自拟降炎平衡方治疗重症颅脑损伤患者临床疗效观察 被引量:1

目的观察降炎平衡散对重症颅脑损伤患者临床疗效。方法将160例重症颅脑损伤患者随机数字表法分为研究组与对照组各80例。入院后2组患者均给予甘露醇、甘油果糖脱水降低颅内压、新凝灵、氨基环酸注射液止血、神经节苷脂营养脑神经、氨溴... 目的观察降炎平衡散对重症颅脑损伤患者临床疗效。方法将160例重症颅脑损伤患者随机数字表法分为研究组与对照组各80例。入院后2组患者均给予甘露醇、甘油果糖脱水降低颅内压、新凝灵、氨基环酸注射液止血、神经节苷脂营养脑神经、氨溴索保护肺功能、泮托拉唑、泮托拉唑抑制胃酸、防治应激性溃疡。发病后24h给予鼻饲肠内营养、给予抗生素控制吸入性肺炎等对症处理,所有患者入院后均给与鼻饲空肠营养管置管。发病24h后研究组患者给予自制方剂降炎平衡散,鼻饲肠内注入。对照组同期给予同等剂量的生理盐水鼻饲管注入。治疗14d后,使用美国国立卫生研究院卒中量表评分(NIHSS评分)、简易智能状态检查量表(MMSE评分)及格拉斯哥昏迷分(GCS评分)观察2组患者神经功能缺损程度;比较2组患者有创气管插管率、吸入性肺炎发生率、应激性溃疡发生率、急性肺损伤发生率、病死率;比较2组患者肝、肾功能异常及Na+水平变化。结果治疗后,研究组患者NIHSS评分低于对照组,GCS、MMSE评分均高于对照组,差异均有统计学意义(P<0.05)。治疗后14d内,研究组患者气管插管率、吸入性肺炎发生率、应激性溃疡发生率、急性肺损伤发生率、病死率均低于对照组,差异有统计学意义(P<0.05)。研究组患者血清中AST、ALT、SCr及Na+水平均低于对照组,差异均有统计学意义(P<0.05)。结论降炎平衡散辅助治疗重症颅脑损伤患者可以明显改善神经功能缺损程度评分,减轻继发脏器功能损伤程度,减少早期患者并发症发生,降低临床病死率。 展开更多

The effect of homogenization time on quench sensitivity of a cast 7085 aluminum alloy was investigated by means of end-quenching test, optical microscope (OM), scanning electron microscope (SEM) and transmission electron microscope (TEM). The results show that with the increase of homogenization time from 48 h to 384 h, quench sensitivity increased slightly as the largest difference in the hardness was increased from 5.2% to 6.9% in the end-quenched and aged specimens. Prolonging homogenization had little effect on the grain structure, but improved the dissolution of soluble T phase and resulted in larger Al3Zr dispersoids with a low number density. Some small quench-induced η phase particles on Al3Zr dispersoids were observed inside grains during slow quenching, which decreased hardness after subsequent aging. The change in the character of Al3Zr dispersoids exerted slight influence on quench sensitivity. 展开更多

Through alloy sampling combined with diffusion triple technique, phase equilibria in Al-Ti-Zr ternary system at 1073 Kwere experimentally determined with electron probe microanalysis (EPMA). Experimental results show that there is a solid solutionβ(Ti,Zr) which dissolves Al up to 16.3% (mole fraction). Ti and Zr can substitute each other in most Ti-Al and Al-Zr binaryintermediate phases to a certain degree while the maximum solubility of Zr in Ti3Al and TiAl reaches up to 17.9% and 4.0% (molefraction), respectively. The isothermal section consists of 16 single-phased regions, 27 two-phased regions and 14 three-phasedregions. No ternary phase was detected. 展开更多

The Intalox metal tower packing was used to simulate an industrial relevant extractive distillation column for purifying azeotropic multicomponent mixture.In order to explain the inconsistencies in the modeling of transfer process in nonideal multicomponent distillation column,a method was developed with equilibrium stage models(EQ)and non-equilibrium model(NEQ)incorporated with Maxwell-Stefan diffusion equations in the framework of AspenONE simulator.Dortmund Modified UNIFAC(UNIFAC-DMD)thermodynamic model was employed to estimate activity coefficients.In addition,to understand the reason for the diffusion against driving force and the different results by EQ and NEQ models,explicit investigations were made on diffusion coefficients, component Murphree efficiency and mass transfer coefficients.The results provide valuable information for basic design and applications associated with extractive distillation. 展开更多

Phase equilibria in Ti?Ni?Pt ternary system have been experimentally determined through diffusion triple technique combined with alloy samples approach.Assisted with electron probe microanalysis(EPMA)and X-ray diffraction(XRD)techniques,isothermal sections at 1073 and 1173 K of this system were constructed and existence of ternary phase Ti2(Ni,Pt)3 was confirmed.In addition,binary compounds Ti3Pt5 and TiPt3-were found to be stable at 1073 and 1173 K,and remarkable ternary solubility in some binary compounds was detected,e.g.,solubility of Pt in TiNi can be up to about 36%(molar fraction)at 1073 K and 40%(molar fraction)at 1173 K.Furthermore,a ternary invariant transition reaction TiNi3+Ti3Pt5→Ti2(Ni,Pt)3+TiPt3+at a temperature between 1073 and 1173 K was deduced. 展开更多

Updated version of local non-equilibrium diffusion model (LNDM) for rapid solidification of binary alloys was considered. The LNDM takes into account deviation from local equilibrium of solute concentration and solute flux fields in bulk liquid. The exact solutions for solute concentration and flux in bulk liquid were obtained using hyperbolic diffusion equations. The results show the transition from diffusion-limited to purely thermally controlled solidification with effective diffusion coefficient →0 and complete solute trapping KLNDM(v)→1 at v→vDb for any kind of solid-liquid interface kinetics. Critical parameter for diffusionless solidification and complete solute trapping is the diffusion speed in bulk liquid vDb. Different models for solute trapping at the interface with different interface kinetic approaches were considered. 展开更多

The discrete element method (DEM) was used to simulate the flow characteristic and strength characteristic of the conditioned sands in the earth pressure balance (EPB) tunneling. In the laboratory the conditioned sands were reproduced and the slump test and the direct shear test of the conditioned sands were implemented. A DEM equivalent model that can simulate the macro mechanical characteristic of the conditioned sands was proposed,and the corresponding numerical models of the slump test and the shear test were established. By selecting proper DEM model parameters,the errors of the slump values between the simulation results and the test results are in the range of 10.3%-14.3%,and the error of the curves between the shear displacement and the shear stress calculated with the DEM simulation is 4.68%-16.5% compared with that of the laboratory direct shear test. This illustrates that the proposed DEM equivalent model can approximately simulate the mechanical characteristics of the conditioned sands,which provides the basis for further simulation of the interaction between the conditioned soil and the chamber pressure system of the EPB machine. 展开更多

In terms of numerical method of Smoluchowski equation the behavior of fission process in diffusion model has been described and analyzed, including the reliance upon time, as well as the deformation parameters at several nuclear temperatures in this paper. The fission rates and the residual probabilities inside the saddle point are calculated for fissile nucleus n+^238U reaction and un-fissile nucleus p+^208Pb reaction. The results indicate that there really exists a transient fission process, which means that the pre-equillbrium fission should be taken into account for the fissile nucleus at the high temperature. Oppositely, the pre-equilibrlum fission could be neglected for the un-fissile nucleus. In the certain case the overshooting phenomenon of the fission rates will occur, which is mainly determined by the diffusive current at the saddle point. The higher the temperature is, the more obvious the overshooting phenomenon is. However, the emissions of the light particles accompanying the diffusion process may weaken or vanish the overshooting phenomenon. 展开更多
