Dysfuncti...AIM: To investigate the role of endogenous pain modulatory mechanisms in the central sensitization implicated by the visceral hypersensitivity demonstrated in patients with irritable bowel syndrome (IBS). Dysfunction of modulatory mechanisms would be expected to also result in changes of somatic sensory function. METHODS: Endogenous pain modulatory mechanisms were assessed using heterotopic stimulation and somatic and visceral sensory testing in IBS. Pain intensities (visual analogue scale, VAS 0-100) during suprathreshold rectal distension with a barostat, cold pressor stimulation of the foot and during both stimuli simultaneously (heterotopic stimulation) were recorded in 40 female patients with IBS and 20 female healthy controls. RESULTS: Rectal hypersensitivity (defined by 95% Cl of controls) was seen in 21 (53%), somatic hypersensitivity in 22 (55%) and both rectal and somatic hypersensitivity in 14 of these IBS patients. Heterotopic stimulation decreased rectal pain intensity by 6 (-11 to -1) in controls, but increased rectal pain by 2 (-3 to +6) in all IBS patients (P 〈 0.05) and by 8 (-2 to +19) in IBS patients with somatic and visceral hypersensitivity (P 〈 0.02). CONCLUSION: A majority of IBS patients had abnormal endogenous pain modulation and somatic hypersensitivity as evidence of central sensitization.展开更多

METHODS:We compared the ...AIM:To compare the response to skin prick tests (SPTs) to food antigens (FAs) and inhalant allergens (IAs) in patients with two subtypes of irritable bowel syndrome (IBS) and healthy controls. METHODS:We compared the results of SPTs for IAs and FAs in 87 volunteers divided into three groups:diarrhea predominant IBS (D-IBS) GroupⅠ(n = 19), constipation predominant IBS (C-IBS) Group Ⅱ (n = 17), and normal controls Group Ⅲ (n = 51). RESULTS:Of the 285 tests (171 for FAs and 114 for IAs) performed in GroupⅠwe obtained 45 (26.3%) positive responses for FA and 23 (20.1%) for IA. Of the 153 tests for FA in Group Ⅱ, we obtained 66 (20.1%) positive responses, and of the 102 tests for IA, we obtained 20 (19.6%) positive responses. Of the 459 tests for FA performed in Group Ⅲ, we obtained 39 (84%) positive responses, and of the 306 for IA, we obtained 52 (16.9%) positive responses. The numbers of positive responses were not significantly different between the three groups, but in the D-IBS group, the number of SPTFA responses differed significantly from those for the other two groups (P < 0.01). CONCLUSION:Despite the small number of cases studied, the higher reactivity to FAs in GroupⅠcompared to Groups Ⅱ and Ⅲ adds new information, and suggests the presence of a possible alteration in intestinal epithelial function.展开更多

This study is an effort to establish information about the pest and, to test the susceptibility of some Mango cultivars (Baladi, Kitchener, Shendi, Alphons, Abu Samaka and Bit abusamaka) to the pest infestation. A survey was carded out at Shendi area, River Nile State, during the period of May 2007 to July 2008, using a home made trap resemble lvnfield traps with attractants (Methyl eugenol, Trimedlure, cue lure and Nu lure) to identify the fruit flies species found in the area and to study the seasonal abundance of the pest species and to determine the infestation percentage of different Mango cultivars. Results indicated that the fruit fly Bactrocera invadens is present all year-round in the study area, with population peak July to September. The study showed that Guava ( Psedium guajava ) represents the best alternative host for the pest. Abu samaka and Bit Abusa amaka mango cultivars were the most susceptible, with 100% infestation, followed by Shendi 70%, Kitchener 54%, Alphons and Baladi (30%, 28%, respectively) were the least susceptible. Also results revealed that females of both species Ceratitis cosyra and Bactrocera invadens out numbered males 4-5 times.展开更多

It is found that the divergent behaviors of the susceptibility take the form of square of logarithm, in contrast with the global ground-state fidelity susceptibility which is power divergence. In order to perform a scaling analysis, we take the square root of the susceptibility and determine the scaling exponent analytically and the result is further confirmed by numerical calculations.展开更多

Methods Sixty-four cases were randomly di...Objective To compare the effects between acupuncture combined with moxibustion on heat-sensitive acupoints and acupuncture on irritable bowel syndrome with diarrhea (IBS-D). Methods Sixty-four cases were randomly divided into an acupuncture and moxibustion group (n=32) and an acupuncture group (n=32). Conventional acupuncture at Tiānshū (天枢 ST 14), Zúsānlǐ (足三里 ST 36), Gōngsūn (公孙 SP 4) and other acupoints was used in the two groups, and moxibustion on heat-sensitive acupoints was added in the acupuncture and moxibustion group. The treatment frequency was 5 times a week, and 4 weeks were a course. After 2 courses, the therapeutic effect was evaluated. Results The clinical symptom scores after treatment in two groups decreased obviously than those before treatment (both P〈0.01). The cured and markedly effective rate in the acupuncture and moxibustion group was 87.5% (28/32), and that in the acupuncture group was 37.5% (12/32), so the effect in the acupuncture and moxibustion group was better than that in the acupuncture group (P〈0.01). The improvement of all kinds of symptom scores in the acupuncture and moxibustion group was all better than that in the acupuncture group (all P〈0.05). Methods: A total of 120 IBS patients were divided into three groups by random sampling method, 40 cases in each group. The heat-sensitive moxibustion group received heat-sensitive moxibustion, the psychological intervention group received psychological intervention, while the observation group received both methods. The therapeutic efficacy was evaluated after 3 courses of treatment. Results: After treatment, the therapeutic efficacy of the observation group was better than that of the heat-sensitive moxibustion group and the psychological intervention group(P〈0.01), and the heat-sensitive moxibsution group was better than the psychological intervention group(P〈0.05). The pathophysiology of irritable bowel syndrome(IBS) is complex and not fully understood, so the aim of this study was to evaluate whether visceral and somatic hypersensitivity, autonomic cardiovascular dysfunction,...The pathophysiology of irritable bowel syndrome(IBS) is complex and not fully understood, so the aim of this study was to evaluate whether visceral and somatic hypersensitivity, autonomic cardiovascular dysfunction, and low-grade inflammation of the gut wall are associated with diarrhea-predominant IBS(D-IBS). Sixty-two patients with D-IBS and 20 control subjects participated in the study. Using the ascending method of limits(AML) protocol, we demonstrated that D-IBS patients had significantly lower sensory thresholds compared with healthy controls(P〈0.001). Using diverse methods, especially the ischemic sensitivity test, for the first time in China, we confirmed that D-IBS patients have somatic hypersensitivity. They had a significantly higher systolic blood pressure and heart rate after a cold stimulus, indicative of autonomic cardiovascular dysfunction. Compared with the control group, D-IBS patients had a significantly higher level of calprotectin(P〈0.001). We also found significant correlations between visceral and somatic hypersensitivity, visceral hypersensitivity and autonomic cardiovascular dysfunction, and somatic hypersensitivity and autonomic cardiovascular dysfunction. Our findings may provide valuable suggestions for the treatment of D-IBS.展开更多
