Parentral treatment of Egyptian patients of bilharziasis contributed to the high incidence of viral hepatitis, and subsequently liver cirrhosis and HCC. CapecJtabJne plus cJsplatJn protocol was evaluated regarding the efficacy and safety in patients with advanced HCC as first line chemotherapy. Methods: One hundred patients with advanced HCC were randomized to receive either capecitabine (1000 mg/m2) twice daily for fourteen days plus intravenous cisplatin (60 mg/m2) on day one to be repeated every three weeks with a maximum of six cycles or placebo (phase III trial). Results: Baseline characteristics were comparable in both groups. According to Barcelona Clinic Liver Cancer Staging System, stage C was the most predominant (82% vs. 75%) in both groups. Median OS was 12 months versus 10 months in favor of the treated group (P value 〈 0.05). Median TTP was significantly higher in the chemotherapy group (7 months vs. 4.5 months) as well as disease control rate (40% vs. 29%), no patient had achieved complete response. Grade 3 toxicity was more pronounced in the treatment group, as regards vomiting and diarrhea (10% vs. 2%), neurotoxicity (6% vs. 2%), elevation of aminotransferase and bilirubin (9.8% vs. 4.9%), hand and foot syndrome reaction was recorded only in chemotherapy group. Conclusion: Capecitabine plus cisplatin regimen showed modest antitumor activity with tolerable toxicity in patients with advanced HCC. Moreover, because of the significantly prolonged time to progression, we demand further attention to this convenient, outpatient, and economic profile based chemotherapy protocol.展开更多

Brain metastasis(BM) is a common complication of NSCLC, with 25%–40% of patients developing BM during the course of the disease. A significant strategy of local disease control in the central nervous system is radiation therapy. With the development of precision medicine,the concept of treating lung cancer BM has gradually changed. In this case, we performed a surgical procedure to obtain enough tumor tissue for the detection of the target gene and other related experiments after the patient was informed. Finally, we found that the patient had both hepatocyte growth factor receptor(MET) gene amplification and kinesin light chain 1-anaplastic lymphoma kinase fusion(KLC1-ALK) through next-generation sequencing and showed sensitivity to the targeted therapy of crizotinib. The patient exhibited good response. Our case was successful and underwent targeted therapy with the guidance of precise diagnosis.展开更多

NF (non-specific factors) involved in treatment are all those effects that do not depend on the pharmacological properties of the drug. Materials and Methods: The job lasted a year. The sample consisted of patients with mental health disorders and was divided into two groups. Treatment compliance was measured with the Morisky-Green Test. Results were compared using the chi square test and relative risk. Results and Discussion: Group A had 23 patients ending 91.3% of them and group B of 22 patients, ending the 77.27%. At the beginning of the study, we found in the group A 0.86 NF/patients while in the group B 0.82 NF/patient. At the end of the study there was a 54.65% decrease in group A while in group B the proportion remained. At the beginning of the study both groups had approximately 40% of compliant patients. Data that remained in the control group rose to 80.95% in group A at the end of the study. Conclusions: The work demonstrates the negative influence of these factors on adherence to treatment.展开更多

Due to the high prevalence of depression in Bosnia and Herzegovina and high work load of psychiatrists, patients that suffer from depression are often marginalised, stigmatised and inadequately treated. Therefore, a pharmacist can influence an adequate selection of a therapy through an interdisciplinary approach and pharmaceutical care and alos provide patient monitoring, counselling about side effects and interactions of the prescribed drugs, which will in the end improve the quality of life for patients that suffer from depression. The aim of this work was to establish the influence of pharmaceutical care on the outcome of the quality of life and the pharmacological treatment of patients suffering from depression. Results showed that the involvement of pharmacist in a multidisciplinary approach to the treatment of depression lead to improvement of the effect of therapy and increased quality of life.展开更多
