METHODS: We performed a retrospective analysis of all patients w...AIM: To evaluate the efficacy of thalidomide in com- bination with other therapies to treat patients with ad- vanced hepatocellular carcinoma (HCC). METHODS: We performed a retrospective analysis of all patients with HCC who were treated with thalido- mide for at least two months. The medical records of patients with HCC who were treated at our institution between April 2003 and March 2008 were reviewed. Image studies performed before and after treatment, tumor response, overall survival, and the decrease in o-fetoprotein (AFP) levels were evaluated. RESULTS: A total of 53 patients with HCC received either 100 or 200 mg/d of thalidomide. The patient population consisted of 9 women and 44 men with a median age of 61 years. Thirty patients (56.6%) were classified as Child-Pugh A, and 12 patients (22.6%) were classified as Child-Pugh B. Twenty-six patients had portal vein thrombosis (49.1%), and 25 patients had extrahepatic metastasis (47.1%). The median duration of thalidomide treatment was 6.0 mo. Six of the 53 patients achieved a confirmed response (11.3%), one achieved a complete response (1.9%) and 5 achieved a partial response (9.4%). The disease control rate (CR + PR + SD) was 28.3% (95% CI: 17.8-42.4), and the median overall survival rate was 10.5 too. The 1- and 2-year survival rates were 45% and 20%, respectively. Only one complete response patient showed an im- proved overall survival rate of 66.8 mo. Sixteen patients (30.2%) showed more than a 50% decrease in their serum AFP levels from baseline, indicating a better re- sponse rate (31.3%), disease control rate (43.8%), and overall survival time (20.7 mo). The therapy was well tolerated, and no significant toxicities were observed. CONCLUSION: Thalidomide was found to be safe for advanced HCC patients, demonstrating anti-tumor ac- tivity including response, survival, and AFP decreases of greater than 50% from baseline.展开更多

AIM: To investigate the therapeutic effects of Fuzheng Yiliu (strengthening the body resistance to inhibit tumor) decoction combined with chemotherapy on the patients with intermediate and late stage gastrointestinal ...AIM: To investigate the therapeutic effects of Fuzheng Yiliu (strengthening the body resistance to inhibit tumor) decoction combined with chemotherapy on the patients with intermediate and late stage gastrointestinal cancer. METHODS: Sixty patients were randomly divided into treatment group (chemotherapy combined with Fuzheng Yiliu decoction) and control group (chemotherapy alone). Four indexes, including the tumor recent remission rate (RR), the change of main symptoms, the toxic and side effects caused by chemotherapy and the change of performance status, were observed in the patients. Peripheral blood contents of CD3+, CD4+, CD8+ cells, CD4+/CD8+ and soluble interleukin-2 receptor (sIL-2R) were tested before and after treatment and the values were compared with those of healthy peoples. RESULTS: The improving rate of main symptoms (69.6%) and performance status (56.7%) were significantly higher in the treatment group than in the control group (34.8%, 26.7%, P<0.05). The occurrence rates of grade II toxic and side-effects on both bone marrow (13.3%) and digestive tract (30%) were lower in the treatment group compared to the control group (36.7%, 63.3%, P<0.05). Before treatment, the proportion of CD3+, CD4+ and CD4+/CD8+ decreased and the proportion of CD8+ and SIL-2R raised markedly both in the control group and treatment group as compared to the healttiy people. After treatment, that increased of CD3+, CD4+, CD4+/CD8+ increased (62.25±10.01% vs 68.31±9.72%, 36.83±10.44% vs 42.6±9.62%, 1.24±0.65 vs l.66±0.85, P<0.05) and the values of CD8+ and sIL-2R decreased obviously (33.06±7.69% vs 29.24±6.25%, 588.23±216.86 U/mL vs 475.87±211.36 U/mL,P<0.05) in the treatment group, whereas these values were opposite in the control group (64.22±6.91% vs60.63±5.75%,35.62±7.49% vs31.53±5.53%, 32.95±8.28% vs 37.14±7.48%, 1.17±0.43 vs 0.94±0.43, 573.63±214.32 U/mL vs 692.17?21.33 U/mL, P<0.05). CONCLUSION: Fuzheng Yiliu decoction can enhance therapeutic effects of chemotherapy on malignant gastrointestinal tumor, and also reduce the toxic and side effects on bone marrow and digestive tract, thereby improving the quality of life and cellular immunity in patients with malignant gastrointestinal tumor.展开更多
AIM:To evaluate the effectiveness of hepatic arterial infusion chemotherapy(HAIC) for advanced hepatocellular carcinoma(HCC) resistant to transarterial chemoembolization(TACE).METHODS:This study was conducted on 42 pa...AIM:To evaluate the effectiveness of hepatic arterial infusion chemotherapy(HAIC) for advanced hepatocellular carcinoma(HCC) resistant to transarterial chemoembolization(TACE).METHODS:This study was conducted on 42 patients who received HAIC for advanced HCC between 2001and 2010 at our hospital.5-fluorouracil(5-FU) was administered continuously for 24 h from day 1 to day 5 every 2-4 wk via an injection reservoir.Intra-arterial cisplatin or subcutaneous interferon was administered in combination with the 5-FU.The patients enrolled in this retrospective study were divided into two groups according to whether or not they fulfilled the criteria for resistance to TACE proposed by the Japan Society of Hepatology in 2010(written in Japanese);one group of patients who did not fulfill the criteria for TACE resistance(group A,n = 23),and another group who fulfilled the criteria for TACE resistance(group B,n = 19).We compared the outcomes in terms of the response and survival rates between the two groups.RESULTS:Both the response rate and tumor suppression rate following HAIC were significantly superior in group A than in group B(response rate:48% vs 16%,P = 0.028,tumor suppression rate:87% vs 53%,P = 0.014).Furthermore,both the progression-free survival rate and survival time were significantly superior in group A than in group B(3-,6-,12-,and 24-mo = 83%,70%,29% and 20% vs 63%,42%,16% and 0%,respectively,P = 0.040,and 9.8 mo vs 6.2 mo,P = 0.040).A multivariate analysis(Cox proportional hazards regression model) showed that resistance to TACE was an independent predictor of poor survival(P = 0.007).CONCLUSION:HAIC administrating 5-FU was not effective against advanced HCC resistant to TACE.Other tools for treatment,i.e.,molecular-targeting agents may be considered for these cases.展开更多

We investigated the clinicopathologi...The incidence of early gastric cancer (EGC) with duodenal invasion is extremely low, although advanced gastric cancer that arises in the antrum occasionally invades the duodenum. We investigated the clinicopathological features of EGC with duodenal invasion and provided strategies for clinical management.A Medline search was performed using the keyword early gastric cancer" and "duodenal invasion': Additional articles were obtained from references within the papers identified by the Medline search. We revealed that EGC with duodenal invasion was of the superficial spreading type of tumor. Tumors 〉 60 mm in size invaded the duodenum more extensively, and the distance of duodenal invasion from the pyloric ring was further in the elevated type than in the depressed type of tumor.There was no significant difference between the length of duodenal invasion and the histological type of the tumor. Gastric cancer located adjacent to the pyloric ring, even if cancer invasion was confined to the mucosa or submucosa, was more likely to invade the duodenum.The present study reveals that the elevated type of EGC is associated with more extensive duodenal invasion when the tumor size is 〉 60 ram, thus highlighting the importance of identification of duodenal invasion in these cases. We also reveal that sufficient duodenal resection with a cancer-free distal surgical margin should be performed in cases of duodenal invasion.展开更多

Methods: One hundred and twenty cases of advanced NSCLC patients treated by intervention chemotherapy through bronchial artery, collected from 2005 to 2008, were randomly divided into two groups: experimental group (group A), 75 cases, 0.25g CMNa was administered before chemical drugs; control group (group B), 45 cases, chemical drugs only. There were no differences of operation procedures and chemo-regimens between the two groups. After intervention, imaging and clinical data were collected periodically and were processed statistically. Results: The effective rate of group A was higher than that of group B (P < 0.05). Responsive rate (RR) in group A was 61% and it was 38% in group B. There were some side effects in the two groups but there were no statistical differences. Conclusion: Low dose CMNa can enhance chemosensitivity during intervention, which can elevate chemotherapeutical effects on NSCLC. Meanwhile the side effects were not increased. This method is worthy of popularizing.展开更多

The aim of this...Objective: Cetuximab is an effective medicine for colorectal cancer bearing wild type Kras. However, few studies of cetuximab on patients with noncolorectal gastrointestinal cancer were available now. The aim of this study was to summarize the efficacy and safety profiles base-on the experiences of 16 patients with noncolorectal gastrointestinal cancer accepted cetuximab-contained regimens treatment. Methods: The clinical data of all patients with noncolorectal gastrointestinal cancer treated by cetuximab combined with chemotherapy and/or radiotherapy from October 1st 2006 to Jun 30th 2009 in Sun Yat-sen University Cancer Center were collected. Results: (1) A total of 16 patients enrolled, including gastric cancer, esophageal carcinoma, pancreatic cancer and cholangiocarcinoma (8, 6, 1 and 1, respectively). The cetuximab was administered 92 cycles totally with median cycle of 4. (2) PR, SD and PD obtained in 4, 3 and 2 patients respectively in cetuximab + chemotherapy group. The disease control rate, median l-rP and median OS were 77.8% (7/9), 3.5 months and 3.5 months, respectively. (3) Both of patients in cetuximab + radiotherapy group achieved PR with median TTP of 4 months and 7 months, with median OS of 14 and 28 months, respectively. (4) in evaluable patients, 3 PR obtained in first-line setting, 1 PD and 1 SD achieved in second-line setting, 2 SD and 1 PD appeared in third-line and more than third-line setting. (5) The commonly side effect was neutropenia, however, rash and hypomagnesemia were the common side effects related to cetuximab. Conclusion: Cetuximab-contained regimens show a promising disease control rate, especially in first-line setting, in patients with noncolorectal gastrointestinal cancer, and the side effects both related to cytotoxic agents and related to cetuximab itself is tolerable and manageable. Of course, more phase Ⅲ clinical trails are needed urgently.展开更多

Methods: 28 patients aged over 65 with advanc...Objective: To investigate the clinical efficacy and toxicity of weekly dose docetaxel monotherapy schedule in elderly with advanced non-small cell lung cancer (NSCLC). Methods: 28 patients aged over 65 with advanced NSCLC were received with docetaxel (Aisu) 35 mg/m^2 on days 1, 8 and 15 every 28 days. A clinical evaluation on effectiveness, quality of life and toxicities was performed. Results: 28 patients were given 86 cycles' chemotherapy altogether. The overall response rate was 35.7% (10/28). The clinical beneficial rate was 64.3% (18/28). Mean KPS was increased from 75.5 at baseline to 87.7 after chemotherapy (P 〈 0.01); lung cancer symptom scale (LCSS) scores of cough, hemoptysis, chest pain and dyspnea were increased from 64, 65, 62 and 65 to 90, 92, 87 and 88, respectively (P 〈 0.01). The median time to progression (TTP) was 5.3 months; median survival time (MST) was 8.5 months. The main toxicities were fatigue, leukopenia and decrease of hemoglobin with well tolerance. Conclusion: Weekly dose docetaxel monotherapy schedule is a feasible, well-tolerated, and active scheme in the treatment of the elderly patients with advanced NSCLC.展开更多

It is also difficult to notice the correlations among the different times of the history without falling in anachronistic mistakes. In fact, it is well-known that a historian must be wise and rational when he finds those correlations, because every society has its own characteristics, structures, and mentality. This brief essay focuses on the social structures in a north eastern italic region, which is called Friuli, between the 12th and 16th centuries. In the above mentioned centuries, it is possible to notice an evolution from a feudal ecclesiastical principality, ruled by prince-patriarchs, ecclesiastical institutions and nobility, to a more fluid and dynamic constitution formed by new classes which rose up in times of urban and economic development. But this development struggles to delete the old system, which persisted until the end of 18th century with the arrival of Napoleon. The transition from the Patriarch's power to Venice's dominion seems to have created two main consequences: by one hand a more bureaucratic and modem structure of the State, by the other hand a consolidation of the old connections between patrons and clients, consequence of the "modem" feudal system that consolidated its roots in the Late Middle Ages centuries.展开更多
To unveil formation mechanism of key sequence boundaries of inland faulting basin and coal accumulation charac- teristics of coal seams in isochrohal stratigraphic framework, sequence stratigraphy, palaeogeographic re...To unveil formation mechanism of key sequence boundaries of inland faulting basin and coal accumulation charac- teristics of coal seams in isochrohal stratigraphic framework, sequence stratigraphy, palaeogeographic recovery and other methods were used to research the sequence stratigraphy and coal accumulation in the example of Banding Basn in the west margin of Yangtze Platform, and the authors advanced a coal accumulation model of Faulting basin on the basis of accommo- dation space changes in the background of palaeogeography of sequence framework. The results show that: normal lacustrine regression and forced lacustrine regression are the main driving forces for the formation of sequence boundaries of Faulting ba- sin; basement subsidence is the main source of accommodation space of Faulting basin; and subsidence disequilibrium is the main cause for the difference in generation rate of accommodation space of Faulting basin. Coal accumulation in Faulting basin is obviously controlled by accommodation space changes in sequence framework and basin evolution. As Faulting basin evolves into depression basin, both subsidence rate of basin basement and generation rate of accommodation space decrease vertically, it appears as a progradational reverse-graded sedimentary sequence, coal accumulation in secondary sequence framework intensifies first and then weakens, and coal accumulation at the middle stage of highstand system tract is the best. During steady rifting period, minable coal seams were mostly developed in initial lacustrine flooding surface of fourth-order sequence and at the middle-late period of highstand system tract, and coal accumulation center lay in palaeogeographic unit of delta plain with moderate available accommodation space. During shrinking period, minable coal seams gradually migrated to the maximum lacustrine flooding surface and coal accumulation center lay in palaeographical unit of lacustrine with large available accommodation space.展开更多
Sedimentary TOC (total organic carbon), TN (total nitrogen) and C/N (carbon/nitrogen) at selected sites of Lake Victoria basin have provided evidence of phytoplankton productivity, input of nitrates and allochth...Sedimentary TOC (total organic carbon), TN (total nitrogen) and C/N (carbon/nitrogen) at selected sites of Lake Victoria basin have provided evidence of phytoplankton productivity, input of nitrates and allochthonous (land) plant materials/catchment destruction in the basin during the late Holocene period (last 4,000 years to present). TOC and TN in the sediment cores were determined using EuroEA3000 Series Elemental Analyser and radiocarbon dating done using AMS (accelerator mass spectrometry) standard method. Results showed that TOC (13.45%-20.78%) and TN (1.07%-1.92%) values were higher at Napoleon Gulf from about 4,186 years before present (ca. 4,186 yr. BP) to present than at inlet of river Kagera (TOC: 3.27%-6.32% and TN: 0.30%-0.59%) from about 190 years before present (ca. 190 yr. BP) to present. Generally TOC and TN had a negative correlation (r = -0.37, p = 0.04, n = 32) at Napoleon Gulf in the last 4,186 years to present signifying that C/N ratios were governed mostly by phytoplankton productivity which markedly increased during some periods in the last 370 years to present possibly as a result of increased input of nitrates. However, the periods from about 4,186 years to 1,684 years before present (ca. 4,186 yr. BP to 1,684 yr. BP) and some periods between the last 370 years to present indicated increased input of allochthonous plant materials/catchment destruction. On overall, there was a positive correlation between C/N and TOC at inlet of river Kagera (r = 0.57, p = 0.01, n = 20) in the last 190 years to present possibly signifying that C/N ratios were governed mainly by input of allochthonous plant materials hence increased catchment destruction. Input of land plant materials (catchment destruction) along Kagera basin declined during some period after 1950 AD. However the most recent years have shown increased input of land plant materials (catchment destruction) in Kagera basin.展开更多

Methods: One hundred and seventeen cases of advanced malignancies were diagnosed and confirmed by histopathological examination, patients were treated with Endostar combined with chemotherapeutic drugs with no cross-resistance. Evaluate the efficacy and adverse reactions after finished two cycles of combination therapy. Results: All the 117 cases of patients were evaluated according to relevant standards, and there were 12 cases of complete remission (CR), 30 cases of partial remission (PR), 58 cases of stable disease (SD), 17 cases of progressive disease (PD). The response rate (RR) was 35.8%, disease control rate (DCR) was 85.4%. Conclusion: The protocol of Endostar combined with chemotherapy could improve the quality of life of patients with malignances, it also has the advantage of low toxicity. Considering the time span of the study, the long-term efficacy remains to be observed.展开更多

Methods: Ninety-two patients who suffered from gastric angle carcinom...Objective: The aim of this study was to explore a new method of lymph node tracing in radical gastrectomy for advanced gastric carcinoma (AGC). Methods: Ninety-two patients who suffered from gastric angle carcinoma with metastasis in No. 3 group lymph nodes were carried out radical gastrectomy. During the operation methylene blue was injected inside or around the circum of the metastatic No. 3 group lymph nodes. Secondary sentinel lymph nodes (SSLNs) were the near- est blue lymph nodes to No. 3 group lymph nodes that were searched in 5 to 10 rain after injecting methylene blue. These SSLNs were resected and carried out hematoxylin-eosin (HE) staining as well as immunohistochemistry (IHC) staining to demonstrate whether there were metastasis. Results: SSLNs were found successfully in 76 patients with a positive rate of 82.6% (76/92) by methylene blue staining, among which, 34 patients were demonstrated existing metastasis in SSLNs by HE staining, 26 patients were demonstrated existing metastasis in SSLNs by IHC staining which were not demonstrated exist- ing metastasis by HE staining. Totally, 60 patients were demonstrated existing metastasis in SSLNs by HE staining and IHC staining with a positive rate of 78.9% (60/76). 136 SSLNs were found in total and 104 SSLNs were demonstrated existing metastasis with a positive rate of 76.4% (104/136). There were 14, 18, 10, 80, 6 and 8 SSLNs in No. 4, No. 5, No. 6, No. 7, No. 8 and No. 9 group lymph nodes respectively. And there were 10, 18, 8, 62, 2 and 4 SSLNs were demonstrated existing metastasis with a positive rate of 71.4%, 100%, 80.0%, 77.5%, 33.3% and 50.0% in No. 4, No~ 5, No. 6, No. 7, No. 8 and No. 9 group lymph nodes respectively. However, there were no significant correlations between the tumor's size and the positive rate of SSLN as well as the degree of tumor's differentiation and the positive rate of SSLN. Conclusion: The technique of SSLN tracing expands the application range of sentinel lymph node (SLN) tracing and provides a new thinking for researching of SLN in AGC.展开更多

Methods: Fifty patients with advanced NSCLC (stages III to IV), who had received therapies in our Center (Department of Biotherapy, Affiliated to Cancer Hospital of Shanxi Medical University, Taiyuan, China) from August 2008 to January 2010, were treated by DC-CIK + chemotherapy as the combined treatment group; fifty advanced NSCLC patients treated with chemotherapy at the same time served as controls. The immunologic function, short-term therapeutic effects, the 1-year survival rate, the life quality, the chemotherapy side effects were compared between the two groups, the safety and therapeutic effects of DC-CIK cells therapy were observed too. Results: There was no obvious change of subsets of T cells in peripheral blood before and after therapy in DC-CIK + chemotherapy group, and IFN-γ was improved after therapy in this group (P < 0.05); in chemotherapy alone group, the ratios of CD3+CD4+, CD3+CD8+, CD3-CD56+ cells and the secretion of IL-2, TNF-α decreased significantly after therapy (P < 0.05); the ratios of CD3+CD8+, CD3+CD56+ were improved after cell culture (P < 0.05). The disease control rate (DCR) of DC-CIK + chemotherapy group was higher than that in the chemotherapy alone group (78.0% vs 56.0%, P < 0.05); the 1-year survival rates of DC-CIK + chemotherapy group and chemotherapy alone group were 50% and 44% respectively, had no significant difference. Compared with chemotherapy alone group, the occurrence of chemotherapy side effects (including bone marrow suppression, nausea and vomiting, peripheral nerve toxicity) was less in the DC-CIK + chemotherapy group (P < 0.05). The physical and appetite were better in DC-CIK + chemotherapy group after therapy. Conclusion: To compare with simple chemotherapy, DC-CIK + chemotherapy for advanced NSCLC is safe and effective, and it can improve patients' life quality and remission rate, and prolong their survival time.展开更多

Here, we present the detailed geochronological, geochemical and petrological analyses fo...The late Permian–Triassic granites in southeastern China have important tectonic significance for the evolution of South China. Here, we present the detailed geochronological, geochemical and petrological analyses for the Jinlongyan(JLY) granite in northwest Fujian Province, southeast China. LA–ICP–MS zircon U–Pb dating yielded a weighted average ^(206)Pb/^(238)U age of 224.1 ±3.3 Ma. The granite is mainly comprised of K-feldspar,plagioclase, quartz, biotite and minor amphibole. It is characterized by enrichments in Rb, Th, REEs(total REE = 295.1–694.3 ppm), and HFSEs(e.g., Zr = 289–520 ppm, Hf = 9.3–15.0 ppm, Y = 36.2–68.2 ppm) but depletions in Ba, Sr, Eu and Ti. The granite is metaluminous to weakly peraluminous and show a clear A-type granite geochemical signature with high SiO_2(70.89 wt%–75.76 wt%), total alkalis(Na_2O + K_2O = 7.51 wt%–8.72 wt%), Ga/Al ratios(10000 Ga/Al = 2.72–3.43). Insitu zircon Hf isotope analysis shows their eHf(t) values ranging from-7.2 to-3.2, with Mesoproterozoic T2DM ages(1308–1525 Ma). Whole-rock Nd isotope data show their eNd(t) values in the range of-9.5 to-9.1 and yield paleoproterozoic TDMages(1606–1985 Ma). These characteristics indicate that the JLY A-type granite magma was formed by the partial melting of Meso-Paleoproterozoic crust rocks in the Cathaysia Block. Structural relationships between the Neoproterozoic rock complexes of a continental massif, island arc and back-arc basin geodynamic affinities are described and considered in this work based on field observations wit...Structural relationships between the Neoproterozoic rock complexes of a continental massif, island arc and back-arc basin geodynamic affinities are described and considered in this work based on field observations within the northeastern segment of the Central Taimyr tectonic zone distinguished in the late Hercynian fold- thrust belt of Taimyr Peninsula. As is established for the first time, rock complexes of the continental massif with the early Late Riphean (Tonian-Cryogenian) volcanogenic-sedimentary cover occur in the study region as the allochthonous syn- and post-sedimentary thrust sheets buried in or thrust over deposits of a back-arc basin, which accumulated in the terminal Late Riphean (Cryogcnian)-initial Vendian (Ediacaran). These and other results of the large-scale structural observations elucidate important details of the region tectonic development in the Late Precambrian, when two lateral ensembles of the Neoproterozoic structures originated in the region. In the first half of the Neoproterozoic, the regional tectonic ensemble included the oceanic plate abut on the continental massif with a primitive volcano-plutonic belt. The subsequent system of an island arc and marginal back- arc basin originated in the second half of the Neoproterozoic and existed approximately till the mid-Vendian (Ediacaran) phase of the intense formation of thrust sheets in the region.展开更多
文摘AIM: To evaluate the efficacy of thalidomide in com- bination with other therapies to treat patients with ad- vanced hepatocellular carcinoma (HCC). METHODS: We performed a retrospective analysis of all patients with HCC who were treated with thalido- mide for at least two months. The medical records of patients with HCC who were treated at our institution between April 2003 and March 2008 were reviewed. Image studies performed before and after treatment, tumor response, overall survival, and the decrease in o-fetoprotein (AFP) levels were evaluated. RESULTS: A total of 53 patients with HCC received either 100 or 200 mg/d of thalidomide. The patient population consisted of 9 women and 44 men with a median age of 61 years. Thirty patients (56.6%) were classified as Child-Pugh A, and 12 patients (22.6%) were classified as Child-Pugh B. Twenty-six patients had portal vein thrombosis (49.1%), and 25 patients had extrahepatic metastasis (47.1%). The median duration of thalidomide treatment was 6.0 mo. Six of the 53 patients achieved a confirmed response (11.3%), one achieved a complete response (1.9%) and 5 achieved a partial response (9.4%). The disease control rate (CR + PR + SD) was 28.3% (95% CI: 17.8-42.4), and the median overall survival rate was 10.5 too. The 1- and 2-year survival rates were 45% and 20%, respectively. Only one complete response patient showed an im- proved overall survival rate of 66.8 mo. Sixteen patients (30.2%) showed more than a 50% decrease in their serum AFP levels from baseline, indicating a better re- sponse rate (31.3%), disease control rate (43.8%), and overall survival time (20.7 mo). The therapy was well tolerated, and no significant toxicities were observed. CONCLUSION: Thalidomide was found to be safe for advanced HCC patients, demonstrating anti-tumor ac- tivity including response, survival, and AFP decreases of greater than 50% from baseline.
文摘Intrahepatic cholestasis of pregnancy (ICP) is a pregnancy-specific liver disorder characterized by maternal pruritus in the third trimester, raised serum bile acids and increased rates of adverse fetal outcomes. The etiology of ICP is complex and not fully understood, but it is likely to result from the cholestatic effects of reproductive hormones and their metabolites in genetically susceptible women. Equally unclear are the mechanisms by which the fetal complications occur. This article reviews the epidemiology, clinical features, diagnosis, etiology and management of ICP.
文摘AIM: To investigate the therapeutic effects of Fuzheng Yiliu (strengthening the body resistance to inhibit tumor) decoction combined with chemotherapy on the patients with intermediate and late stage gastrointestinal cancer. METHODS: Sixty patients were randomly divided into treatment group (chemotherapy combined with Fuzheng Yiliu decoction) and control group (chemotherapy alone). Four indexes, including the tumor recent remission rate (RR), the change of main symptoms, the toxic and side effects caused by chemotherapy and the change of performance status, were observed in the patients. Peripheral blood contents of CD3+, CD4+, CD8+ cells, CD4+/CD8+ and soluble interleukin-2 receptor (sIL-2R) were tested before and after treatment and the values were compared with those of healthy peoples. RESULTS: The improving rate of main symptoms (69.6%) and performance status (56.7%) were significantly higher in the treatment group than in the control group (34.8%, 26.7%, P<0.05). The occurrence rates of grade II toxic and side-effects on both bone marrow (13.3%) and digestive tract (30%) were lower in the treatment group compared to the control group (36.7%, 63.3%, P<0.05). Before treatment, the proportion of CD3+, CD4+ and CD4+/CD8+ decreased and the proportion of CD8+ and SIL-2R raised markedly both in the control group and treatment group as compared to the healttiy people. After treatment, that increased of CD3+, CD4+, CD4+/CD8+ increased (62.25±10.01% vs 68.31±9.72%, 36.83±10.44% vs 42.6±9.62%, 1.24±0.65 vs l.66±0.85, P<0.05) and the values of CD8+ and sIL-2R decreased obviously (33.06±7.69% vs 29.24±6.25%, 588.23±216.86 U/mL vs 475.87±211.36 U/mL,P<0.05) in the treatment group, whereas these values were opposite in the control group (64.22±6.91% vs60.63±5.75%,35.62±7.49% vs31.53±5.53%, 32.95±8.28% vs 37.14±7.48%, 1.17±0.43 vs 0.94±0.43, 573.63±214.32 U/mL vs 692.17?21.33 U/mL, P<0.05). CONCLUSION: Fuzheng Yiliu decoction can enhance therapeutic effects of chemotherapy on malignant gastrointestinal tumor, and also reduce the toxic and side effects on bone marrow and digestive tract, thereby improving the quality of life and cellular immunity in patients with malignant gastrointestinal tumor.

文摘AIM:To evaluate the effectiveness of hepatic arterial infusion chemotherapy(HAIC) for advanced hepatocellular carcinoma(HCC) resistant to transarterial chemoembolization(TACE).METHODS:This study was conducted on 42 patients who received HAIC for advanced HCC between 2001and 2010 at our hospital.5-fluorouracil(5-FU) was administered continuously for 24 h from day 1 to day 5 every 2-4 wk via an injection reservoir.Intra-arterial cisplatin or subcutaneous interferon was administered in combination with the 5-FU.The patients enrolled in this retrospective study were divided into two groups according to whether or not they fulfilled the criteria for resistance to TACE proposed by the Japan Society of Hepatology in 2010(written in Japanese);one group of patients who did not fulfill the criteria for TACE resistance(group A,n = 23),and another group who fulfilled the criteria for TACE resistance(group B,n = 19).We compared the outcomes in terms of the response and survival rates between the two groups.RESULTS:Both the response rate and tumor suppression rate following HAIC were significantly superior in group A than in group B(response rate:48% vs 16%,P = 0.028,tumor suppression rate:87% vs 53%,P = 0.014).Furthermore,both the progression-free survival rate and survival time were significantly superior in group A than in group B(3-,6-,12-,and 24-mo = 83%,70%,29% and 20% vs 63%,42%,16% and 0%,respectively,P = 0.040,and 9.8 mo vs 6.2 mo,P = 0.040).A multivariate analysis(Cox proportional hazards regression model) showed that resistance to TACE was an independent predictor of poor survival(P = 0.007).CONCLUSION:HAIC administrating 5-FU was not effective against advanced HCC resistant to TACE.Other tools for treatment,i.e.,molecular-targeting agents may be considered for these cases.
文摘The incidence of early gastric cancer (EGC) with duodenal invasion is extremely low, although advanced gastric cancer that arises in the antrum occasionally invades the duodenum. We investigated the clinicopathological features of EGC with duodenal invasion and provided strategies for clinical management.A Medline search was performed using the keyword early gastric cancer" and "duodenal invasion': Additional articles were obtained from references within the papers identified by the Medline search. We revealed that EGC with duodenal invasion was of the superficial spreading type of tumor. Tumors 〉 60 mm in size invaded the duodenum more extensively, and the distance of duodenal invasion from the pyloric ring was further in the elevated type than in the depressed type of tumor.There was no significant difference between the length of duodenal invasion and the histological type of the tumor. Gastric cancer located adjacent to the pyloric ring, even if cancer invasion was confined to the mucosa or submucosa, was more likely to invade the duodenum.The present study reveals that the elevated type of EGC is associated with more extensive duodenal invasion when the tumor size is 〉 60 ram, thus highlighting the importance of identification of duodenal invasion in these cases. We also reveal that sufficient duodenal resection with a cancer-free distal surgical margin should be performed in cases of duodenal invasion.
文摘Objective: The aim of this study was to explore whether low dose Glycididazolum natrium (CMNa) used during intervention for advanced non-small cell lung cancer (NSCLC) patients can enhance chemo-sensitivity and to evaluate its clinical value. Methods: One hundred and twenty cases of advanced NSCLC patients treated by intervention chemotherapy through bronchial artery, collected from 2005 to 2008, were randomly divided into two groups: experimental group (group A), 75 cases, 0.25g CMNa was administered before chemical drugs; control group (group B), 45 cases, chemical drugs only. There were no differences of operation procedures and chemo-regimens between the two groups. After intervention, imaging and clinical data were collected periodically and were processed statistically. Results: The effective rate of group A was higher than that of group B (P < 0.05). Responsive rate (RR) in group A was 61% and it was 38% in group B. There were some side effects in the two groups but there were no statistical differences. Conclusion: Low dose CMNa can enhance chemosensitivity during intervention, which can elevate chemotherapeutical effects on NSCLC. Meanwhile the side effects were not increased. This method is worthy of popularizing.

文摘Objective: Cetuximab is an effective medicine for colorectal cancer bearing wild type Kras. However, few studies of cetuximab on patients with noncolorectal gastrointestinal cancer were available now. The aim of this study was to summarize the efficacy and safety profiles base-on the experiences of 16 patients with noncolorectal gastrointestinal cancer accepted cetuximab-contained regimens treatment. Methods: The clinical data of all patients with noncolorectal gastrointestinal cancer treated by cetuximab combined with chemotherapy and/or radiotherapy from October 1st 2006 to Jun 30th 2009 in Sun Yat-sen University Cancer Center were collected. Results: (1) A total of 16 patients enrolled, including gastric cancer, esophageal carcinoma, pancreatic cancer and cholangiocarcinoma (8, 6, 1 and 1, respectively). The cetuximab was administered 92 cycles totally with median cycle of 4. (2) PR, SD and PD obtained in 4, 3 and 2 patients respectively in cetuximab + chemotherapy group. The disease control rate, median l-rP and median OS were 77.8% (7/9), 3.5 months and 3.5 months, respectively. (3) Both of patients in cetuximab + radiotherapy group achieved PR with median TTP of 4 months and 7 months, with median OS of 14 and 28 months, respectively. (4) in evaluable patients, 3 PR obtained in first-line setting, 1 PD and 1 SD achieved in second-line setting, 2 SD and 1 PD appeared in third-line and more than third-line setting. (5) The commonly side effect was neutropenia, however, rash and hypomagnesemia were the common side effects related to cetuximab. Conclusion: Cetuximab-contained regimens show a promising disease control rate, especially in first-line setting, in patients with noncolorectal gastrointestinal cancer, and the side effects both related to cytotoxic agents and related to cetuximab itself is tolerable and manageable. Of course, more phase Ⅲ clinical trails are needed urgently.
文摘Objective: To investigate the clinical efficacy and toxicity of weekly dose docetaxel monotherapy schedule in elderly with advanced non-small cell lung cancer (NSCLC). Methods: 28 patients aged over 65 with advanced NSCLC were received with docetaxel (Aisu) 35 mg/m^2 on days 1, 8 and 15 every 28 days. A clinical evaluation on effectiveness, quality of life and toxicities was performed. Results: 28 patients were given 86 cycles' chemotherapy altogether. The overall response rate was 35.7% (10/28). The clinical beneficial rate was 64.3% (18/28). Mean KPS was increased from 75.5 at baseline to 87.7 after chemotherapy (P 〈 0.01); lung cancer symptom scale (LCSS) scores of cough, hemoptysis, chest pain and dyspnea were increased from 64, 65, 62 and 65 to 90, 92, 87 and 88, respectively (P 〈 0.01). The median time to progression (TTP) was 5.3 months; median survival time (MST) was 8.5 months. The main toxicities were fatigue, leukopenia and decrease of hemoglobin with well tolerance. Conclusion: Weekly dose docetaxel monotherapy schedule is a feasible, well-tolerated, and active scheme in the treatment of the elderly patients with advanced NSCLC.
文摘It is a very hard job for historians to keep themselves neutrals in from of the events they find during their studies. It is also difficult to notice the correlations among the different times of the history without falling in anachronistic mistakes. In fact, it is well-known that a historian must be wise and rational when he finds those correlations, because every society has its own characteristics, structures, and mentality. This brief essay focuses on the social structures in a north eastern italic region, which is called Friuli, between the 12th and 16th centuries. In the above mentioned centuries, it is possible to notice an evolution from a feudal ecclesiastical principality, ruled by prince-patriarchs, ecclesiastical institutions and nobility, to a more fluid and dynamic constitution formed by new classes which rose up in times of urban and economic development. But this development struggles to delete the old system, which persisted until the end of 18th century with the arrival of Napoleon. The transition from the Patriarch's power to Venice's dominion seems to have created two main consequences: by one hand a more bureaucratic and modem structure of the State, by the other hand a consolidation of the old connections between patrons and clients, consequence of the "modem" feudal system that consolidated its roots in the Late Middle Ages centuries.

文摘To unveil formation mechanism of key sequence boundaries of inland faulting basin and coal accumulation charac- teristics of coal seams in isochrohal stratigraphic framework, sequence stratigraphy, palaeogeographic recovery and other methods were used to research the sequence stratigraphy and coal accumulation in the example of Banding Basn in the west margin of Yangtze Platform, and the authors advanced a coal accumulation model of Faulting basin on the basis of accommo- dation space changes in the background of palaeogeography of sequence framework. The results show that: normal lacustrine regression and forced lacustrine regression are the main driving forces for the formation of sequence boundaries of Faulting ba- sin; basement subsidence is the main source of accommodation space of Faulting basin; and subsidence disequilibrium is the main cause for the difference in generation rate of accommodation space of Faulting basin. Coal accumulation in Faulting basin is obviously controlled by accommodation space changes in sequence framework and basin evolution. As Faulting basin evolves into depression basin, both subsidence rate of basin basement and generation rate of accommodation space decrease vertically, it appears as a progradational reverse-graded sedimentary sequence, coal accumulation in secondary sequence framework intensifies first and then weakens, and coal accumulation at the middle stage of highstand system tract is the best. During steady rifting period, minable coal seams were mostly developed in initial lacustrine flooding surface of fourth-order sequence and at the middle-late period of highstand system tract, and coal accumulation center lay in palaeogeographic unit of delta plain with moderate available accommodation space. During shrinking period, minable coal seams gradually migrated to the maximum lacustrine flooding surface and coal accumulation center lay in palaeographical unit of lacustrine with large available accommodation space.
文摘Sedimentary TOC (total organic carbon), TN (total nitrogen) and C/N (carbon/nitrogen) at selected sites of Lake Victoria basin have provided evidence of phytoplankton productivity, input of nitrates and allochthonous (land) plant materials/catchment destruction in the basin during the late Holocene period (last 4,000 years to present). TOC and TN in the sediment cores were determined using EuroEA3000 Series Elemental Analyser and radiocarbon dating done using AMS (accelerator mass spectrometry) standard method. Results showed that TOC (13.45%-20.78%) and TN (1.07%-1.92%) values were higher at Napoleon Gulf from about 4,186 years before present (ca. 4,186 yr. BP) to present than at inlet of river Kagera (TOC: 3.27%-6.32% and TN: 0.30%-0.59%) from about 190 years before present (ca. 190 yr. BP) to present. Generally TOC and TN had a negative correlation (r = -0.37, p = 0.04, n = 32) at Napoleon Gulf in the last 4,186 years to present signifying that C/N ratios were governed mostly by phytoplankton productivity which markedly increased during some periods in the last 370 years to present possibly as a result of increased input of nitrates. However, the periods from about 4,186 years to 1,684 years before present (ca. 4,186 yr. BP to 1,684 yr. BP) and some periods between the last 370 years to present indicated increased input of allochthonous plant materials/catchment destruction. On overall, there was a positive correlation between C/N and TOC at inlet of river Kagera (r = 0.57, p = 0.01, n = 20) in the last 190 years to present possibly signifying that C/N ratios were governed mainly by input of allochthonous plant materials hence increased catchment destruction. Input of land plant materials (catchment destruction) along Kagera basin declined during some period after 1950 AD. However the most recent years have shown increased input of land plant materials (catchment destruction) in Kagera basin.
文摘Objective: The aim of this study was to evaluate the recent efficacy and adverse reactions of recombinant human endostatin (Endostar) plus chemotherapy in the treatment of advanced malignancies. Methods: One hundred and seventeen cases of advanced malignancies were diagnosed and confirmed by histopathological examination, patients were treated with Endostar combined with chemotherapeutic drugs with no cross-resistance. Evaluate the efficacy and adverse reactions after finished two cycles of combination therapy. Results: All the 117 cases of patients were evaluated according to relevant standards, and there were 12 cases of complete remission (CR), 30 cases of partial remission (PR), 58 cases of stable disease (SD), 17 cases of progressive disease (PD). The response rate (RR) was 35.8%, disease control rate (DCR) was 85.4%. Conclusion: The protocol of Endostar combined with chemotherapy could improve the quality of life of patients with malignances, it also has the advantage of low toxicity. Considering the time span of the study, the long-term efficacy remains to be observed.

文摘Objective: The aim of this study was to explore a new method of lymph node tracing in radical gastrectomy for advanced gastric carcinoma (AGC). Methods: Ninety-two patients who suffered from gastric angle carcinoma with metastasis in No. 3 group lymph nodes were carried out radical gastrectomy. During the operation methylene blue was injected inside or around the circum of the metastatic No. 3 group lymph nodes. Secondary sentinel lymph nodes (SSLNs) were the near- est blue lymph nodes to No. 3 group lymph nodes that were searched in 5 to 10 rain after injecting methylene blue. These SSLNs were resected and carried out hematoxylin-eosin (HE) staining as well as immunohistochemistry (IHC) staining to demonstrate whether there were metastasis. Results: SSLNs were found successfully in 76 patients with a positive rate of 82.6% (76/92) by methylene blue staining, among which, 34 patients were demonstrated existing metastasis in SSLNs by HE staining, 26 patients were demonstrated existing metastasis in SSLNs by IHC staining which were not demonstrated exist- ing metastasis by HE staining. Totally, 60 patients were demonstrated existing metastasis in SSLNs by HE staining and IHC staining with a positive rate of 78.9% (60/76). 136 SSLNs were found in total and 104 SSLNs were demonstrated existing metastasis with a positive rate of 76.4% (104/136). There were 14, 18, 10, 80, 6 and 8 SSLNs in No. 4, No. 5, No. 6, No. 7, No. 8 and No. 9 group lymph nodes respectively. And there were 10, 18, 8, 62, 2 and 4 SSLNs were demonstrated existing metastasis with a positive rate of 71.4%, 100%, 80.0%, 77.5%, 33.3% and 50.0% in No. 4, No~ 5, No. 6, No. 7, No. 8 and No. 9 group lymph nodes respectively. However, there were no significant correlations between the tumor's size and the positive rate of SSLN as well as the degree of tumor's differentiation and the positive rate of SSLN. Conclusion: The technique of SSLN tracing expands the application range of sentinel lymph node (SLN) tracing and provides a new thinking for researching of SLN in AGC.

文摘Objective: The aim of the study was to evaluate the safety and therapeutic effects of autologous dendritic cells co-cultured with cytokine-induced killer cells (DC-CIK) combined with chemotherapy in advanced non-small cell lung cancer (NSCLC) patients. Methods: Fifty patients with advanced NSCLC (stages III to IV), who had received therapies in our Center (Department of Biotherapy, Affiliated to Cancer Hospital of Shanxi Medical University, Taiyuan, China) from August 2008 to January 2010, were treated by DC-CIK + chemotherapy as the combined treatment group; fifty advanced NSCLC patients treated with chemotherapy at the same time served as controls. The immunologic function, short-term therapeutic effects, the 1-year survival rate, the life quality, the chemotherapy side effects were compared between the two groups, the safety and therapeutic effects of DC-CIK cells therapy were observed too. Results: There was no obvious change of subsets of T cells in peripheral blood before and after therapy in DC-CIK + chemotherapy group, and IFN-γ was improved after therapy in this group (P < 0.05); in chemotherapy alone group, the ratios of CD3+CD4+, CD3+CD8+, CD3-CD56+ cells and the secretion of IL-2, TNF-α decreased significantly after therapy (P < 0.05); the ratios of CD3+CD8+, CD3+CD56+ were improved after cell culture (P < 0.05). The disease control rate (DCR) of DC-CIK + chemotherapy group was higher than that in the chemotherapy alone group (78.0% vs 56.0%, P < 0.05); the 1-year survival rates of DC-CIK + chemotherapy group and chemotherapy alone group were 50% and 44% respectively, had no significant difference. Compared with chemotherapy alone group, the occurrence of chemotherapy side effects (including bone marrow suppression, nausea and vomiting, peripheral nerve toxicity) was less in the DC-CIK + chemotherapy group (P < 0.05). The physical and appetite were better in DC-CIK + chemotherapy group after therapy. Conclusion: To compare with simple chemotherapy, DC-CIK + chemotherapy for advanced NSCLC is safe and effective, and it can improve patients' life quality and remission rate, and prolong their survival time.

文摘The late Permian–Triassic granites in southeastern China have important tectonic significance for the evolution of South China. Here, we present the detailed geochronological, geochemical and petrological analyses for the Jinlongyan(JLY) granite in northwest Fujian Province, southeast China. LA–ICP–MS zircon U–Pb dating yielded a weighted average ^(206)Pb/^(238)U age of 224.1 ±3.3 Ma. The granite is mainly comprised of K-feldspar,plagioclase, quartz, biotite and minor amphibole. It is characterized by enrichments in Rb, Th, REEs(total REE = 295.1–694.3 ppm), and HFSEs(e.g., Zr = 289–520 ppm, Hf = 9.3–15.0 ppm, Y = 36.2–68.2 ppm) but depletions in Ba, Sr, Eu and Ti. The granite is metaluminous to weakly peraluminous and show a clear A-type granite geochemical signature with high SiO_2(70.89 wt%–75.76 wt%), total alkalis(Na_2O + K_2O = 7.51 wt%–8.72 wt%), Ga/Al ratios(10000 Ga/Al = 2.72–3.43). Insitu zircon Hf isotope analysis shows their eHf(t) values ranging from-7.2 to-3.2, with Mesoproterozoic T2DM ages(1308–1525 Ma). Whole-rock Nd isotope data show their eNd(t) values in the range of-9.5 to-9.1 and yield paleoproterozoic TDMages(1606–1985 Ma). These characteristics indicate that the JLY A-type granite magma was formed by the partial melting of Meso-Paleoproterozoic crust rocks in the Cathaysia Block. 文摘Structural relationships between the Neoproterozoic rock complexes of a continental massif, island arc and back-arc basin geodynamic affinities are described and considered in this work based on field observations within the northeastern segment of the Central Taimyr tectonic zone distinguished in the late Hercynian fold- thrust belt of Taimyr Peninsula. As is established for the first time, rock complexes of the continental massif with the early Late Riphean (Tonian-Cryogenian) volcanogenic-sedimentary cover occur in the study region as the allochthonous syn- and post-sedimentary thrust sheets buried in or thrust over deposits of a back-arc basin, which accumulated in the terminal Late Riphean (Cryogcnian)-initial Vendian (Ediacaran). These and other results of the large-scale structural observations elucidate important details of the region tectonic development in the Late Precambrian, when two lateral ensembles of the Neoproterozoic structures originated in the region. In the first half of the Neoproterozoic, the regional tectonic ensemble included the oceanic plate abut on the continental massif with a primitive volcano-plutonic belt. The subsequent system of an island arc and marginal back- arc basin originated in the second half of the Neoproterozoic and existed approximately till the mid-Vendian (Ediacaran) phase of the intense formation of thrust sheets in the region.
