Deprescribing antihypertensive treatment in nursing home patients and the effect on blood pressure 被引量:2

Background It is debatable whether treating multimorbid nursing home patients with antihypertensive drugs produces beneficial effects. Most cardiovascular guidelines promote treatment; few have advice on how to deprescribe when treatment may no longer be necessary. We investigated the effect of medication review on antihypertensive drug use and the association between cognition, blood pressure, and prescribing. Methods From August 2014 to December 2015, 765 patients from 72 units (clusters) in 32 Norwegian nursing homes were included in a 4-month, multicentre, cluster-randomized, controlled trial, with 9-month follow-up. Patients ≥ 65 years old with antihypertensive treatment (n = 295, 39%) were randomized to systematic medication review where the physician received support from peers (collegial mentoring) or were given care as usual (control condition). Outcome measures were the number of antihypertensive drugs, systolic blood pressure, and pulse. We used hospitalizations and deaths as criteria to assess harm. Results At baseline, each patient used 9.2 ± 3.5 regular drugs, and 1.6 ± 0.7 antihypertensives. Mean blood pressure was 128/71 mmHg and 9% had a systolic pressure ≥ 160 mmHg. Between baseline and month four, antihypertensives were deprescribed to a significantly higher extent in the intervention group (n = 43, 32%) compared to control (n = 11, 10%); Incidence Rate Ratio = 0.8, 95% CI = 0.7?0.9. In the intervention group, there was an immediate increase in systolic blood pressure when antihypertensives were reduced, from baseline 128 ± 19.5 mmHg to 143 ± 25.5 mmHg at month four. However, at month nine, the blood pressure had reverted to baseline values (mean 134 mmHg). Deprescription did not affect pulse and systolic pressure. The number of hospitalizations was higher in control patients at month four (P = 0.031) and nine (P = 0.041). Conclusion A systematic medication review supported by collegial mentoring significantly decreased the use of antihypertensive drugs in nursing home patients without an effect on the systolic blood pressure over time. 展开更多

With the increase of network complexity,the flexibility of network control and management becomes a nontrivial problem.Both Software Defined Network(SDN) and Autonomic Network technologies are sophisticated technologies for the network control and management.These two technologies could be combined together to construct a software defined self-managing solution for the future network.An autonomic QoS management mechanism in Software Defined Network(AQSDN) is proposed in this paper.In AQSDN,the various QoS features can be configured autonomically in an OpenFlow switch through extending the OpenFlow and OF-Config protocols.Based on AQSDN,a novel packet context-aware QoS model(PCaQoS) is also introduced for improving the network QoS.PCaQoS takes packet context into account when packet is marked and managed into forwarding queues.The implementation of a video application's prototype which evaluates the self-configuration feature of the AQSDN and the enhancement ability of the PCaQoS is presented in order to validate this design. 展开更多

Base on the theory of energy minimization, a numerical algorithm is established to calculate load distribution, and the relationship curve of spur gear load distribution is obtained, and the load distribution ratio changes from 033 to 067 in double contact zone. This theory is adopted to compute the load distribution of helical gear along time-varying contact line, and the load distribution varies with the instantaneous position of the meshing point and the length of contact line, and the maximum value of load appears at the pitch point. Compared with the load distribution results, the helical gear changes more smoothly than spur gear. The load distribution provides a basis for calculate tooth bending deformation and critical stress. 展开更多

In order to solve the problem of artificial generation and low efficiency of test sequences for zone controller (ZC), a model-based automatic generation method of test sequence is proposed. Firstly, the timed automata model is established based on function analysis of the zone controller, and the correctness of the model is verified by UPPAAL. Then by parsing the timed automata model files, state information and transition conditions can be extracted to generate test case sets. Finally, according to the serialization conditions of test cases, the test cases are serialized into test sequences by using the improved depth first search algorithm. A case, the ZC controls the train running within its jurisdiction, shows that the method is correct and can effectively improve the efficiency of test sequence generation. 展开更多

Namely, structure box is local social ecological environment. Structure box is a closed area which is full of anger. Therefore, in such environment with too much pressure, we need an outlet to release pressure. Anger is the social and psychological basis of the emotional resonance of onlooker, when the fuse incident occurred, so that the widespread anger in structure box will find the spotlight. The words and actions of the leaders in the informal group will produce a great social infection and circulatory response, and groups’ stirring will cause emotional resonance which is easy to burst. Thus, irrational behavior may caused by people. This processcan use five links to describe: occurrence of fuse events, information diffusion, emotional resonance, situational catalysis, control failure, and occurrence of public security emergency. This study focused on the automatic versus controlled nature of the underlying cognitive processes of ERPs (Event-Related Potentials) effects during speech perception. ERPs to the final word of sentences (half being semantically incongruent with the sentence and half congruent) were presented to French natives under 4 levels of degradation: no degradation, mild degradation (2 levels), and strong degradation. Under mild degradation allowing controlled sentence-level processing according to behavioral data, the N400 effect (i.e., the N400 to incongruent words minus the N400 to congruent words) and a late positive complex were delayed and the latter effect was reduced. Under strong degradation allowing only automatic sentence processing according to behavioral data, no ERP effects remained. These results suggest that, unlike contextual effect found with single words (e.g., using word-pair or word-list paradigms), ERP effects elicited by more complex contexts such as full sentences are generated by controlled but not by automatic mechanisms of speech processing. 展开更多

Nowadays the flexible configuration of manufacturing cells becomes to an important requirement especially at small and medium sized companies. This method can make the production fast and effective at small series or frequent manufacturing changes. The shop-floor control method, presented in this paper, offers a solution for the facing problem of fast and easy reconfiguration. The hardware of the controller designed modular with software components for online configuration. This solution allows sensor integration on different levels for every part of the manufacturing cell. With unified programming language and the machine specific controllers (post-processing) the cells can be defined easily by different types of human-machine interaction. The shop-floor control architecture is implemented and validated on an Adept SCARA (selective compliance assembly robot arm) robot.
