布地奈德治疗老年慢性肺阻型疾病的临床效果观察

目的:探讨布地奈德治疗对慢性肺阻型疾病的临床疗效。方法:选取我院呼吸内科于2017年8月至2019年1月收治的老年慢性阻塞性肺疾病患者84例,随机分为观察组与对照组,每组42例;对照组患者进行常规药物治疗,察组患者在对照组基础上使用布地... 目的:探讨布地奈德治疗对慢性肺阻型疾病的临床疗效。方法:选取我院呼吸内科于2017年8月至2019年1月收治的老年慢性阻塞性肺疾病患者84例,随机分为观察组与对照组,每组42例;对照组患者进行常规药物治疗,察组患者在对照组基础上使用布地奈德气雾剂,每日3次,每次2揿,二组连续治疗1周;观察临床疗效,检测患者治疗前后动脉血气变化。结果:临床疗效结果显示观察组显效5例,无效18例,总有效率达到57.1%,而对照组显效仅有2例,无效23例,总有效率为45.2%,二组数据差异有统计学意义(P<0.05);二组治疗后PaO2较治疗前显著提高,而PaCO2指标显著下降,二组治疗后差异比较有统计学意义(P<0.05)。结论:布地奈德对老年慢性阻塞性肺病患者疗效显著,有效提高治疗效率,值得临床应用。 展开更多

OBJECTIVE: To observe effect of Liuweibuqi Capsule, a Traditional Chinese Medicine (TCM), on the janus kinase (JAK)/signal transducer and activator of transcription (STAT) pathway and matrix metalloproteinases (MMPs) in a chronic obstructive pulmonary disease (COPD) rat model with lung deficiency in terms of TCM's pattern differentiation. METHODS: Rats were randomly divided into a normal group, model group, Liuweibuqi group, Jinshuibao group, and spleen aminopeptidase group (n= 10). Aside from the normal group, all rats were ex-posed to smoke plus lipopolysaccharide tracheal instillation to establish the COPD model with lung deficiency. Models were established after 28 days and then the normal and model groups were given normal saline (0.09 g/kg), Liuweibuqi group was given Liuweibuqi capsule (0.35 g/kg), Jinshuibao group was given Jinshuibao capsules (0.495 g/kg), and the spleen group was given spleen aminopeptidase (0.33 mg/kg), once a day for 30 days. Changes in symptoms, signs, and lung histology were observed. Lung function was measured with a spirometer. Serum cytokines were detected using enzyme-linked immunosorbent assay, and changes in the JAK/STAT pathway, MMP-9, and MMPs inhibitor 1 (TIMP1) were detected by immunohistochemistry, RT-PCR, and western blotting, respectively.RESULTS: Compared with the normal group, lung tissue was damaged, and lung function was reduced in the model control group. Additionally, the levels of interleukin (IL)-1β, y interferon (IFN-γ), and IL-6 were higher, while IL-4 and IL-10 were lower in the model control group than those in the normal group. The expressions of JAK1, STAT3, ρ-STAT3, and MMP-9 mRNA and protein in lung tissue were higher, and TIMP1 mRNA and protein was lower in the model group compared with the normal group. After treatment, compared with the model group, the expression of inflammatory cytokines was lower in each treatment group, and expressions of JAK/ STAT pathway, MMPs were lower. Compared with the positive control groups, the Jinshuibao and spleen aminopeptidase groups, lung function was better, and JAK1, STAT3, and p-STAT3 protein were lower and TIMP1 was higher in the Liuweibuqi group.CONCLUSION: Liuweibuqi capsules can improve the symptoms of COPD possibly by regulating the expression of the JAK1/STAT3 pathway and MMP9/ TIMP1. 展开更多

OBJECTIVE: To study the effects of cold-dryness on pulmonary and immunologic function of peripheral T-lymphocytes in chronic obstructive pulmonary disease (COPD) model rats, and to provide references for the prevention and treatment of cold-dryness COPD in the Xinjiang region. METHODS: The COPD model was established with an elastase drip into the trachea combined with smoking. The cold-dryness COPD model was developed by stressing with a cold-dry environment. Success of the model was determined by observation of pathologic lung sections. Rats were sacrificed by exsanguination from the femoral artery and changes of peripheral blood CD4+, CD8+, and CD4+/CD8+ were detected by flow cytometryo Data were analyzed with SAS 11.5 statistical software. RESULTS: On the ninetieth day after ending the ex- periment, Peak expiratory flow in the cold-dryness COPD group was lower than that in the COPD and normal control groups (P〈0.01). The time of inspiration in the cold-dryness COPD group was higher than that in the COPD and normal groups (P〈0.05). Time of expiration (Te) in the cold-dryness COPD group was higher than that in the COPD and normal groups (P〈0.01). 50% tidal volume expiratory flow (EFS0) in the cold-dryness COPD group was lower than that in the COPD and normal groups (P〈 0.01), and EFS0 in the COPD group was lower than that in the normal group (P〈0.05). CD4+ content of peripheral blood in the cold-dryness COPD group was lower than that in the COPD and the normal groups (P〈0.05). CD8+ content in the cold-dryness COPD and COPD groups was higher than that in the normal control group (P〈0.01), and CD8+ content in the cold-dryness COPD group was higher than that in the COPD group (P〈0.01). CD4+/CD8+ in the cold-dryness COPD group and the COPD group was lower than that in the normal control group (P〈0.01), and CD4+/CD8+ in the cold-dryness COPD group was lower than that in the COPD group (P〈0.05). CONCLUSION: In the cold-dryness COPD model, CD8+ increased and CD4+/CD8+ decreased. Moreover, cold-dryness may aggravate this state. The effects of cold-dryness on pulmonary function main- ly manifested as prolongation of Te and decrease of EF50, which could be one of causes of cold-dryness environment in the northwest of China leading to COPD with region characteristics. 展开更多
