土壤湿度年际变化对中国区域极端气候事件模拟的影响研究 Ⅱ.敏感性试验分析 被引量:41

利用NCARCAM3.1大气环流模式,设计了有、无土壤湿度年际异常的两组数值试验,探讨了土壤湿度年际异常对极端气候事件模拟的可能影响。结果表明,模式模拟的极端气候事件对土壤湿度异常十分敏感,土壤湿度异常对极端气候指标的多年平均空间... 利用NCARCAM3.1大气环流模式,设计了有、无土壤湿度年际异常的两组数值试验,探讨了土壤湿度年际异常对极端气候事件模拟的可能影响。结果表明,模式模拟的极端气候事件对土壤湿度异常十分敏感,土壤湿度异常对极端气候指标的多年平均空间分布、年际变率以及年际变化均具有重要影响。当不考虑土壤湿度的年际异常时:(1)模拟的暖夜日数、暖昼日数和热浪持续指数的发生频次在全国范围内均明显减少,而霜冻日数则明显增加。极端降水指标的响应表现出明显的空间差异,极端降水频次在江淮流域明显减小,而极端降水强度则表现为东北减弱、长江流域增强;中雨日数和持续湿期在我国大部分地区减少。(2)极端气温指标的年际变率在我国大部分地区呈减小趋势;而极端降水事件的变化则较为复杂,极端降水频次和极端降水强度的年际变率在长江以南有所增强,而北方地区则有所减弱。中雨日数和持续湿期的年际变率在我国呈现出较为一致的减少趋势。(3)模式对暖夜日数、霜冻日数的年际变化的模拟能力明显下降,并对4个极端降水指标的年际变化的模拟能力在全国多数区域均有不同程度的下降。 展开更多

目的 探讨细菌培养鉴定与药物敏感性试验分析在合理使用抗生素中的作用。方法 收集2020年1月至2021年3月本院收治的156例肺部感染患者为研究对象。所选患者均接受细菌培养鉴定及药物敏感性试验分析,进而对病原菌的耐药性开展深入分析。... 目的 探讨细菌培养鉴定与药物敏感性试验分析在合理使用抗生素中的作用。方法 收集2020年1月至2021年3月本院收治的156例肺部感染患者为研究对象。所选患者均接受细菌培养鉴定及药物敏感性试验分析,进而对病原菌的耐药性开展深入分析。结果 在156例患者中,共检出126株病原菌,其中革兰阴性杆菌98例,占77.8%,革兰阳性球菌28例,占22.2%;革兰阴性杆菌对三代头孢菌素的耐药性均较高,而对头孢哌酮舒巴坦、阿米卡星的耐药性则较低;革兰阳性球菌对红霉素、青霉素及克林霉素的耐药性均较高,而对万古霉素、左氧氟沙星及利奈唑胺的耐药性则较低。结论 细菌培养鉴定与药物敏感性试验分析的联合应用能够做到对病原菌分布状况及其耐药性的准确了解,为临床合理使用抗生素提供参考,使细菌耐药性降低的同时,还可获得较为显著的抗感染效果,有利于患者病情的控制。 展开更多

Present and future of prophylactic antibiotics for severe acute pancreatitis 被引量:39

AIM: To investigate the role of prophylactic antibiotics in the reduction of mortality of severe acute pancreatitis (SAP) patients, which is highly questioned by more and more randomized controlled trials (RCTs) and metaanalyses. METHODS: An updated meta-analysis was performed. RCTs comparing prophylactic antibiotics for SAP with control or placebo were included for meta-analysis. The mortality outcomes were pooled for estimation, and re-pooled estimation was performed by the sensitivity analysis of an ideal large-scale RCT. RESULTS: Currently available 11 RCTs were included. Subgroup analysis showed that there was significant reduction of mortality rate in the period before 2000, while no significant reduction in the period from 2000 [Risk Ratio, (RR ) = 1.01, P = 0.98]. Funnel plot indi-cated that there might be apparent publication bias in the period before 2000. Sensitivity analysis showed that the RR of mortality rate ranged from 0.77 to 1.00 with a relatively narrow confidence interval (P < 0.05). However, the number needed to treat having a minor lower limit of the range (7-5096 patients) implied that certain SAP patients could still potentially prevent death by antibiotic prophylaxis. CONCLUSION: Current evidences do not support prophylactic antibiotics as a routine treatment for SAP, but the potentially benefited sub-population requires further investigations. 展开更多

Experimental study of gemcitabine combination with radiation in vitro on a highly metastatic human ovarian cancer cell line HO-8910PM

Objective: The aim of the study was to investigate the mechanism of gemcitabine (GEM) combination with radiation on the high metastasis human ovarian cancer cell line (HO-8910PM). Methods: Human ovarian cancer cell line HO- 8910PM was treated with different concentrations of gemcitabine for 24 h, then the cells were counted. In the study of GEM combination with radiation, an efficiency of colony formation was observed; the cell cycle and apoptosis were analyzed by flow cytometry; the experiment of depend on the time and its radio sensitivity were observed by using mitotic index with the cells for each 24, 48, 72 and 96 h after experiment. Results: It suggested that the GEM had an inhibition effect on the human ovarian cancer cell line. The alive cell numbers were decreased by following a height of GEM concentration. When GEM in combina- tion with a radiation, the suppression was significantly increased than that of single GEM therapy. The efficiency of colony formation was significantly lower, under this condition the cell could be arrested at G0-G1 phase and could be decreased to enter into the S phase; the apoptosis percentage could be significantly increased; especially, under the 4 Gy and 6 Gy doses the cell apoptosis was more obvious. GEM combination with radiation had depended on the time to the cells; mitotic index of the calls in combination group was observed significantly lower than that of single GEM therapy or single radiation, and this showed that it had an effect of radiosensitivity. Conclusion: The GEM has a significant growth inhibition on the human ovarian cancer cells, GEM combination with radiation could induce HO-8910PM cell occurred arrested and apoptosis. It has depended on the time and has a radiosensitivity effect. The result shows that it is a better method to treat the human ovarian cancer by using radiotherapy combined with gemcitabine. 展开更多
