维持性血液透析病人干体重预测方程的构建和验证 被引量:6

[目的]采用Basile的“四步法”构建基于生物电阻抗分析的维持性血液透析病人干体重预测方程,并对方程进行验证。[方法]便利抽取上海市某三级甲等医院的138例维持性血液透析病人作为血液透析组,选取上海市某三级甲等医院的816名体检者作为对照组,选取上海市其他两所医院的35例维持性血液透析病人作为验证组。对Basile的血液透析病人干体重预测方程构建方法进行调整,利用血液透析组、对照组、验证组人群数据构建并验证适用我国维持性血液透析病人的干体重预测方程。[结果]多元线性回归分析结果显示:397名正常体质指数(BMI)的男性体检者体重与其年龄、身高、50 kHz电阻值(R50)呈线性相关(R2=0.783,P<0.001),378名正常BMI的女性体检者体重与身高、R50呈线性相关(R2=0.662,P<0.001)。矫正后男性维持性血液透析病人干体重=(0.859427×身高-0.025143×年龄-0.034933×R50-59.867424)×透析后BMI/21.76;矫正后女性维持性血液透析病人干体重=(0.697978×身高-0.032994×R50-34.080396)×透析后BMI/20.44。经验证组病人数据验证,结果显示:验证组男性和女性干体重实际值和预测值比较,差异均无统计学意义(P>0.05)。[结论]构建的维持性血液透析病人干体重预测方程对病人干体重的预测值和实际值间有较好一致性,生物电阻抗分析法可以与临床上的其他方法结合起来更好地指导医护人员对病人进行容量管理。 展开更多

目的:探讨传统腹腔镜直肠癌低位前切除术(L-LAR)患者术后吻合口瘘(AL)发生的相关危险因素分析。方法:回顾性选取2017年5月—2022年5月334例天水市第一人民医院普通外科接受L-LAR术治疗的直肠癌患者的临床资料。统计所有研究对象的一般资... 目的:探讨传统腹腔镜直肠癌低位前切除术(L-LAR)患者术后吻合口瘘(AL)发生的相关危险因素分析。方法:回顾性选取2017年5月—2022年5月334例天水市第一人民医院普通外科接受L-LAR术治疗的直肠癌患者的临床资料。统计所有研究对象的一般资料,通过单因素及多因素Logistic回归分析,筛选出影响L-LAR患者术后AL发生的相关因素。结果:发生组中男性患者、有糖尿病、有术前肠梗阻、有术后贫血、有血管浸润、术前血清Alb<30g/L、术中出血量≥100ml、手术时间≥180min、术后血清Alb<30g/L的患者占比高于未发生组(P<0.05);多因素非条件Logistic回归模型分析结果显示,男性患者、有术前肠梗阻、有血管浸润、术前血清Alb<30g/L、手术时间≥180min、术后血清Alb<30g/L均为行L-LAR术患者术后AL发生的危险因素(均P<0.05)。结论:L-LAR术后吻合口瘘发生诱导因素较多,临床上可采取相应干预策略来预防,改善患者的预后。 展开更多

With a more complex pore structure system compared with clastic rocks, carbonate rocks have not yet been well described by existing conventional rock physical models concerning the pore structure vagary as well as the influence on elastic rock properties. We start with a discussion and an analysis about carbonate rock pore structure utilizing rock slices. Then, given appropriate assumptions, we introduce a new approach to modeling carbonate rocks and construct a pore structure algorithm to identify pore structure mutation with a basis on the Gassmann equation and the Eshelby-Walsh ellipsoid inclusion crack theory. Finally, we compute a single well's porosity using this new approach with full wave log data and make a comparison with the predicted result of traditional method and simultaneously invert for reservoir parameters. The study results reveal that the rock pore structure can significantly influence the rocks' elastic properties and the predicted porosity error of the new modeling approach is merely 0.74%. Therefore, the approach we introduce can effectively decrease the predicted error of reservoir parameters. 展开更多

The finite-element (FE) model and the Rosenthal equation are used to study the thermal and microstructural phenomena in the laser powder-bed fusion of lnconel 718. A primary aim is to comprehend the advantages and disadvantages of the Rosenthal equation (which provides an analytical alternative to FE analysis), and to investigate the influence of underlying assumptions on estimated results. Various physical characteristics are compared among the FE model, Rosenthal equation, and experiments. The predicted melt pool shapes compared with reported experimental results from the literature show that both the FE model and the analytical (Rosenthal) equation provide a reasonably accurate estimation. At high heat input, under conditions leading to keyholing, the reported melt width is narrower than predicted by the analytical equation. Moreover, a sensitivity analysis based on choices of the absorptivity is performed, which shows that the Rosenthal approach is more sensitive to absorptivity, compared with the FE approach. The primary reason could be the effect of radiative and convective losses, which are assumed to be negligible in the Rosenthal equation. In addition, both methods predict a columnar solidification microstructure, which agrees well with experimental reports, and the primary dendrite arm spacing (PDAS) predicted with the two approaches is comparable with measurements. 展开更多

This paper describes a structural equation modeling (SEM) analysis of factors influencing architects' trust in project design teams. We undertook a survey of architects, during which we distributed 193 questionnaires in 29 A-level architectural design institutes selected radomly from the altogether 59 ones in Shenzhen, P. R. China, and received 130 valid questionnaires. We used Amos 6.0 for SEM to identify significant personal construct based factors affecting interpersonal trust. The results show that only social interaction between architects significantly affects their interpersonal trust. The explained variance of trus~ is not very high in the model. Therefore, future research should add more factors into the current model. The practical implication is that team managers should promote the social interactions between team members such that the interpersonal trust level between team members can be improved. 展开更多

This paper investigates the hypotheses of influences of CPTED on FOC (fear of crime) and SOC (sense of community). A total of 171 respondents from the designated residential areas were involved in this study. The structural equation modeling technique was employed and the results indicate that CPTED has a direct influence on fear of crime and sense of community. Previous studies on the relationship between exPosure to televised violence and viewers' aggressive behaviours have produced mixed results. Some studies have found significant effect sizes while others have found low and non-significant effect sizes. Television effects scholars have postulated that these mixed results are caused by the inconsistencies of research methodology employed and the inability to control major mediating variables in the studies. The present study is designed with the objectives to overcome these shortcomings. The respondents for this study were 514 students aged between 13 and 18 from 10 schools in the State of Perak, Malaysia. Exploratory data analyses provided evidence that the employed instruments achieved sound psychometric properties. Hypotheses testing suggested that exposure to the Contents of Television Violence (TVCN) did not have a direct relationship with adolescents' aggressive behaviours; instead, Contextual Features of Television Violence (TVCX) totally mediated this relationship. Some other mediating variables then mediated totally and some mediated partially the relationship between TVCX and adolescents' aggressive behaviours. To conclude, this study provides valuable information for parents, the television industry, and the policy makers in recognizing pro-violent and anti-violent features of television violence programmes for Malaysian school-going adolescents. It also provides a new perspective for future studies of television violence in Malaysia. 展开更多

Based on relating equation group, a simplified method was presented in terms of the matrix displacement method, which can be conveniently used to study the re-distribution of the internal forces and displacement of truss structures due to the removal of members. Such removal is treated as though adding a load case to the original truss, and the re-distribution can be calculated without modifying the original global stiffness matrix. The computational efficiency of the presented method is faster by square times than that of the matrix displacement method. The results from the two methods are identical. 展开更多
