Management of acquired bronchobiliary fistula:A systematic literature review of 68 cases published in 30 years

AIM:To outline the appropriate diagnostic methods and therapeutic options for acquired bronchobiliary fistula(BBF).METHODS:Literature searches were performed in Medline,EMBASE,PHMC and LWW(January 1980August 2010)using the following keywords:biliobronchial fistula,bronchobiliary fistula,bronchobiliary fistula,biliarybronchial fistula,tracheobiliary fistula,hepatobronchial fistula,bronchopleural fistula,and biliptysis.Further articles were identified through crossreferencing.RESULTS:Sixtyeight cases were collected and reviewed.BBF secondary to tumors(32.3%,22/68),including primary tumors(19.1%,13/68)and hepatic metastases(13.2%,9/68),shared the largest proportion of all cases.Biliptysis was found in all patients,and other symptoms were respiratory symptoms,such as irritating cough,fever(36/68)and jaundice(20/68).Half of the patients were treated by lessinvasive methods such as endoscopic retrograde biliary drainage.Invasive approaches like surgery were used less frequently(41.7%,28/67).The outcome was good at the end of the followup period in 28 cases(range,2 wk to 72 mo),and the recovery rate was 87.7%(57/65).CONCLUSION:The clinical diagnosis of BBF can be established by sputum analysis.Careful assessment of this condition is needed before therapeutic procedure.Invasive approaches should be considered only when noninvasive methods failed.

The Impacts of Tax Procedures Reform on Business Environment in Vietnam： Achievements and Further Reforms

Tax procedures are factors that strongly affect the business environment. A simple and convenient tax procedures system will contribute to a good business environment by easing of tax returns preparation, reducing tax compliance costs, and preventing corruption. This paper examines the results and impacts of previous tax procedures reform on Vietnam＇s business environment, identifies challenges of tax procedures reform now and the coming years, and discusses areas where further reforms are needed to sustain tax revenue and reduce compliance costs including： （1） wansparent and comprehensive guidance to all tax laws and other legal tax documents; （2） widening the application of risk management in tax administration; （3） continuing simplification of tax procedures; and （4） development of IT in tax administration.

Solid Waste Generation and Composition in Mexico： A Comparison between Rural and Urban Communities

This paper reports a generation and composition analysis of household solid waste in Mexico, using as a reference the characterization study performed in two communities, one urban and one rural. Data obtained are fundamental parameters for the planning of urban solid waste management systems in Mexico, complying this way with the established policy in the National Program for Waste Management and Prevention 2009-2012. The study was carried out during five days and two main parameters were assessed： （1） the daily generation rate per person and （2） the waste composition. The per capita waste generation was 0.958 kg for the urban community and 0.631 kg for the rural. The results showed that the mean composition of waste from both locations was statistically similar （a = 0.05）. In both locations the most important contribution was the fraction of food waste. In both locations more than 60% of the waste is potentially recyclable. The results of this study will be used by the municipal waste authorities in order to establish an integrated waste management plan.

How PDCA cycle implementation improved surgical time-out compliance rate

Objective： To explore the effect of PDCA-cycle in improving time-out compliance rate and improvement in quality and safety management of surgery. Methods： We improved time-out compliance rate by using PDCA-cycle （a quality improvement tools）. Data were collected before and after the implementation of PDCA-cycle. The results were analyzed. Results： After implementation of PDCA-cycle, over all time-out compliance rate was increased from 80% to 99.8% （P=0.000）. The compliance rate of identifying patient, performing site marking before surgery, verification of correct-site and correct procedure were increased from 85.1%, 79.2%, 89.7% and 89.1% to 99.8%, 99.9%, 100% and 100% （P〈0.05） respectively. Conclusion： Our study validated the PDCA-cycle methodology improves time-out compliance rate, and it is one of the most effective quality improvement tools for hospital management.

Roadroid： Continuous Road Condition Monitoring with Smart Phones

Road condition is an important variable to measure in order to decrease road and vehicle operating/maintenance costs, but also to increase ride comfort and traffic safety. By using the built-in vibration sensor in smart phones, it is possible to collect road roughness data which can be an indicator of road condition up to a level of Class 2 or 3 in a simple and cost efficient way. Since data collection therefore is possible to be done more frequently, one can better monitor roughness changes over time. The continuous data collection can also give early warnings of changes and damage, enable new ways to work in the operational road maintenance management, and can serve as a guide for more accurate surveys for strategic asset management and pavement planning. Collected measurement data are wirelessly transferred by the operator when needed via a web service to an internet mapping server with spatial filtering functions. The measured data can be aggregated in preferred sections, as well as exported to other GlS （geographical information systems） or road management systems. Our conclusion is that measuring roads with smart phones can provide an efficient, scalable, and cost-effective way for road organizations to deliver road condition data.
