This study aimed to perform the economic evaluation of lifelong annual influenza vaccination for cardiovascular events and well-established pneumonia prevention. Methods Lifetime costs, life-expectancy, and quality-adjusted live years (QALYs) were estimated beyond one-year cycle length of a six-health states Markov model condition on whether a hospitalization for ACS, stroke, heart failure, pneumonia, no hospitalizations occurred, or death. The comparison of three age-groups of 40-49, 50-65, and 〉 65 years scenario was performed. Incremental cost-effectiveness ratio (ICER) and net monetary benefit (NMB) were presented as a societal perspective in 2016. The model robustness was determined by one-way and prob- abilistic sensitivity analyses. Results The influenza vaccination was cost-effective in all age-groups, by dominant ICERs (lower cost with higher effectiveness) which was completely lower than acceptable willingness-to-pay threshold of Thailand [160,000 THB (4,466.8 USD) per QALYs], with a great incremental value of NMB. Especially, the 50-year-old-and- above scenario was shown as the most benefit at 129,092 THB (3,603.9 USD) for each patient. Conclusions The annually additional influenza vaccination to standard treatment in ACS was cost-effective in all age-groups, which should be considered in clinical practice and health-policy making process.展开更多

The combined structure of inducer, centrifugal impeller, and regenerative impeller is presented, and a theoretical parallel combinatorial hydraulic design method is investigated. The comparative experimental results show that efficiency in smaller capacity region, head coefficient and efficiency in larger capacity region of HCRPs is few lower, much higher and lower than those of high-speed centrifugal pumps, respectively, and that the suction performance of HCRPs is determined only by inducer. HCRPs can be more suitably applied to deliver small-capacity high-head liquids in chemical and petrochemical industries.展开更多

To make efficient use of data center resources, the key challenge is to map customer demands (modeled as virtual data center, VDC) to the physical data center effectively. In this paper, we focus on this problem. Distinct with previous works, our study of VDC embedding problem is under the assumption that switch resource is the bottleneck of data center networks (DCNs). To this end, we not only propose relative cost to evaluate embedding strategy, decouple embedding problem into VM placement with marginal resource assignment and virtual link mapping with decided source-destination based on the property of fat-tree, but also design the traffic aware embedding algorithm (TAE) and first fit virtual link mapping (FFLM) to map virtual data center requests to a physical data center. Simulation results show that TAE+FFLM could increase acceptance rate and reduce network cost (about 49% in the case) at the same time. The traffie aware embedding algorithm reduces the load of core-link traffic and brings the optimization opportunity for data center network energy conservation.展开更多

The concern about the local adverse effects of intramus-cular injection, particularly in CVD patients receiving antithrombotic therapy, is one of the important impediments. This study was con-ducted to assess the safety, side effects and tolerability of intradermal influenza vaccine in CVD patients. Methods This was an observa-tional study in adult CVD patients who had undergone vaccination against seasonal influenza by intradermal vaccination between May 16th and May 30th, 2012 at Maharaj Nakorn Chiang Mai Hospital. The medical history, patients' acceptability and adverse effects were collected using a written questionnaire completed by the patient immediately following vaccination and by a telephone survey eight days later. Results Among 169 patients, 52.1%were women and the mean age was 63 ± 12 years. Coronary artery disease, valvular heart disease and dilated cardiomyopathy were present in 121 (71.6%), 40 (23.7%) and 8 (4.7%), respectively. Antithrombotics were used in 89.3%. After vaccination, the pain score was 0, 1 or 2 (out of 10) in 44.4%, 15.1%, and 27.6%of the patients, respectively. Eight days after vaccination, the common adverse reactions were itching 19 (11.9%), swelling 9 (5.7%) and fatigue (4.7%). No hematoma or bruising was reported. Conclusions The intradermal influenza vaccination is safe and well tolerates with high rates of satisfaction in CVD patients. This technique should be useful in expanding influenza vaccine coverage.展开更多

Eccentricity related faults as well as clutch wobbling constitute major portions of the faults related to induction motors. This paper presents the effect of clutch wobbling and mixed eccentricity on induction machine stator currents and the possibility of distinguishing between each ailment via comparing the abnormal harmonics contained in stator current spectrums in each case. In this paper, the current spectrum of a four pole-pairs, 550 kW, induction machine were calculated for the cases of full symmetry, clutch wobbling, and mixed eccentricity. The calculations involve integration of 93 electrical plus four mechanical ordinary differential equations. Electrical equations account for variable inductances affected by slotting and eccentricities. The calculations were followed by Fourier analysis of the stator currents. Zooms of the current spectrums, around the 50 Hz fundamental harmonic as well as of the main slot harmonic zone, of both calculated and measured currents are included. The spectrums presented in this paper rely on calculations that are performed with dedicated software which is owned by AGH University of Science and Technology.展开更多
