Multimodality imaging using transthoracic echocardiography (TTE) or transesophageal echocardiography (TOE) and multislice CT (MSCT) constitute cornerstone techniques for the pre-operative management, peri-procedural guidance, follow up and recognition of possible transcatheter valve related complications. CT angiography is much more accurate regarding the total definition of aortic annulus diameter and circumferential area. Two-dimensional (2D) echocardiography, underestimates the aortic valve annulus diameter compared to 3D imaging techniques (MSCT, MRI and 3D TOE). Three-dimensional TOE imaging provides measurements of the aortic valve annulus similar to those delivered by MSCT. The pre-procedural MSCT constitutes the gold standard modality minimizing the presence of paravalvular aortic regurgitation, one of the most frequent complications. TOE/TTE and MSCT performance could predict the possibility of pacemaker implantation post-procedural. The presence of a new transient or persisting MR can be assessed well by TOE. Both TTE and TOE, consist initially the basic examination for post TAVI evaluation. In case of transcatheter heart valve failure, the MSCT could be used as additional imaging technique.展开更多

We utilize the fuzzy linguistic scale to construct a linguistic preference matrix, and propose a fuzzy induced ordered weighted geometric averaging (FIOWGA) operator to aggregate linguistic preference information. A method based on the fuzzy linguistic scale and FIOWGA operator for decision-making problems is presented. Since the approval of rituximab in 1997, monoclonal antibodies(mAbs) have become an increasingly important component of therapeutic regimens in oncology. The success of mAbs as a therapeutic class is a result of great...Since the approval of rituximab in 1997, monoclonal antibodies(mAbs) have become an increasingly important component of therapeutic regimens in oncology. The success of mAbs as a therapeutic class is a result of great strides that have been made in molecular biology and in biotechnology over the past several decades. Currently, there are 14 approved mAb products for oncology indications, and there are ten additional mAbs in late stages of clinical trials. Compared to traditional chemotherapeutic agents, mAbs have several advantages, including a long circulating half-life and high target specificity. Antibodies can serve as cytotoxic agents when administered alone, exerting a pharmacologic effect through several mechanisms involving the antigen binding(Fab) and/or Fc domains of the molecule, and mAbs may also be utilized as drug carriers, targeting a toxic payload to cancer cells. The extremely high affinity of mAbs for their targets, which is desirable with respect to pharmacodynamics(i.e., contributing to the high therapeutic selectivity of mAb), often leads to complex, non-linear, target-mediated pharmacokinetics. In this report, we summarize the pharmacokinetic and pharmacodynamics of mAbs that have been approved and of mAbs that are nearing approval for oncology indications, with particular focus on the molecular and cellular mechanisms responsible for their disposition and efficacy.展开更多

Simulation results of the WDM6 scheme and the Thompson scheme,both of which are commonly-used double-moment bulkmicrophysics schemes,are compared within theWeather Research and Forecasting model.The purpose of the com...Simulation results of the WDM6 scheme and the Thompson scheme,both of which are commonly-used double-moment bulkmicrophysics schemes,are compared within theWeather Research and Forecasting model.The purpose of the comparison is to study the difference in the aspects of the warm-rain hydrometeor number concentrations,the droplet size distributions,and the budgets of the rain mixing ratio and number concentration.It is found that the WDM6 scheme overestimates the ratio and the amount of large precipitation,and underestimates those of small precipitation,compared to the Thompson scheme.The cloud number concentration(CNC)predicted in the WDM6 scheme is one to three orders ofmagnitude smaller than that of the Thompson scheme,which is set to the specific valueof CNC.The cloud droplet spectra of the WDM6 scheme are broader.The WDM6 scheme produces a larger rain number concentration and smaller mass mean diameter of raindrops under the influence of both warm and cold rain processes—specifically,autoconversion and melting of snow and graupel.The WDM6 scheme produces a larger autoconversion rate and smaller total melting rate of snow and graupel than the Thompson scheme in the rain mixing ratio budget.The sign of the difference in the rain-cloud collection varieswith region,and the rain-cloud collection process together with evaporation of rain have a major influence on the sign of the surface precipitation difference between the two schemes.展开更多

Objective: To investigate the clinical and pathological characteristics, treatment methods, and prognosis of synchronous primary cancer of the endometrium and ovary. Methods: The clinical data of 43 patients with sy...Objective: To investigate the clinical and pathological characteristics, treatment methods, and prognosis of synchronous primary cancer of the endometrium and ovary. Methods: The clinical data of 43 patients with synchronous primary cancer of endometrium and ovary were retrospectively reviewed. The survival was calculated by Kaplan-Meier method and compared using the log-rank test. Results: The median age of the patients at diagnosis was 49 years (range, 28-73 years). The most common symptoms were abnormal vaginal bleeding (69.8%) and abdominal or pelvic pain (44.2%). Pelvic masses were found in 39.5% of the patients and enlarged corpus in 27.9% at physic examination, while pelvic masses were found in 67.4% of the 43 patients (29 cases) and thickening or abnormal endometrium in 23.3% (10 cases) during ultrasound examination. Of 25 patients examined by CT/MRI, pelvic masses were found in 13 cases and enlarged uterus in 11 cases. All 15 patients who underwent endometrial biopsies were proven to have endometrioid carcinomas. Serum CA125 level was found to be elevated in 22 of the 34 examined cases (64.7%) with median value 500 U/mL (range, 39-3439 U/mL). FIGO stages of endometrial carcinomas: IA 18 cases, IB 20 cases, IC 2 cases, and ⅡA 3 cases; Stages of ovarian carcinomas: IA 19 case, IB 4 cases, IC 7 cases, Ⅱ 4 cases, and ⅢC 9 cases. Twenty-four patients (55.8%) were in stage I both endometrial and ovarian carcinomas. Thirty-one patients underwent total hysterectomy plus bilateral salpingo-oophorectomy with omentectomy and appendectomy, meanwhile, 12 patients had pelvic lymph nodes dissection. Thirty-eight of the 43 patients (88.4%) had a pathologically proven endometrial adenocarcinomas. The predominant ovarian histologies were endometrioid or mixed tumors with endometrioid components (30/43, 69.8%). Postoperatively, 26 patients (60.5%) received adjuvant chemotherapy alone, 12 had chemotherapy plus radiotherapy, only one patients had radiation alone and the remaining 4 cases received no adjuvant treatment. The 3-year and 5-year survival rates of the group were 87.4% and 71.1% respectively. The 3-year and 5-year survival rates of patients with endometrioid carcinoma at both endometrial and ovarian were higher than that of those with non-endometrioid or mixed histologic subtypes (93.8%, 82% vs 79.7%, 69%). The 3-year and 5-year survival rates of patients with early stages disease were better than those of other patients (93.3%, 93.3% vs 69.7%, 36.7%). Recurrence developed in 15 patients (34.9%). It was showed by univariate analysis that lower CA125 level, early FIGO stage, and adjuvant chemotherapy plus radiotherapy significantly and positively affected the 5-year survival rate, while only early FIGO stage and chemotherapy plus radiotherapy were revealed by multivariate analysis as independent prognostic factors. Conclusion: Synchronous primary cancers of the endometdum and ovary were different from either the primary endometrial or ovarian cancer, while usually it can be detected in early stage with a good prognosis. The impact of the CA125 level on prognosis needs to be further studied. Surgery treatment alone may be enough for early stage patients. Chemotherapy plus radiotherapy may be necessary for advanced patients.展开更多

The adsorption behavior of the functionalized chelating fibers for Hg(Ⅱ) was investigated by static adsorption experiments, and the effects of some essential factors on adsorption of Hg(Ⅱ) were examined, such as pH, initial concentration, adsorption time, coexisting cations, and temperature, The results showed that the adsorptive equilibrium could be achieved in 10 min, and the equilibrium adsorption quantity of PP-g-AA-TU fibers was 20 times that of PP-g-AA fibers. The PP-g-AA-TU fibers showed a very high adsorption rate and a good selectivity for Hg(Ⅱ) over a wide range of pH. The adsorption isotherm can be well described with Langmuir model, with the maximum adsorption capacity for Hg(Ⅱ) up to 52.04 mg.g-1 and the removal of Hg(Ⅱ) more than 97%. The kinetic data indicate that the adsorption process is best-fitted into the pseudo-second-order model.展开更多

The slip boundary condition is simulated by specular reflection boundary and the no-slip boundary condition is simulated by bounce back boundary. The first scheme aims to generating multipartite entanglement for vibrational modes of trapped ions, which only requires a single laser beam tuned to the ionic carrier frequency. Our scheme works in the mediated excitation regime, in which the corresponding Rabi frequency is equal to the trap frequency. Beyond their fundamental importance, these states may be of interest for experimental studies on decoherence since the present scheme operates in a fast way. The second scheme aims to preparing the continuous variable multimode maximal1y Greenberger-Horne-Zeilinger state. The distinct advantage is that the operation time is only limited by the available laser intensity, not by the inherent mechanisms such as off-resonant excitations. This makes it promising to obtain entanglernent of multiple coherent and squeezing states with desired amplitudes in a reasonable time.展开更多

In the scheme, a member of the group signs the message on behalf of the group while the receiver verifies the signature's validity with the aid of the trusty group manager who plays a crucial role when a possible dispute arises. A probabilistic encryption algorithm is generally adopted in public-key encryption protocols. Here we consider the way it increases the unicity distance of both classical and quantum private-key encryption schemes. The security of quantum probabilistic privatekey encryption schemes against two kinds of attacks is analyzed. By using the no-signalling postulate, we show that the scheme can resist attack to the key. The scheme's security against plaintext attack is also investigated by considering the information-theoretic indistinguishability of the encryption scheme. AIM:To evaluate the trends in the eradication rate of Helicobacter pylori(H.pylori) over the past 11 years in a single center.METHODS:This retrospective study covered the period from January 2000 to December 2010.We e...AIM:To evaluate the trends in the eradication rate of Helicobacter pylori(H.pylori) over the past 11 years in a single center.METHODS:This retrospective study covered the period from January 2000 to December 2010.We evaluated 5746 patients diagnosed with gastric ulcers(GU),duodenal ulcers(DU),GU + DU,or nonpeptic ulcers associated with an H.pylori infection.We treated them annually with the 2 wk standard first-line triple regimen,proton pump inhibitor(PPI) + amoxicilin + clarithromycin(PAC;PPI,clarithromycin 500 mg,and amoxicillin 1 g,all twice a day).The follow-up test was performed at least 4 wk after the completion of the 2 wk standard H.pylori eradication using the PAC regimen.We also assessed the eradication rates of 1 wk second-line therapy with a quadruple standard regimen(PPI b.i.d.,tripotassium dicitrate bismuthate 300 mg q.i.d.,metronidazole 500 mg t.i.d.,and tetracycline 500 mg q.i.d.) after the failure of the first-line therapy.Statistical analysis was performed with 95%CI for the differences in the annual eradication rates.RESULTS:A total of 5746 patients [2333 males(58.8%),1636 females(41.2%);mean age of males vs females 51.31 ± 13.1 years vs 52.76 ± 13.6 years,P < 0.05,total mean age 51.9 ± 13.3 years(mean ± SD)] were investigated.Among these patients,1674 patients were excluded:35 patients refused treatment;18 patients ceased H.pylori eradication due to side effects;1211 patients had inappropriate indications for H.pylori eradication,having undergone stomach cancer operation or chemotherapy;and 410 patients did not undergo the follow-up.We also excluded 103 patients who wanted to stop eradication treatment after only 1 wk due to poor compliance or the side effects mentioned above.Finally,we evaluated the annual eradication success rates in a total of 3969 patients who received 2 wk first-line PAC therapy.The endoscopic and clinical findings in patients who received the 2 wk PAC were as follows:gastric ulcer in 855(21.5%);duodenal ulcer in 878(22.1%);gastric and duodenal ulcer in 124(3.1%),erosive,atrophic gastritis and functional dyspepsia in 2055(51.8%);and other findings(e.g.,MALToma,patients who wanted to receive the therapy even though they had no abnormal endoscopic finding) in 57(0.5%).The overall eradication rate of the 2 wk standard firstline triple regimen was 86.5%.The annual eradication rates from 2000 to 2010 were 86.7%,85.4%,86.5%,83.3%,89.9%,90.5%,88.4%,84.5%,89.1%,85.8%,and 88.3%,sequentially(P = 0.06).No definite evidence of a significant change in the eradication rate was seen during the past eleven years.The eradication rates of second-line therapy were 88.9%,82.4%,85%,83.9%,77.3%,85.7%,84.4%,87.3%,83.3%,88.9%,and 84%(P = 0.77).The overall eradication rate of 1 wk quadruple second-line therapy was 84.7%.There was no significant difference in the eradication rate according to the H.pylori associated diseases.CONCLUSION:This study showed that there was no trend change in the H.pylori eradication rate over the most recent 11 years in our institution.展开更多

文摘Recently, Xiu et al. [Commun. Theor. Phys. 49 (2008) 905] proposed two schemes of teleporting an N particle arbitrary and unknown state when N groups of three particle general W states are utilized a.s quantum channels. They gave the maximal probability of successful teleportation. Here we find that their operation is not the optimal and the successful probability of the teleportation is not maximum. Moreover, we give the optimal schemes operation and obtain maximal successful probability for teleportation.
文摘Objective: To investigate the clinical and pathological characteristics, treatment methods, and prognosis of synchronous primary cancer of the endometrium and ovary. Methods: The clinical data of 43 patients with synchronous primary cancer of endometrium and ovary were retrospectively reviewed. The survival was calculated by Kaplan-Meier method and compared using the log-rank test. Results: The median age of the patients at diagnosis was 49 years (range, 28-73 years). The most common symptoms were abnormal vaginal bleeding (69.8%) and abdominal or pelvic pain (44.2%). Pelvic masses were found in 39.5% of the patients and enlarged corpus in 27.9% at physic examination, while pelvic masses were found in 67.4% of the 43 patients (29 cases) and thickening or abnormal endometrium in 23.3% (10 cases) during ultrasound examination. Of 25 patients examined by CT/MRI, pelvic masses were found in 13 cases and enlarged uterus in 11 cases. All 15 patients who underwent endometrial biopsies were proven to have endometrioid carcinomas. Serum CA125 level was found to be elevated in 22 of the 34 examined cases (64.7%) with median value 500 U/mL (range, 39-3439 U/mL). FIGO stages of endometrial carcinomas: IA 18 cases, IB 20 cases, IC 2 cases, and ⅡA 3 cases; Stages of ovarian carcinomas: IA 19 case, IB 4 cases, IC 7 cases, Ⅱ 4 cases, and ⅢC 9 cases. Twenty-four patients (55.8%) were in stage I both endometrial and ovarian carcinomas. Thirty-one patients underwent total hysterectomy plus bilateral salpingo-oophorectomy with omentectomy and appendectomy, meanwhile, 12 patients had pelvic lymph nodes dissection. Thirty-eight of the 43 patients (88.4%) had a pathologically proven endometrial adenocarcinomas. The predominant ovarian histologies were endometrioid or mixed tumors with endometrioid components (30/43, 69.8%). Postoperatively, 26 patients (60.5%) received adjuvant chemotherapy alone, 12 had chemotherapy plus radiotherapy, only one patients had radiation alone and the remaining 4 cases received no adjuvant treatment. The 3-year and 5-year survival rates of the group were 87.4% and 71.1% respectively. The 3-year and 5-year survival rates of patients with endometrioid carcinoma at both endometrial and ovarian were higher than that of those with non-endometrioid or mixed histologic subtypes (93.8%, 82% vs 79.7%, 69%). The 3-year and 5-year survival rates of patients with early stages disease were better than those of other patients (93.3%, 93.3% vs 69.7%, 36.7%). Recurrence developed in 15 patients (34.9%). It was showed by univariate analysis that lower CA125 level, early FIGO stage, and adjuvant chemotherapy plus radiotherapy significantly and positively affected the 5-year survival rate, while only early FIGO stage and chemotherapy plus radiotherapy were revealed by multivariate analysis as independent prognostic factors. Conclusion: Synchronous primary cancers of the endometdum and ovary were different from either the primary endometrial or ovarian cancer, while usually it can be detected in early stage with a good prognosis. The impact of the CA125 level on prognosis needs to be further studied. Surgery treatment alone may be enough for early stage patients. Chemotherapy plus radiotherapy may be necessary for advanced patients.

文摘A fast and selective adsorbent for Hg(ll) from aqueous solutions using thiourea (TU) functionalized polypropylene fiber grafted acrylic acid (PP-g-AA), PP-g-AA-TU fibers, was characterized by Fourier transform infrared spectroscopy and X-ray photoelectron spectroscopy. The adsorption behavior of the functionalized chelating fibers for Hg(Ⅱ) was investigated by static adsorption experiments, and the effects of some essential factors on adsorption of Hg(Ⅱ) were examined, such as pH, initial concentration, adsorption time, coexisting cations, and temperature, The results showed that the adsorptive equilibrium could be achieved in 10 min, and the equilibrium adsorption quantity of PP-g-AA-TU fibers was 20 times that of PP-g-AA fibers. The PP-g-AA-TU fibers showed a very high adsorption rate and a good selectivity for Hg(Ⅱ) over a wide range of pH. The adsorption isotherm can be well described with Langmuir model, with the maximum adsorption capacity for Hg(Ⅱ) up to 52.04 mg.g-1 and the removal of Hg(Ⅱ) more than 97%. The kinetic data indicate that the adsorption process is best-fitted into the pseudo-second-order model.
文摘Endoscopic submucosal dissection(ESD) is a widely accepted treatment for early gastric cancer(EGC),especially in Korea and Japan.The criteria for the therapeutic use of ESD for EGC have been expanded recently.However,attention should be drawn to the technical feasibility of the ESD treatment which depends on a lesion's location,size or fibrosis level,or operator's experience.In the case of a lesion with a high level of difficulty,a more experienced operator is required.Thus,the treatment for a lesion with a high level of difficulty should be performed according to the degree of the operator's experience.In this paper,the authors describe the ESD procedure for lesions with a high level of difficulty.

文摘The effect of viscosity and viscosity difference and boundary patterned slip on mixing in a micro mixer has been numerically studied using lattice Boltzmann method (LBM). The slip and no-slip ratio is not constant and varies irregularly, and viscosity is altered by changing the relaxation time in LBE equation. The slip boundary condition is simulated by specular reflection boundary and the no-slip boundary condition is simulated by bounce back boundary. It has been found that it is feasible to optimize the micro mixer design by combining the viscosity effect and boundary patterned ratio altogether.

文摘We propose two schemes to prepare entanglement for the vibronic collective states of multiple trapped ions. The first scheme aims to generating multipartite entanglement for vibrational modes of trapped ions, which only requires a single laser beam tuned to the ionic carrier frequency. Our scheme works in the mediated excitation regime, in which the corresponding Rabi frequency is equal to the trap frequency. Beyond their fundamental importance, these states may be of interest for experimental studies on decoherence since the present scheme operates in a fast way. The second scheme aims to preparing the continuous variable multimode maximal1y Greenberger-Horne-Zeilinger state. The distinct advantage is that the operation time is only limited by the available laser intensity, not by the inherent mechanisms such as off-resonant excitations. This makes it promising to obtain entanglernent of multiple coherent and squeezing states with desired amplitudes in a reasonable time.

文摘A quantum group signature(QGS) scheme is proposed on the basis of an improved quantum chaotic encryption algorithm using the quantum one-time pad with a chaotic operation string. It involves a small-scale quantum computation network in three phases, i.e. initializing phase, signing phase and verifying phase. In the scheme, a member of the group signs the message on behalf of the group while the receiver verifies the signature's validity with the aid of the trusty group manager who plays a crucial role when a possible dispute arises. Analysis result shows that the signature can neither be forged nor disavowed by any malicious attackers.

文摘We present a quantum probabilistic encryption algorithm for a private-key encryption scheme based on conjugate coding of the qubit string. A probabilistic encryption algorithm is generally adopted in public-key encryption protocols. Here we consider the way it increases the unicity distance of both classical and quantum private-key encryption schemes. The security of quantum probabilistic privatekey encryption schemes against two kinds of attacks is analyzed. By using the no-signalling postulate, we show that the scheme can resist attack to the key. The scheme's security against plaintext attack is also investigated by considering the information-theoretic indistinguishability of the encryption scheme. Finally, we make a conjecture regarding Breidbart's attack.
文摘Today, Social Networking Sites (SNS), online forum, and e-mail communication have become a daily phenomenon in Indonesia. Thousands and even millions of the people have been using Blog, Facebook, MySpace, Twitter, and others as a communication medium. Many of them are mobile connected and have made it a daily practices. Interestingly, all these changes are not followed by Onmi International Hospital which it ultimately in 2009 led to the crisis. This crisis was beginning from the customer, Prita Mulyasari, who felt disappointed at the hospital service, and wrote an e-mail to her friend. Suddenly, in a short time the e-mail was disseminated widely on the Internet and produced a lot of support and public humiliation to the Omni International Hospital. This paper aims at explaining the cybcr crisis experienced by Omni International Hospital and how to overcome the problem at corporate level.
文摘AIM:To evaluate the trends in the eradication rate of Helicobacter pylori(H.pylori) over the past 11 years in a single center.METHODS:This retrospective study covered the period from January 2000 to December 2010.We evaluated 5746 patients diagnosed with gastric ulcers(GU),duodenal ulcers(DU),GU + DU,or nonpeptic ulcers associated with an H.pylori infection.We treated them annually with the 2 wk standard first-line triple regimen,proton pump inhibitor(PPI) + amoxicilin + clarithromycin(PAC;PPI,clarithromycin 500 mg,and amoxicillin 1 g,all twice a day).The follow-up test was performed at least 4 wk after the completion of the 2 wk standard H.pylori eradication using the PAC regimen.We also assessed the eradication rates of 1 wk second-line therapy with a quadruple standard regimen(PPI b.i.d.,tripotassium dicitrate bismuthate 300 mg q.i.d.,metronidazole 500 mg t.i.d.,and tetracycline 500 mg q.i.d.) after the failure of the first-line therapy.Statistical analysis was performed with 95%CI for the differences in the annual eradication rates.RESULTS:A total of 5746 patients [2333 males(58.8%),1636 females(41.2%);mean age of males vs females 51.31 ± 13.1 years vs 52.76 ± 13.6 years,P < 0.05,total mean age 51.9 ± 13.3 years(mean ± SD)] were investigated.Among these patients,1674 patients were excluded:35 patients refused treatment;18 patients ceased H.pylori eradication due to side effects;1211 patients had inappropriate indications for H.pylori eradication,having undergone stomach cancer operation or chemotherapy;and 410 patients did not undergo the follow-up.We also excluded 103 patients who wanted to stop eradication treatment after only 1 wk due to poor compliance or the side effects mentioned above.Finally,we evaluated the annual eradication success rates in a total of 3969 patients who received 2 wk first-line PAC therapy.The endoscopic and clinical findings in patients who received the 2 wk PAC were as follows:gastric ulcer in 855(21.5%);duodenal ulcer in 878(22.1%);gastric and duodenal ulcer in 124(3.1%),erosive,atrophic gastritis and functional dyspepsia in 2055(51.8%);and other findings(e.g.,MALToma,patients who wanted to receive the therapy even though they had no abnormal endoscopic finding) in 57(0.5%).The overall eradication rate of the 2 wk standard firstline triple regimen was 86.5%.The annual eradication rates from 2000 to 2010 were 86.7%,85.4%,86.5%,83.3%,89.9%,90.5%,88.4%,84.5%,89.1%,85.8%,and 88.3%,sequentially(P = 0.06).No definite evidence of a significant change in the eradication rate was seen during the past eleven years.The eradication rates of second-line therapy were 88.9%,82.4%,85%,83.9%,77.3%,85.7%,84.4%,87.3%,83.3%,88.9%,and 84%(P = 0.77).The overall eradication rate of 1 wk quadruple second-line therapy was 84.7%.There was no significant difference in the eradication rate according to the H.pylori associated diseases.CONCLUSION:This study showed that there was no trend change in the H.pylori eradication rate over the most recent 11 years in our institution.
