We report two cases of colorectal perforation that occurred during manometry in the patients undergoing restorative proctectomy for distal rectal cancer. In the first patient, computed tomography showed an extraperitoneal perforation in the pelvic cavity and a rupture of the rectal wall. A localized perforation into the retroperitoneum was managed conservatively. In the second patient, a 3 cm linear colon rupture was detected above the anastomotic site. A primary closure of the perforated colon and proximal ileostomy were conducted, but the patient died 2 wk later. We hypothesize that the perforation induced by anorectal manometry may be associated with the relative weakening of the proximal bowel wall due to anastomosis, decreased compliance, and abnormal rectal sensation. We suggest that measurement of the maximum tolerable volume should not be routinely performed alter restorative proctectomy for distal rectal cancer.展开更多

Recent rapid rise of indoor location based services for smartphones has further increased the importance of precise localization of Wi-Fi Access Point(AP).However,most existing AP localization algorithms either exhibi...Recent rapid rise of indoor location based services for smartphones has further increased the importance of precise localization of Wi-Fi Access Point(AP).However,most existing AP localization algorithms either exhibit high errors or need specialized hardware in practical scenarios.In this paper,we propose a novel RSSI gradient-based AP localization algorithm.It consists of the following three major steps:firstly,it uses the local received signal strength variations to estimate the direction(minus gradient) of AP,then employs a direction clustering method to identify and filter measurement outliers,and finally adopts triangulation method to localize AP with the selected gradient directions.Experimental results demonstrate that the average localization error of our proposed algorithm is less than 2meters,far outperforming that of the weighted centroid approach.展开更多

Based on negative selection algorithm, the mature detectors are generated. By introducing clonal selection algorithm, the detectors with higher affinity are selected to undergo a proliferation and somatic hyper-mutation process, so that more excellent detector offspring can be generated. Experimental result shows that the detection model has a higher detection rate for unknown smartphone malware, and better detection performance can be achieved by increasing the clone generation.展开更多

The uniqueness and scalability of coding are guaranteed by the rational arrangement of code bits. The recognition of coded targets is realized by cross-ratio invariance restriction, space coordinates transform of feature points based on spacial pose estimation algorithm, recognition of code bits and computation of coding values. Objective To explore whether acupuncture can improve sleep disturbance,cognitive impairment and emotional disorders caused by sleep deprivation,and its association with the attenuation of oxidative stress injury in pr...Objective To explore whether acupuncture can improve sleep disturbance,cognitive impairment and emotional disorders caused by sleep deprivation,and its association with the attenuation of oxidative stress injury in prefrontal cortex.Methods Fifty-two male Sprague-Dawley rats were randomly divided into a control group(n=10),a model group(n=14),a manual acupuncture(MA)group(n=14),and a sham-MA group(n=14).All the groups were established as sleep deprivation models via the modified multiple platform method,except for the control group.Rats in both the MA group and the sham-MA group received corresponding intervention,respectively.After modeling and intervention,the four groups received three behavioral tests,namely sleep monitoring,by comprehensive lab animal monitoring system(CLAMS),Morris water maze(MWM)test and open-field test(OFT),followed by oxygen free radical level test and Western blot(WB)detection for the expression levels of Bax and Bcl-2.Results The MA group derived more sleep time within 24 h than either the model group or the sham-MA group(both P<0.05).On MWM orientation navigation test day 1,there were no significant differences in escape latency among the control,MA and sham-MA groups(P>0.05),and the escape latency was significantly shorter in these three groups than that in the model group(all P<0.05).On test day 4,the escape latency was markedly shorter in the MA group than that in either the model group or the sham-MA group(both P<0.05);meanwhile,the MA group showed significantly better performance compared with these two groups in space probe test(both P<0.05).In OFT,compared with the control group,there was a significant decline in the horizontal movement score in the other three groups(all P<0.05),and the decrease was more significant in the model group and the sham-MA group than that in the MA group(both P<0.05).The superoxide dismutase(SOD)content was markedly higher and the malondialdehyde(MDA)content was markedly lower in the MA group than those in the model group and the sham-MA group(all P<0.05).Compared with the model group and the sham-MA group,the expression of Bax was significantly lower and the expression of Bcl-2 was significantly higher in the MA group(all P<0.05).Conclusion MA therapy can lengthen the sleep time in sleep-deprived rats and improve learning and memory impairments induced by sleep deprivation,and the underlying mechanism may be associated with the enhancement of antioxidant capacity in the prefrontal cortex and the inhibition of hippocampal neuronal apoptosis.展开更多

Methods: This study was perform...Objective: To define the preoperative and intraoperative variables which may affect the immediate postoperative outcome in surgically managed patients with unstable pelvic fractures. Methods: This study was performed prospectively from January 2009 to June 2011 on 36 consecutive patients admitted to the trauma ward of Postgraduate Institute of Medical Education and Research, Chandigarh, with unstable pelvic injuries. Results: In the present study of 36 patients, 29 were managed surgically. Surgical duration was 2 hours in pa- tients operated on within 1 week and 3.4 hours in those operated on after 1 week. The blood loss was 550 ml when surgery was done after a week, but when done within a week it was 350 ml. The average blood loss through Pfanenstial approach was 360 ml, through posterior approach was 408 ml and through combined approach was 660 ml which was significantly high. Conclusion: Anterior approach to the pelvis would cause significantly more amount of blood loss than poste- rior approach and extemal fixation. Surgical approaches do not have any influence on the surgical duration or the infec- tion rate. The blood loss significantly increases when the surgical time is more than 1 h. The infection rate is not influ- enced by the duration of surgery. Presence or absence of associated injuries to the head, chest or abdomen is the main determinants of patient's survival and it greatly influ- ences the duration of hospital stay.展开更多
Objective: To determine whether six weeks' of acupuncture has a positive effect on plantar flexion muscle strength and whether electroacupuncture (EA) has a greater effect than manual acupuncture (MA) on plantar...Objective: To determine whether six weeks' of acupuncture has a positive effect on plantar flexion muscle strength and whether electroacupuncture (EA) has a greater effect than manual acupuncture (MA) on plantar flexion, so that the most beneficial method of acupuncture can be established for strengthening muscles. Methods: A randomized controlled trial with parallel groups. A total of 20 participants were randomly assigned to three groups: a control/non-acupuncture group (n=6), a manual acupuncture group (MAG) (n=8) and an electroacupuncture group (EAG) (n=6). MAG and EAG received three 25-minute treatments per week for 6 weeks. The control group continued with their normal training activity and received no treatment. The maximal voluntary contraction (MVC) of the ankle plantar-flexors at 30~ per second was measured using Biodex system 3, Results: Both MA and EA had a positive effect in increasing plantar flexion strength. The likelihood for this happening was high; 69% for MA and 92% for EA when compared with the control group. However the effect size was small for both groups. There was only a trivial difference in the effect size between the EAG and the MAG in regards to average plantar flexion torque. Conclusion: EA and MA have a beneficial effect on plantar flexion muscle strength compared with the control group in healthy population.展开更多
Objective To observe the efficacy of acupuncture combined with sitting-position knee-adjustment manipulations in treating patellofemoral osteoarthritis.Methods Ninety-two patients with patellofemoral osteoarthritis we...Objective To observe the efficacy of acupuncture combined with sitting-position knee-adjustment manipulations in treating patellofemoral osteoarthritis.Methods Ninety-two patients with patellofemoral osteoarthritis were randomized into an observation group and a medication group,with 46 cases in each group.The observation group received acupuncture and sitting-position knee-adjustment manipulations,and the medication group received oral celecoxib capsules.After 8-week treatment,changes in the short-form McGill pain questionnaire(SF-MPQ)and Lysholm knee scoring scale(LKSS)scores were observed,and the clinical efficacy was compared.Results The total effective rate was 87.0%in the observation group and 63.0%in the control group;the between-group difference was statistically significant.Before treatment,there were no significant differences in the SF-MPQ score or LKSS score(P>0.05).After 8-week treatment,the SF-MPQ[including pain rating index(PRI),visual analog scale(VAS),and present pain intensity(PPI)]and LKSS scores showed notable changes in both groups(P<0.05);the SF-MPQ and LKSS scores in the observation group were significantly different from those in the control group(P<0.05).Conclusion Combining acupuncture and sitting-position knee-adjustment manipulations can reduce pain and ameliorate joint function in patients with patellofemoral osteoarthritis,producing more significant efficacy than oral celecoxib capsules.展开更多
