目的:分析探究异丙酚与异氟醚对上腹部手术患者围术期应激变化的影响。方法:将80例择期行上腹部手术患者随机均分为异丙酚组(A组)和异氟醚组(B组),分别应用异丙酚微泵维持麻醉与异氟醚吸入维持麻醉。连续监测血流动力学指标,并于麻醉诱... 目的:分析探究异丙酚与异氟醚对上腹部手术患者围术期应激变化的影响。方法:将80例择期行上腹部手术患者随机均分为异丙酚组(A组)和异氟醚组(B组),分别应用异丙酚微泵维持麻醉与异氟醚吸入维持麻醉。连续监测血流动力学指标,并于麻醉诱导前(T0)、切皮后10 min(T1)、开腹探查时(T2)、术后24 h(T3)采集患者的静脉血,采用放射免疫分析法检测血清皮质醇和血糖指标。结果:两组患者在T1和T2时间点血糖和皮质醇均显著升高,且B组血糖和皮质醇升高更为显著(P<0.05);两组患者在T3时间点血糖和皮质醇均降低,且A组血糖和皮质醇升高更为显著(P<0.05);A组的T1和T2时间点的HR显著降低(P<0.05),而B组无显著差异(P>0.05)。结论:异丙酚更能抑制上腹部手术围术期应激变化,但对血流动力学影响较大。 展开更多

目的探究地佐辛对老年患者七氟醚吸入诱导气管插管的影响。方法选取2017年1月~2018年9月我院收治的50例择期全麻手术并于术中行七氟醚吸入诱导气管插管的老年患者作为研究对象,按照随机数字表法将其分为实验组(25例)和对照组(25例)。... 目的探究地佐辛对老年患者七氟醚吸入诱导气管插管的影响。方法选取2017年1月~2018年9月我院收治的50例择期全麻手术并于术中行七氟醚吸入诱导气管插管的老年患者作为研究对象,按照随机数字表法将其分为实验组(25例)和对照组(25例)。实验组患者麻醉诱导前静脉注射0.2 mg/kg地佐辛,对照组患者麻醉诱导前静脉注射10 ml生理盐水,比较两组患者注射药物前(T_1)、注射药物后10 min(T_2)、气管插管前1 min(T_3)、气管插管后1 min(T_4)及气管插管后10 min(T_5)的血流动力学指标[心脏指数(CI)、中心静脉压(CVP)和血氧饱和度(SpO_2)]、应激变化指标[心率(HR)、收缩压(SBP)、舒张压(DBP)、呼吸频率(RR)]及不良反应发生情况。结果两组患者T_1的CI、CVP及SpO_2比较,差异无统计学意义(P>0.05);两组患者T_2、T_3、T_4、T_5的CI、CVP及SpO_2均低于T_1,差异有统计学意义(P<0.05);两组患者T_2、T_3、T_4、T_5的CI、CVP比较,差异无统计学意义(P>0.05);实验组患者T_2、T_3、T_4、T_5的SpO_2均高于对照组,差异有统计学意义(P<0.05)。两组患者T_1的HR、SBP、DBP及RR比较,差异无统计学意义(P>0.05);实验组患者T_2、T_3、T_4、T_5的HR、SBP、DBP及RR均低于T_1,差异有统计学意义(P<0.05);对照组患者T_3、T_4、T_5的HR、SBP、DBP及RR均低于T_1,差异有统计学意义(P<0.05);对照组患者T_1与T_2的HR、SBP、DBP及RR比较,差异无统计学意义(P>0.05);实验组患者T_2、T_3、T_4、T_5的HR、SBP、DBP及RR均低于对照组,差异有统计学意义(P<0.05)。实验组患者麻醉期间的不良反应总发生率为4.00%,明显低于对照组的32.00%,差异有统计学意义(P<0.05)。结论地佐辛用于老年七氟醚吸入诱导气管插管能够改善患者的SpO_2,有稳定的血流动力学指标,插管后应激反应较小,药物安全性高。 展开更多

New strain induced melt activation(new SIMA) method was employed to prepare high-quality semisolid billet of AZ61 magnesium alloy.Optical microscopy and tensile test were used to study the microstructure and mechanical properties of the thixo-extruded component.The results showed that the optimal process parameters for achieving the complete filling status involved the applied pressure of 784 MPa,the pressure holding time of 90 s and the die temperature of 450 ℃.Compared to semisolid isothermal treatment,high mechanical properties such as the tensile strength of 300.5 MPa and elongation of 22% and fine microstructure were obtained in the thixo-extruded parts.With increasing the isothermal temperature and holding time,the tensile strength and elongation were increased firstly and then decreased.When the press pass was increased from 1 to 4,the tensile strength and elongation of the thixo-extruded parts were greatly enhanced and microstructure was refined obviously. 展开更多

During transient electromagnetic method (TEM) exploration of a copper mine, we detected the late-channel TEM signal reversal phenomenon (a voltage change from positive to negative) caused by the influence of the induced polarization (IP) effect, which affects the depth and precision of the TEM detection. The conventional inversion method is inefficient because it is difficult to process the data. In this paper, the Cole-Cole model is adopted to analyze the effect of Dc resistivity, chargeability, time constant, and frequency exponent on the TEM response in an homogeneous half space model. Singular Value Decomposition (SVD) is used to invert the measured TEM data, and the Dc resistivity, chargeability, time constant and frequency exponent were extracted from the measured TEM data in the mine area. The extracted parameters are used for interpreting the detection result as a supplement. This reveals why the TEM data acquired in the area has a low resolution. It was found that the DC resistivity and time constant do not significantly change the results, however, the chargeability and frequency exponent have a significant effect. Because of these influences, the SVD method is more accurate than the conventional method in the apparent resistivity profile. The area of the copper mine is confined accurately based on the SVD inverted data. The conclusion has been verified by drill and is identical to the practical geological situation. 展开更多

The effects of Al-8B grain refiner on microstructure and tensile properties of an Al-12Zn-3Mg-2.5Cu alloy produced by modified strain induced melt activation process were investigated. Pre-deformation of 60% was used by hot working at 300 ℃. After pre-deformation, the samples were heated to a temperature above the solidus and below the liquidus point and maintained in the isothermal conditions at three different temperatures(500, 550 and 590 ℃) for varying time(10, 20 and 40 min). It was observed that strain induced melt activation has caused the globular morphology of α(Al) grains. Microstructural study was carried out on the alloy by using optical microscope and scanning electron microscope in both unrefined and B-refined conditions. The results showed that for the desired microstructures of the alloy during SIMA process, the optimum temperature and time are 550 ℃ and 10 min, respectively. After the T6 heat treatment, the average tensile strengths increased from 278 to 585 MPa and 252 to 560 MPa for samples refined with 3.75% Al-8B before and after SIMA process, respectively. The ultimate strength of SIMA specimens is lower than that of B-refined specimens. 展开更多

The paper aims to provide new information that may elucidate the modulatory effect of estrogen on the bone-resorbing cytokines RANKL and its anti-resorptive factor OPG secrected by HPLFs which are already force-stimulated. The expression of OPG mRNA is rising after mechanical loading either with or without stimulated by estrogen before. But HPDL cells exposured to estrogen for 24 h before loaded tend to express more OPG mRNA. Compared with the no-estrogen group, the inhibit trend of RANKL mRNA is much more apparent in with-estrogen group. Moreover, estrogen and mechanic force time-dependently increased OPG expression and attenuated the RANKL expression. 展开更多
