METHODS: Thirty-five patients with gastric or duodenal ulcers positive for H pylori were randomized either to 1 wk amoxicillin, clarithromycin, lansoprazole (Group 1) or to the same regimen supplemented with CBM 7 d ahead of the triple therapy (Group 2). Stool samples were collected before and 2, 4, 7, 15, and 22 d after the starting eradication therapy, and were examined intestinal flora. Patients were required to keep a diary record of their condition. RESULTS: Obligate anaerobes decreased significantly on d 2, 4, 8 and 15 in Group 1. On the other hand, they did not decrease significantly in Group 2. The Escherichia coli was dominant bacterium in Enterobacteriaceae, but that was replaced by other species such as Klebsiella and Enterobacter after eradication in Group 1. The change was suppressed in Group 2. Abdominal symptoms were less frequent in Group 2 than in Group 1. CONCLUSION: The combined use of CBM reduced the changes in the intestinal flora and decreased the incidence of gastrointestinal side effects.展开更多

Studies have provided indirect evidence that cellulolytic activity of some anaerobic bacteria is repressed by carbohydrates, such as glucose. This effect is known as carbon catabolite repression (CCR). Previous work...Studies have provided indirect evidence that cellulolytic activity of some anaerobic bacteria is repressed by carbohydrates, such as glucose. This effect is known as carbon catabolite repression (CCR). Previous work has found that cellulolytic activity of Clostridium cellulovorans and Eubacterium cellulosolvens are regulated. Many cellulolytic systems of these organisms are expressed only in the presence of cellulose or cellobiose (the disaccharide of cellulose). Some of these cellulose-induced systems also appear subject to CCR when more soluble substrates, such as glucose, are also available. To determine if such repression directly effects cellulolytic activity of C. cellulovorans and E. cellulosolvens, these organisms were cultivated in media containing a glucose analog. We then measured the ability of low levels of analog to inhibit growth of the organisms when cellobiose or cellulose were the energy substrates. Our results found that growth of both C. cellulovorans and E. cellulosolvens in cellobiose-containing medium are strongly inhibited by glucose analogs. In addition, both organisms exhibited delayed and slower growth in cellulose-containing medium when a glucose analog was added. These results provide direct demonstration that these cellulolytic bacteria are subject to CCR. This repression of cellulolysis may affect both of these organisms' ability to serve as industrial platforms for biomass degradation, and may interfere with the contribution of E. cellulosolvens toward animal digestion of cellulose. These results were also in sharp contrast to what has been reported regarding CCR activity in Clostridium cellulolyticum, which actively expresses cellulases in the presence of low levels of glucose.展开更多

Patients: Serum inflammation markers of fifty-five outpatients (29 females, 26 males, aged 78 + 8.5 years) were analyzed. Stool specimens and thus data on gut microbiota were available from a subgroup of 23 individuals (9 females and 14 males). Results: Global cerebral atrophy was found in all magnet resonance tomography scans. Mean mini-mental-score examination in Alzheimer's disease patients was 18.8 ± 7.1, in patients with mild cognitive impairment 27.8 ± 1.5. Serum neopterin concentrations correlated with concentrations of fecal S100A12 (p 〈 0.001) and cq-antitrypsin (p 〈 0.05). Faecalibacterium prausnitzii correlated with MMSE (p 〈 0.05), with Akkermansia muciniphila (p 〈 0.01) and with serum neopterin (p 〈 0.05). Fecal zonulin correlated inversely with Clostridium cluster I (p 〈 0.02). Conclusions: Our results underline earlier in vitro and animal studies that cognitive decline associates with age-related changes in the intestinal microbiota and neuroinflammation. The disease is rarely diagnosed in cats, as a consequence of their increased resistance to the neurotoxin, tetanospasmin. Mortality in animals is generally high (80%). To date these authors have not been able to locate any reports of mortality rates specific to cats. Clinical diagnosis is based on clinical signs and a history of an untreated penetrating wound. This case report describes the development of moderate generalized tetanus in an approximately six month old female stray cat, found with an untreated wound on the side of its neck. Histiocytes have a pivotal role in wound repair and intestinal epithelial recovery-the most important goal to sustain gut functionality.Yet,an in vivo description of colonic histiocytes by confocal laser endomicroscop...Histiocytes have a pivotal role in wound repair and intestinal epithelial recovery-the most important goal to sustain gut functionality.Yet,an in vivo description of colonic histiocytes by confocal laser endomicroscopy(CLE) is missing.Here,we report the case of a 45-yearsold male patient who was referred to our clinic with weight loss and a history of two consecutive Clostridium difficile colitis episodes,the latter cured 3 wk before present admission.Stool microbiology was negative.Conventional colonoscopy showed atrophy and a light mucosal oedema in the distal colon.During on-going endoscopy,we performed a fluorescein-aided CLE which revealed large polygonal(histiocytes-like) cells with copious cytoplasm and large nuclei in the lamina propria of the sigmoid colon as well as regenerative epithelial changes.Histopathological assessment of biopsies from the same areas confirmed the endomicroscopical findings:Periodic acid-Schiff-and CD68-positive foamy histiocytes in the colonic lamina propria and an advanced epithelial recovery.Since stool microbiology was repeatedly negative and polymerase chain reaction-analysis from colonic biopsies could not detect any mRNA for Thropheryma whippleii and common pathogens,we interpreted this particular setting as a mucosal healing process after consecutive Clostridium difficile infections.In conclusion,by describing these colonic histiocytes,we highlight the clinical usefulness of CLE in describing the entity of histiocytes in vivo and in real-time during the process of post-infectious mucosal healing in the colon.展开更多
