目的利用加权逼近理想值排序法(TOPSIS)进行泮托拉唑注射液的药物利用分层评价。方法利用合理用药监测系统随机抽取使用泮托拉唑注射液的患者的出院病历,结合药物利用评价(DUE法)建立评价标准,利用层次分析模型(AHM)赋权法计算评价指标... 目的利用加权逼近理想值排序法(TOPSIS)进行泮托拉唑注射液的药物利用分层评价。方法利用合理用药监测系统随机抽取使用泮托拉唑注射液的患者的出院病历,结合药物利用评价(DUE法)建立评价标准,利用层次分析模型(AHM)赋权法计算评价指标的相对权重,并运用TOPSIS数据处理法进行计算、患者属性分层和排序,评价各病历医嘱的合理性。结果300份病历与完全合理值之间的距离Ci=0.7393,Ci≥0.6的病历占70.3%,应用基本合理。按适应证分层排序,Ci≥0.6的适应证有10个;Ci<0.6的适应证有2个,为手术预防应激性溃疡和防止胃酸反流合并吸入性肺炎。按年龄分层,超高龄和高龄患者应用最合理。结论运用加权TOPSIS法对泮托拉唑注射液的使用情况进行整体排序、按患者属性分层排序,并评价是合理可行的。本院在泮托拉唑的应用超高龄和高龄患者的治疗性应用合理性更高,大部分应用基本合理,但仍存在很多问题。 展开更多

Personal health record (PHR) enables patients to manage their own electronic medical records (EMR) in a centralized way, and it is oRen outsourced to be stored in a third-party server. In this paper we propose a novel secure and scalable system for sharing PHRs. We focus on the multiple data owner scenario, and divide the users in the system into multiple security domains that greatly reduce the key management complexity for owners and users. A high degree of patient privacy is guaranteed by exploiting hierarchical and multi- authority attribute-sets based encryption (HM- ASBE). Our system not only supports compound attributes due to flexible attribute sets combinations, but also achieves fine-grained access control. Our scheme supports efficient on-demand user/attribute revocation and break-glass access under emergency scenarios. 展开更多
