Current strategies for improved survival in patients with Barrett's adenocarcinoma focus on detection of dysplasia. This can be obtained by screening programs in high-risk cohorts of patients and/or endoscopic biopsy surveillance of patients with known Barrett's esophagus (BE). Several therapies have been developed in attempts to reverse BE and reduce cancer risk. Aggressive medical management of acid reflux, lifestyle modifications, antireflux surgery, and endoscopic treatments have been recommended for many patients with BE. Whether these interventions are cost-effective or reduce mortality from esophageal cancer remains controversial. Current treatment requires combinations of endoscopic mucosal resection techniques to eliminate visible lesions followed by ablation of residual metaplastic tissue. Esophagectomy is currently indicated in multifocal high-grade neoplasia or mucosal Barrett's carcinoma which cannot be managed by endoscopic approach.展开更多

Objectives Even though guidelines are available to guide dysphagia identification and management practice,there is still a gap between evidence and practice,which requires improvement.The purpose of this study was to ...Objectives Even though guidelines are available to guide dysphagia identification and management practice,there is still a gap between evidence and practice,which requires improvement.The purpose of this study was to determine the effect of using tailored,multifaceted strategies to improve evidence-based post-stroke dysphagia identification and management practice in a community hospital.Methods Guided by the Knowledge to Action framework,the tailored,multifaceted strategies were developed and implemented for 5 months in a community hospital using a before-and-after study design.These strategies consisted of training intervention,policy intervention,and audit and feedback intervention.Nurses’level of knowledge and adherence,were collected in March 2019 and again in January 2020.Patients’quality of life and satisfaction were evaluated during the pre-intervention period(between February 2019 and April 2019)and the post-intervention period(between November 2019 and January 2020).Results A total of 55 patients with post-stroke dysphagia(28 in the pre-intervention period and 27 in the post-intervention period)and 17 registered nurses were recruited.Following implementation,there were statistically significant improvements in patients’outcomes(quality of life and satisfaction)and nurses’outcomes(level of knowledge and adherence).Conclusions This study assists in closing the research-practice gap by using tailored,multifaceted strategies to increase the use of evidence-based nursing care for dysphagia identification and management practices.展开更多

Many high-risk medications are ordered and administered at patients’ bedsides without be...Introduction: The department of emergency medicine (DEM) has a high-risk environment due to its unique and complex workflow. Many high-risk medications are ordered and administered at patients’ bedsides without being checked by a pharmacist first, which may lead to an increase in the incidence of patient medication errors (MEs). Objective: The current study evaluated the needs of the clinical pharmacy service in the DEM at King Saud University Medical City (KSUMC), Riyadh, Saudi Arabia. Methods: A cross-sectional retrospective study was conducted between Jan 2016 to Dec 2017 and the documentation of clinical pharmacist interventions was extracted from Esihi database. Results: A total of 2,255 interventions for 862 patients were documented. The recommended interventions were as follows: 645 (dose adjustments), 108 (therapeutic substitutions), and 354 interventions (initiating drug therapy). Adverse drug reactions (ADRs) were reported in 16 patients, and drug interactions were managed in 26 patients. The DEM responded to 713 information inquires and 290 pharmacokinetic consultations. Drug discontinuations included 39 incidents (where unjustified drug prescription occurred), 37 (where contraindications were involved), and 19 (where duplicate therapy was involved). The most common interventions were related to the following drugs: antibiotics (34%), anticoagulants (15%), and anticonvulsants (10%). The acceptance rates for the EM clinical pharmacist recommendations increased from 93.9% in 2016 to 99% in 2017. The most common outcome for interventions was to optimize the therapeutic effects of the drugs that were administered (73%). Reconciliation was done in 796 patients. Conclusions: The clinical pharmacy service plays a critical role in the management of patients in the emergency department (ED).展开更多

Methods: A questionnaire survey was conducted in the test group (n = 98) by filling out a symptom checklist 90 (SCL-90). And then the survey result was compared with the normal group (n = 1388) contained in relevant references. Results: Except for hostile factor that was similar to normal level (P 〉 0.05), other 8 factors were higher than in normal group (P 〈 0.01). And the cancer patients were classified with the survey results. Conclusion: Compared with normal persons, cancer patients have more psychological problems and related intervention measures are sought necessarily to improve the quality of life, to advance the immune function and to prolong the survival time of patients.展开更多
Engineering facilities subjected to natural hazards(such as winds and earthquakes) will result in risk when any designed system(i.e.capacity) will not be able to meet the performance required(i.e.demand).Risk might be...Engineering facilities subjected to natural hazards(such as winds and earthquakes) will result in risk when any designed system(i.e.capacity) will not be able to meet the performance required(i.e.demand).Risk might be expressed either as a likelihood of damage or potential financial loss.Engineers tend to make use of the former(i.e.damage).Nevertheless,other non-technical stakeholders cannot get useful information from damage.However,if financial risk is expressed on the basis of probable monetary loss,it will be easily understood by all.Therefore,it is necessary to develop methodologies which communicate the system capacity and demand to financial risk,Incremental dynamic analysis(IDA) was applied in a performance-based earthquake engineering context to do hazard analysis,structural analysis,damage analysis and loss analysis of a reinforced concrete(RC) frame structure.And the financial implications of risk were expressed by expected annual loss(EAL).The quantitative risk analysis proposed is applicable to any engineering facilities and any natural hazards.It is shown that the results from the IDA can be used to assess the overall financial risk exposure to earthquake hazard for a given constructed facility.The computational IDA-EAL method will enable engineers to take into account the long-term financial implications in addition to the construction cost.Consequently,it will help stakeholders make decisions.展开更多

Reducing the quantities of these waste streams can significantly redu...Industrial battery manufacturing facilities generate large quantities of hazardous waste, which must be properly treated before it can be disposed. Reducing the quantities of these waste streams can significantly reduce the cost of treatment and lead to competitive advantages. Waste minimization at these facilities is beneficial for the stakeholders and the environment. The quantities of hazardous waste can be minimized by upgrading the facility's technology or substituting hazardous substances, which are used in the battery manufacturing process, with more environmentally friendly options. Separation of waste streams will allow for additional reuse opportunities and revenue generation from the sale of these materials, which will enhance the financial performance of the facility. This paper provides a case study of comprehensive waste minimization in a battery manufacturing plant in Ohio, USA. Source reduction, recovery, and recycling methods are taken into account with consideration given to economic impacts. The goal of the study was to develop an understanding of the facility's waste generating processes, to suggest methods to reduce to the waste generation and finally to select an appropriate waste minimization option to suggest the facility's management team. Some of the suggested methods are currently being practiced while others are at the initial stage of development.展开更多

The aim of this study is to provide an overview the most common procurement methods used in constructing infrastructure transport projects and analyze how these methods contribute to the desired quality of the final product in relation to client competence. An on-line survey of construction actors was carried out to ascertain quality level of Swedish infrastructure transport projects and determinant factors of quality problems. An equal number of respondents indicated that the quality of infrastructure projects has either increased or remained same level over the past twenty years. Respondents also pointed out lack of client competence that is vital in realizing the desired quality level through proper procurement, monitoring and evaluation procedures. Public clients heavily rely on traditional design-build procurement that requires considerable client involvement of a project. Thus, the association of quality problems and lack of client competence may not be a mere coincidence but an overlooked outcome of current situation.展开更多
Objective: To evaluate the applicability of the modified physiological and operative severity score for enumeration of mortality and morbidity (POSSUM) scoring system in predicting mortality in the patients undergo...Objective: To evaluate the applicability of the modified physiological and operative severity score for enumeration of mortality and morbidity (POSSUM) scoring system in predicting mortality in the patients undergoing hip joint arthroplasty. Methods: A total of 295 patients with hip fractures were analyzed using the modified POSSUM surgical scoring system. The mean ages of the patients were 66.59 years in the complicative group, 62.28 years in noncomplicative group, 77.89 years in the death group and 63.25 years in the living group, respectively. The comparisons between the observed and predicted morbidity, between the observed and predicted mortality were made within 30 days after operation. Results: The average physiological scores and operative severity scores was 18.96 ± 4.83 and 13.47 ± 2.01 in complicative group, while 15.65 ± 3.66 and 11.74 ± 2.26 innoncomplicative group (P〈0.05). The average physiological scores and operative severity scores was 25.56 ± 3.78 and 14.22 ± 0.67 in death group, while 16.46 ± 4.09 and 12.25 ± 2.33 in living group (P〈0.05). Though POSSUM scoring system over-predicted the overall risk of death, its estimate was very close in the high risk groups (〉10%). There was perfect consistence between the observed and the predicted morbidity as calculated by published predictor equation for morbidity, and consistence for mortality in the high risk band. Conclusions: Modified POSSUM scoring system may be used to predict the morbidity in patients with hip fracture. Furthermore, POSSUM scoring system overpredicts the overall risk of death, but its estimate is close to the actual data in the high risk band (〉10%).展开更多
