SVR12 rates for subgroups were: 92% for SOF/DCV, 93% for each SOF/SMV, SOF/PR, 94% for SOF/LDV, 100% for 3D, 94% for pretreated, 87% for liver cirrhosis, 82% for patients with platelets < 100/n L, 88% post-liver transplantation, 95% for GT1 a, 93% for GT1 b, 90% for GT3, 100% for GT2, 4, and 6. 12 patients suffered from relapse, 6 prematurely discontinued treatment, of which 4 died. Negative predictors of SVR12 were a platelet count < 100/nL, MELD score ≥ 10(P < 0.0001), liver cirrhosis(P = 0.005) at baseline. In Interferonfree treatment GT3 had significantly lower SVR rates than GT1(P = 0.016). Side effects were mild. CONCLUSION Excellent SVR12 rates and the favorable side-effect profile of DAA-combination therapy can be well translated into "real-world". Patients with advanced liver disease, signs of portal hypertension, especially with platelets < 100/n L and patients with GT3 are in special need for further research efforts to overcome comparatively higher rates of virological failure.展开更多

But both the necessity of repeated injections to maintain corrections and the question of adverse allergic reactions developing from the use of a xenogenic collagen have been an area of serious concern. To overoome these adverse effects, we have developed injectable collagen preparations from human placenta. Gamma irradiation was used for sterilization and crosslinkins of the collagen. We observed the mouse immune respose to gamma-irradiated human placenta soluble and insoluble collagen following multiple injections. After six injections of these materials, no total IgG level increase was found, nor was antibody specifically directed against human collagen found.Mouse antibody levels were also observed following Zyderm II and Zyplast repetitive injections and following repetitive implantations of coated vicryl and chromic gut. No humoral immune response was found in this heterologous type system.展开更多

Based on the cumulative fatigue criterion and the stress-life equation, the key meshing states of the gear pair were investigated for the contact fatigue and the bending fatigue. Then, the reliability of the proposed model was proved by comparing the calculation result with the simulation result. Further study was performed to analyze the variation of the contact fatigue stress and the bending fatigue stress under different loads. Furthermore, the roles of the driving pinion and the driven gear pair were evaluated in the fatigue life of the straight bevel gear pair and the main fatigue failure mode was determined for the significant gear. The results show that the fatigue failure of the driving pinion is the main fatigue failure for the straight bevel gear pair and the bending fatigue failure is the main fatigue failure for the driving pinion.展开更多

Varying results for this treatment underpin the need for a synthesis of evidence. Methods: We conducted a literature-based meta-analysis to quantify the magnitude of the benefit comparing irinotecan in combination with cisplatin (IP) with etoposide in combination with cisplatin (EP) in ED-SCLC patients. The primary outcome was overall survival (OS) and progression-free survival (PFS); secondary outcomes included overall response rate, 1- and 2-year survival rates, disease control rate and toxicity. Results: Four trials including 1,541 patients were identified in the analysis. No positive results (P〈0.05) were seen: OS (HR=0.85, CI95%=0.71-1.01; P=-0.08) with high heterogeneity (Chi2=7.76, dr=-3 [P=-0.05]; I2=61%), PFS (HR=0.91, CI95%=0.74-1.28; P=-0.36) with high heterogeneity (Chi2=11.96, df=3 [P=-0.008]; I2=75%), overall response rate(OR=1.16; CI95%=0.79-1.70; P=0.45), disease control rate (OR=1.01; CI95%=0.74-1.38; P=0.95), 1-year survival rate (OR = 1.30; CI95%=0.98-1.72; P=0.07) and 2-year survival rate (OR=1.97; CI95%=0.95-4.09; P=-0.07). Fewer patients who received IP suffered severe hematologic toxicities (grade≥3), such as neutropenia, thrombocytopenia and leucopenia. However, severe non-hematologic toxicities (grade≥3), such as diarrhea, nausea, vomiting, fatigue, anorexia, and dehydration, were more common among patients who received IP. Conclusion: IP does not lengthen the overall survival or progression-free survival compared with EP in patients with ED-SCLC Fewer patients receiving IP had grade ≥ 3 hematological toxicities of nentropenia, leucopenia and thrombocytopenia, but more had grade≥3 diarrhea, nausea, vomiting, fatigue, anorexia and dehydration.展开更多

The theoretical model of conjugate-curve gear drive is briefly introduced. The general calculation formulas of sliding ratios are developed according to the conjugate curves. The applications to the circular arc gears based on conjugate curves and the novel involute-helix gears are studied. A comparison on the sliding coefficient with the conventional corresponding gear drive is also carried out. The influences of gear parameters such as spiral parameter, gear ratio and modulus on the sliding ratios of gear drive are discussed. Brief description of manufacturing method for conjugate-curve gear pair is given. The research results show that the sliding ratios of gear pair become smaller with the increase of spiral parameter and gear ratio, respectively. And it will be greater with the increase of modulus for the tooth profiles. The meshing characteristics of conjugate-curve gears are further reflected and the optimization design of tooth profiles with high performance may be obtained.展开更多

Methods: Twenty-four cases of advanced NSCLC (stage Ill-IV) patients were treated with A regimen (DDP 35 mg/m^2 d1-3; GEM 1250 mg/m^2 d1, 8 ). The other 31 cases were treated with B (GEM 1250 mg/m^2 d1,8; NVB 25 mg/m^2 d1, 8 ). Repeat every 3 weeks for 6 courses. Results: In A regimen group, the overall response rate was 45.8% (CR + PR = 11), median response time was 5.5 months, median survival time was 11 months and 1-year survival rate was 41.7%. In B regimen group, the overall response rate was 48.4% (CR + PR = 15) and median response time, survival time and 1-year survival rate were respectively 6.5 and 10 months and 41.9%. The major toxicities were nausea/vomiting, myelosuppression in A regimen group, myelosuppression and phlebitis in B regimen group, respectively. Conclusion: A regimen and B regimen for advanced NSCLC have similar response rate (P 〉 0.05). B regimen, a two-drug combination without a platinum drug is of less toxicity and more safety than A regimen, a three-drug platinum-based regimen and is recommended to be a regimen in the first-line treatment for advanced NSCLC.展开更多

Three-dimensional conformal radiation therapy requires the precise definition of the target volume. We aimed to compare the treatment results and radiation toxicities between three-dimensional conformal radiotherapy (3D-CRT) and conventional radiotherapy (CR) for patients with cervical and thoracic esophageal carcinoma. Methods: From June 2004 to December 2006, a random study was performed on 106 patients treated with 3D-CRT or CR, 53 patients in each group. The patients in CR group received conventional radiotherapy in 2.0 Gy/f, 5 f/week and total dose was 66-70 Gy in 6.5 or 7 weeks. The patients in 3D-CRT group were treated by 3D-CRT in 2.0 Gy/f, 5 f/week and total dose was 64-70 Gy in 6 weeks. The local control rates, survival rates and radiation toxicities for the two groups were investigated. Results: The 1-year and 3-year local control rates were 83.0% and 60.4% in 3D-CRT group and 64.2% (x^2 = 4.853, P = 0.028) and 32.1% (x^2 = 9.812, P = 0.002) in CR group. The 1-year and 3-year survival rates were 73.6% and 50.9% in 3D-CRT group and 54.7% (x^2 = 4.102, P = 0.043) and 32.1% (x^2 = 3.886, P = 0.049) in CR group. Between the two groups, there was no significant difference in radiation toxicities. Conclusien: The 1-year and 3-year local control rates and survival rates of patients with esophageal carcinoma treated by 3D-CRT is superior to CR. However, longer-term results and radiation toxicity need further study which involves more patients and prolonged follow-up.展开更多

This technique makes a compromise between electrolysis prevention and high flow rate performance. This technique should eliminate electrolytic bubble generation, electrodes wear and fluid propriety modification. All these side phenomena are prevented by considering isolated electrodes. The numerical presented results in this paper demonstrate that continuous MHD pumping is possible with isolated electrodes. The MHD excitation combines a high frequency altering current with a low frequency altering magnetic field. In order to validate our results, two independent theoretical methods for computing flow rate are followed. The two presented independent approaches show that high flow rate is possible even with isolated electrodes. To overcome the problem of dimensioning this kind of pumps, a generic numerical analysis is proposed. Hence, the pump performances as functions of the external parameter are studied and tools to calculate for a given fluid and the optimal high frequency regime are provided.展开更多

Between the two groups, there was no significant difference in radiation toxicities. Conclusien: The 1-year and 3-year local control rates and survival rates of patients with esophageal carcinoma treated by 3D-CRT is superior to CR. However, longer-term results and radiation toxicity need further study which involves more patients and prolonged follow-up.
文摘In this work, a numerical study for designing a new kind of MHD (Magneto-Hydrn-Dynamic) pumps is presented. Hence, the pump performances as functions of the external parameter are studied and tools to calculate for a given fluid and the optimal high frequency regime are provided.

文摘Objective: The aim of this study was to evaluate the anti-tumor activity and safety of Gemcitabine (GEM) combined with Vinorelbine (NVB) in patients with advanced TNABC after chemotherapy. Methods: Thirty-seven patients with immunohistochemical proved TNABC were enrolled. The patients received 21-day cycles of NVB 25mg/m^2 i.v. with GEM 1000 mg/m^2 i.v. on days 1 and 8. Results: A total of 136 cycles were given to 37 patients(median 4 cycles, ranged 2-6 cycles). The treatment response was evaluable in all patients. Of the 37 patients, 1 received complete remission (CR), 8 received partial remission (PR), 20 had stable disease (SD), 9 had progressive disease (PD). Overall objective response (CR+ PR) were 24.3 %. The median time to progress (TTP) was 6 months (95% CI, 4-6 months). The median overall survival was 24 months (95% CI, 11-37 months). The median 1-year survival rate was (66.24±8.43)%. The median 3-year survival rate was (28.77±11.96)%. The major adverse events were grade Ⅰ-Ⅱ myelosuppression, peripheral neurologic toxicities, nausea and vomiting. Some patients had rash and hepatic dysfunction. A total of 40% of patients experienced flu-like symptoms. Alopecia and diarrhea were rare. Conclusion: The combination of GEM and NVB is an effective and well tolerated regimen for the patients with TNABC.
文摘Replication is an approach often used to speed up the execution of queries submitted to a large dataset.A compile-time/run-time approach is presented for minimizing the response time of 2-dimensional range when a distributed replica of a dataset exists. The aim is to partition the query payload(and its range) into subsets and distribute those to the replica nodes in a way that minimizes a client's response time. However, since query size and distribution characteristics of data(data dense/sparse regions) in varying ranges are not known a priori, performing efficient load balancing and parallel processing over the unpredictable workload is difficult.A technique based on the creation and manipulation of dynamic spatial indexes for query payload estimation in distributed queries was proposed. The effectiveness of this technique was demonstrated on queries for analysis of archived earthquake-generated seismic data records.

文摘In order to study the stability of friction and contact of the rotating tribological pair system, considering the influence of the changeable factors on the stability, the system dynamics analysis model based on the Lagrange equation is firstly established. The surface contact stiffness model is determined on the basis of the fractal theory. The model of the friction torque with velocities is created by using the Stribeck friction effect. The Lyapunov indirect method is employed to explore the eigenvalue problem of the system state equation. The effects of the applied load, the fractal dimension, the fractal scaling coefficient and the Stribeck coefficient on the system stability are investigated in detail. The numerical simulation results demonstrate that the tribological pair system is prone to causing system instability at low speed, and the system instability boundary value decreases when the Stribeck coefficient decreases. The fractal dimension and the fractal scaling coefficient impact the system stability slightly when fractal dimensions are large, and the system instability can be reduced by properly increasing the surface smoothness. Moreover, the system instability evidently increases with the increase in the applied load and the Stribeck coefficient. These achievements can provide a reference and theoretical support for the analysis of the dynamic performance of the tribological pair system.
