As a leading cause of hospitalizations among patients aged 65 years or older, HF is a major consumer of healthcare resources, creating a substantial strain on the healthcare system. This paper discusses the epidemiology of HF, financial impact, and multifaceted predicaments in end-stage HF care. A search was conducted on the U.S National Library of Medicine website (www.pubmed.gov) using keywords such as end-stage heart failure, palliative care, ethical dilemmas. Despite the poor prognosis of HF (worse than that for many cancers), many HF patients, caregivers, and clinicians are unaware of the poor prognosis. In addition, the unpredictable clinical trajectory of HF complicates the planning of end-of-life care, such as palliative care and hospice, leading to underutilization of such resources. In conclusion, ethical dilemmas in end-stage HF are numerous, embroiling not only the patient, but also the caregiver, healthcare team, and society.展开更多

Apart from being the repository of biological diversity, these PAs serve as the source of subsistence livelihoods to the local communities. While all the PAs are managed by the state Forest Department (FD), with a view to ensure sustainability, a recent approach of co-management has been initiated in five PAs as a pilot project with an aid of development partners incorporating stakeholders' participation. Strategic decision-making in the management of human recourses in the field of environmental protection is crucial for the countries in transition. Lack of financial and technological resources can be compensated with other potentials. Therefore, it is necessary to emphasize the decision-making process. Optimization of the existing resources under real conditions is needed in order to get better results of the implementation of adopted strategies. However, rationally looking at how different the impact of foreign economic activities on China's economy and society is economic workers' main task. From 1982 to 2006, based on six categories of China's environmental indicators, the paper improves the entropy method to calculate the environmental pollution index which can represent China's overall environmental situation, uses the VAR model of the impulse response function and variance decomposition method to study the impact of foreign trade, FDI on China's environmental pollution in the long-term. The results show that, FDI and import trade is conducive to the improvement of the quality of our environment and export trade is the deterioration of our environment, "pollution haven" hypothesis is proved in China. Clinical data are generated both from the documentation of physicians' interactions with the patient and by diagnostic systems. During the care process, a number of different actors and roles (physicians, specialists, nurses, etc.) have the need to access patient data and document clinical activities in different moments and settings. Thus, data sharing and flexible aggregation based on different users' needs have become more and more important for supporting continuity of care at home, at hospitals, at outpatient clinics. In this paper, the authors identify and describe needs and challenges for patient data management at provider level and regional- (or inter-organizational-) level, because nowadays sharing patient data is needed to improve continuity and quality of care. For each level, the authors describe state-of-the-art Information and Communication Technology solutions to collect, manage, aggregate and share patient data. For each level some examples of best practices and solution scenarios being implemented in the Italian Healthcare setting are described as well.展开更多

Semi-dry grasslands have been planted on cleared areas of forest in the Pannon Mountains, which would be reclaimed by forest without use of the areas by humans. In our work we analysed cleared grasslands. The questions we tried to answer were which changes were caused by different land use, how favourable mowing can be for the purpose of grassland management and nature protection, do the composition of the plant species and the diversity vary within two years when conditions of precipitation are different. In four sampling areas, in 4 × 10 quadrats we registered the plant species and their cover values. We performed a site assessment in April, June and October, 2013-2014. We analysed the data by using cluster and ordination processes and we compared the sampling areas on the basis of the humidity preference and Shannon's index of diversity. According to the results, the composition of species and diversity on the analysed areas used for different purposes considerably deviate from each other. Further, we found a difference between two portions of the area whose had minor deviation in water management.展开更多

The aim is to conserve biodiversity, alleviate poverty and develop the country. Participation and involvement of local population in natural resources management, consultation with all sectors and responsibilization of regional and local authorities in protected areas management are all the basic principles of MSPA. The paper brought focus in the understanding of this participation of local actors (populations, local authorities) and in their interests to participate in the protected area management in Anjozorobe Angavo. The research has been conducted by doing investigations and meetings with local actors. A typology of participations had been done. It has been indeed noticed that the understanding was differentiated then motivations to participate were not always the environmental one.
