There are no screening recommendations for patients unless they have Barrett's esophagitis or a significant family history of this disease. Often, esophageal cancer is not diagnosed until patients present with dysphagia, odynophagia, anemia or weight loss. When symptoms occur, the stage is often stage Ⅲ or greater. Treatment of patients with very early stage disease is fairly straight forward using only local treatment with surgical resection or endoscopic mucosal resection. The treatment of patients who have locally advanced esophageal cancer is more complex and controversial. Despite multiple trials, treatment recommendations are still unclear due to conflicting data. Sadly, much of our data is difficult to interpret due to many of the trials done have included very heterogeneous groups of patients both histologically as well as anatomically. Additionally, studies have been underpowered or stopped early due to poor accrual. In the United States, concurrent chemoradiotherapy prior to surgical resection has been accepted by many as standard of care in the locally advanced patient. Patients who have metastatic disease are treated palliatively. The aim of this article is to describe the multidisciplinary approach used by an established team at a single high volume center for esophageal cancer, and to review the literature which guides our treatment recommendations.展开更多

Methods Seventy-five cases were randomized into 3 groups. Of which, in the combined therapy group of moxibustion and medicine (22 cases), the pricking moxibustion and routine western medicine treatment were provided in combination; in the Chinese herbal medicine group (29 cases), oral administration of Chinese herbal medicine and routine western medicine treatment were provided; in the western medicine group (24 cases), routine western medicine treatment was provided. After 7 days treatment, the therapeutic effects of 3 groups were judged by comparisons of skin rush, oral mucosa herpes, constipation (or loose stools), indigestion and anorexia, and sore throat. Results The total effective rate was 95.5% in combined therapy group of moxibustion and medicine, 86.2% in Chinese herbal medicine group, 83.3% in western medicine group. The total effective rate in combined therapy group was superior to that in either Chinese herbal medicine group or western medicine group (both P〈0.05). Comparing the fading durations of skin rush, oral mucosa herpes, constipation (or loose stools), indigestion and anorexia between combined therapy group and other two groups, significant differences were presented statistically (both P〈0. 05). Conclusion In the combined therapy group of moxibustion and medicine, the symptoms of alimentary canal were relieved, the disease courses were shortened and the pains were reduced in hand-foot-mouth disease patients.展开更多

In the acupuncture group, after chemotherapy, acupuncture was given at Bǎihui(百会 GV 20), Hégǔ(合谷 LI 4), Wàiguān(外关 TE 5), Zúsānlǐ(足三里ST 36) and Ashi points. The needles were retained for 30 min in each treatment, once a day, totally for 2 weeks. In the Vitamin B6 group, after chemotherapy, Vitamin B6 was taken orally, 300 mg a day, totally for 2 weeks. Separately, on the day of enrollment and after 2 weeks of treatment, the scores of Karnofsky performance scale(KPS) and the quality of life(QLQ-C30) were evaluated in the acupuncture group and the Vitamin B6 group. After 2 weeks of treatment, the therapeutic effects were evaluated. Results: The effective rate of chemotherapy-induced HFS was 70.0% in the acupuncture group and was36.7% in the Vitamin B6 group. The result in the acupuncture group was higher than the Vitamin B6 group, indicating the significant difference(P0.05). After treatment, the KPS scores(84.23 ± 10.49) in the acupuncture group were higher obviously than the Vitamin B6 group(77.84 ± 10.33), indicating the significant difference(P0.05). Regarding QLQ-C30 evaluation, after 2-week treatment, the scores in the items of physical function, role function, emotional function, cognitive function and social function were all reduced remarkably as compared with those before treatment in the two groups, indicating the significant differences(all P〈0.05). After treatment, the scores of physical function(7.13 ± 1.56 vs 16.72 ± 4.29),emotional function(4.89 ± 0.87 vs 11.76 ± 1.29) and cognitive function(6.32 ± 1.23 vs 18.17 ± 4.1) in the acupuncture group were all lower obviously than those in the Vitamin B6 group, indicating the significant differences(all P0.05); and the scores of role function and social function were not different significantly as compared with the VB6 group(all P〈0.05).Conclusion: Acupuncture is effective for HFS induced by the chemotherapy with capecitabine and its effects are better than the oral administration of Vitamin B 6 in terms of physical function, emotional function and cognitive function.展开更多

Methods: Male SD rats anesthetized with chloral hydrate were randomly divided into 3 groups which were treated with manual acupuncture (MA), injection of lidocaine followed by MA and injection of normal saline (NS) followed by MA, respectively. The right carotid artery was canulated for persistent measurement of the blood pressure and meanwhile nerve discharges from the nerve-tract were recorded for analysis with amplitude spike counts for every 5 s. Results: The results showed that there were significant nerve discharges recorded from the nerve-tract when applying MA at Zusanli (ST 36) and simultaneous decrease in the MAP, while there was no response when inserting a needle into the Zusanli (ST 36) without manipulation (P〈0.05). Furthermore, the reduction of MAP during MA could be completely abolished after blockade of peripheral nerve discharges with an injection of lidocaine into the tissue around Zusanli (ST 36) but not with that of normal saline (NS). Conclusion: These results indicate that MA at Zusanli (ST 36) can elicit the peripheral nerve discharges from the nerve innervating the acupoint; such kind of nerve discharges may contain acupuncture signal regulating blood pressure via somato-cardiovascular reflex.展开更多

Mild moxibustion scale was used to count the score. Results: Warm was the main moxibustion sensation, burning pain and soreness decreased with the rise of distance; for the same acupoint, score of mild moxibustion scale increased with the decrease of distance; score ranged between 5.5 and 6.5at distance 3 cm, which was the most comfortable distance for volunteers. Conclusion: The distance of 3 cm is the most comfortable distance in mild moxibustion.展开更多

The observation group was intervened by ear acupuncture plus injection at Zusanli(ST 36), and the control group was intervened by oral administration of Ibuprofen, 10 d as a treatment course. The clinical efficacies of the two groups were compared after 2 treatment courses; the visual analogue scale(VAS),present pain intensity(PPI) and 36-item short-form health survey(SF-36) were measured before and after the treatment;the changes of interleukin(IL)-6 and IL-10 after the treatment were also observed. Results: The VAS and PPI scores were significantly changed after the treatment in both groups(both P〈0.01). After the treatment, the VAS score in the observation group was significantly different from that in the control group(P〈0.05). The component scores of SF-36 were significantly changed after the treatment in both groups(P〈0.01); after the treatment, the scores of physical functioning(PF), bodily pain(BP), social functioning(SF), and mental health(MH) in the observation group were significantly different from those in the control group(all P〈0.05). The contents of IL-6 and IL-10 dropped significantly after the intervention in both groups(both P〈0.01), and the between-group differences were also statistically significant(both P〈0.01). The total effective rate of the observation group was higher than that of the control group(P〈0.05).Conclusion: Ear acupuncture plus injection at Zusanli(ST 36) can significantly improve the shoulder pain after laparoscopic gynecological surgery, down-regulate the expressions of IL-6 and IL-10, and boost the recovery.展开更多
