Both breath test and routine liver test, serum levels of cholic and chenodeoxycholic acid conjugates were evaluated.RESULTS: Methacetin breath test, expressed as 60 min cumulative percent of oxidation, discriminated the hepatic functional capacity not only between controls and liver disease patients, but also between different categories of chronic liver disease patients. Methacetin breath test was correlated with liver function tests and serum bile acids.Furthermore, methacetin breath test, as well as serum bile acids, were highly predictive of Child-Pugh scores. The diagnostic power of phenylalanine breath test was always less than that of methacetin breath test.CONCLUSION: Methacetin breath test represents a safe and accurate diagnostic tool in the evaluation of hepatic functional mass in chronic liver disease patients.展开更多

Methods:Respiratory impedance was measured by impulse oscillometry (lOS) in 4...Objective :To evaluate the applicable value of respiratory impedance for the stage 0 of chronic obstructive pulmonary disease (COPD). Methods:Respiratory impedance was measured by impulse oscillometry (lOS) in 41 cases at stage 0 of COPD (NHLBI/WHO, 2001 Standard) and their conventional pulmonary function values were compared with 42 healthy subjects. Both groups had no significant deviation in age, stature and avoirdupois, etc. Master-Screen pulmonary function test system (Jaeger Co, Germany) were used to determine IOS parameters including viscous resistance of 5Hz, 20 Hz and 35Hz (R5, R20, R35), reactance of 5Hz, 35 Hz (X3, X35), resonant frequency (Fres) ,total respiratory impedance (Zrs) and routine pulmonary function values including forced expiratory volume in one second to predicted value (FEV1 % ), forced expiratory volume in one second to forced vital capacity ratio( FEV1/ FVC% ), maximal mid expiratory flow (MMEF%),V23% and V50%. Results:Both groups had no significant deviation in FEV1%, FEV1/FVC% and X35(P〉0.05). It was increased significantly in viscous resistance of 5Hz, 20 Hz and 35Hz (R5, R20, R35) in COPD group than that in healthy group (P〈0.01). So were Fres and Zrs (P〈0.01). MMEF%,V25%,V50% and reactance of 5Hz (X5) in COPD group were marked lower than that in healthy group (P〈0.01). The sensitivity of MMEF%, V25%,V50% was higher than others, but its specificity was lower. In parameters of IOS, Fres was the most sensitive index for diagnosis of the small airway function and its specificity was higher than that of MMEF%, V25% ,V50%. Conclusion:In the risk case at the stage 0 of COPD, MMEF%, V25% and V50% could be decreased, but Fres, R5 ,R5-20 could be increased in spite of FEV1% and FEV1/FVC% in normal range.展开更多
OBJECTIVE: To investigate the effects of Yiqibushenhuoxue decoction on stable chronic obstructive pulmonary disease(COPD) by observing its influences on patients' quality of life and airway inflammation.METHODS: S...OBJECTIVE: To investigate the effects of Yiqibushenhuoxue decoction on stable chronic obstructive pulmonary disease(COPD) by observing its influences on patients' quality of life and airway inflammation.METHODS: Seventy patients with stable COPD were randomly divided into a treatment group(n=35) treated with Yiqibushenhuoxue decoction plus Seretide and a control group(n=35) treated with Seretide only.The dosage of Yiqibushenhuoxue decoction was 100 mL each time, twice a day, and the dosage of Seretide was salmeterol 50 μg/fluticasone 250 μg twice a day. Both groups were treated for 12 weeks. Before and after the treatment, St George's respiratory disease questionnaire(SGRQ)scores,forced expiratory volume,and forced expiratory volume in 1 second/forced vital capacity(FEV1/FVC)were measured.RESULTS:The SGRQ scores in both groups were significantly lower than those before treatment(P<0.05). After treatment, the total SGRQ scores and each subscore in the treatment group were significantly lower than those in the control group(P<0.05). The percentage of the predicted FEV1% and FEV1/FVC were higher in both groups, but no statistical differences were detected from before to after the treatment or between the two groups(P>0.05).CONCLUSION: Yiqibushenhuoxue decoction could significantly decrease the SGRQ scores in patients with stable COPD, which suggests that it is able to improve patient symptoms.展开更多
Objective: To observe the clinical effect of acupuncture on chronic obstructive pulmonary disease (COPD), and the improvements of patients' pulmonary ventilation function and 6-minute walk test (6-MWT) distance....Objective: To observe the clinical effect of acupuncture on chronic obstructive pulmonary disease (COPD), and the improvements of patients' pulmonary ventilation function and 6-minute walk test (6-MWT) distance. Methods: A total of 80 COPD patients [grade 3-4 in Global Initiative for Chronic Obstructive Lung Disease (GOLD), qi deficiency of the lung and kidney in traditional Chinese medicine (TCM) pattern] were randomly allocated into a treatment group (n=40) and a control group (n=40). Salmeterol Xinafoate and Fluticasone Propionate powder (Seretide, 50 μg/250 μg) for inhalation was used for basic treatment in both groups (once in the morning and once in the evening). Patients in the treatment group received acupuncture at Feishu (BL 13), Shenshu (BL 23), O.ihai (CV 6), Guanyuan (CV 4), Dingchuan (EX-B 1), Danzhong (CV 17) and Zusanli (ST 36] twice a week for 3 months. After 3 months of treatment, clinical effects, lung ventilation functions and 6-MWT distance were observed and compared in the two groups. Results: After 3 months of treatment, the total effective rate was 95.0% in the treatment group, versus 80.0% in the control group, showing a statistical difference (P〈O.05); the phlegm expectoration, dyspnea and shortness of breath were more significantly improved in the treatment group than those in the control group (P〈0.01, P〈0.05); and the 6-MWT distance and forced expiratory volume in 1 second percentage of predicted value (FEV1%) were more significantly improved in the treatment group than those in the control group (P〈0.05, P〈0.01). Conclusion: Seretide inhaler combined with acupuncture can improve signs and symptoms in COPD patients, increase the 6-MWT distance, imorove FEV1% and obtain better results than Seretide alone.展开更多
OBJECTIVE:To examine the clinical effects of a mixture of Chinese Yam and Epimedium in patients with stable moderate or severe chronic obstructive pulmonary disease(COPD).METHODS:Forty-nine patients with COPD were ran...OBJECTIVE:To examine the clinical effects of a mixture of Chinese Yam and Epimedium in patients with stable moderate or severe chronic obstructive pulmonary disease(COPD).METHODS:Forty-nine patients with COPD were randomly allocated to a group whose usual treatment was supplemented with oral Chinese Yam-Epimedium mixture,or a control group given placebo.For each patient,body mass index,airflow obstruction,dyspnea,and exercise capacity were measured and converted into the BODE index before treatment and at one and three months after initiation of treatment.Participants also completed the St George's Respiratory Questionnaire(SGRQ) at the same intervals.RESULTS:After one month,improvements were seen in the BODE index and SGRQ of participants taking Chinese Yam-Epimedium mixture compared to controls.There were statistically significant differences in the SGRQ:three of its components and the total SGRQ scores were significantly decreased(P<0.05),respiratory symptom scores had improved(P<0.01),and the dyspnea component of the BODE index had significantly decreased(P<0.05).Similar improvements were observed after three months of treatment,but exercise tolerance had also improved:the six-minute walking distance had significantly increased(P<0.05) in the treatment group when compared with controls.CONCLUSION:Chinese Yam-Epimedium mixture can significantly improve dyspnea,exercise capacity,and the quality of life of patients with stable moderate or severe COPD.展开更多

Cases in the control group received routine Western medical treatment, whereas cases in the treatment group conducted breathing and Daoyin exercises in addition to routine Western medical treatment. Patients in both groups were treated for a total of 3 months. Then the observation was made on changes in pulmonary ventilation function, major clinical symptoms, modified Medical Research Council scale(m MRC), distance in 6-minute walk test(6-MWT), COPD assessment test(CAT) and efficacy satisfaction questionnaire for COPD(ESQ-COPD) before and after treatment. Results: After treatment, the total effective rate was 80.0% in the treatment group, versus 66.7% in the control group, showing a statistical difference(P〈0.05). Patients in the treatment group obtained more significant improvement in coughing, sputum production, dyspnea and shortness of breath than those in the control group(P〈0.05). Patients in the treatment group obtained more significant elevation in the forced expiratory volume in 1 second percentage of predicted value(FEV1%) and peak expiratory flow rate(PEF%) than those in the control group(P〈0.05). Patients in the treatment group obtained lower m MRC score than those in the control group(P〈0.05). Patients in the treatment group obtained longer 6-MWT distance than those in the control group(P〈0.05). Patients in the treatment group obtained lower CAT score(P〈0.01) and higher ESQ-COPD score(P〈0.05) than those in the control group. Conclusion: Breathing and Daoyin exercises combined with routine Western medical treatment are effective for stable COPD(grade II-III) due to deficiency of the lung and kidney and can improve the patients' quality of life.展开更多
