed total hip arthroplasty (THA) in acetabular fracture patients. This study systematically reviewed the literature for outcomes of THA in patients with PTA and prior acetabular fracture. Methods: Pubmed, EMBASE, SCOPUS, and Cochrane library were searched for articles containing the keywords "acetabular", "fracture", "arthroplasty", and "post traumatic arthritis" published between 1995 and August 2017. Studies with less than 10 patients, less than 2 years of follow-up, conference abstracts, and non-English language articles were excluded. Data on patient demographics, surgical characteristics, and outcomes of delayed THA, including implant survival, complications, need for revision, and functional scores, was collected from eligible studies. Results: With 1830 studies were screened and data from 10 studies with 448 patients were included in this review, The median patient age on date of THA was 51.5 years, ranging from 19 to 90 years. The median time from fracture to THA was 37 months, with a range of 27-74 months. Mean follow-up times ranged from 4 to 20 years, The mean Harris hip scores (HHS) improved from 41.5 pre-operatively, to 87.6 post-operatively, The most prevalent postoperative complications were heterotopic ossification (28% -63%), implant loosening (1%-24%), and infection (0%-16%). The minimum 5-year survival of implants ranged from 70% to 100%. Revision rates ranged from 2% to 32%. Conclusion: Despite the difficulties associated with performing THA in patients with PTA from previous acetabular fracture (including soft tissue scarring, existing hardware, and acetabular bone loss) and the relatively high complication rates, THA in patients with VIA following prior acetabular fracture leads to significant improvement in pain and function at 10-year follow-up. Further high quality randomized controlled studies are needed to confirm the outcomes after delayed THA in these patients.展开更多
Giant earthquakes generate rich signals that can be used to explore the characteristics of the hierarchical structure of the Earth’s interior associated with the eigenfrequencies of the Earth.We employ the spectral e...Giant earthquakes generate rich signals that can be used to explore the characteristics of the hierarchical structure of the Earth’s interior associated with the eigenfrequencies of the Earth.We employ the spectral element method,incorporated with large-scale parallel computing technology,to investigate the characteristics of global seismic wave propagation excited by the2011 Mw9.0 Tohoku earthquake.The transversely isotropic PREM model is employed as a prototype of our numerical global Earth model.Topographic data and the effect of the oceans are taken into consideration.Wave propagation processes are simulated by solving three-dimensional elastic wave governing equations with the seismic moment tensor obtained from the Global Centroid Moment Tensor Catalog.Three-dimensional visualization of our computing results displays the nature of the global seismic wave propagation.Comparative analysis of our calculations with observations obtained from the Incorporated Research Institutions for Seismology demonstrates the reliability and feasibility of our numerical results.We compare synthetic seismograms with incorporated and unincorporated ocean models.First results show that the oceans have obvious effects on the characteristics of seismic wave propagation.The peak displacement and peak velocity of P waves become relatively small under the effect of the ocean.However,the effect of the ocean on S-waves is complex.The displacement and velocity of S waves decrease rapidly over time using an unincorporated ocean model.Therefore,the effects of the ocean should be incorporated when undertaking quantitative earthquake hazard assessments on coastal areas.In addition,we undertake comparative analysis on the characteristics of the Earth’s oscillation excited by the 2004 Sumatra-Andaman,2008 Wenchuan,and 2011Tohoku earthquakes that incorporate the effect of the Earth’s gravitational potential.A comparison of the amplitude spectra of the numerical records indicates that energy released by the three big earthquakes is different.Our comparative analysis realizes that the computing results can accurately reproduce some eigenfrequencies of the Earth,such as toroidal modes 0T2 to 0T13and spheroidal modes 0S7 to 0S31.These results demonstrate that numerical simulations can be successfully used to investigate the Earth’s oscillations.We propose that numerical simulations can be used as one of the major tools to further reveal how the Earth’s lateral heterogeneities affect the Earth’s oscillations.展开更多
