A desmoid tumor,also known as aggressive fibromatosis,is a rare benign neoplasm that arises from fascial or musculoaponeurotic tissues.It can occur in any anatomical location,most commonly the abdominal wall,shoulder ...A desmoid tumor,also known as aggressive fibromatosis,is a rare benign neoplasm that arises from fascial or musculoaponeurotic tissues.It can occur in any anatomical location,most commonly the abdominal wall,shoulder girdle and retroperitoneum.The typical clinical presentation is a painless mass with a slow and progressive invasion of contiguous structures.It is associated with a high local recurrence rate after resection.Many issues regarding the optimal treatment of desmoid tumors remain controversial.Aggressive surgical resection with a wide margin(2-3 cm) remains the gold standard treatment with regard to preserving quality of life.Radiotherapy alone has been shown to be effective for the control of unresectable or recurrent lesions.Desmoid tumors tend to be locally infiltrative,therefore,the fields must be generous to prevent marginal recurrence.The radiation dose appropriate for treating desmoid tumors remains controversial.We present a 25-year-old Caucasian man with local recurrence of a desmoid tumor after repeated surgical resection,treated with radiotherapy.The patient achieved complete tumor regression at 4 mo after radiotherapy,and he is clinically free of disease at 12 mo after the end of treatment,with an acceptable quality of life.The patient developed short bowel syndrome as a complication of second surgical resection.Consequently,radiotherapy might have worsened an already present malabsorption and so led to steatohepatitis.展开更多

In this disease the risk of many malignancies is increased the most important of which being non-Hodgkin lymphoma of small intestine. Other malignancies include adenocarcinoma of small intestine and squamous cell carcinoma of esophagus and melanoma. As to our knowledge so far only one case of ceJiac disease associated with hypopharyngeaJ squamous ceil car- cinoma has been reported. In this article we presented a patient suffering from celiac disease with squamous cell carcinoma of hypopharynx. She underwent chemotherapy and radiation therapy, unfortunately however she died because of progress of disease. So, in patients with celiac disease we should pay attention to various malignancies and when cases of cancers are accompanied by malabsorption we must think of celiac disease involvement.展开更多
Using Cobb-Douglas production function and Solow Residual, this study establishes a discriminant index to measure the intensive index of coal-production at the section-level, so as to analyze the transfer trend of the...Using Cobb-Douglas production function and Solow Residual, this study establishes a discriminant index to measure the intensive index of coal-production at the section-level, so as to analyze the transfer trend of the growth pattern of Jiangsu's coal-production since 1990s. The research shows that the transition of coal production in Jiangsu Province has mainly experienced three phases, which are the quick transition phase from extensive growth to intensive growth (from 1990 to 1994), the fluctuation phase (from 1995 to 1999), and the transition back phase from intensive growth to extensive growth (from 2000 to 2003). On the whole, the coal production in Jiangsu Province nowadays is still featured by extensive growth pattern and largely dependent upon capital inputs. Finally, from the aspect of the technology progress, improving the qualities of labor, changing product structures and improving enterprise management, this study puts forward suggestions on how to transfer the growth pattern of Jiangsu's coal-production into intensive type.展开更多
This paper examines the effect of equalizing ignition delay in a compression ignition engine.Two sets of tests were conducted,i.e.a set of constant injection timing tests with start of fuel injection at 10°crank ...This paper examines the effect of equalizing ignition delay in a compression ignition engine.Two sets of tests were conducted,i.e.a set of constant injection timing tests with start of fuel injection at 10°crank angle degree(CAD)before top dead center(BTDC)and a set of constant ignition timing tests while also keeping the 10℃AD BTDC injection and adding ignition improver(2-ethylhexylnitrate-,2-EHN)to the fuel mixtures.Soot particles were characterized using DMS-500 instrument in terms of mass,size,and number.The experimental results showed that adding 2-EHN to the model fuel blends reduced the soot surface area,soot mass concentration and soot mean size.Replacing 20 vol%of a C 7-heptane with 20 vol%methyl-decanoate(an oxygenated C 11 molecule)did not affect the ignition delay or rate of fuel air premixing,the peak in-cylinder pressure or heat release rates.Toluene addition(0−22.5 vol%)to heptane increased the mean size of the soot particles generated by only 3%while also resulted in a slight increase in the peak cylinder pressure and peak heat release rates.Blending toluene and methyl-decanoate into heptane without adding 2-EHN increased the premix phase fraction by at least 13%.However,by adding 2-EHN(4×10^(−4)−1.5×10^(−3)),the premixed phase fraction decreased by at least 11%.展开更多
