Application of endoscopic hemoclips for nonvariceal bleeding in the upper gastrointestinal tract 被引量:6

AIM: To investigate acute nonvariceal bleeding in the upper gastrointestinal (GI) tract and evaluate the effects of endoscopic hemoclipping. METHODS: Sixty-eight cases of acute nonvariceal bleeding in the upper GI tract were given endoscopic treatment with hemoclip application. Clinical data, endoscopic findings, and the effects of the therapy were evaluated. RESULTS: The 68 cases (male:female = 42:26, age from 9 to 70 years, average 54.4) presented with hernatemesis in 26 cases (38.2%), melena in nine cases (13.3%), and both in 33 cases (48.5%). The causes of the bleeding included gastric ulcer (29 cases), duodenal ulcer (11 cases), Dieulafoy's lesion (11 cases), Mallory-Weiss syndrome (six cases), post-operative (three cases), post-polypectomy bleeding (five cases), and post-sphincterotomy bleeding (three cases); 42 cases had active bleeding. The mean number of hemoclips applied was four. Permanent hemostasis was obtained by hemoclip application in 59 cases; 6 cases required emergent surgery (three cases had peptic ulcers, one had Dieulafoy's lesion, and two were caused by sphincterotomy); three patients died (two had Dieulafoy's lesion and one was caused by sphincterotomy); and one had recurrent bleeding with Dieulafoy's lesion 10 mo later, but in a different location. CONCLUSION: Endoscopic hemoclip application was an effective and safe method for acute nonvariceal bleeding in the upper GI tract with satisfactory outcomes. 展开更多

Top-k ranking of websites according to traffic volume is important for Internet Service Providers(ISPs) to understand network status and optimize network resources. However, the ranking result always has a big deviation with actual rank for the existence of unknown web traffic, which cannot be identified accurately under current techniques. In this paper, we introduce a novel method to approximate the actual rank. This method associates unknown web traffic with websites according to statistical probabilities. Then, we construct a probabilistic top-k query model to rank websites. We conduct several experiments by using real HTTP traffic traces collected from a commercial ISP covering an entire city in northern China. Experimental results show that the proposed techniques can reduce the deviation existing between the ground truth and the ranking results vastly. In addition, we find that the websites providing video service have higher ratio of unknown IP as well as higher ratio of unknown traffic than the websites providing text web page service. Specifically, we find that the top-3 video websites have more than 90% of unknown web traffic. All these findings are helpful for ISPs understanding network status and deploying Content Distributed Network(CDN). 展开更多

Continued application of Endostar combined with chemotherapy in advanced hemangioendothelioma of bone

By one case of hemangioendothelioma of bone accompanying pulmonary metastasis was treated with rh-endostatin injection (Endostar) combined with chemotherapy. The patient got partial response (PR) for 3 years after the application of Endostar maintenance therapy and Endostar combined with taxane-based chemotherapy. During the period of using Endostar as monotherapy, the patient got long-term disease control and good quality of life. There was no drug related adverse event during the therapy of Endostar. Suggested continued using of Endostar combined with chemotherapy could achieve an convinced therapeutic effect. Then using Endostar as maintenance treatment after patient got the optimal efficacy was feasible and profitable. This treatment strategy of long-term administration of Endostar was worthy of further observation, to explore the feasibility for long-term administration of combined with chemotherapy in the treatment of hemangioendothelioma of bone accompanying pulmonary metastasis. 展开更多

Aiming at this problem,a parallelization approach was proposed with six memory optimization schemes for CG,four schemes of them aiming at all kinds of sparse matrix-vector multiplication (SPMV) operation. Conducted on IBM QS20,the parallelization approach can reach up to 21 and 133 times speedups with size A and B,respectively,compared with single power processor element. Finally,the conclusion is drawn that the peak bandwidth of memory access on Cell BE can be obtained in SPMV,simple computation is more efficient on heterogeneous processors and loop-unrolling can hide local storage access latency while executing scalar operation on SIMD cores. 展开更多

Status of Renewable Energy Applications in the Malaysian Energy Mix

This has created energy supply and environmental sustaina... Demand for power in Malaysia is projected to double the current demand in 2030. The current fuel mix for power generation is heavily dependent on fossil fuels. This has created energy supply and environmental sustainability concerns to the industry. Furthermore, at the recent United Nations Climate Change Conference 2009-15th Conference of Parties (COP15), Malaysia had given the commitment to voluntary reduction of up to 40% in terms of emissions intensity of GDP by the year 2020 compared to 2005 levels. Renewable energy (RE) provides the best alternative in addressing these issues. In 2001, RE was regarded as the fifth fuel in the new Five Fuel Strategy of the National Energy Policy. Small Renewable Energy Program (SREP) was launched with the main objective of supporting the government's strategy to intensify the development of RE as the fifth fuel resource. Biomass, the largest amongst the main RE resources available, can be used for heat and power generation. With optimum utilization, the value ofbiomass resources in Malaysia is estimated to be more than RM 500 billion (USD 156 billion) over the next 20 years. Malaysia has the capacity to use renewable energy resources to address the national energy agenda. 展开更多

Continuous usage of bioreactor causes early degradation of most bioreactor liner materials due to the effects of various chemicals, consequently resulting in contamination in the bioprocess. Performance of PP-ternary nanocomposite (PPTN) for its potential application in the fabrication of bioreactor liner material was investigated in this study. The chemical resistance of the composite samples obtained was tested by exposing them to chemicals such as acid, alkaline, water and bacterial solutions, according to ASTM 543-06, and their effects on the composite samples were carefully observed. Specifically, the investigation focused on the changes in the physico-mechanical properties of PPTN following long term of exposure to these chemicals. The results show slight increase in the weight and dimensions of samples in the first few days, followed by constant reading for the period of 4 weeks. The performance in terms of physical properties was in the range of PPTN with 0.61% MWCNT > PPTN 0.45% > PPTN 0.17%. The maximum percentage change in tensile properties, observed in this study, was approximately 10% against PPTN (0.17%), which indicates stable mechanical properties of the composite and invariably suggests that the nanocomposites could serve as a better alternative for bioreactor liner fabrication. 展开更多
