Although heart transplant is the "gold standard" therapy for HF, due to the limited availability of organs, many patients died when waiting for the transplant. Left ventricular assist device (LVAD), as a mechanical circulatory support, has become a new light for patients with HF. With the technical advancements, LVADs work not only as a bridge to transplant, but also assist heart recovery and even as a destination therapy in long-term treatment. This observation paper reviewed the development of LVAD and its clinical roles. The challenges and possible solutions in nursing care for pa- tients with LVAD at different stage of implantation were discussed. The healthcare professionals could obtain a better understanding about the LVAD treatment for HF patients.展开更多

Methods The pharmacological management of 677 patients (fema...Background To document the pharmacotherapy of chronic heart failure (CHF) and to evaluate the adherence to treatment guidelines in Australian population. Methods The pharmacological management of 677 patients (female 46.7%, 75.5 ±11.6 years) with CHF was retrospectively analyzed. Results The use of angiotensin converting enzyme (ACE) inhibitors/angiotensin receptor blockers (ARB) and β-blockers were 58.2 % and 34.7 %, respectively. Major reasons for non-use of ACE inhibitors/ARBs were hyperkalemia and elevated serum creatinine level. For patients who did not receive β-blockers, asthma and chronic obstructive pulmonary disease were the main contraindications. Treatment at or above target dosages for ACE inhibitors/ARBs and β-blockers was low for each medication (40.3% and 28.9%, respectively). Conclusions Evidenced-based medical therapies for heart failure were under used in a rural patient population. Further studies are required to develop processes to improve the optimal use of heart failure medications.展开更多

Two groups of patients were compared, depending on the treatment they have got group, which was taking 75 mg of enteric aspirin per day and the group, where pantoprazole has been added to aspirin in the usual dose. Mucosa assessment was studied using fibrogastroduodenoscopy and Lanza score. Also, the relationship between aggression factors that cause NSAID gastropathy and mucosal protection agents has been studied. It was proved that the pantoprazole influence reduces the ulcerative-erosive lesions amount. Stomach erosive lesions percentage decreased from 58.18% to 42.42%, stomach ulcers from 14.55% to 6.06%, duodenum erosive lesions decreased from 34.55% to 24.24%, duodenum ulcers deceased from 9.09% to 3.03%. A positive correlation between LTB4 and Lanza scale was checked after pantoprazole treatment, indicating an impact on the LTB4 reduction in ulcers healing in patients with NSAID gastropathy.展开更多
