As in most of Africa, marriage is a valued institution that ensures the continuation of the family tree. The emergence of CWS is gathering momentum fast in many communities in the country much to the dislike,of the church and custodians of tradition who perceive CWS to be a cheap union form that breaches marriage norms. This paper seeks to address whether the emergence of CWS has affected marriage perceptions in MD. Initially, quantitative data were collected via online survey disseminated via Facebook. The time-efficiency, economic viability, convenience and anonymity of this method made it a viable method for this study. Later, however, due to a low response to the Facebook link, the online survey was constructed into a questionnaire and administered to the same target population. Questionnaires were relevant to this study because they could be self-administered. However, the questionnaires were limited as they did not allow for attention to details. The data were analysed using content analysis.展开更多

We treated a 22-yearold male patient with malnutrition as a result of PDPV and a previously performed operation for scoliosis, who s...A preduodenal portal vein (PDPV) is known to be a rare cause of duodenal stenosis. We treated a 22-yearold male patient with malnutrition as a result of PDPV and a previously performed operation for scoliosis, who showed an improvement in quality of life after being treated with a combination of nutritional support and surgery. The patient with PDPV had been admitted to our department with duodenal stenosis, ranging from the first to third portions. He had suffered from vomiting since 1 year of age, and he developed malnutrition during the last 6-too period after orthopedic surgery for scoliosis. The stenosis was related to both the PDPV and the previously performed operation for scoliosis. After receiving nutritional support for 6 mo, a gastrojejunostomy with Braun's anastomosis for the first portion and a duodenojejunostomy for the second and third portions were performed. The postoperative course was almost uneventful. Three months later, he was discharged and able to attend university. In patients with widespread duodenal stenosis, there may be a complicated cause,such as PDPV and duodenal stretching induced by previous spinal surgery.展开更多
The pitch of the spiral finned tube influences seriously to the acoustic resonance in the heat exchanger.In this research,the flow characteristics in relating to the aeolian tone from the finned cylinder are studied b...The pitch of the spiral finned tube influences seriously to the acoustic resonance in the heat exchanger.In this research,the flow characteristics in relating to the aeolian tone from the finned cylinder are studied by the numerical simulation.It is observed that the tonal noise generated from the finned tube at two pitch spaces.The ratio of the fin pitch to the cylinder diameter is changed at 0.11 and 0.27.The tone level increases and the frequency decreases with the pitch shorter.The separation flow from the cylinder generates the span-wise vortices,Karman vortices,and the separation flow from the fin generates the stream-wise vortices.When the fin pitch ratio is small,the stream-wise vortices line up to span-wise and become weak rapidly.Only the Karman vortices are remained and integrate in span.So the Karman vortex became large.This causes the low frequency and the large aeolian tone.展开更多

Methods: Twenty six patients (23 males, 3 females) with lumbosacral SCI and 13 normal volunteers were enrolled into this...To investigate the anorectal status in patients with lumbosacral spinal cord injury (SCI). Methods: Twenty six patients (23 males, 3 females) with lumbosacral SCI and 13 normal volunteers were enrolled into this study as controls. The median age was 43.7 years (ranging 17-68 years) and the median time of patients since injury was 59.1 months ( ranging 8 months-15 years). They were diagnosed as complete lumbosacral SCI (n=2, American Spinal Injury Association (ASIA) score A), or incomplete lumbosacral SCI (n =24, ASIA score B- D) with mixed symptoms of constipation and/or fecal incontinence, and were studied by anorectal manometry. None of the patients had any medical treatments for neurogenic bowel prior to this study. Results: The maximum anal resting pressure in lumbosacral SCI patients group was slightly lower than that in control group ( One-way ANOVA: P = 0. 939 ). During defecatory maneuvers, 23 of 26 ( 88. 5 %) patients with lumbosacral SCI and I of 13 ( 7. 7 % ) in the control group showed pelvic floor dysfunction (PFD) (Fisher' s exact test: P〈0.0001). Rectoanal inhibitory reflex (RAIR) was identified in both patients with lumbosacral SCI and the controls. The rectal volume for sustained relaxation of the anal sphincter tone in lumbosacral SCI patients group was significantly higher than that in the control group (Independent-Samples t test: P〈0.0001). The mean rectal volume to generate the first sensation was 92.7 ml ± 57.1 mi in SCI patients, 41. S ml ± 13.4 ml in the control group ( Independent-Samples t test: P 〈0.0001 ). Conclusions: Most of the patients with lumbosacral SCI show PFD during defecatory maneuvers and their rectal sensation functions are severely damaged. Some patients exhibit abnormal cough reflex. Anorectai manometry may be helpful to find the unidentified supraconal lesions. RAIR may be modulated by central nervous system (CNS).展开更多
