Objective: To analyze the tolerance of lung cancer patients with moderate pulmonary hypofunction to open-chest operation. Methods: The postoperative complications of 31 patients with moderate pulmonary hypofunction ...Objective: To analyze the tolerance of lung cancer patients with moderate pulmonary hypofunction to open-chest operation. Methods: The postoperative complications of 31 patients with moderate pulmonary hypofunction (study group) and 62 patients with normal pulmonary function (control group) were reviewed. Statistical t test and χ^2 test were set to examine the data, and logistic regression was performed to find the associated factors. Results: In patients with moderate pulmonary hypofunction, hypoxemia (41.9%), arrhythmia and cardiac dysfunction (25.8%), and pulmonary inflammation (25.8%) were common postoperative complications, and respiratory failure and cardiac failure (9.2%) severe complications. In-hospital mortality rate was 3.2%. In the control group, the incidence of the above 4 postoperative complications was 16.1%, 8.1%, 9.6% and 3.2% accordingly, and no case died in hospital. Regression analysis showed that age and resection range were associated with the occurrence of the complications. Conclusion: Common postoperative complications (hypoxemia) in patients with moderate pulmonary hypofunction are remarkably higher than those in control group, but severe complications and in-hospital mortality rate are not significantly high. Patients with older age and larger resection range undergo higher complication rate.展开更多

Methods: The clinical data and endocrine hormones level of 25 patients with acute pituitary apoplexy who underwent transsphenoidal surgery from Jan. 2002 to June 2004 were retrospectively analyzed. Results: 13 cases underwent surgery within 3 days after admission and 22 cases within 1 week. Of the 25 cases, 9 patients suffered the impairment of pituitary-thyroidal function, 14 cases of pituitary-adrenal function and 11 cases of pituitary-gonadal function before surgery. After surgery, 5/9, 8/14 and 7/11 were recovered from the corresponding hypopituitarism. Conclusion: Hypopituitarism is a major manifestation of acute pituitary apoplexy. Urgent surgery decompression contributed to the improvement of pituitary function. Patients with hypopituitarism after surgery required the corresponding hormones replacement therapy.展开更多

This procedure has been performed by select surgeons at specialized centers and concerns over local recurrence, sexual dysfunction and appropriate training measures have further hindered widespread acceptance. Data for laparoscopic rectal resection now supports its continued implementation and widespread usage by expeienced surgeons for select patients. AIM: To report our experience using mini-laparotomy for the resection of rectal cancer using the total meso- rectal excision (TME) technique, METHODS: Consecutive patients with rectal cancer who underwent anal-col...AIM: To report our experience using mini-laparotomy for the resection of rectal cancer using the total meso- rectal excision (TME) technique, METHODS: Consecutive patients with rectal cancer who underwent anal-colorectal surgery at the authors' hospital between March 2001 and June 2009 were included, In total, 1415 patients were included in the study, The cases were divided into two surgical proce- dure groups (traditional open laparotomy or mini-lap- arotomy), The mini-laparotomy group was defined as having an incision length ≤ 12 cm. Every patient un- derwent the TME technique with a standard operation performed by the same clinical team. The multimodal preoperative evaluation system and postoperative fast track were used. To assess the short-term outcomes, data on the postoperative complications and recovery functions of these cases were collected and analysed. The study included a plan for patient follow-up, to ob- tain the long-term outcomes related to 5-year survival and local recurrence. RESULTS: The mini-laparotomy group had 410 pati- ents, and 1015 cases underwent traditional laparoto- my. There were no differences in baseline characteris- tics between the two surgical procedure groups. The overall 5-year survival rate was not different between the rnini-laparotorny and traditional laparotorny groups (80.6% vs 79.4%, P = 0.333), nor was the 5-year local recurrence (1.4% vs 1.5%, P = 0.544). However, 1-year mortality was decreased in the rnini-laparotorny group compared with the traditional laparotorny group (0% vs 4.2%, P 〈 0.0001). Overall 1-year survival rates were 100% for Stage Ⅰ, 98.4% for Stage Ⅱ, 97.1% for Stage Ⅲ, and 86.6% for Stage Ⅳ. Local recurrence did not differ between the surgical groups at 1 or 5 years. Local recurrence at 1 year was 0.5% (2 cases) for mini-laparotorny and 0.5% (5 cases) for traditional laparotorny (P = 0.670). Local recurrence at 5 years was 1.5% (6 cases) for mini-laparotorny and 1.4% (14 cases) for traditional laparotorny (P = 0.544). Days to first ambulation (3.2 ± 0.8 d vs 3.9 ± 2.3 d, P = 0.000) and passing of gas (3.5 ± 1.1 d vs 4.3 ± 1.8 d, P = 0.000), length of hospital stay (6.4 ± 1.5 d vs 9.7 ± 2.2 d, P = 0.000), anastomotic leakage (0.5% vs 4.8%, P = 0.000), and intestinal obstruction (2.2% vs 7.3%, P = 0.000) were decreased in the rrnini-laparotorny group compared with the traditional laparotorny group. The results for other postoperative recovery function indi- cators, such as days to oral feeding and defecation, were similar, as were the results for immediate post- operative complications, including the physiologic and operative severity score for the enumeration of mortal- ity and morbidity score. CONCLUSION: Mini-laparotomy, as conducted in a sin- gle-centre series with experienced TME surgeons, is a safe and effective new approach for minimally invasive rectal cancer surgery. METHODS: We performed Roux-en-Y gastric bypass with reserved gastric volume, a procedure in which gastrointestinal continuity was reestablished while excluding the entire duodenum and proximal jejunal loop. We observed the procedural success rate, long-term survival, and histopathological sequelae associated with a number of technical modifications. These included: use of anatomical markers to precisely identify Treitz's ligament; careful dissection along surgical planes; careful attention to the choice of regional transection sites; reconstruction using full-thickness anastomoses; use of a minimally invasive procedure with prohemostatic pretreatment and hemorrhage control; prevention of hypo-thermic damage; reduction in the length of the procedure; and accelerated surgical recovery using fast-track surgical modalities such as perioperative permissive underfeeding and goal-directed volume therapy. RESULTS: The series of modif ications we adopted reduced operation time from 110.02 ± 12.34 min to 78.39 ± 7.26 min (P < 0.01), and the procedural success rate increased from 43.3% (13/30) to 90% (18/20) (P < 0.01), with a long-term survival of 83.3% (15/18) (P < 0.01). CONCLUSION: Using a number of fast-track and damage control surgical techniques, we have successfully established a stable model of gastric bypass in diabetic rats.展开更多

In order to determine the specificity of the structure of the tested anthropological dimensions, the researchers examined 200 judo and karate athletes, members ofjudo and karate clubs of Serbia (about 100 judokas and about 100 karatekas), aged 18 to 27 years. To assess motor abilities, the researchers used 16 motor tests selected according to the structural model, defined as a mechanism for movement structuring, mechanism for functional synergies and tonus regulation, mechanism for excitation intensity regulation, and a mechanism for excitation duration regulation. All the data in this research were processed at the Multidisciplinary Research Center, Faculty of Sport and Physical Education, University of Pristina, through the system of data processing software programs developed by Popovic. The algorithms and programs realized in the research are fully presented, and the results of the programs are analyzed.展开更多

The paper introduces initial notions on applying linguistic criteria as the basic ones in the process of creating ME textbooks. Through the historical development and under the influence of extra linguistic factors (globalization of the shipping industry, in particular), ME has accumulated and stored various resources being in current use: vocabulary, grammar and phonology, operational and semiotic systems, specific means of communication, functional models, patterns of narration and description, etc.. The proper linguistic analysis makes a secure foundation for methodology which can guarantee a better quality of ME course books for the 1st and 2nd year maritime students all over the world.展开更多
