观察3种不同视频喉镜在模拟困难气道模型气管插管的效果 被引量:1

目的 初次应用3种不同视频喉镜于模拟困难气道模型气管插管时的效果,为临床初次处理困难气道时提供技术数据参考。方法 选择在麻醉科实习1~3个月的实习学生32名,均在调整好的AirSim模拟困难气道模型上使用Macintosh喉镜(M组),GlideScop... 目的 初次应用3种不同视频喉镜于模拟困难气道模型气管插管时的效果,为临床初次处理困难气道时提供技术数据参考。方法 选择在麻醉科实习1~3个月的实习学生32名,均在调整好的AirSim模拟困难气道模型上使用Macintosh喉镜(M组),GlideScope(G组)、Hc(H组)和Airtraq(A组)进行气管插管。记录气管插管总时间、获得最佳视野的时间、气管导管置入的时间、气管导管置入次数、喉部暴露分级,同时使用VAS评分评估每种喉镜最佳暴露时使用力量的大小和评价潜在创伤。结果 气管插管总时间最短的是M组。获得最佳视野的时间最短的是M组。气管导管置入时间最短的是M组;声门暴露C/LⅢ级以上,M组、G组、H组和A组暴露情况分别为46.9%、6.2%、9.3%、12.4%。反复置管次数最少的是H组。最佳暴露时A组声门最佳暴露时使用力量最小。结论 对未使用过视频喉镜的志愿者来说,初次应用3种视频喉镜在模拟困难气道模型气管插管时,与Macintosh喉镜相比,3种视频喉镜能明显改善模拟困难气道下的喉部暴露分级,但会延长插管的时间,对急救是不利的。 展开更多

目的 研究比较模拟气道和虚拟现实模拟装置在纤维支气管镜(简称纤支镜)插管培训中的应用效果。方法选取2020年9月至2021年9月在川北医学院附属医院麻醉科接受住院培训的230名麻醉学住院医师作为研究对象,采用随机数字表法分为试验组(n=1... 目的 研究比较模拟气道和虚拟现实模拟装置在纤维支气管镜(简称纤支镜)插管培训中的应用效果。方法选取2020年9月至2021年9月在川北医学院附属医院麻醉科接受住院培训的230名麻醉学住院医师作为研究对象,采用随机数字表法分为试验组(n=115)和对照组(n=115)。对照组医师采用模拟气道完成纤支镜插管培训,试验组医师采用虚拟现实模拟装置完成纤支镜插管培训。比较两组教学效果、操作效果、培训满意程度。结果 两组住院医师在纤支镜插管考核的5次纤支镜插管操作中均插管成功。试验组在第2、3、4、5次纤支镜插管操作考核中的插管时间短于对照组,差异有统计学意义(P <0.05);试验组在第1、2、3、4、5次纤支镜插管操作考核中的操作能力评分高于对照组,差异有统计学意义(P <0.05);试验组对镜头空间认知能力培训效果、镜头运动控制能力培训效果、本次培训过程结果趣味程度、本次培训结束后能力提升、本次培训结束后自学效果的满意度评分高于对照组,差异有统计学意义(P <0.05)。结论 虚拟现实模拟装置和虚拟气道在纤支镜插管培训中均有良好的应用效果,但虚拟现实模拟装置在培训中的应用能有效缩短住院医师纤支镜插管时间,提升住院医师纤支镜插管操作能力,提升住院医师培训满意程度。 展开更多

困难气道模拟教学在气道管理培训中的应用

目的探讨喉罩置入在外科住院医师规范化培训气道危机情境模拟教学中的应用效果。方法2017年7月至2018年6月,将在皖南医学院弋矶山医院麻醉科进行外科住院医师规范化培训轮转的48名学员按照入科顺序分为试验组与对照组,每组24名学员。试... 目的探讨喉罩置入在外科住院医师规范化培训气道危机情境模拟教学中的应用效果。方法2017年7月至2018年6月,将在皖南医学院弋矶山医院麻醉科进行外科住院医师规范化培训轮转的48名学员按照入科顺序分为试验组与对照组,每组24名学员。试验组实施情境模拟下的喉罩置入培训,对照组实施单纯的任务训练器喉罩置入培训。培训结束后,比较两组学员理论考试成绩和在模拟临床危机事件下急救气道处理能力,包括喉罩置入成功率等技术能力和解决问题能力等非技术能力;对两组学员开展培训满意度的问卷调查。结果试验组与对照组学员理论成绩差异无统计学意义[(88.0±4.2)分比(86.0±3.4)分,P=0.076];与对照组学员相比,试验组学员喉罩置入成功率更高[75.0%(18/24)比45.8%(11/24),P=0.039)],解决问题能力评分更高[(6.4±0.5)分比(2.1±1.2)分,P<0.01],教学总体满意度更高[(4.1±0.5)分比(3.5±0.6)分,P<0.01],差异均具有统计学意义。结论气道危机情境模拟喉罩置入培训的应用,可以有效提高外科住院医师规范化培训学员急救气道管理能力。 展开更多

Using the technique of smooth filtering and cylindrical filtering,the initial vortex circulation and large-scale environmental field were separated from the background field.Then the separated initial vortex circulation was translated and reinserted in the location where it was observed.This led to the determination of a method of initial vortex relocation.For seven tropical cyclones at 23 points of measurement time in the years of 2006 and 2007,two schemes,either directly adding a tropical cyclone bogus model in the background or adding it after the relocation of the initial vortex in the background field,were employed.Simulation experiments were compared.The results showed that the mean errors of the simulated tropical cyclone tracks at 24 and 48 hours were both smaller with the scheme of adding tropical cyclone bogus model after the relocation of the initial vortex in the background field.The relocation method of the initial vortex decreases the error caused by the deviation of the initial tropical cyclone location in tropical cyclone models.The relocation method is conducive to improving the track forecast of tropical cyclone models and has a good perspective for operational application. 展开更多

Based on use of multi-dimensional models,this investigation simulates the performance of a proton exchange membrane fuel cell by varying the channel pattern.In the one-dimensional model,the porosity of the gas diffusion layer is 0.3.The model reveals the water vapor distribution of the fuel cell and demonstrates that the amount of water vapor increases linearly with the reduction reaction adjacent to the gas channel and the gas diffusion layer.Secondly,four novel tapered gas channels are simulated by a two-dimensional model.The model considers the distributions of oxygen,the pressure drop,the amount of water vapor distribution and the polarization curves.The results indicate that as the channel depth decreases,the oxygen in the tapered gas channel can be accel-erated and forced into the gas diffusion layer to improve the cell performance.The three-dimensional model is employed to simulate the phenomenon associated with four novel tapered gas channels.The results also show that the best performance is realized in the least tapered gas channel.Finally,an experimentally determined mechanism is found to be consistent with the results of the simulation. 展开更多

Numerical computation models of air cooling heat transfer and flow behaviors in triangular wavy fin channels(TWFC) were established with structural parameters of fins considered.The air side properties of heat transfer coefficient and pressure drop are displayed with variable structural parameters of fins and inlet velocities of cooling air.Within the range of simulation,TWFC has the best comprehensive performance when inlet velocity vin=4-10 m/s.Compared with those of straight fins,the simulation results reveal that the triangular wavy fin channels are of higher heat transfer performances especially with the fin structural parameters of fin-height Fh=9.0 mm,fin-pitch Fp=2.5-3.0 mm,fin-wavelength λ=14.0-17.5 mm and fin-wave-amplitude A=1.0-1.2 mm.The correlations of both heat transfer factor and friction factor are presented,and the deviations from the experimental measurements are within 20%. 展开更多

A semi-empirical gas entrainment model was proposed for the ventilated cavity in vertical pipe, based on which, a complete numerical scheme was established by coupling with the Eulerian-Eulerian two-fluid model to predict the multiscale flow field created by ventilated cavity. Model predictions were validated against experimental measurements on void fraction and bubble size distributions. Simulations were carried out to explore the effect of ventilation rate and inlet turbulence intensity on the macroscale cavity shape and the bubbly flow downstream of the ventilated cavity. As the ventilation rate increasing, a reverse trend was observed for the void fraction and bub-ble size distributions. It is concluded that the average void fraction in the pipe flow region is determined by the vo-lumetric ratio between liquid and gas. However, the bubble size evolution is dominated by the breakage effect induced by turbulence in the vortex region. Furthermore, simulations were conducted to analyze geometric scale effect based upon Froude similitude. The results imply that the velocity distributions were properly scaled. Slight scale effect was seen for the void fraction caused by faster dispersion of bubbles in the larger size model. The comparatively greater bubble size was predicted in the smaller model, implying significant scale effects in terms of tur-bulence and surface tension effect. It reveals that empirical correlations valid in wide range are required for the extrapolation from small-size laboratory models. 展开更多

By PHONENICS program, the numerical simulation is processed, and the changing rule of airflow temperature with various parameters in drifting tunnel is derived. In this study, the change of free surface vor- tex and air absorption in a pump intake has been investigated by the Volume of Fraction (VOF) method with steady multiphase flow model in order to represent the behavior of the free surface vortex exactly. The homoge- neous free surface model is used to apply interactions of air and water. The results show that air intake by the free surface vortex motion can be visualized using the iso-surface of air volume fraction. The vortices make an air column from the free surface to the pump intake. Also, it was found that the flee surface vortex can be controlled by installing curtain walls. 展开更多
