目的:观察早期拔管加面罩辅助呼吸用于预防气管拔管时心血管反应。方法:将40例ASAⅠ-Ⅱ级择期普胸手术患者,随机分为正常组和早拔组。结果:早拔组拔管前后心血管反应较轻,正常组拔管前后心血管反应较重。收缩压(SBP)、舒张压(DB... 目的:观察早期拔管加面罩辅助呼吸用于预防气管拔管时心血管反应。方法:将40例ASAⅠ-Ⅱ级择期普胸手术患者,随机分为正常组和早拔组。结果:早拔组拔管前后心血管反应较轻,正常组拔管前后心血管反应较重。收缩压(SBP)、舒张压(DBP)、HR与拔管前5 min比较差异有统计学意义(P〈0.05和P〈0.01)。结论:早期拔管加面罩辅助呼吸能有效预防气管拔管时的心血管反应。 展开更多

迷走神经对心血管活动通常呈现抑制性影响,但许多动物实验观察证实,迷走神经对心血管活动还具有兴奋性影响,当刺激迷走神经外周端时,可看到血压升高,或在刺激停止后出现短时期的窦性心动过速,Copen等称之为迷走后加速(Postvagal tachyca... 迷走神经对心血管活动通常呈现抑制性影响,但许多动物实验观察证实,迷走神经对心血管活动还具有兴奋性影响,当刺激迷走神经外周端时,可看到血压升高,或在刺激停止后出现短时期的窦性心动过速,Copen等称之为迷走后加速(Postvagal tachycardia,PVT)。关于这种迷走心血管兴奋效应的机理,目前有以下一些认识:①乙酰胆硷对心肌有直接兴奋作用,②迷走神经的胆硷能效应,③迷走神经通过乙酰胆硷转而引起儿茶性胺释放的肾上腺素能机制。迷走心兴奋作用可能与临床所见心绞痛在晚间发作有关,是值得进一步研究的问题。 展开更多

The roles of androgens on cardiovascular physiology and pathophysiology are controversial as both beneficial and detrimental effects have been reported. Although the reasons for this discrepancy are unclear, multiple factors such as genetic and epigenetic variation, sex-specificity, hormone interactions, drug preparation and route of administration may contribute. Recently, growing evidence suggests that androgens exhibit beneficial effects on cardiovascular function though the mechanism remains to be elucidated. Endothelial cells (ECs) which line the interior surface of blood vessels are distributed throughout the circulatory system, and play a crucial role in cardiovascular function. Endothelial progenitor cells (EPCs) are considered an indispensable element for the reconstitution and maintenance of an intact endothelial layer. Endothelial dysfunction is regarded as an initiating step in development of atherosclerosis and cardiovascular diseases. The modulation of endothelial functions by androgens through either genornic or nongenomic signal pathways is one possible mechanism by which androgens act on the cardiovascular system. Obtaining insight into the mechanisms by which androgens affect EC and EPC functions will allow us to determine whether androgens possess beneficial effects on the cardiovascular system. This in turn may be critical in the prevention and therapy of cardiovascular diseases. This article seeks to review recent progress in androgen regulation of endothelial function, the sex-specificity of androgen actions, and its clinical applications in the cardiovascular system. 展开更多
