Three-dimensional endoanal ultrasonographic assessment of an anal fistula with and without H_2O_2 enhancement 被引量:25

AIM: To evaluate the effectiveness of three-dimensional endoanal ultrasound (3D-EAUS) in the assessment of anal fistulae with and without H202 enhancement. METHODS: Sixty-one patients (37 males, aged 17-74 years... AIM: To evaluate the effectiveness of three-dimensional endoanal ultrasound (3D-EAUS) in the assessment of anal fistulae with and without H202 enhancement. METHODS: Sixty-one patients (37 males, aged 17-74 years) with anal fistulae, which were not simple low types, were evaluated by physical examination and 3D-EAUS with and without enhancement. Fistula classification was determined with each modality and compared to operative findings as the reference standard. RESULTS: The accuracy of 3D-EAUS was significantly higher than that of physical examination in detecting the primary tract (84.4% vs 68.7%, P = 0.037) and secondary extension (81.8% vs 62.1%, P = 0.01) and localizing the internal opening (84.2% vs 59.7%, P = 0.004). A contrast study with H202 detected several more fistula components including two primary suprasphincteric fistula tracks and one supralevator secondary extension, which were not detected on non-contrast study. However, there was no significant difference in accuracy between 3D-EAUS and H202- enhanced 3D-EAUS with respect to classification of the primary tract (84.4% vs 89.1%, P = 0.435) or secondary extension (81.8% vs 86.4%, P = 0.435) or localization of the internal opening (84.2% vs 89.5%, P = 0.406). CONCLUSION: 3D-EAUS was highly reliable in the diagnosis of an anal fistula. H2O2 enhancement was helpful at times and selective use in difficult cases may be economical and reliable. 展开更多

The answer is a key aspect of a longwall mining feasibility study when the consequences of inadequately rated shields are considered.This paper addresses this question based on the measured nature of the loading environment in which shields are required to operate,the various geological and geometrical controls of that environment and the various links between their load rating,a range of other relevant shield design factors and the loss event they are required to prevent a major roof collapse on the longwall face.The paper concludes that despite the tremendous advances that have been made in shield design and load rating over the past50 years,the same drivers that caused longwall miners of the past to seek improved roof control on the longwall face via the use of ever-higher rated shields,are still as relevant today.However at the current time,the limits of the largest available longwall shields have yet to be tested,therefore industry focus for the foreseeable future should possibly be in achieving the maximum level of roof control on the face via their optimum operational use rather than considering further shield rating increases and incurring the inevitable downsides in terms of capital cost and shield weight. 展开更多

This article investigates the role of the body in Jean-Luc Nancy's powerful essay Corpus, and critiques it from the standpoint of Agamben's biopolitics. For Nancy, the body becomes the privileged site of both existence and sense in a way that threatens to obscure the logic of exceptional decision that Agamben takes from Carl Schmitt. As an alternative to Nancy's understanding of the body, we can see in Deleuze a series of bodies that works in parallel to a series of sense or language, where sense and body do not get collapsed into each other. At the same time, contemporary continental philosophy resists the idealistic separation of thinking and embodiment. Deleuze calls this relation between sense and body a cut, but we could also consider a parallax, following Slavoj Zizek. Finally, African-American historian of religion Charles H. Long's work complements some of Deleuze's insights in a more explicitly postcolonial context. 展开更多
